When to Deliver Women with Diabetes in Pregnancy?

The prevalence of pregestational diabetes and the incidence of gestational diabetes have both increased over recent years. One component of the management of diabetes in pregnancy is the timing of delivery in the late-preterm, early-term, or full-term periods. Recent guidance from the National Institute for Child Health and Human Development, Society for Maternal-Fetal Medicine, and American College of Obstetricians and Gynecologists has lacked specificity, for example, recommending delivery for women with pregestational diabetes with poorly controlled glucose levels to be from 34 to 39 weeks'

[The "Adaptive Intelligence Diagnostic Test" in clinical practice: 3 case examples].

After the main advantages of the new German intelligence test-battery "Adaptive Intelligence Diagnosticum" (AID) are stated, this paper gives several case-studies in order to illustrate AID's support to clinical psychology. The case-studies are all quoted from a Child-Guidance-like institution. They allow inferences which might not be likely to be gained by some other tests. Besides, they serve for pursuing practical handling, computation, and interpretation of the AID.

Minor physical anomalies in normal, neurotic, learning disabled, and severely disturbed children.

A high incidence of minor physical anomalies in a childhood schizophrenic population has been previously reported by Goldfarb. In the present study, 108 boys from four different clinical populations were examined, utilizing a standardized anomaly scoring system for which a high interrater reliability was obtained. The patient populations were: general pediatric ward patients (n = 31), psychoneurotic outpatients at a university child guidance clinic (n = 26), learning diabled children (n = 23), and autistic, borderline, and atypical children (n = 28) from two residential treatment centers. [A case report of benign paroxysmal vertigo of childhood complicated with patent foramen ovale and emotional disorder].

Pediatric vertigo presents with complex etiologies, and its clinical diagnosis and management can be challenging due to children's limited ability to describe their symptoms. Recurrent vertigo episodes in children, who are at a critical stage of development, can readily lead to emotional disorders in both the children themselves and their guardians. This article reports a case of benign paroxysmal vertigo of childhood(BPVC) in a child who sought care from multiple providers. The child was found to have a patent foramen ovale(PFO) and mild emotional disorder. By analyzing challenges encountered

The Role of Early Maladaptive Schemas in the Appearance of Psychological Symptomatology in Adult Women Victims of Child Abuse.

Child abuse affects people's ways of thinking, feeling, and observing the world, resulting in dysfunctional beliefs and maladaptive schemas. Thus, consequences of child abuse may persist during adulthood. Therefore, the aim of this study was to analyze the psychological consequences (anxiety, phobic anxiety, depression, and hopelessness) of different types of maltreatment (physical, sexual, and emotional abuse and physical and emotional neglect) and to study the role of early maladaptive schemas in the onset of symptomatology in adult female victims of child abuse. The sample consisted of 75 w

Fractures in suspected child abuse: A critical review of the evidence cited by the American Academy of Pediatrics and Royal College of Paediatrics and Child Health.

We critically evaluated the evidence cited by the American Academy of Pediatrics (AAP) and the Royal College of Paediatrics and Child Health (RCPCH) supporting the diagnostic significance of fractures in suspected physical child abuse. Primary studies cited by the AAP Clinical Report and the RCPCH Systematic Review were reviewed with respect to study design, methods used to classify cases as abuse, and the potential for incorporation bias and outcome misclassification. Sixty-seven studies were identified, of which 35 reported measures of diagnostic utility. All were observational, predominantl

[Therapy of childhood schizophrenia].

Due to the multifacet genesis and variability of clinical phenomenology, the therapy of childhood schizophrenia must be multidimensional. Formerly applied techniques like electroshock- and insulin-therapy are now replaced by pharmaco-therapy, primarily with phenothiazines, butyrophenones and chlorprothixens. The dosage depends on age, body weight or body surface. Because of extrapyramidal motor side effects, combinations with anticholinergic drugs may be necessary. Psychopharmaco-therapy alone, however, is insufficient. High emphasis must be placed on psychotherapy and educational guidance and

Need for and Steps Toward a Clinical Guideline for the Telemental Healthcare of Children and Adolescents.

OBJECTIVE: This article identifies and describes key considerations toward the development of a clinical guideline intended to optimize telemental healthcare (TMH) of children and adolescents. METHODS: The literature was searched with key terms and title words. Of 2824 articles that met primary or secondary key word search criteria, 326 met both criteria, and 118 thematically related directly to child and adolescent TMH. Only 44 studies met levels of evidence I-III and expert recommendation criteria used in clinical guidelines; review of their references found 8 additional studies (52 total).

[Life-threatening macroglossia following cleft palate palatoplasty].

The case of a 13-month-old child who developed a life-threatening macroglossia with airway obstruction following palatoplasty for a cleft palate is reported. As direct laryngoscopy was not feasible a laryngeal mask (LM) was inserted to secure the airway. Under fiber optic guidance an endotracheal tube was then introduced via the LM. In this article the incidence, pathophysiology, clinical dynamics, options for emergency anesthesia management and organizational implications of this rare but typical complication in the field of oral and craniomaxillofacial surgery are reported.

Trends in childrens' ability to consent to a dental examination and the potential impact on reported caries indices.

