Applicability of laparoscopic surgery for colorectal disease.

BACKGROUND: The advantages of laparoscopic colorectal surgery for selected patients have been well established. However, the applicability of laparoscopic surgery in the whole population of patients with colorectal disease is not well known. METHODS: A single-institution medical records review of 269 patients subjected to colorectal surgery was made. Of these, 206 open colorectal procedures were performed, and data were reviewed retrospectively. In addition, 63 patients were subjected to laparoscopy, and their data were recorded prospectively. An analysis of the existence of factors that cont

[Prevention of anastomotic dehiscence in colorectal surgery].

Anastomotic leakage is a major cause of mortality in colorectal surgery. Some variables associated with a high-risk level for anastomotic leakage have been identified, including denutrition, obesity, smoking, level of the anastomoses, qualification of the surgeon. Several methods have been evaluated in order to prevent anastomotic leakage. Prophylactic antibiotic therapy has been recommended, but has no effect on the rate of anastomotic leakage. Optimal preoperative colonic preparation appears to be obtained with povidone iodine enemas associated with a low residue diet. Stapling gives better

Problems and complications associated with colorectal surgery.

Successful colorectal surgery requires a combination of thorough patient evaluation and surgical judgment to determine the correct surgical approach and to properly apply the general principles of colorectal surgery. These principles include gentle tissue handling, preservation of blood and nerve supply, proper suture selection and suture techniques, postoperative monitoring, and early recognition and treatment of complications.

The application of regenerative medicine in colorectal surgery.

Tissue reconstruction and regeneration represent one of the greatest challenges in any surgical field. Regenerative medicine combined with stem cell-based therapy is a novel and promising field of medicine. Stem cells possess the ability to differentiate into specialized cells and to decrease inflammation and therefore can play a role in repair or regeneration of damaged tissues. Colorectal surgery often deals with infected, poorly vascularized, radiated, and inflamed tissue, as well as instances where imperfect healing might have grave implications. This problem has led researchers to study u

[The application of transversostomy in colorectal cancer surgery].

UNLABELLED: Defunctioning stomas (ileostomy and transversostomy) are the most common methods of the decompression of distal digestive tract. The aim of this study was to assess the results of transverse loop colostomy in colorectal cancer patients. MATERIAL AND METHOD: Between 1998 and 2007, 119 patients with colorectal cancer underwent transversostomy. In 97 patients the colostomy was formed during the primary operation as a planned prevention from further complications (1st group) and in 22 previously operated patients due to postoperative complications as a second procedure (2nd group). [Use of antibiotics in colorectal surgery in Denmark].

INTRODUCTION: The aim of the study was to investigate the use of antibiotics in scheduled and acute colorectal surgery in Denmark and on the basis of this and a literature research, to recommend possible antibiotic strategies. MATERIAL AND METHODS: In 2004, a written questionnaire regarding the antibiotic treatment in scheduled operations for colorectal cancer and acute colorectal surgery was submitted to all 39 surgical departments in Denmark which at the time performed colorectal surgical procedures. RESULTS: The response rate was 97%. 17 different antibiotic regimes using five different k

Prophylaxis of infection for elective colorectal surgery.

BACKGROUND: Influenced by the key results of the clinical trials conducted in the early 1970s by Condon, Nichols, and Gorbach, surgeons have adopted the routine use of mechanical bowel prep and antimicrobial prophylaxis prior to elective colorectal procedures as a widely established practice. Recent clinical trial data, however, led us to reexamine the benefits of mechanical bowel preparation, methods of antimicrobial prophylaxis and to assess the role of new, specific risk factors for surgical site infection after colorectal operations. METHODS: Pertinent studies on antimicrobial prophylaxis

[The Knight-Griffen technique in colorectal surgery].

The use of stapling devices has changed the practice of colorectal surgery. The authors report their experience with the "double stapling" technique introduced by Knight and Griffen in 1980 for the performance of low colorectal anastomosis. They treated 42 patients, with a mean age of 63.2 years. In 34 surgery was performed for rectal adenocarcinoma, while 8 patients were being reoperated on for closure of a colostomy after Hartmann's operation. In one patient a diverting colostomy was fashioned because of poor bowel cleansing, secondary to a stenosing rectal carcinoma. Mortality was nil. Ther

Bowel preparation or not for elective colorectal surgery.

The importance of bowel preparation in elective colorectal surgery has often been stressed. We have surveyed the current practice of bowel preparation and chemoprophylaxis amongst surgeons in Wales and the south-west of England. From this we identified one surgeon who used no preoperative preparation and his results are reported in detail. One hundred consecutive colorectal cases were studied from one surgeon (J.M.P-T.). There was a 7% incidence of wound infection and one anastomosis leaked. None of these patients had mechanical bowel preparation but all had antibiotic chemoprophylaxis. Mechan

Prophylaxis with doxycycline (Vibramycin) in colorectal surgery.

A prospective randomized double-blind study comprising 118 patients was performed to evaluate the effects of doxycycline as a prophylactic antimicrobial in elective colonic surgery. Fifty-eight patients were treated and 60 were controls. 200 mg of active substance or placebo was given 4-6 hours before operation and 100 mg daily for the next five postoperative days. Levels of doxycycline in serum and tissues were determined and related to the MIC-values of the contaminants. A significantly lower incidence of abdominal wound sepsis, intra-abdominal complications and septicaemia was found in the

Patients' related sexual outcomes in colorectal surgery.

BACKGROUND: Patients undergoing colorectal surgery (CRS) have an increased risk of developing sexual disorders, attributed to different mechanisms. In this context, sexual function (SF) assessment of patients before and after surgery is essential: to identify risk factors for sexual disorders as well as to minimize their impact on overall quality of life (QoL), allowing them a satisfying relationship and sexual life. MATERIAL AND METHODS: Patients over 18 years of age who underwent a CRS in the University Hospital of Geneva, Switzerland, between June 2014 and February 2016 were included. Our

Mechanical bowel preparation for elective colorectal surgery.

BACKGROUND: For over a century the presence of bowel content during surgery has been linked to anastomotic leakage. Mechanical bowel preparation has been considered an efficient agent against leakage and infectious complications. This dogma is not based on solid evidence, but on observational data and expert's opinions. OBJECTIVES: To determine the effectiveness and safety of prophylactic mechanical bowel preparation for morbidity and mortality rates in elective colorectal surgery. SEARCH STRATEGY: We searched MEDLINE, EMBASE, LILACS, and the Cochrane Central Register of Controlled Trials. W
