Chronic complete tears of the ulnar collateral ligament of the thumb metacarpophalangeal joint often require surgical reconstruction. The indications, materials used, and the methods for reconstruction remain controversial. We describe a technique for reconstruction that utilizes a slip of the extensor retinaculum for reconstruction. The advantages of extensor retinaculum as a graft choice are that it matches the native ligament with a synovial and nonsynovial surface, has elasticity similar to the ligament and minimal donor site morbidity. Reconstruction of the ulnar collateral ligament with

BACKGROUND: The increase in ulnar collateral ligament (UCL) elbow reconstructions over the past 20 years has affected younger athletes more than any other age group. Although return to play and postoperative performance have been extensively studied in professional baseball players, outcomes in collegiate baseball players are less known.
PURPOSE/HYPOTHESIS: The purpose of this study was to characterize return to play and changes in performance after UCL reconstruction (UCLR) in collegiate baseball players. We hypothesized that collegiate baseball players would have similar return-to-play rate

BACKGROUND: Medial ulnar collateral ligament (MUCL) reconstruction outcomes are well described in competitive throwers but not in nonthrowers. This investigation elucidated epidemiologic variables, functional outcomes, and prognostic factors after MUCL reconstruction in young active patients.
METHODS: United States military service members undergoing MUCL reconstruction were isolated using the Management Analysis and Reporting Tool (M2) database from 2009 to 2016. Demographics, injury characteristics, and surgical variables were extracted. Multivariate analysis was performed, discerning varia

PURPOSE: Acute elbow injuries that disrupt the lateral ulnar collateral ligament and result in posterolateral rotatory instability usually require surgical treatment. The 2 technical options reported, direct repair and use of a palmaris longus tendon graft, have usually favored the use of the graft. To balance this emphasis, we report our experience with direct repair of the humeral origin in cases of trauma, whether acute, delayed, or recurrent. It was our hypothesis that because the humeral origin is the point of failure and separation, restoration of this attachment is sufficient to restore

BACKGROUND: Currently, several graft options have been described for reconstruction of the medial ulnar collateral ligament of the elbow. Palmaris longus, gracilis, plantaris, toe extensor, and even Achilles tendon autografts have been well documented. To our knowledge, no study has investigated the clinical outcomes following the use of allograft tendon for primary medial ulnar collateral ligament reconstruction. It is our hypothesis that medial ulnar collateral ligament reconstruction with hamstring allograft provides results similar to those reported with autograft without the potential com

OBJECTIVES: Rupture of the ulnar collateral ligament (UCL) is a frequent injury of the hand. When untreated, this injury may lead to decreased pinch strength, pain, instability, and osteoarthritis. There is currently no consensus on treatment of acute or chronic UCL injuries. Our primary purpose was to compare nonoperative treatment with surgical repair and surgical reconstruction of thumb UCL injuries. A secondary purpose was to compare graft choice and surgical technique for reconstruction.
DATA SOURCES: A systematic review of multiple medical databases was performed using PRISMA (Preferred

Injury to the ulnar collateral ligament of the thumb metacarpophalangeal joint is commonly encountered in clinical practice, particularly among athletes. Although repair using suture anchors is currently the gold standard, recent evidence suggests suture tape augmentation offers superior initial construct strength, allowing for earlier mobilization. This technical note describes a reproducible technique for thumb ulnar collateral ligament repair using suture tape augmentation. The procedure involves primary repair with a 1.3-mm Suturetape (Arthrex, Naples, FL) followed by internal bracing usin

BACKGROUND: The ulnar collateral ligament (UCL) is essential for elbow stability during pitching. In professional baseball, the fastball (FB) is the most commonly used pitch, making postrecovery FB performance after UCL reconstruction (UCLR) a crucial aspect to consider.
HYPOTHESES: (1) Pitchers undergoing UCLR would show no significant changes in performance metrics compared with nonoperated pitchers with similar FB velocity and spin rate, and (2) no significant variance would be found in these metrics within the operated pitchers concerning their preinjury anthropometric characteristics and

BACKGROUND: Elbow ulnar collateral ligament (UCL) repair with suture brace augmentation shows good time-zero biomechanical strength and a more rapid return to play compared with UCL reconstruction. However, there are concerns about overconstraint or stress shielding with nonabsorbable suture tape. Recently, a collagen-based bioinductive absorbable structural scaffold has been approved by the Food and Drug Administration for augmentation of soft tissue repair.
PURPOSE/HYPOTHESIS: This study aimed to assess the initial biomechanical performance of UCL repair augmented with this scaffold. We hyp

