Reducing False Arrhythmia Alarms Using Different Methods of Probability and Class Assignment in Random Forest Learning Methods.

The literature indicates that 90% of clinical alarms in intensive care units might be false. This high percentage negatively impacts both patients and clinical staff. In patients, false alarms significantly increase stress levels, which is especially dangerous for cardiac patients. In clinical staff, alarm overload might lead to desensitization and could result in true alarms being ignored. In this work, we applied the random forest method to reduce false arrhythmia alarms and specifically explored different methods of probability and class assignment, as these affect the classification accura

[Reducing the Invalid Alarm Rate for Fetal Heartbeat Monitoring in High-Risk Pregnancies in the Delivery Room].

BACKGROUND & PROBLEMS: Fetal heartbeat (FHB) monitoring is used to monitor the condition of a fetus in the uterus and estimate the expected time of delivery. It is crucial to accurately and comprehensively monitor fetal heart beats in high-risk pregnancies. However, invalid alarms frequently occur in clinical environments, easily promoting alarm fatigue among nurses and causing anxiety and distrust in the laboring women and their families. Health professionals may ignore valid alarms due to alarm fatigue and miss the opportunity to save a fetus' life. A survey in our hospital identified invali

A Case of Hydroxocobalamin-Induced False Blood Leak Alarm on Dialysis Machine.

Hemodialysis machines are equipped with a blood leak detector/alarm to prevent loss of blood following rupture of semipermeable membrane; the blood leak alarms could also be triggered by sensor malfunction or presence of air bubbles in the system. Hydroxocobalamin is a Food and Drug Administration-approved rapid-acting antidote to cyanide poisoning that converts cyanide to nontoxic cyanocobalamin. Side effects are reddish discoloration of skin and body fluids, urticarial rash, and rarely anaphylaxis. In this article, a case of false blood leak alarm following treatment of cyanide poisoning wit

A Frontal Account of False Alarms.

Prior research has demonstrated that the frontal lobes play a critical role in the top-down control of behavior, and damage to the frontal cortex impairs performance on tasks that require executive control [Burgess, P. Associations between the self-reported frequency of hearing chemical alarms in theater and regional brain volume in Gulf War Veterans.

BACKGROUND: We previously reported evidence of reduced cortical gray matter (GM), white matter (WM), and hippocampal volume in Gulf War (GW) veterans with predicted exposure to low-levels of nerve agent according to the 2000 Khamisiyah plume model analysis. Because there is suggestive evidence that other nerve agent exposures may have occurred during the Gulf War, we examined the association between the self-reported frequency of hearing chemical alarms sound during deployment in the Gulf War and regional brain volume in GW veterans. METHODS: Ninety consecutive GW veterans (15 female, mean ag

Impact of increasing current drain in a pacemaker operating during activation of the elective replacement indicator.

The occurrence of a 13.8 s episode of ventricular asystole in a patient whose VVIR pacemaker displayed the elective replacement indicator (ERI) is reported. Increasing battery current drain by VARIO testing and programming of the emergency VVI mode markedly increased battery current drain with a resultant decrease in the battery voltage below the pacing threshold. The prolonged lack of capture occurred because the lowered battery voltage could not return instantaneously to its previous level after the demand for a higher battery current drain had ceased. Rather, the battery voltage increased p

The application value of intraoperative neurophysiological monitoring in cervical spinal canal stenosis decompression surgery.

BACKGROUND CONTEXT: Although intraoperative neurophysiological monitoring (IONM) has been widely recognized and used in spine surgery, its characteristics vary for different types of spinal disorders, necessitating the development of tailored monitoring strategies. Cervical spinal stenosis presents complex clinical symptoms and carries significant surgical risks, creating a critical need to clarify the monitoring features, alert patterns, and their relationship with outcomes in such surgeries. A comprehensive assessment and the development of a refined IONM monitoring plan throughout the perio

Very long apnea events in preterm infants.

Apnea is nearly universal among very low birth weight (VLBW) infants, and the associated bradycardia and desaturation may have detrimental consequences. We describe here very long (>60 s) central apnea events (VLAs) with bradycardia and desaturation, discovered using a computerized detection system applied to our database of over 100 infant years of electronic signals. Eighty-six VLAs occurred in 29 out of 335 VLBW infants. Eighteen of the 29 infants had a clinical event or condition possibly related to the VLA. Most VLAs occurred while infants were on nasal continuous positive airway pressure

Digitalizing Ventilator Safety: Enhancing Workflow Efficiency and Competency in Critical Care.

