A case report of cholinesterase inhibitor poisoning: cholinesterase activities and analytical methods for diagnosis and clinical decision making.

Suicidal ingestion of organophosphorus (OP) or carbamate (CM) compounds challenges health care systems worldwide, particularly in Southeast Asia. The diagnosis and treatment of OP or CM poisoning is traditionally based on the clinical appearance of the typical cholinergic toxidrome, e.g. miosis, salivation and bradycardia. Yet, clinical signs might be inconclusive or even misleading. A current case report highlights the importance of enzymatic assays to provide rapid information and support clinicians in diagnosis and rational clinical decision making. Furthermore, the differentiation between

In silico Exploration of Bioactive Phytochemicals Against Neurodegenerative Diseases Via Inhibition of Cholinesterases.

Neurodegenerative disorders are estimated to become the second leading cause of death worldwide by 2040. Despite the widespread use of diverse allopathic drugs, these brain-associated disorders can only be partially addressed and long term treatment is often linked with dependency and other unwanted side effects. Nature, believed to be an arsenal of remedies for any illness, presents an interesting avenue for the development of novel neuroprotective agents. Interestingly, inhibition of cholinesterases, involved in the breakdown of acetylcholine in the synaptic cleft, has been proposed to be ne

Synthesis of Hantzsch Adducts as Cholinesterases and Calcium Flux inhibitors, Antioxidants and Neuroprotectives.

We report herein the design, synthesis, biological evaluation, and molecular modelling of new inhibitors of acetylcholinesterase (AChE) and butyrylcholinesterase (BuChE), able to block Ca+2 channels also showing antioxidant and neuroprotective activities. The new MTDL, dialkyl 2,6-dimethyl-4-(4-((5-aminoalkyl)oxy)phenyl)-1,4-dihydropyridine-3,5-dicarboxylate 3a-p, have been obtained via Hantzsch reaction from appropriate and commercially available precursors. Pertinent biological analysis has prompted us to identify MTDL 3h [dimethyl-4-(4-((5-(4-benzylpiperidin-1-yl)pentyl)oxy)phenyl)-2,6-dime

Effects of hycanthone and praziquantel on monoamine oxidase and cholinesterases in Schistosoma mansoni.

The effects of hycanthone and praziquantel on the activities of monoamine oxidases and cholinesterases were studied in the 600-g supernatant from homogenates of Schistosoma mansoni and mouse liver or brain. Hycanthone was shown to be a very potent inhibitor of monoamine oxidases from worms and mouse liver. Hycanthone also inhibited the specific and nonspecific cholinesterases of S. mansoni, but cholinesterase from mouse brain was not affected significantly by this drug. Praziquantel showed no effect on monoamine oxidase from mouse liver or the parasite; however, it was slightly inhibitory to S

Effect of metrifonate on blood cholinesterases in children during the treatment of schistosomiasis.

In a controlled clinical trial, Tanzanian schoolchildren with urinary schistosomiasis, many of whom had coexisting hookworm infection, were randomly allocated to one of three groups that were treated with doses of 7.5, 10.0, and 12.5 mg per kg of body weight, respectively, of metrifonate, orally, up to 3 times at 14-day intervals. No serious side effects were observed during or after the administration of single or repeated doses. A few hours after medication, plasma cholinesterase was almost completely inhibited, regardless of the dose given, while erythrocyte cholinesterase was almost comple

Proline-Based Carbamates as Cholinesterase Inhibitors.

Series of twenty-five benzyl (2S)-2-(arylcarbamoyl)pyrrolidine-1-carboxylates was prepared and completely characterized. All the compounds were tested for their in vitro ability to inhibit acetylcholinesterase (AChE) and butyrylcholinesterase (BChE), and the selectivity of compounds to individual cholinesterases was determined. Screening of the cytotoxicity of all the compounds was performed using a human monocytic leukaemia THP-1 cell line, and the compounds demonstrated insignificant toxicity. All the compounds showed rather moderate inhibitory effect against AChE; benzyl (2S)-2-[(2-chloroph

Initiation of cholinesterase inhibitors in an inpatient setting.