BACKGROUND: Ulnar collateral ligament injuries are common in the overhead-throwing athlete as the anterior band of this ligament is the primary soft tissue static stabilizer to valgus stress during a throwing motion. Various surgical techniques have been described over the past 30 years to address these injuries including Jobe, modified Jobe, docking, 3-stranded docking, Graftlink, and anatomic repairs with good to excellent results. Nevertheless, modifications to these techniques may increase the strength and survival of the reconstruction, which may expedite and optimize postoperative rehabi

BACKGROUND: Medial ulnar collateral ligament (UCL) injury is increasingly prevalent in professional baseball pitchers, and significant research has been devoted to understanding the risk factors and prevention strategies associated with it. To date, no study has investigated what the players themselves believe causes and prevents the injury.
PURPOSE: To evaluate the opinions of UCL injuries among pitchers, including professional athletes.
STUDY DESIGN: Cross-sectional study.
METHODS: A total of 214 baseball pitchers (45 high school/college, 169 Major League Baseball [MLB]/Minor League Baseb

Shoulder and elbow injuries have been described in baseball players as early as the 1940s. Ulnar collateral ligament (UCL) tears have been recognized as a significant source of disability for baseball players and have been seen in increasing frequency as training regimens and level of play have become more intense and rigorous. Our understanding and treatment of these injuries have also evolved over time. This article summarizes the evolution of the treatment of UCL tears and discusses future directions for the treatment and prevention of these injuries.

BACKGROUND: A variety of techniques currently exist for ulnar collateral ligament (UCL) reconstruction in symptomatic overhead athletes, all with the potential complication of fracture about the humeral or ulnar tunnels. Far cortical button fixation is a reproducible, biomechanically attractive option; however, no clinical series has been published on this technique to date. This study reports the clinical outcomes, with minimum 2-year follow-up, of a dual far cortical button suspension technique for UCL reconstruction in athletes.
METHODS: A retrospective evaluation was performed of 23 conse

INTRODUCTION: Elbow ulnar collateral ligament (UCL) reconstruction has become the standard of care for the throwing athlete with a symptomatic ruptured or insufficient ligament and the desire to resume competitive play. Since Jobe's initial description of UCL reconstruction, the technique has evolved. A novel modification was the "docking" technique developed by Altchek. Subsequently, the docking technique as originally described was slightly modified. Arthroscopy is no longer routinely performed, and in some cases, a 3-strand graft is used.
MATERIALS AND METHODS: We treated 21 overhand athle

Thumb hypoplasia represents a spectrum of deficiencies from a slightly smaller thumb to a completely absent thumb. The clinical features of a type II hypoplastic thumb include a narrow thumb-index web space, absent thenar muscles, and instability of the thumb metacarpophalangeal joint. This article will focus thumb reconstruction using the ring finger flexor digitorum superficialis to provide opposition and restore metacarpophalangeal joint stability. The technique, postoperative protocol, outcome, and complications will be emphasized.

Over 10 years in our unit, 127 patients underwent surgical repair of an acute thumb ulnar collateral ligament (UCL) rupture for clinically unstable injuries or displaced avulsion fractures. Ultrasound was used when clinical diagnosis was uncertain. The male/female ratio was 3:2, with a mean age of 40 years (range: 12-81 years). Most of them (> or =66%) were hyper-extension/abduction injuries. The most common cause was a fall (49%), followed by sports injuries; skiing accounted for only 2.4%. Ultrasound was 92% sensitive for UCL ruptures (positive predictive value (PPV) 99%). Over 99% of patien

BACKGROUND: The kinetic chain of the throwing motion functions to optimize efficiency of proximal segments to decrease force loads seen at smaller, distal segments such as the ulnar collateral ligament. Several studies have shown that shoulder internal rotation forms the physiologic counter to the valgus torque generated during the late cocking phase of throwing. Previous studies have implicated decreased glenohumeral internal rotation as a cause of shoulder internal impingement. To date, an association between pathologic glenohumeral internal rotation deficit and elbow injury has not been exh

Complete thumb ulnar collateral ligament (UCL) injuries usually require primary repair. The ulnar collateral ligament is often torn from its insertion site and reattachment is difficult. Seven patients underwent repair with the Mitek bone anchor (Mitek Surgical Products, Norwood, MA) for complete ulnar collateral ligament disruptions. A Stener lesion was found in four patients. Follow-up examination was at approximately 1 year. All patients regained a stable metacarpophalangeal joint to valgus stress. X-ray films demonstrated accurate placement of the bone anchor with protraction of the metall

PURPOSE: A flat metacarpal head of the thumb is thought to predispose to ulnar collateral ligament ruptures because of limited range of motion. In this study, we quantitatively compared the roundness of the thumb metacarpal head in individuals with an ulnar collateral ligament rupture to the roundness in patients without any thumb pathology using conventional thumb metacarpophalangeal joint anteroposterior radiographs.
METHODS: Patient data were extracted from a single institutional system, including six urban hospitals in the United States. Eighty-two patients with a thumb ulnar collateral l