BACKGROUND: Noninvasive ventilation is increasingly delivered outside intensive care units; however, ventilator alarm management, documentation, and competency validation remain inconsistent when paper-based processes are used. Delayed or inconsistent responses to actionable alarms may contribute to clinical deterioration and escalation of care. LOCAL PROBLEM: Nurses and respiratory therapists reported fragmented documentation, limited access to standardized troubleshooting guides, and time-consuming competency tracking for noninvasive ventilation devices in acute medical units that provide i

Representation Learning Approaches to Detect False Arrhythmia Alarms from ECG Dynamics.

The high rate of intensive care unit false arrhythmia alarms can lead to disruption of care and slow response time due to desensitization of clinical staff. We study the use of machine learning models to detect false ventricular tachycardia (v-tach) alarms using ECG waveform recordings. We propose using a Supervised Denoising Autoencoder (SDAE) to detect false alarms using a low-dimensional representation of ECG dynamics learned by minimizing a combined reconstruction and classification loss. We evaluate our algorithms on the PhysioNet Challenge 2015 dataset, containing over 500 records (over

Effects of the Visual Patient Avatar on vital sign deviations and audible alarm burden in perioperative care: a dual-centre, quasi-experimental pre-post large-scale observational study protocol (NewYork-Presbyterian/Weill Cornell and University Hospital Zurich).

INTRODUCTION: Perioperative monitoring requires clinicians to integrate multiple physiological data streams while managing interruptions and audible alarms. The Visual Patient Avatar (VPA) is an avatar-based monitor visualisation that integrates vital signs and sensor states into an animated virtual patient. This protocol evaluates whether VPA implementation is associated with objective changes in monitoring and alarm outcomes during routine perioperative care. METHODS AND ANALYSIS: This investigator-initiated, dual-centre, quasi-experimental pre-post observational study uses routine perioper

Impact of Noise Levels of the Dialysis Machine Alarm on Patient Anxiety.

INTRODUCTION AND BACKGROUND: Hemodialysis machine alarms are warning noises, which are essential for patient safety, but the psychological impact of these alarms is often overlooked. This study aims to assess the relationship between hemodialysis machine alarm noise levels and anxiety among patients undergoing thrice weekly hemodialysis. METHODS: This observational, cross-sectional, analytical study was conducted on 53 participants who were undergoing hemodialysis. A Benetech GM1351 digital sound-level meter was used to measure the noise level of hemodialysis machine alarms. Socio-demographic

Mechanistic insights into alarm therapy for nocturnal enuresis: a retrospective cohort study.

PURPOSE: The gold standard treatment for nocturnal enuresis includes alarm treatment, although the exact mechanism behind its effectiveness remains uncertain. The aim of this study was to investigate whether the time to alarm activation progressively increases during treatment, and to explore whether alarm treatment causes a transition from enuresis episodes to nocturia. METHODS: We included children with nocturnal enuresis undergoing successful alarm treatment from 01.01.2016 - 10.01.2023 at Aalborg University Hospital, Denmark. Data, including alarm times and episodes of nocturia, was obtai

Heightened false alarms of conditioned threat predict longitudinal increases in GAD and SAD symptoms over the first year of college.

Lab-based fear-conditioning studies have repeatedly implicated exaggerated threat reactivity to benign (unreinforced) stimuli as concurrent markers of clinical anxiety, but little work has examined the strength of false alarms as a longitudinal predictor of anxiety problems. As such, we tested whether heightened false alarms of conditioned threat assessed in participants' first semester of college predicted second-semester symptoms of generalized anxiety disorder (GAD) and social anxiety disorder (SAD) - two anxiety conditions that are common in college students, have been associated with exce

Improved pointer in auditory alarms enhances response accuracy.

BACKGROUND: Auditory alarms are crucial in clinical settings, alerting clinicians to events requiring immediate attention. However, multitasking can lead to missed alarms and disrupt patient care. Enhancing auditory alarms can improve patient safety and clinician satisfaction. METHODS: In a controlled laboratory study, we recruited 26 clinicians (residents, fellows, advanced practice providers) and 19 non-clinicians (undergraduate students) to compare our previously validated alarm with an improved design. The improved alarm incorporates a 'pointer' (a short sound burst indicating acuity leve