We examined initiation of cholinesterase inhibitors (ChEIs) to determine whether ChEIs were being newly prescribed without sufficient evaluation for dementia and/or delirium and to explore whether there are differences in outcomes, such as mortality, hospital readmission rates, and duration of hospitalization, between patients newly started on ChEI and those who continued such medications prior to admission. Patients hospitalized in fiscal year 2008 and prescribed ChEI were identified. We reviewed electronic medical records. Of 282 patients, 15.6% (44) were new-starts and 84.4% (238) were cont

Cholinesterase inhibitors for mild cognitive impairment.

BACKGROUND: Mild cognitive impairment is hypothesised to represent a pre-clinical stage of dementia but forms a heterogeneous group with variable prognosis. OBJECTIVES: To assess the safety and efficacy of cholinesterase inhibitors in people with mild cognitive impairment. SEARCH METHODS: Trials were identified from the Cochrane Dementia and Cognitive Improvement Group's Specialised Register, which is frequently updated from the major healthcare databases (MEDLINE, EMBASE, CINAHL, PsycINFO and Lilacs) as well as trial registers and grey literature. SELECTION CRITERIA: Double-blind, placebo-

Cholinesterase inhibitors: beyond Alzheimer’s disease.

Cholinesterase inhibitors (ChEIs) are widely licensed for the symptomatic treatment of Alzheimer’s disease, but their use has also been examined in a wide variety of neurological disorders besides Alzheimer’s disease, and this article reviews these uses. The evidence currently available suggests that ChEIs may possibly have a role in the treatment of some patients with dementia with Lewy bodies and Parkinson’s disease dementia, but at this point in time there would seem to be only a limited case for recommending ChEIs in mild cognitive impairment, Down syndrome, progressive supranuclear palsy,

Cholinesterase inhibitors stabilize Alzheimer's disease.

During the last decade, a systematic effort to develop a pharmacological treatment for Alzheimer disease (AD) has resulted into three drugs being registered for the first time in the USA and Europe for this specific indication. All three are cholinesterase inhibitors (ChEI). The major therapeutic effect of ChEI on AD patients is to maintain cognitive function at a constant level during a six-month to one-year period of treatment, as compared to placebo. Additional drug effects might slow cognitive deterioration and improve behavioral and daily living conditions. Comparison of clinical effects

Interaction of 2,4-diaminopyridine with cholinesterase inhibitors.

The interaction between 2,4 diaminopyridine (2,4-DAP) and neostigmine or edrophonium was studied in vivo in the rat sciatic nerve-tibialis anterior muscle preparation using the constant infusion technique of pancuronium. The ED50 value (dose of drug which produces 50% antagonism) decreased from 17 to 7 micrograms/kg with neostigmine and from 70 to 20 micrograms/kg with edrofonium, when these 2 compounds were co-administered with 40 micrograms/kg 2,4-DAP. The ED50 value of 2,4-DAP decreased from 160 to 90 micrograms/kg when 3.5 micrograms/kg neostigmine was added. From these experiments we conc

Brain cholinesterase activity in animals and birds.

Normal cholinesterase activity in brain tissue was measured in various mammalian and avain species. The cholinesterase activity in the cerebrum of cattle, swine, sheep and horses was approximately 2-3 umoles/min/g of tissue in each instance. The whole brain cholinesterase activity of small feral mammals was approximately 2 to 5 fold greater than the domestic animals. Considerable interspecies variability was present in the feral mammals. Similar variability was also observed in the avian brain cholinesterase determinations. The avian whole brain cholinesterase activities ranged from 9.78 to 21

Distribution of cholinesterase in canine venous system.

Cholinesterase (ChE) activity was examined in 15 veins from dogs by pharmacological and histochemical procedures. Potentiation of acetylcholine (ACh)-induced contractile response by neostigmine was observed in helical and longitudinal strips of portal, mesenteric veins and middle segment of the inferior vena cava, but not in the other 12 veins. Histochemical studies with an electron-microscope (the method of Karnovsky-Roots) revealed that ChE activity was identified as high density granules around the smooth muscle cells only in the former three veins. The visible ChE activity was abolished by
