Modified Blood Patch Used to Treat a High Output Chyle Leak After McKeown Esophagectomy.

Chyle leaks after esophagectomy are associated with significant morbidity and mortality. High-output fistulas are particularly difficult to manage, as the likelihood of spontaneous closure with conservative management is low. Leaks that fail to resolve with conservative management are referred for thoracic duct ligation or embolization. Some patients, however, are not candidates for these procedures or have persistent output despite intervention. We report a case of a post-McKeown esophagectomy patient with a high-output chyle leak despite intraoperative thoracic duct ligation. Treatment was s

Relationship Between Early Oral Intake Post Pancreaticoduodenectomy and Chyle Leakage: A Retrospective Cohort Study.

BACKGROUND: Early oral intake is strongly recommended according to the enhanced recovery after surgery (ERAS) guidelines because it can reduce complications and improve recovery. However, early oral intake has been indicated to be associated with chyle leakage (CL) after pancreatic surgery, which may lead to worsening of existing malnutrition and impeded recovery. This study investigated the relationship between early oral intake and CL and identified risk factors for CL to reduce its occurrence and promote recovery after pancreaticoduodenectomy. MATERIALS AND METHODS: All patients who underw

Finger-pressing: a simple and efficient way to stop chyle leak post neck dissection.

PURPOSE: Postoperative chyle leak (CL) is a rare but severe complication after neck dissection. Conservative first-line treatments, such as total parenteral nutrition (TPN), octreotide and pressure dressing, can significantly prolong the hospital stay. Efficient and well-tolerated conservative treatment options are still lacking. METHODS: We have developed a new maneuver to treat CL called "finger-pressing." A thumb is pressed against the point of leakage between the two muscle heads of the sternocleidomastoid. Finger-pressing continues for about 24 h until CL stops. We used this maneuver to

Operative management for refractory chyle leakage after pancreatectomy: A case report and literature review.

Post-pancreatectomy chyle leak is a common complication typically managed conservatively. However, rare ISGPS grade C leaks that produce high output can lead to significant morbidity and often resist non-operative treatments. We report two cases of such refractory leaks successfully addressed through surgical intervention. We describe two complex cases of postoperative chyle leakage. The first case involves a 66-year-old female who underwent pylorus-preserving pancreaticoduodenectomy with portal vein resection. The second case is a 67-year-old female who underwent completion total pancreatecto

Best evidence summary for nutritional management of cancer patients with chyle leaks following surgery.

BACKGROUND: Chyle leaks (CL) is a significant postoperative complication following lymph node dissection in cancer patients. Persistent CK is related to a series of adverse outcomes. Nutritional management is considered an effectively strategy that treat CL. However, the existing evidence on nutritional management for this patient cohort fails to provide actionable clinical guidance. AIM: This study was aimed to establish an evidence-based framework for nutritional management, offering reliable basis for clinical nursing practice. METHODS: Utilizing the "6S" mode, we conducted a systematic s

Fluorescence-guided surgery with indocyanine green to identify an idiopathic chyle leak-case report.

BACKGROUND: Chylothorax is the leakage of chyle into the pleural space and is associated with up to 50% morbidity. Although, the identification of traumatic chylothoraces is well described, non-traumatic chylothoraxes, mostly idiopathic, present therapeutic challenges as they are difficult to localize. We describe an attempt at localizing and treating an idiopathic chylothorax refractory to conservative and minimally invasive techniques. This was done using indocyanine green (ICG) and was a joint case between a thoracic surgeon and an interventional radiologist. CASE DESCRIPTION: A 50-year-ol

Chyle Leak after Right Axillary Lymph Node Dissection in a Patient with Breast Cancer.

BACKGROUND: A female patient was diagnosed with a right-sided chyle leak following right skin sparing mastectomy, axillary lymph node dissection, and immediate tissue expander placement in the setting of invasive ductal carcinoma status post neoadjuvant chemotherapy. Summary. Our patient underwent a level I and II right axillary lymph node dissection followed by an axillary drain placement. On the first postoperative day, a change from serosanguinous to milky fluid in this drain was noted. The patient was diagnosed with a chyle leak based on the milky appearance and elevated triglyceride level

Thoracoscopic thoracic duct ligation for persistent cervical chyle leak: utility of immediate pathologic confirmation.

