Patient-Reported Experience and Preferences with Long-Axial-Field-of-View and Total-Body PET/CT: A Prospective, Multicenter Cross-Sectional Survey.

Total-body and long-axial-field-of-view PET/CT scanners differ in geometry, scan capability, and access requirements. Understanding patient preferences is essential for protocol design and service planning. Methods: Patients undergoing PET/CT at 3 centers across 3 scanner types completed a structured survey covering scanner perception, physical comfort, claustrophobia, scan duration tolerance, radiation dose and image quality preferences, and travel/access priorities. Group differences were assessed using the Fisher-Freeman-Halton exact test with Cramér φ for effect size. Results: In total, 22

Using ambient audiovisual experiences to reduce the need for sedation in claustrophobic MRI patients.

INTRODUCTION: Claustrophobia is a major cause of discontinued magnetic resonance imaging (MRI) procedures and the use of sedation for MRIs incurs significant costs to both the hospital and the patient. This service evaluation assesses the effectiveness of an audiovisual ambient experience (AE) system in reducing the need for sedation for claustrophobic MRI patients in a hospital. METHODS: This is a service evaluation that was conducted in two phases: 1. A comparison was made of the claustrophobia-related MRI discontinuation rates among five outpatient scanners (one AE scanner and four non-AE

Reduction of motion sickness in prehospital trauma care.

Motion sickness adds to the discomfort of many patients being transported by ambulance. Recent research has demonstrated the effectiveness of oxygen therapy in reducing motion sickness during transport to hospital. However, patients reported negative reactions to wearing a facemask, which produced feelings of claustrophobia and anxiety. We therefore tested the hypothesis that supplemental oxygen inhaled from a new oxygen delivery device OxyArm, which avoids direct contact with the patient's skin, may reduce the incidence of motion sickness and increase patient satisfaction. Forty patients suff

Patient preferences for development in MRI scanner design: a survey of claustrophobic patients in a randomized study.

OBJECTIVE: To investigate which magnetic resonance imaging (MRI) scanner designs claustrophobic patients prefer. MATERIAL/METHODS: We analyzed questionnaires completed by 160 patients at high risk for claustrophobia directly after a scan in either a short-bore or open panoramic scanner as part of a prospective randomized trial Enders et al (BMC Med Imaging 11:4, 2011). Scanner preferences were judged based on schematic drawings of four scanners. Information on the diagnostic performance of the depicted scanners was provided, too. RESULTS: A majority of patients suggested upright open (59/160

Use of a 1.0 Tesla open scanner for evaluation of pediatric and congenital heart disease: a retrospective cohort study.

BACKGROUND: Open cardiovascular magnetic resonance (CMR) scanners offer the potential for imaging patients with claustrophobia or large body size, but at a lower 1.0 Tesla magnetic field. This study aimed to evaluate the efficacy of open CMR for evaluation of pediatric and congenital heart disease. METHODS: This retrospective, cross-sectional study included all patients ≤18 years old or with congenital heart disease who underwent CMR on an open 1.0 Tesla scanner at two centers from 2012-2014. Indications for CMR and clinical questions were extracted from the medical record. Studies were quali

Sedation for magnetic resonance imaging in the prone position - A report of four cases.

BACKGROUND: Magnetic resonance imaging (MRI) is a useful tool, but it can be difficult to perform in those with claustrophobia as it requires being enclosed in a noisy cylindrical space. Being in the prone position is essential to spread breast tissue. However, sedation in a prone position is challenging because of the possibility of respiratory depression and the difficulty in manipulating the airway. CASE: Four patients with claustrophobia were sedated using dexmedetomidine, has minimal effect on respiration. Dexmedetomidine also enables the patient's cooperation in assuming the prone posit

Determining the efficacy of low-dose oral benzodiazepine administration and use of wide-bore magnet in assisting claustrophobic patients to undergo MRI brain examination.

PURPOSE: Claustrophobia remains a challenging barrier for a significant number of patients to successfully complete a Magnetic Resonance Imaging (MRI) examination. While use of wide-bore machines and pre-exam administration of a low-dose benzodiazepine are commonly employed, there is little published research to determine which modality is the most efficacious based on the patient's specific degree of claustrophobia. This retrospective case-control study examines the efficacy of using a low-dose oral benzodiazepine and wide-bore magnet to successfully aid the claustrophobic patient in completi

'The dungeon of thyself': the claustrum as pathological container.

