Clinical results of external fixation for unstable Colles' fractures.

The objective of this study is to evaluate clinical and radiological outcomes of unstable or intra-articular Colles' fractures using the Pennig model of external fixation. The Pennig model of dynamic external fixation was used on 88 patients with 92 Colles' fractures. In 35 wrists, exercise was not performed during external fixation. In 57 wrists, exercise was initiated three weeks after surgery. Regarding radiological measurements, the degrees of volar tilt (VT), radial inclination (RI), radial length (RL) and ulnar variance (UV) were serially measured and evaluated. The clinical results were

[Coincidental ipsilateral Colle's fracture and scaphoid fracture after low energy trauma].

Distal radius fractures are frequently seen in the emergency room. Scaphoid fractures are frequently seen as well. However, coincidental occurrence of these two types of fracture is quite rare. A 66-year-old healthy man was taken to the emergency room after a fall. He had pain in his right wrist, and an X-ray examination revealed a Colles' fracture with a 30 degree dorsal angulation and an undislocated transverse fracture of the scaphoid bone. The case illustrates that special attention should be given to the examination of the wrist and the first finger beam to exclude associated injuries.

Is ulnar variance suitable for a parameter of Colles' fracture pre-operatively?

BACKGROUND: To evaluate ulnar variance (UV) as a parameter for Colles' fracture as a measure of the true length of the distal radius pre-operatively, we measured UV pre-operatively in 20 patients with Colles' fractures being treated surgically using the method of perpendiculars. Because the distal fragment is by definition dorsi-flexed, the dorsal edge of the most distal part of the radius is seen proximally and the volar edge of it is seen distally on true postero-anterior x-ray of the wrist. METHODS: We measured three different UVs in x-rays. UVd is the distance from the dorsal edge of the

Irreducible skin penetration of the ulnar head in Colles fracture--case report.

Three cases out of four of skin penetration by the ulnar head associated with Colles fractures were found to be irreducible. On exploration they had identical pathologic anatomy, viz, buttonholing of the ulnar head between the flexor carpi ulnaris, flexor digitorum profundus, pronator quadratus, and the flexor retinaculum. Incising the flexor retinaculum facilitated reduction.

Minor axial shortening of the radius affects outcome of Colles' fracture treatment.

Functional disability after a Colles' fracture was correlated to the degree of axial shortening of the radius measured at the site of distal radioulnar joint. According to the sum of demerit points, the functional end result was unsatisfactory (fair or poor) in 4% of the patients with an acceptable anatomic result, in 25% of the patients with grade 1 shortening (corresponding to a shortening of 3 to 5 mm), and in 31% of the patients with grade 2 shortening (shortening of more than 5 mm). The results suggest that even a minor axial shortening of the radius, without concomitant malalignment of t

Factors affecting the outcome of Colles' fracture: an anatomical and functional study.

A prospective radiological and functional assessment has been performed on 235 consecutively treated displaced Colles' fractures. The age and sex of the patients, as well as the initial displacement, involvement of articular surface and the adequacy of reduction of the fracture, were all analysed. Because of their greater potential for collapse, the better reduced fractures tended to lose more position during treatment. Despite this, in general, the better the reduction, the better was the final anatomical position of the fractures. The functional result at 3 months was related to the severity

Metacarpal radiogrammetry by computed radiography in postmenopausal women with Colles' fracture and vertebral crush fracture syndrome.

Based on the hypothesis that the underlying osteoporotic mechanism of Colles' fracture in postmenopausal women is similar to that of other osteoporotic fractures, that is, cortical bone resorption as opposed to cancellous bone resorption, the rate of corticoendosteal bone loss was compared in 40 normal postmenopausal women [average age 68.4 +/- 7.1 years; 20 +/- 4 years since menopause (YSM)], in 35 postmenopausal women with Colles' fracture (age 69.4 +/- 7.5 years, 22 +/- 8 YSM), in 35 normal postmenopausal women with vertebral crush fracture (age 69.4 +/- 7.5 years, 22 +/- 8 YSM, and in 35 n

Follow-Up Effect of Wechat Group on Discharged Patients with Colles Fracture.

This work explores the effect of a Wechat group on follow-up and continuation of nursing for discharged patients with Colles fracture. A total of 96 patients with Colles fracture are enrolled and randomized into two groups by the random number table method. The control group is followed up and guided by traditional methods. Based on the follow-up method adopted in the control group, a Wechat group is combined with Wechat constant tracking. The regular follow-up rate, a satisfaction of with continuing care, Gartland-Werley wrist score, the exercise of self-care agency score (ESCA score), and co

Pulsed electromagnetic field during cast immobilization in postmenopausal women with Colles' fracture.