OBJECTIVE: Chylous fistulas can occur after neck surgery. Both nonoperative measures and direct fistula ligation may lead to fistula resolution. However, a refractory fistula requires upstream thoracic duct ligation. This can be accomplished minimally invasively. Success depends on lymphatic flow interruption where the duct enters the thorax. We report on the utility of frozen section confirmation in achieving this goal. METHODS: Persistent chylous fistulas occurred in 2 patients after left cervical operations. In the first patient, attempted direct fistula ligation and sclerosant application

Nutritional Management in Patients with Chyle Leak following Head and Neck Surgery: A Systematic Review.

OBJECTIVE: We aim to investigate nutritional management of chyle leaks by a systematic review of the published literature. METHODS: We searched the following databases (Medline, Embase, Cochrane) for literature incorporating nutritional aspects in chyle leak management. We excluded non-English papers, paediatric studies, conference papers, and studies without nutritional intervention. RESULTS: Among 260 patients (out of 26,270), 81.5.% (n=212/260) achieved chyle leak resolution with nutritional management, while 18.5% (n=48/260) required surgery. A case report of an unusual presentation of chyle leak after repair of a perforated gastric ulcer.

INTRODUCTION AND IMPORTANCE: Chyle leak is an uncommon complication of abdominal surgeries. There are no reported cases of a chyle leakage following an omental patch repair for a perforated gastric ulcer before. A chyle leak from such a case however can successfully be treated conservatively. CASE PRESENTATION: We report an unusual case of a 57-year-old male presented with a perforated gastric ulcer who underwent a laparotomy and omental patch repair, which subsequently presented with a chyle leak from his abdominal wound. CLINICAL DISCUSSION: Post operative chyle leak usually occurs from th

The postoperative drain lymphocyte count as a novel predictor of chyle leakage after pancreatic surgery.

Chyle leakage is a recognized complication of pancreatic surgery. However, the risk factors for this condition have not been fully elucidated. This study aimed to evaluate whether the postoperative drain lymphocyte count can serve as a predictor of chyle leakage after pancreatic surgery. A total of 132 patients who underwent pancreatic resection, including pancreaticoduodenectomy, distal pancreatectomy, and total pancreatectomy, between November 2021 and March 2025 were retrospectively analyzed. Patient characteristics and perioperative factors, including drain lymphocyte count, were examined

A case of severe retroperitoneal chyle leakage and subsequent chylothorax secondary to partial laparoscopic nephrectomy.

Retroperitoneal chyle leakage combined with chylothorax after nephrectomy is rare in clinical practice. We report a 41-year-old female who developed severe chyloretroperitoneum along with subsequent chylothorax after partial nephrectomy. The patient underwent nutritional control, received octreotide treatment and had a complete recovery after 58 days. A damage to the cisterna chylic could account for the leakage. The chylothorax was a displacement through the diaphragmatic hiatus. Comprehensive review showed that near 30 % of such patients would require re-surgery or interventional treatment.

Chyle leak after major pancreatic surgery. Is this an underestimated complication? A narrative review of the literature.

PURPOSE: Postoperative chyle leak, termed 'chylous ascites', is a rare complication with a reported frequency of only one in 20464 abdominal operations. The purpose of this study was to summarize the available scientific data reviewing the most relevant studies for this type of postoperative complication after pancreatic surgery, highlighting at the same time the necessity for pancreatic surgeons to retain a high level of clinical suspicion for the early diagnosis and its therapeutic management. METHODS: A thorough literature search in Pubmed and Google Scholar, under the terms' chylous ascit

Chyle leak following total colectomy for ulcerative colitis: a case report and review of the literature.

Chyle leak is a rare complication in colorectal surgery. It occurs due to disruption of the lymphatic drainage network in the abdomen or retroperitoneum. We describe the first reported case of chyle leak following total colectomy for inflammatory bowel disease. Our patient underwent total colectomy for severe ulcerative colitis not responsive to medical treatment. Four days postoperatively, a milky fluid was noted in the drainage bag. Analysis of the fluid confirmed chyle. The patient remained well and was successfully managed conservatively with a fat-free elemental diet and was discharged fr

Chyle leakage after axillary node sampling in a patient with breast cancer: a case report.

BACKGROUND: Chyle leakage is a well-known complication after thoracic surgery, such as esophagectomy, cardiac surgery, mediastinal lymph node dissection, and neck surgery. However, chyle leakage is a rare complication after dissections of the lateral or subclavian axillary nodes for breast surgery. It is particularly unusual for chyle leakage to occur after minimally invasive dissection of the axillary nodes. Most cases of chyle leakage subside with conservative management, but some cases require surgery. CASE REPORT: An 80-year-old woman had invasive lobular cancer of the left breast (cT1 [1