UNLABELLED: New guidance on consent for England and Wales suggests that children aged over 11 should be asked to consent to the NHS child dental survey examinations. If they are "Gillick competent" then they can provide consent. Whether they are "Gillick competent" is a matter of clinical judgment of the examining dentist. This paper explores the level of understanding expressed after the examination by children apparently "Gillick competent". It considers issues how a dentist judges a child competent to make a decision to participate in a dental survey. OBJECTIVE: The objective of this inves

When Primary Ciliary Dyskinesia Is Diagnosed in Utero: Insights from Two Families.

INTRODUCTION: Primary ciliary dyskinesia (PCD) is a rare genetic disorder affecting ciliary function, leading to chronic respiratory ilness. Prenatal diagnosis remains uncommon, with most cases identified postnatally through clinical symptoms and genetic testing. However, advancements in prenatal screening have enabled earlier detection, presenting parents with complex medical and ethical dilemmas. This study explores parental decision-making following such a diagnosis, identifying key influencing factors and highlighting the role of healthcare providers in offering informed guidance. METHODS

What matters most to pediatric rheumatologists in deciding whether to discontinue biologics in a child with juvenile idiopathic arthritis? A best-worst scaling survey.

OBJECTIVES: Care for JIA patients has been transformed in the biologics era; however, biologics carry important (although rare) risks and are costly. Flares after biological withdrawal are seen frequently, yet there is little clinical guidance to identify which patients in clinical remission can safely have their biologic discontinued (by stopping or tapering). We examined what characteristics of the child or their context are important to pediatric rheumatologists when making the decision to discuss withdrawal of biologics. METHODS: We conducted a survey including a best-worst scaling (BWS)

Does the use of ultrasound guidance during percutaneous Achilles tendon tenotomy improve outcomes of clubfoot management? A randomized controlled trial.

Achilles tenotomy is a crucial aspect in the non-operative management of clubfoot as per Ponseti's casting protocol. Achilles tenotomy is routinely carried out percutaneously in a blind manner without any image guidance. This study aimed to determine the role of ultrasound-guided percutaneous Achilles tenotomy (PAT) in improving outcomes in clubfoot management. There are no previous studies that have explored the role of image guidance in PAT. This prospective, single-center randomized control trial included 50 clubfoot cases (74 feet) planned for PAT at a mean age of 30.2 months. A single exp

Parent psychological adjustment, donor conception and disclosure: a follow-up over 10 years.

STUDY QUESTION: What is the relationship between parent psychological adjustment, type of gamete donation (donor insemination, egg donation) and parents' disclosure of their use of donated gametes to their children. SUMMARY ANSWER: Disclosure of donor origins to the child was not always associated with optimal levels of psychological adjustment, especially for fathers in donor insemination families. WHAT IS KNOWN ALREADY: Cross-sectional analyses have found mothers and fathers who conceived a child using donated sperm or eggs to be psychologically well-adjusted, with few differences emerging

Use of procalcitonin in therapeutic decisions in the pediatric intensive care unit.

Procalcitonin (PCT) is frequently used by clinicians in children with suspected bacterial infections and sepsis. However interpretation in the critically ill child may be challenging due to the complexity of underlying conditions and its impact on PCT values. Herein, we propose a guidance for the use of procalcitonin in critically ill children, supported by a comprehensive analysis of the literature, to help the clinician for interpreting PCT in the various clinical conditions encountered in pediatric intensive care units. We describe the importance of the clinical context, timing of measureme

Contact with child protection services during pregnancy: a cross-sectional study using the eLIXIR Born in South London, UK maternity-child data linkage.

BACKGROUND: In the last decade, rates of children with child protection agency involvement have increased in many high-income countries, including the UK. Disparities in both maternal and child health outcomes as well as child welfare referrals have been widely evidenced, yet no previous research has investigated contact with child protection agencies in the UK (Children’s Social Care, CSC) during pregnancy using linked maternity and mental health records. The aim of this study was to investigate characteristics of pregnant women when child protection agencies are involved and investigate what

End-of-life medical decision-making for children in custody: A collaborative, multi-stakeholder practical approach.

BACKGROUND: Parents have a constitutionally-protected, fundamental right to make decisions concerning the health and well-being of their children, afforded by the Due Process Clause of the Fourteenth Amendment. However, parental rights are not absolute, and may be curtailed after a finding of parental "unfitness" including perpetration of egregious child abuse/neglect. Court intervention may be necessary to assert "parens patriae" authority to protect a child's well-being. Disagreements over medical care for a child (particularly when parent maltreatment resulted in life-altering clinical cond

Minimum intervention oral care management of early childhood caries: a 17-year follow-up case report.

BACKGROUND: Early childhood caries (ECC) has a profound impact on a child's quality of life, and its management remains a challenge for the paediatric dentist, mainly because it depends on radical changes in the child/carers' daily behaviour and any dental treatment must be provided to very young child. CASE REPORT: This case report describes the on-going care of a 2-year-old child presenting with ECC and management until permanent dentition is complete. All patient-focused, teamdelivered care was delivered using the minimum intervention oral care framework, implementing non-invasive and mini
