BACKGROUND: The use of laparoscopy in liver surgery is well established and considered as the gold standard for small resections. The laparoscopic resections have lower morbidity and better cosmetic results, but still require an incision to remove the surgical specimen. The possibility of remove the specimen through natural orifices and avoid an abdominal incision may further improve the benefits offered by minimally invasive procedures.

OBJECTIVE: To introduce a combined laparovaginal metroplasty approach to the treatment of bicornuate uterus.
DESIGN: Case report.
SETTING: Private community hospital.
PATIENT: A nulliparous patient with three recurrent spontaneous midtrimester abortions and a double uterus on hysterosalpingogram was evaluated laparoscopically.
INTERVENTIONS: A laparoscopic-assisted transvaginal wedge metroplasty for the surgical correction of a symptomatic bicornuate uterus was performed. Intraoperative hysteroscopy also was used.

This report describes the use of an Argyle trocar catheter for the drainage of pelvic abscesses following transvaginal ultrasound-directed follicle aspiration (UDFA) for oocyte collection. Three patients treated with this technique recovered uneventfully. We conclude that the use of an Argyle trocar catheter for colpotomy in the management of pelvic abscesses appears to be simple and effective.

Complications associated with cold-knife conization are mainly hemorrhage, genital and urinary infections. We presented two cases of accidental posterior colpotomy with Douglas pouch wound initially unnoticed at the time of the cold-knife conization, and secondary complicated by a pelviperitonitis for the first case and by an incarceration of the bowel loop in the vaginal wound for the second case.

STUDY OBJECTIVE: To compare 2 different types of uterine manipulators (i.e., tight fitting vs loose fitting) used for total laparoscopic hysterectomy (TLH).

Fallopian tube prolapse into the vaginal vault is a rare complication after hysterectomy with adnexal preservation. It can occur following vaginal and abdominal hysterectomy, and rarely following interposition and colpotomy. Histopathology is the only means of definitive diagnosis. Prevention of tubal prolapse can be achieved by suturing the adnexae high in the pelvis at abdominal hysterectomy, and the incidence decreases if the pelvic peritoneum is closed properly.

A series is described of 150 cases of sterilization through posterior colpotomy. The operation was mainly elective or postabortal. The average hospital stay was short--1 or 2 days and complications were few-mainly pelvic peritonitis. There was only 1 death in the series covering 5 years--from peritonitis. BACKGROUND AND OBJECTIVES: To evaluate whether the route and surgical technique by which hysterectomy is performed influence the incidence of vaginal cuff dehiscence.

PURPOSE: We report the detailed technique and results of transvaginal extraction of the kidney following laparoscopic nephrectomy.
PATIENTS AND METHODS: From August 2013 onward, we planned to perform transvaginal extraction of the kidney following laparoscopic nephrectomy in a total of 6 female patients. Of the 6 patients, 5 underwent the procedure. After completion of the primary laparoscopic nephrectomy and entrapping the removed kidney, the patients were placed in the supine lithotomy position. A transverse posterior colpotomy was created transvaginally at the apex of the posterior fornix.

Morcellation allows minimally invasive approaches to surgery even in the presence of large uteri or myomas. Recent restrictions in the use of power morcellation, as well as concerns regarding the potential for morcellation to disseminate malignant tissue, have initiated investigation and innovation to find safer methods. This review examines current techniques for tissue extraction during uterine surgery, with a focus on contained power morcellation and contained manual morcellation via mini-laparotomy or colpotomy.
