Carotid endarterectomy improves cognitive function in patients with exhausted cerebrovascular reserve.

OBJECTIVE: To investigate changes in cognitive function following carotid endarterectomy (CEA). DESIGN: Prospective study with controls. METHODS: CEA patients (n=159) were compared to a urology surgery control group (n=20). In CEA patients cerebrovascular reserve (CVR) was measured preoperatively. During surgery emboli and blood flow velocity in the middle cerebral artery were measured by transcranial Doppler (TCD) and cerebral oxygen saturation (CsO2) by near infrared spectroscopy. Cognitive function was measured preoperatively and at 5 days and 8 weeks postoperatively using a standardised

Mechanisms and Impact of Cognitive Reserve in Normal Aging and Alzheimer's Disease.

Age-related cognitive decline and individual differences in dementia susceptibility are increasingly explained through the concept of cognitive reserve (CR). CR reflected the brain's adaptive capacity to sustain cognitive performance despite Alzheimer's disease (AD)-related pathology, extending beyond traditional biomarkers that captured the molecular or structural changes, but often failed to account for clinical heterogeneity. This review provided a comprehensive synthesis of how CR was operationalized through three major methodological approaches: sociobehavioral proxies, residual variance

An intervention for delirium superimposed on dementia based on cognitive reserve theory.

Delirium superimposed on dementia (DSD) accelerates the trajectory of functional decline and results in prolonged hospitalization, re-hospitalization, premature nursing home placement, and death. In this article we propose a theory-based intervention for DSD that is derived from the literature on cognitive reserve and neuroplasticity. We begin by defining cognitive reserve, the guiding framework for our hypothesis. We review the pathophysiology and neuropsychology of delirium noting the similarities with dementia-these two conditions reflecting acute and chronic reductions in cognitive reserve

Cognitive reserve and the risk of postoperative neurocognitive disorders in older age.

BACKGROUND: Postoperative delirium (POD) and postoperative cognitive dysfunction (POCD) are postoperative neurocognitive disorders (PNDs) that frequently occur in the aftermath of a surgical intervention. Cognitive reserve (CR) is a concept posited to explain why cognitive health varies between individuals. On this qualitative understanding of cognitive health, factors like IQ, education level, and occupational complexity can affect the impact of neuropathological processes on cognitive outcomes. METHODS: We investigated the association between CR and POD and CR and POCD on data from 713 pati

Delaying the onset of Alzheimer disease: bilingualism as a form of cognitive reserve.

OBJECTIVES: There is strong epidemiologic evidence to suggest that older adults who maintain an active lifestyle in terms of social, mental, and physical engagement are protected to some degree against the onset of dementia. Such factors are said to contribute to cognitive reserve, which acts to compensate for the accumulation of amyloid and other brain pathologies. We present evidence that lifelong bilingualism is a further factor contributing to cognitive reserve. METHODS: Data were collected from 211 consecutive patients diagnosed with probable Alzheimer disease (AD). Patients' age at onse

Is cognitive reserve associated with the prevention of cognitive decline after stroke? A Systematic review and meta-analysis.

OBJECTIVE: To conduct a systematic review and meta-analyses of the effect of socio-behavioral cognitive reserve (CR) proxies on cognitive decline after stroke. METHOD: Three journal search and indexing databases (PubMed, Scopus and Web of Sciences) were crossed to examine the scientific evidence systematically. In addition, meta-analytic techniques, using mixed-effect methods, were carried out to estimate the impact (pooled effect size) of CR proxies on either dementia incidence or cognitive decline after stroke. RESULTS: Twenty-two studies were included in the systematic revision, whereas n

Cognitive reserve and rate of change in Alzheimer's and cerebrovascular disease biomarkers among cognitively normal individuals.

We examined whether cognitive reserve (CR) impacts level of, or rate of change in, biomarkers of Alzheimer's disease (AD) and small-vessel cerebrovascular disease in >250 individuals who were cognitively normal and middle-aged and older at the baseline. The four primary biomarker categories commonly examined in studies of AD were measured longitudinally: cerebrospinal fluid measures of amyloid (A) and tau (T); cerebrospinal fluid and neuroimaging measures of neuronal injury (N); and neuroimaging measures of white matter hyperintensities (WMHs) to assess cerebrovascular pathology (V). CR was in

Cognitive reserve and adaptive resilience: a conceptual analysis of a cross-species framework for neuropsychology and cognitive aging.

Why do some older adults maintain effective cognitive functioning despite substantial age-related brain change or neuropathology, whereas others with comparable neural burden show marked impairment? Related questions arise in comparative research, where organisms exposed to similar biological or environmental challenges may show markedly different adaptation patterns. Although these phenomena have traditionally been examined in separate literatures, they converge on a broader problem: how living systems maintain, recover, or reorganize function under challenge. This article develops a cross-sp

Frontotemporal structure preservation underlies the protective effect of lifetime intellectual cognitive reserve on cognition in the elderly.

BACKGROUND: Cognitive decline with age has heterogeneous, which might be related to the accumulation of protective factors called cognitive reserve, especially intellectual engagement factors over the life course. However, how lifetime intellectual cognitive reserve (LICR) protects cognitive function in the elderly remains unclear. We aimed to examine the relationship between LICR and cognition and the mild cognitive impairment (MCI) risk, as well as the neural mechanism of LICR on cognition. METHODS: A total of 5126 participants completed extensive neuropsychological tests, with LICR indicat

Association of Cognitive Reserve and Preoperative Cognitive Function in Patients with Adult Moyamoya Disease: A Preliminary Study.

