[Clinical reasoning and decision-making in practice. A woman with hypokalaemia].

A 39-year-old woman had been investigated elsewhere due to symptomatic hypokalaemia, renal potassium wasting and metabolic alkalosis. Vomiting was considered to be the underlying cause but the patient repeatedly denied this behaviour. After extensive investigations, Bartter's syndrome was finally diagnosed. Eighteen months later the patient was readmitted due to progressive renal insufficiency. On the basis of a very low urinary chloride excretion and the aforementioned laboratory results, it was concluded that the metabolic disturbances were due to vomiting and the diagnosis of Bartter's synd

A Windows-based tool for the study of clinical decision-making.

Studies of health-provider decision-making, and of their practice patterns, play a central role in efforts to improve the quality and effectiveness of care and in decreasing costs of healthcare delivery systems. Researchers from a variety of disciplines have studied a broad range of clinical conditions, using a number of methodological approaches and measurement tools, including self-report, written clinical vignettes, simulated clinical encounters using actors as patients and analysis of medical records and administrative data. Although these provide information about the outcomes of clinical

Clinical decision making in the management of the late sequelae of poliomyelitis.

Years after the initial onset of poliomyelitis, patients often report new problems, including fatigue, weakness, pain, breathing difficulty, decreased endurance, problems with swallowing, choking, increased sensitivity to cold, and psychological problems. The premise of this article is that underlying pathophysiology of these problems can be assessed on the basis of a detailed history, a multisystem physical examination, and supplemental information provided by investigative tests. The indications for treatment of the late sequelae of poliomyelitis based on the underlying pathophysiology and t

Decision making with clinical diagnostic tests in terms of statistical decision theory and information theory.

The value of a diagnostic test for an individual patient is expressed in terms of the test's ability to determine whether the patient does or does not have a disease. For diagnostic tests with a continuous diagnostic variable, various decision thresholds can be selected to adjust the sensitivity and specificity of the diagnostic test to conform with the clinician's concerns (e.g., health costs, financial costs, and diagnostic information of the test). Three methods of statistical decision theory and information theory are presented for critically evaluating the value of a clinical diagnostic t

Care-pathway decision support improves non-specialist vet clinical decision making in dogs with myelopathy.

BACKGROUND: Non-specialist vets are often required to make decisions in the absence of specialist knowledge and support. Care pathways (CPs) use knowledge engineering to convert expert knowledge into a computer-interpretable format, delivered to clinicians at the point-of-care via patient-specific clinical decision recommendations. HYPOTHESIS/OBJECTIVES: Non-specialist vets make diagnostic and management decisions that are more consistent with specialists when they have access to CP decision support. ANIMALS: Clinical information from 8 historical cases of dogs with myelopathy presenting to

A clinical decision support system for AI-assisted decision-making in response-adaptive radiotherapy (ARCliDS).

Involvement of many variables, uncertainty in treatment response, and inter-patient heterogeneity challenge objective decision-making in dynamic treatment regime (DTR) in oncology. Advanced machine learning analytics in conjunction with information-rich dense multi-omics data have the ability to overcome such challenges. We have developed a comprehensive artificial intelligence (AI)-based optimal decision-making framework for assisting oncologists in DTR. In this work, we demonstrate the proposed framework to Knowledge Based Response-Adaptive Radiotherapy (KBR-ART) applications by developing a

Transient ischaemic attack clinic: an evaluation of diagnoses and clinical decision making.

The diagnosis of transient ischaemic attack (TIA) is based largely on the patient's symptom recall and clinical judgement. This decision-making process is highly subjective and the inter-observer reliability of TIA diagnosis is at best moderate, even among neurologists. The aim of this study is to examine the presenting features and final diagnoses of referrals to a TIA clinic and to evaluate characteristics that favoured the diagnosis of TIA over other TIA "mimics". Consecutive new referrals to a tertiary metropolitan hospital TIA clinic over a 9month period were examined. Characteristics bet

[Integration of clinical, biological and imaging data in medical decision-making].

This issue of the Revue Médicale de Liège brings together clinical observations, therapeutic reviews and public health studies. They share a common message: medical decision-making relies on the integration of clinical, biological and imaging data, weighed against scientific evidence and each patient's context.

Masterclass: Running-related injuries - bringing evidence together for clinical decision-making.

Running is one of the most accessible and widely practised forms of physical activity promoting multiple health benefits. However, running-related injuries (RRI) are common. In novice runners, the incidence is estimated as 17.8 (95% CI 16.7 to 19.1) injuries per 1000 running hours. In contrast, in recreational runners it is approximately 7.7 (95% CI 6.9 to 8.7) injuries per 1000 running hours. RRIs have significant individual, physical, social, and economic consequences, including moderate pain, withdrawal from running training, increased use of medical resources (e.g., healthcare consultation

Four clinical dilemmas in surgical decision making for low rectal cancer.

