Heart rate and skin conductance in four-year-old children with aggressive behavior.

Autonomic underarousal, indicated by low heart rate (HR) and skin conductance level (SCL), is related to childhood aggression. However, results are inconsistent in preschoolers. We assessed HR, SCL, heart rate reactivity and skin conductance reactivity in four-year-old children. Comparisons were made between children with a high level and with a low level of aggressive behavior according to the Child Behavior Checklist 1 1/2-5 as well as between children who were diagnosed with Oppositional Defiant Disorder or Conduct Disorder (ODD/CD) and children with a low level of aggression. Preschool chi

Development and Examination of the Reactive Attachment Disorder and Disinhibited Social Engagement Disorder Assessment Interview.

The fifth edition of the Diagnostic and Statistical Manual (DSM) categorizes reactive attachment disorder (RAD) and disinhibited social engagement disorder (DSED) as two separate disorders, and their criteria are revised. For DSED, the core symptoms focus on abnormal social disinhibition, and symptoms regarding lack of selective attachment have been removed. The core symptoms of RAD are the absence of attachment behaviors and emotional dysregulation. In this study, an international team of researchers modified the Child and Adolescent Psychiatric Assessment for RAD to update it from DSM-IV to

Effect of Childhood Disease on Hospital Presentation: A Survey of Pediatricians.

BACKGROUND: Determining when caregivers should take their children to a hospital is crucial in ensuring the health and safety of children. Because children cannot make these decisions on their own, caregivers bear the core responsibility for the wellness of their children. The aim of this study was to determine how disease, disability, and child behavior affect when and how often caregivers take their children to a hospital. METHODS: A structured anonymous online survey was circulated to pediatricians in Japan. Pediatricians were queried about the characteristics of their patients, including

[Depressive reactions in children following the loss of a paternal figure, by death or divorce].

A review of different viewpoints on the theme "Death and the child" is made. When the child loses a loved object, he has to accept it in the outside world and manage the changes in his inner world. If the lost object is very needed, his ability to comprehend death is obstructed. When and how it is possible to help the child to understand death, is also examined. Depending of the age, development stage, previous relationships with the dead person, each child reacts differently. Effects of divorce on children are studied. A method that provides better avenues for ellaboration to children is pres

[DRESS syndrome and agranulocytosis, a rare combination].

INTRODUCTION: Drug reaction with eosinophilia and systemic symptoms (DRESS) is a severe toxidermia that can lead to death from multivisceral failure. We report a case of DRESS associated with febrile agranulocytosis in a child. OBSERVATION: An 8-year-old child was hospitalized for diffuse maculopapular exanthema with edema of the extremities and face associated with cheilitis and febrile agranulocytosis. This symptomatology began 1month after the introduction of carbamazepine for partial epilepsy. The clinical picture was a multisystemic disease with colitis, interstitial pneumonitis, hepatic

Maternal infection - but not inflammation - is associated with attention-deficit/hyperactivity disorder symptoms during childhood: a population-based cohort study.

BACKGROUND: Studies are equivocal on whether prenatal exposure to infection and inflammation is linked to higher risk of attention-deficit hyperactive disorder (ADHD) in the offspring. The current study investigated the relationship between prenatal exposure to maternal cytokines, C-reactive protein (CRP), fever and the risk of ADHD in childhood and early adolescence in the general pediatric population. METHOD: Data came from 6,555 mother-child pairs enrolled in Generation R, a Dutch prospective population-based cohort. The primary predictors were 1) self-reported fever and/or infections in e

Reactive Attachment Disorder and Disinhibited Social Engagement Disorder in School-Aged Foster Children--A Confirmatory Approach to Dimensional Measures.

This study aimed to investigate the factor structure and external correlates of the constructs Reactive Attachment Disorder (RAD) and Disinhibited Social Engagement Disorder (DSED) from the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). The following were addressed: First, do our data support the DSM-5 conceptualization of RAD/DSED as two separate constructs? Second, are RAD and DSED distinct from other well-established dimensions of child psychopathology? Third, what are the external correlates of RAD/DSED in this sample? The study sample included 122 foster children aged 6-10

A Neonate With Cytokine Storm Managed With Steroids, Therapeutic Plasma Exchange, and Tocilizumab.

Neonatal cytokine storms, though rare, can induce hyperinflammation due to elevated interleukin-6 (IL-6), triggering multiorgan failure. We present the case of a term male neonate necessitating extracorporeal membrane oxygenation (ECMO) post-birth for persistent pulmonary hypertension due to meconium aspiration syndrome. Three days after weaning from ECMO support, steroids and therapeutic plasma exchange were initiated due to deteriorating thrombocytopenia, oxygenation, hemodynamic instability, and increased C-reactive protein (CRP) and ferritin levels. Elevated IL-6 prompted tocilizumab admin

Attachment disorders diagnosed by community practitioners: a replication and extension.

BACKGROUND: While considered a rare diagnosis, reactive attachment disorder (RAD) is simultaneously the subject of considerable debate. A recent report suggested that RAD is overdiagnosed in community settings and that conduct problems may be used to make a diagnosis of RAD (Woolgar & Baldock, Child and Adolescent Mental Health, 20, 2015, 34-40). This study seeks to replicate and extend these findings. METHOD: Clinical assessment data from 100 consecutive admissions of maltreated foster and adopted children (ages 3-17) to a specialty treatment clinic in the United States were reviewed. Measur

Aggression, ADHD symptoms, and dysphoria in children and adolescents diagnosed with bipolar disorder and ADHD.