The author discusses the claustrum as an aspect of pathological containment within inner space and its relation to Bion's (1962a, b) container-contained concept. Having outlined the early psychoanalytic conceptual foundations of claustrophobia and the claustrum, the author charts the term from its introduction by Erikson in 1937 through its divergent developmental trajectories within the conceptual vocabulary of the classical, Independent and Kleinian schools up to Meltzer's (1992) contemporary reworking. The vicissitudes of the transmission of ideas within and between these groups is stressed

Wide, short bore magnetic resonance at 1.5 t: reducing the failure rate in claustrophobic patients.

PURPOSE: Claustrophobic reactions in patients undergoing magnetic resonance imaging (MRI) have a significant impact on the workflow, patient acceptance and ultimately the costs involved in obtaining a diagnostic scan. The purpose of this study was to determine if the use of a wide, short bore MRI scanner could reduce the need for general anesthesia assistance in these cases. METHODS: Between September 2006 and March 2008, all patients for whom MRI examinations of the head and/or spine were canceled or prematurely terminated due to claustrophobia on a standard 60 cm bore, 1.5 T scanner were sc

A machine learning approach for detection of claustrophobic brain activity in electroencephalography.

Claustrophobia, a phobia with a specific unreasonable and excessive fear of enclosed spaces, can have a considerable impact on an individual's life. Electroencephalography (EEG) has been a tool with potential for studying neural processes in anxiety disorders including claustrophobia. In this work, a machine learning algorithm for differentiation between claustrophobic and healthy controls using EEG signals is presented. EEG data were collected from 22 participants under controlled conditions, and preprocessing included filtering, artifact removal, and feature extraction using relative Power S

Evaluation of audiovisual and auditory interventions on exam anxiety during whole-body magnetic resonance imaging in cancer patients: a quasi-experimental study.

BACKGROUND: WB-MRI is a radiation-free imaging technique widely used in oncology. Despite clinical advantages, WB-MRI can cause significant patient anxiety and claustrophobia, affecting scan completion and patient adherence. Non-invasive interventions to reduce MRI-related distress are still limited in clinical practice. This study evaluates the use of audiovisual and auditory entertainment for reducing anxiety during WB-MRI. METHODS: In this prospective, quasi-experimental trial, 213 cancer patients (median age: 52.0[44-60]; female 70%) undergoing WB-MRI were equally assigned to Video (natur

Does bore size matter?-A comparison of the subjective perception of patient comfort during low field (0.55 Tesla) and standard (1.5 Tesla) MRI imaging.

The purpose of the present study was to evaluate the subjectively perceived patient comfort during magnetic resonance imaging (MRI) examinations and to assess potential differences between a recently introduced low field MRI scanner and a standard MRI scanner. Among other characteristics, the low field MRI scanner differs from the standard MRI scanner by offering more space (wider bore size of 80 centimeter diameter) and producing less noise, which may influence the patient comfort. In total, 177 patients were surveyed after MRI scans with either the low field MRI scanner (n = 91, MAGNETOM Fre

Early Palliative Care in Heart Transplant Evaluation.

Palliative care (PC) is becoming increasingly integrated into the care of patients with advanced heart failure, although it is not yet widely incorporated into the heart transplant evaluation process. Patients undergoing evaluation for heart transplant carry a high burden of mental health morbidity, which can affect their ability to complete the evaluation and undergo successful transplantation. We describe the case of a 67-year-old male with ischemic cardiomyopathy on continuous inotropic support for decompensated heart failure, who initially declined to complete a transplant evaluation and e

Pelvimetry and patient acceptability compared between open 0.5-T and closed 1.5-T MR systems.

Our objective was to compare maternal pelvimetry and patient acceptability between open low-field (0.5-T) and closed 1.5-T MR systems. Thirty women referred for pelvimetry (pregnant: n=15) were scanned twice in the supine position, once in the vertical open system and once in the closed system. Each patient completed a comfort and acceptability questionnaire. Pelvimetric and questionnaire data were compared between systems. Total scan time was double in the open system (7:52+/-1:47 vs 3:12+/-1:20 min). Poor image quality in the open system prevented assessment of interspinous and intertuberous