INTRODUCTION: Many patients with moyamoya disease (MMD) exhibit cognitive decline; however, the link between cognitive reserve (CR) and cognitive function in those who have not undergone revascularization remains unexplored. We aimed to evaluate preoperative cognitive impairment in such patients and to explore the relationship between CR, measured using the Cognitive Reserve Index questionnaire (CRIq), and cognitive abilities across different domains, determined using neuropsychological tests. METHODS: Demographic, clinical, CRIq, and neuropsychological assessment data were gathered from pati

The influence of cognitive reserve in the protection of the cognitive status after an acquired brain injury: A systematic review.

BACKGROUND: Cognitive Reserve (CR) hypothesis was introduced to account for the variability in cognitive performance of patients with similar degrees of brain injury or pathology. The individual variability of CR is modulated by the interaction of innate capacities and exposures throughout life, which can act as protectors against neuropathology's clinical effects. Individuals with higher CR appear to have better cognitive performance after a brain injury. The present review aimed to identify and map the scientific evidence available in literature regarding CR's influence in protecting the cog

Association of lifespan cognitive reserve indicator with the risk of mild cognitive impairment and its progression to dementia.

INTRODUCTION: The association of lifespan cognitive reserve (CR) with mild cognitive impairment (MCI) remains controversial. We aimed to examine the association of lifespan CR indicator with the risk of MCI and its progression to dementia, taking brain pathologies into account. METHODS: In a community-based cohort study (mean age, 79 years) with annual follow-up (median, 5.16 years; maximum, 20 years), a cognitively intact group (n = 1182) and an MCI group (n = 420) were identified at baseline. During the follow-up, 611 participants died and underwent autopsies. CR indicator encompassing educ

The role of pre-morbid intelligence and cognitive reserve in predicting cognitive efficiency in a sample of Italian elderly.

BACKGROUND: Models of cognitive reserve in aging suggest that individual's life experience (education, working activity, and leisure) can exert a neuroprotective effect against cognitive decline and may represent an important contribution to successful aging. AIM: The objective of the present study is to investigate the role of cognitive reserve, pre-morbid intelligence, age, and education level, in predicting cognitive efficiency in a sample of healthy aged individuals and with probable mild cognitive impairment. METHODS: Two hundred and eight aging participants recruited from the provincia

Associations between cognitive reserve, traumatic events, post-traumatic stress symptoms, and mid- to late-life cognitive performance.

Exposure to potentially traumatic events is common across the lifespan, yet individual differences in cognitive outcomes following trauma exposure in older adults remain poorly understood. Cognitive reserve (CR) may protect against trauma-related cognitive deficits, but CR's moderating role in relationships between trauma exposure, post-traumatic stress symptoms (PTSS), and cognition is unclear. We examined 745 community-dwelling middle-aged to older adults (50-86 years, mean age = 64.5, 69.1% female) who completed assessments of traumatic event exposure (Life Events Checklist for DSM-5), post

Cognitive reserve in subjective cognitive decline with worry: DMN-VN connectivity supports episodic memory under structural vulnerability.

BACKGROUND: The occurrence of worry in subjects with Subjective Cognitive Decline(SCD) confers a greater risk of future AD and cognitive decline. However, it is not known whether these individuals have distinct memory and neuroimaging profiles. Therefore, we aimed to identify brain imaging characteristics associated with episodic memory in elderly adults with subjective cognitive decline and related worry (SCD+) in a population-based cohort. METHODS: A total of 418 individuals from the BABRI cohort underwent structural MRI (n = 418) and episodic memory-related task-based fMRI (n = 153) scans.

Modeling functional loss in Alzheimer's Disease through cognitive reserve and cognitive state: A panel data longitudinal study.

Cognitive Reserve (CR) refers to the brain's ability, supported by active and modifiable forms of lifestyle compensation, to cope with neural changes due to age or disease, delaying the onset of cognitive deficits. In CR studies, neuropsychological performances and functional autonomy are considered alternative outcomes. While decreased functional independence gains importance in dementia diagnosis and monitoring, cognitive functioning may play a role in staging its severity. The main aim of the present study was to test a longitudinal model of Alzheimer's Disease (AD), in which CR (years of e

Do cognitive reserve levels affect brain glucose metabolism and amyloid-β depositions in subjective cognitive decline subjects?

Subjective cognitive decline (SCD) is the preclinical stage of Alzheimer's disease (AD). Abnormal brain glucose metabolism and Aβ depositions can be observed in SCD subjects. Nevertheless, there was not cognitive impaired performance in standardized neuropsychological in these subjects. Cognitive reserve (CR) could be the reason to explain this phenomenon. However, correspondence between CR and SCD was still uncleared. In this study, we attained 74 subjects underwent 18FFDG PET scans (SCD1 group) and 38 patients underwent 18FAV45 PET scans (SCD2 group) from Xuanwu Hospital, Beijing, China. Fir

Role of cognitive reserve in cognitive variability in euthymic individuals with bipolar disorder: cross-sectional cluster analysis.

BACKGROUND: People with bipolar disorder have moderate cognitive difficulties that tend to be more pronounced during mood episodes but persist after clinical remission and affect recovery. Recent evidence suggests heterogeneity in these difficulties, but the factors underlying cognitive heterogeneity are unclear. AIMS: To examine whether distinct cognitive profiles can be identified in a sample of euthymic individuals with bipolar disorder and examine potential differences between subgroups. METHOD: Cognitive performance was assessed across four domains (i.e. processing speed, verbal learnin