BACKGROUND: This study had two objectives: (1) examine characteristics of aggression in children and adolescents diagnosed with bipolar disorder and (2) determine whether the CBCL pediatric bipolar disorder profile differentiated youngsters with bipolar disorder from youngsters with ADHD. METHOD: Children and adolescents referred to a pediatric psychopharmacology clinic were systematically evaluated for psychopathology using a psychiatrist-administered diagnostic interview, parent- and teacher-report rating scales assessing the child's behavior, and child-completed self-report scales. Irritability and Emotion Dysregulation Predict Escitalopram Response in Adolescents with Generalized Anxiety Disorder: A Secondary Analysis of a Randomized, Double-Blind, Placebo-Controlled Trial.

OBJECTIVE: Irritability is increasingly recognized as an important aspect of emotion dysregulation in pediatric anxiety disorders. However, the relationship between irritability and treatment outcome in adolescents with generalized anxiety disorder (GAD) remains poorly understood. We examined whether irritability predicts clinical outcome and whether changes in irritability are associated with treatment-related improvement. METHODS: This secondary analysis utilized data from a randomized, double-blind, placebo-controlled trial of escitalopram in adolescents (12-17 years) with GAD (N = 51). Ir

Adjustment disorder in children and adolescents.

The literature on adjustment disorder in children and adolescents is reviewed to evaluate the empirical and conceptual basis of this disorder as defined in DSM-III-R, and to determine whether revisions, are indicated in DSM-IV. Existing studies suggest that adjustment disorder is a disorder of high prevalence in all settings, which carries significant morbidity and poor outcome in children and adolescents. Problems identified with the DSM-III-R definition include low reliability, the predominance of mixed rather than discrete symptom presentations in children and adolescents, and the persisten

Sex differences in the associations between vagal reactivity and oppositional defiant disorder symptoms.

BACKGROUND: Vagal reactivity to stress in children has been associated with future psychiatric outcomes. However, results have been mixed possibly because these effects are in opposite direction in boys and girls. These sex differences are relevant in the context of development of psychopathology, whereby the rates of psychiatric disorders differ by sex. In this study, we aimed to examine the association between vagal reactivity, assessed as a reduction in respiratory sinus arrhythmia (RSA) in response to a challenge, and the development of future oppositional defiant disorder (ODD) symptoms i

Unmasking masked depression in children and adolescents.

The authors examined depressive symptoms and behavior disorders in 102 systematically interviewed children aged 7 to 17 years to elucidate the category of masked depression. They found that it was possible to diagnose these children using adult research criteria and that more children with depression were identified using a systematic interview than were identified using standard evaluation procedures. Although children with a depressive disorder may also exhibit behavior disorders that overshadow the depression, an alert clinician conducting a thorough interview should be able to identify the

The interaction of BDNF Val66Met, PTSD, and child abuse on psychophysiological reactivity and HPA axis function in a sample of Gulf War Veterans.

INTRODUCTION: While the BDNF Val66Met polymorphism has been linked to various psychological disorders, limited focus has been on its relationship to posttraumatic stress disorder (PTSD) and early traumas such as child abuse. Therefore, we assessed whether Val66Met was associated with fear potentiated psychophysiological response and HPA axis dysfunction and whether PTSD status or child abuse history moderated these outcomes in a sample of Veterans. METHODS: 226 and 173 participants engaged in a fear potentiated acoustic startle paradigm and a dexamethasone suppression test (DST) respectively.

Electrocortical Reactivity to Emotional Faces in Youth of Depressed Mothers: The Moderating Role of Emotion Regulation Styles.

Children of depressed mothers are at significantly high risk (HR) for developing major depressive disorder (MDD) compared to their low risk (LR) counterparts. Evidence shows that HR youth exhibit a reduced late positive potential (LPP), an electroencephalogram (EEG) marker of emotional reactivity, in response to social-emotional stimuli. However, it remains unknown how emotion regulation (ER) styles may impact LPP responses in HR and LR offspring. The current study sought to examine the interplay of ER strategies (i.e., rumination, cognitive reappraisal, and suppression) and maternal history o

Trauma From the Eye of the Beholder: Reporter Discordance in Children's Trauma, Psychopathology, and Neurobiology.

BACKGROUND: Childhood trauma is a risk factor for adolescent psychopathology, including posttraumatic stress disorder (PTSD). Most research has relied on caregiver (i.e., parent or legal guardian) reports of child trauma. In this study, we investigated the impact of reporter (child vs. caregiver) in assessing neurobiological correlates of trauma and PTSD in children. METHODS: Two independent samples of youths (original sample: N = 76, 47% female, mean [SD] age = 9.39 [0.492] years; replication sample: N = 98, 51% female, mean [SD] age = 9.38 [0.089]) and their caregivers were interviewed rega

Reduced visual cortex grey matter volume in children and adolescents with reactive attachment disorder.

Child maltreatment increases the risk for psychiatric disorders throughout childhood and into adulthood. One negative outcome of child maltreatment can be a disorder of emotional functioning, reactive attachment disorder (RAD), where the child displays wary, watchful, and emotionally withdrawn behaviours. Despite its clinical importance, little is known about the potential neurobiological consequences of RAD. The aim of this study was to elucidate whether RAD was associated with alterations in grey matter volume (GMV). High-resolution magnetic resonance imaging datasets were obtained for child