INTRODUCTION: Although Colles' fracture i.e. a dorsally displaced distal radius fracture (DRF) is one of the most common fractures, there is no enough evidence to determine the best form of rehabilitation. OBJECTIVE: To assess whether the use of pulsed electromagnetic field (PEMF) therapy during cast immobilization of DRF provides beneficial effects on pain, edema, wrist range of motion and function, as well as on the frequency of complications immediately after cast removal. METHODS: The prospective randomized controlled study included 60 women over the age of 55 years with extra-articular

Colles' fracture: dorsal splint or complete cast during the first 10 days?

In a prospective randomised trial of conservatively treated Colles' fractures, we compared radiological outcomes and complication rates for two immobilisation methods, either complete cast (34 cases) or dorsal splint (38 cases), during the first 10 days after reduction. Radiographs were evaluated for dorsal tilt, radial tilt and radial shortening; results were almost identical in the two groups. There was a mean difference of 3.4 degrees in dorsal angulation in favour of dorsal splinting 10 days after reduction and a mean difference of 1.6mm in radial length in favour of complete casting at 5

Intramedullary bone cementing for the treatment of Colles fracture in elderly patients.

Eighteen patients with Colles fractures, mean age 70 years (range 55-91), were treated by intramedullary bone cementing. The best indication for this technique was an unstable extra-articular Colles fracture with osteoporosis. Bone cement was packed into the canal that was made by curettage of intramedullary cancellous bone from the dorsal fracture site. Intramedullary cementing caused little bleeding from the medullary canal and no irritation of the extensor tendons. Because of rigid fixation, patients could use the affected hand for light activities without any external orthosis the day afte

Long-term prognosis of displaced Colles' fracture: a 10-year prospective review.

In all, 85 per cent of Colles' fractures are clinically satisfactory after 10 years. Clinical results improve little after 3 months. Displaced intraarticular fracture, wrist deformity and algodystrophy are associated with an unsatisfactory outcome.

Do Kirschner wires maintain reduction of displaced Colles' fractures?

Fifty-three patients underwent closed reduction and longitudinal k-wiring of displaced Colles' fractures and were reviewed after a mean of 26 months. Radiographs taken at the time of injury, after reduction and k-wiring, and at fracture union were compared for radial shortening and dorsal angulation. Manipulation significantly improved fracture position (P<0.001). Dorsal angulation was successfully corrected by manipulation in 98%, and this position was maintained to fracture union in all cases. Seventy-three percentage of fractures manipulated for radial shortening >2 mm were adequately reduc

Treatment of unstable Colles' fractures with a new radius mini-fixator.

An external fixator has been developed that is confined to the radius. It is lightweight, has a low profile, and its application is uncomplicated. Because the fixator provides rigid fixation of the fracture fragments, it has the important advantage of allowing immediate active wrist exercises, thus facilitating early restoration of mobility. The radius mini-fixator has been effective in the treatment of unstable Frykman Type I and II Colles' fractures that account for 36.3% in Frykman's series. It has also been used to stabilize osteotomies of the radius for malunited Colles' fractures.

Carpal tunnel pressures and wrist position in patients with colles' fractures.

Intracarpal canal interstitial fluid pressures were determined in the wrists of 22 patients with 23 Colles' fractures. Mean values were 18 mm Hg in neutral wrist position, 27 mm Hg in 20 degrees flexion, 47 mm Hg in 40 degrees flexion, and 35 mm Hg in 20 degrees extension. Ten of 23 wrists (45%) had pressures greater than 40 mm Hg in 40 degrees of flexion. Because of the high incidence of median neuropathy associated with immobilization of wrist fractures in a position of marked flexion, an awareness of the magnitude of increased interstitial fluid pressure should lead to alternative methods o

Comminuted displaced Colles' fractures. Treatment with intramedullary methylmethacrylate stabilisation.

Four women aged 87, 77, 74 and 69 years with severely comminuted, displaced and intraarticular Colles' fractures of the dominant hand, where conservative treatment had failed, were treated by use of intramedullary methylmethacrylate stabilisation in order to retain optimal anatomical position of the fracture. The patients were followed by serial clinical, radiological and 99Technetium-scintimetrical examinations for at least 1 year. Normal mobility of the wrist and power of the grip was obtained within 6 weeks postsurgery in all patients. Radiological cortical bone healing occurred within norm
