Comparison of Nationwide Volumes for Five Major Gastrointestinal Surgical Procedures in Japan: Implications From Publicly Available NCD and NDB Open Data.

AIM: In Japan, the National Clinical Database (NCD), a clinician-reported registry, and the National Database of Health Insurance Claims and Specific Health Checkups of Japan Open Data (NDB Open Data), an administrative claims database, are both widely used as nationwide surgical databases, but their consistency in capturing national surgical volumes has not been systematically evaluated. METHODS: We compared publicly available data from the NCD annual surgical statistics and the NDB Open Data from 2014 to 2023. Annual case numbers were extracted for five major gastrointestinal procedures: ch

[Surgical site infection following proximal GIT procedures].

INTRODUCTION: Surgical site infections (ICHM) remain a serious problem in the proximal GIT surgery, with incidence rates from 5% to 26% in planned procedures, and up to 40% in urgent procedures. Surgical site infections result in increased postooperative morbidity and mortality rates, prolonged hospitalization and increased treatment costs. Minimalization of the surgical site infection rates is an important epidemiological and therapeutical objective. AIM: A retrospective study assessed surgical site infections rates in urgent and elective procedures on the proximal GIT, and in a selected gro

Safety and utility of the modified Pfannenstiel incision in laparoscopic and robot-assisted colorectal surgery.

PURPOSE: We modified the Pfannenstiel (PS) incision by relocating it to the left lower abdomen, to combine the advantages of the PS incision with those of intracorporeal anastomosis in colorectal cancer surgery. We conducted this study to evaluate the safety and utility of our modified Pfannenstiel incision (mPS) incision in laparoscopic and robotic surgery. METHODS: We reviewed, retrospectively, 357 colorectal cancer patients who underwent laparoscopic and robotic surgery between 2022 and 2025. The patients were divided into two groups based on the surgical incision used: those with the mPS

Minimally invasive approach to colorectal surgery.

While the development of laparoscopic surgery over the last two decades was amazingly fast, its adoption was neither uniform nor universal. Some procedures, like laparoscopic cholecystectomy, rapidly became the standard of care throughout the surgical community. Laparoscopy for colorectal surgery gained much less acceptance. Factors such as technical complexity, cost, duration of surgery and concerns about oncologic safety influenced the hesitancy in performing this surgery, and it took the surgical community more than a decade to admit that the laparoscopic option is legitimate: it is safe, a

Research Progress on the Optimal Timing for Colostomy Reversal.

This review comprehensively summarizes the latest research progress on the optimal timing for colostomy reversal, a critical surgical intervention aimed at restoring normal bowel function and improving the quality of life of patients with colostomies. Colostomy is commonly performed for various gastrointestinal conditions, including colorectal cancer, diverticulitis, and inflammatory bowel diseases, but its long-term presence imposes physical and psychological burdens on patients. The timing of reversal directly affects postoperative recovery, complication rates, and overall patient outcomes,

[Standardized surgical procedure of proximally extended resection and sphincter-preserving surgery (Tianhe procedure®) for rectal cancer after radiotherapy (2025 version)].

Tianhe procedure® is a functional sphincter-preserving surgical approach developed for rectal cancer patients following radiotherapy. This technique involves extended proximal resection of the colon beyond the pelvic cavity, followed by anastomosis of the non-irradiated proximal colon to the distal rectum or anal canal. This strategy aims to reduce the incidence of anastomotic complications and postoperative bowel dysfunction. However, there is currently a lack of standardized practice guideline for implementing Tianhe procedure® in China. Therefore, the Chinese Radiation Intestinal Injury Res

Current status of robot-assisted surgery.

The introduction of robot-assisted surgery, and specifically the da Vinci Surgical System, is one of the biggest breakthroughs in surgery since the introduction of anaesthesia, and represents the most significant advancement in minimally invasive surgery of this decade. One of the first surgical uses of the robot was in orthopaedics, neurosurgery, and cardiac surgery. However, it was the use in urology, and particularly in prostate surgery, that led to its widespread popularity. Robotic surgery, is also widely used in other surgical specialties including general surgery, gynaecology, and head

Complications in colorectal surgery.

Colorectal pediatric surgery is a diverse field that encompasses many different procedures. The pullthrough for Hirschsprung disease, the posterior sagittal anorectoplasty for anorectal malformations including complex cloaca reconstructions and the ileal pouch anal anastomosis for ulcerative colitis and familial adenomatous polyposis present some of the most technically challenging procedures pediatric surgeons undertake. Many children prevail successfully following these surgical interventions, however, a small number of patients suffer from complications following these procedures. Anticipat

What Do Population-based Studies Tell Us about the Ideal Timing of Surgical Resection for Colorectal Cancer?

Timely access to curative-intent colorectal cancer (CRC) surgery is a central component of surgical quality. Yet contemporary evidence challenges historical assumptions that shorter wait times always confer better outcomes. Drawing on population-based studies which offer the most robust and generalizable data this article synthesizes what is known about the relationship between surgical delay and oncologic outcomes in CRC. We outline key conceptual considerations in defining wait-time intervals, discuss lessons from the coronavirus disease 2019 pandemic, and identify gaps for future research.

[Postoperative peritonitis. Patients, causes, therapy, prognosis].

1. The most frequent causes of postoperative peritonitis are circumscribed abscesses, the postoperative ileus and the anastomotic leakage or breakdown of the digestive suture. 2. The mortality is 33%; procedures in upper abdominal surgery are accompanied by higher mortality than in colorectal surgery in cases of postoperative peritonitis. 3. The most frequent cause of death after reoperation is the generalized sepsis. 4. A mixed flora of spores are found in most cases; obligate anaerobic spores are found mostly after colorectal procedures, while typical "hospitalized spores" were present equal

[Laparoscopic surgery for benign colorectal diseases].

Since 1991 when the first cases of laparoscopic colectomy were reported, this technique has become widespread in colorectal surgery. Its reliability in colorectal cancer has been demonstrated but indications must still be limited to strictly controlled clinical trials that will determine its impact in terms of cancerology outcome. Inversely, for benign colorectal disease, the efficacy, low morbidity and the generally observed clinical benefit of laparoscopy compared with laparotomy, particularly in terms of patient comfort in the post-operative period and delay to normal activities, has been c

Internal herniation following laparoscopic left hemicolectomy: an underreported event.

PURPOSE: Laparoscopic colorectal surgery creates fewer adhesions compared with open colorectal resection. Small bowel obstruction after laparoscopic colorectal resection may be caused by internal herniation of the small bowel through a colomesenteric defect, probably related to a lack of adhesion formation. This is seen especially after left colonic resections. METHODS: A literature review was conducted using the key words "intestinal obstruction" and "laparoscopic left colonic resection," and 9 documented case reports of obstruction following laparoscopic left colorectal resection were ident

Novel adjunctive technologies for the prevention of colorectal anastomotic leakage: a systematic review.

BACKGROUND: Anastomotic leakage (AL) remains one of the most serious complications following colorectal surgery, particularly after low anterior resection. In recent years, several adjunctive technologies have been introduced to reinforce anastomotic integrity during the early phases of healing. This systematic review aimed to evaluate the safety, feasibility, and clinical effectiveness of novel mechanical and biological devices designed to prevent AL in colorectal surgery. METHODS: A systematic literature search was conducted in PubMed, Embase, and Scopus. Human studies evaluating innovative