Advances in neoadjuvant therapy, imaging, and surgical techniques over the past three decades have significantly transformed the management of low rectal cancer. However, in this anatomically and functionally sensitive region, where decisions intersect with continence, identity, and quality of life, strict adherence to standard protocols is often inadequate. This article explores four recurring dilemmas that influence contemporary surgical decision-making: whether to preserve the sphincter or perform an abdominoperineal resection, whether to create a stoma or avoid it, whether to use minimally

Hidradenitis Suppurativa from Preconception to Lactation: Clinical Treatment Decision-Making Review.

Hidradenitis suppurativa is a chronic, inflammatory skin disease that predominantly affects women of reproductive age and may require continuous therapeutic control during pregnancy and lactation. This review summarizes current evidence on the safety and practical use of topical, systemic, biologic, metabolic, and procedural therapies for HS throughout the preconception, gestational, peripartum, and breastfeeding periods. Topical clindamycin, benzoyl peroxide, chlorhexidine, and selected beta-lactam antibiotics appear to represent preferred options for mild disease or acute flares, whereas tet

Biomarker-guided decision making in clinical drug development for neurodegenerative disorders.

Neurodegenerative disorders are characterized by complex neurobiological changes that are reflected in biomarker alterations detectable in blood, cerebrospinal fluid (CSF) and with brain imaging. As accessible proxies for processes that are difficult to measure, biomarkers are tools that hold increasingly important roles in drug development and clinical trial decision making. In the past few years, biomarkers have been the basis for accelerated approval of new therapies for Alzheimer disease and amyotrophic lateral sclerosis as surrogate end points reasonably likely to predict clinical benefit

Remimazolam in pediatric anesthesia: a systematic review for clinical decision-making.

BACKGROUND: Remimazolam's role in pediatric anesthesia is evolving. We systematically reviewed 2024-2025 evidence to establish a clinical decision-making framework for its use. METHODS: Following PRISMA guidelines, a systematic search identified 23 studies (15 RCTs) involving 2,847 pediatric patients for narrative synthesis. RESULTS: Remimazolam demonstrated superior hemodynamic stability vs. propofol (cardiovascular complications: RR 0.30, 95% CI 0.20-0.46) and reduced emergence delirium by 61% (RR 0.39, 95% CI 0.21-0.70). Autoimmune neuropathies: New guidelines and expert insights into clinical decision making.

INTRODUCTION: Guillain-Barré syndrome (GBS) and chronic demyelinating polyneuropathy (CIDP) are immune-mediated demyelinating neuropathies that may have similar symptoms in their early stages. GBS is generally monophasic with acute, progressive, flaccid paralysis of the extremities accompanied by reduced/absent reflexes. The onset of GBS is often 1-2 weeks after a precipitating event and reaches its nadir by 4 weeks. With CIDP the progressive phase by definition lasts more than 2 months, although it can be shorter in acute-onset forms which initially resemble GBS. AREAS COVERED: In this revie

Decision making in clinical trials: Interim analyses, innovative design, and biomarkers.

The efficient and accurate execution of clinical trials testing novel treatments for Alzheimer's disease (AD) is a critical component of the field's collective efforts to develop effective disease-modifying treatments for AD. The lengthy and heterogeneous nature of clinical progression in AD contributes to the challenges inherent in demonstrating a clinically meaningful benefit of any potential new AD therapy. The failure of many large and expensive clinical trials to date has prompted a focus on optimizing all aspects of decision making, to not only expedite the development of new treatments,

Restorative dentistry clinical decision-making for hypodontia: managing missing premolar teeth.

Hypodontia is a relatively common clinical condition and the second premolar tooth is the most common tooth that fails to develop (excluding third molars). For some patients, no treatment is required, as there is little, if any, consequence of the condition. For other patients, the missing teeth are part of a more complex dental presentation, requiring specialist dental management. This paper describes the clinical decision-making process and explores the options for managing this presentation of hypodontia.

Understanding clinical decision-making in mesothelioma care: a mixed methods study.

INTRODUCTION: Malignant pleural mesothelioma is a rare, incurable cancer arising from previous asbestos exposure; patients have a poor prognosis, with a median survival rate of 8-14 months. Variation in mesothelioma clinical decision-making remains common with a lack of multidisciplinary knowledge sharing, leading to inconsistencies in treatment decisions. The study aimed to explore which factors impacted on clinicians' decision-making in mesothelioma care, with a view to optimising the mesothelioma care pathway. METHODS: This mixed methods study consisted of documentary analysis of local and

Three-dimensional Surface Imaging for Clinical Decision Making in Pectus Excavatum.

To evaluate pectus excavatum, 3-dimensional surface imaging is a promising radiation-free alternative to computed tomography and plain radiographs. Given that 3-dimensional images concern the external surface, the conventional Haller index, and correction index are not applicable as these are based on internal diameters. Therefore, external equivalents have been introduced for 3-dimensional images. However, cut-off values to help determine surgical candidacy using external indices are lacking. A prospective cohort study was conducted. Consecutive patients referred for suspected pectus excavatu
