Treatment Recommendations for Clinical Deterioration on the Wards: Development and Validation of Machine Learning Models.

BACKGROUND: Clinical deterioration in general ward patients is associated with increased morbidity and mortality. Early and appropriate treatments can improve outcomes for such patients. While machine learning (ML) tools have proven successful in the early identification of clinical deterioration risk, little work has explored their effectiveness in providing data-driven treatment recommendations to clinicians for high-risk patients. OBJECTIVE: This study established ML performance benchmarks for predicting the need for 10 common clinical deterioration interventions. This study also compared

A clinical practice review: augmentation of meniscoligamentous structures for primary prevention of knee joint deterioration.

Meniscus and ligament injuries of the knee can have significant implications on both short-term activity level as well as long-term knee health. While great advancements in technique and treatment strategy for these injuries have been made in recent years, clinical failure and progressive joint deterioration remain significant concerns. At the same time, technical advancements are being made throughout orthopedics, especially in the field of sports medicine. Many of these developing technologies focus on augmenting stability in the early postoperative period while optimizing biology, with the

Comparison of Deep Learning Approaches Using Chest Radiographs for Predicting Clinical Deterioration: Retrospective Observational Study.

BACKGROUND: The early detection of clinical deterioration and timely intervention for hospitalized patients can improve patient outcomes. The currently existing early warning systems rely on variables from structured data, such as vital signs and laboratory values, and do not incorporate other potentially predictive data modalities. Because respiratory failure is a common cause of deterioration, chest radiographs are often acquired in patients with clinical deterioration, which may be informative for predicting their risk of intensive care unit (ICU) transfer. OBJECTIVE: This study aimed to c

Frontline Nurses' clinical judgment in recognizing, understanding, and responding to patient deterioration: A qualitative study.

BACKGROUND: Early warning systems and rapid response teams have been widely implemented in hospitals worldwide to facilitate early recognition and response to patient deterioration. Unfortunately, evidence suggests that these interventions have made little impact on unexpected cardiac or respiratory arrest, hospital mortality, unplanned admission to intensive care units, or hospital length of stay. These programs depend on nurses recognizing at risk patients and initiating a timely response. Although physiologic abnormalities commonly precede serious adverse events, nurses often fail to recogn

Development and usability testing of a machine learning clinical decision support tool for suggesting etiologies and treatment of clinical deterioration.

BACKGROUND: Early warning score clinical decision support (CDS) systems identify patients at risk of clinical deterioration, but do not provide insights on the underlying cause. Machine learning (ML) models may address this limitation, but how to convey such outputs to clinicians remains unclear. This study elicited clinicians' design requirements and assessed their impact on ML-CDS usability in a controlled lab setting. METHODS: This sequential exploratory mixed-methods approach began with focus groups to obtain design requirements for the ML-CDS interface. After revising the prototype, we c

Clinical features of patients with Legionnaires disease showing initial clinical improvement but radiological deterioration: A retrospective single-center analysis.

Patients with Legionnaires disease occasionally experience initial clinical improvement but radiological progression. However, data on this issue are so far limited. The aim of this study was to investigate changes in chest radiograph findings in patients with Legionnaires disease who showed initial clinical improvement and to identify risk factors and outcomes in these patients. All patients diagnosed with Legionnaires disease at a tertiary hospital in South Korea between March 2011 and May 2020 were retrospectively enrolled. Legionnaires disease was defined as abnormal chest radiographs accom

Pulmonary Restriction in Patients With a Fontan Circulation: Determinants, Deterioration Over Time, and Clinical Implications.

PURPOSE: Pulmonary restriction is common in patients with a Fontan circulation and is associated with impaired quality of life. Studies often fail to accurately define pulmonary restriction, and few have investigated its predictors, longitudinal course, and clinical implication. We investigate the prevalence and determinants of pulmonary restriction in patients with a Fontan circulation, the evolution of pulmonary function over time, and the relationship between pulmonary function and exercise capacity. METHODS: This longitudinal study included 85 patients with a Fontan circulation who underw

Neurological deterioration in cauda equina syndrome is probably progressive and continuous. Implications for clinical management.

Fifty-six human and animal studies of cauda equina syndrome (CES) were reviewed. The evidence from human studies was poor (level IV). Evidence from animal studies and limited evidence from human studies suggest that structural and functional neurological losses are a progressive, continuous process. The longer the cauda equina nerve roots are compressed the greater the harm and the poorer the extent of recovery. This should prompt diagnosis and surgery for all CES patients as soon as practicably possible.

Progressive deterioration of brainstem auditory evoked potentials in Creutzfeldt-Jakob disease: clinical and electroencephalographic correlation.

Brainstem auditory evoked potentials (BAEP) were recorded and correlated with the clinical and electroencephalographic (EEG) findings in three verified cases of Creutzfeldt-Jakob disease (CJD). In contrast to previous reports, there was progressive deterioration of the BAEPs, through initial asymmetry of wave latencies to abnormal wave morphology, and increased interpeak latencies. Consequently, in two of the patients, disorganization of the wave pattern and generalized conduction-time slowing appeared by the time there was a fully developed clinical picture of CJD with periodic EEG waves. Serum Procalcitonin as an Indicator of Disease Severity and Clinical Deterioration in Hospitalized Patients With Acute Cholangitis.

BACKGROUND: Acute cholangitis (AC) results from bile stasis and subsequent infection in the biliary tract, requiring urgent evaluation to determine the timing of biliary decompression. Procalcitonin (PCT) and lactic acid (LAC) may be reliable indicators of disease severity to suggest early biliary decompression. We seek to assess the utility of PCT and LAC to predict severe AC and adverse in-hospital outcomes such as ICU transfer, vasopressor use, or mortality. METHODS: This is a retrospective observational study of patients admitted with AC to a tertiary hospital in the United States between

Deterioration of long-term clinical results after the Elmslie-Trillat procedure for dislocation of the patella.

We examined 39 patients (45 knees) who had undergone an Elmslie-Trillat procedure for recurrent or habitual dislocation of the patella with a follow-up of more than ten years. The mean age at the time of surgery was 18.4 years; the mean follow-up was 161 months (120 to 238). Using Fulkerson's functional knee score, 41 knees (91%) had an excellent or good result at a mean follow-up of 45 months, and this was maintained in 29 (64%) at the final review. The main cause of deterioration in the clinical results was the onset or worsening of patellofemoral joint pain, not patellar instability.
