Predictors of timely follow-up after abnormal cancer screening among women seeking care at urban community health centers.

BACKGROUND: We sought to measure time and identify predictors of timely follow-up among a cohort of racially/ethnically diverse inner city women with breast and cervical cancer screening abnormalities. METHODS: Eligible women had an abnormality detected on a mammogram or Papanicolaou (Pap) test between January 2004 and December 2005 in 1 of 6 community health centers in Boston, Massachusetts. Retrospective chart review allowed us to measure time to diagnostic resolution. We used Cox proportional hazards models to develop predictive models for timely resolution (defined as definitive diagnosti

Beyond the emergency: Centering mental health in community disaster preparedness and recovery.

BACKGROUND: Extreme weather events are increasing in frequency and severity, disproportionately affecting marginalized and under‑resourced communities. In Houston, Texas, where insurance coverage and mental health access remain limited, successive disasters compound existing inequities, leaving residents with unmet psychological needs. Yet little is known about how chronically impacted communities conceptualize their mental health experiences, barriers to accessing care, and priorities for support following repeated disaster events. METHODS: We conducted a qualitative study using town hall-st

Extreme Heat Exposure and Blood Pressure in Community Health Center Patients.

IMPORTANCE: Extreme heat has well-documented cardiovascular effects, yet its acute association with blood pressure (BP) in routine clinical settings remains incompletely characterized, particularly in large, diverse outpatient populations. OBJECTIVE: To assess the association between extreme heat exposure and BP among patients receiving care at community health centers (CHCs). DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study was conducted using electronic health record data from a national network of 2554 CHCs across 40 US states during the summer months (June through August) of

Telemedicine Use and Hypertension Control in an Urban Community Health Center Cohort.

BACKGROUND: Real-time or synchronous telemedicine can be a valuable adjunctive strategy for chronic disease management, but few studies have assessed its impact on hypertension control among safety-net populations. OBJECTIVE: To evaluate whether telemedicine is associated with blood pressure (BP) control. DESIGN: Retrospective cohort study. Mixed-effects logistic regression models clustered by the patient estimated associations between telemedicine and BP control after adjusting for patient factors and neighborhood context. PARTICIPANTS: Patients seeking care at an urban, multisite communit

Integration of a Community Opioid Treatment Program Into a Federally Qualified Health Center.

OBJECTIVES: With the increasing rates of opioid overdose deaths in the United States, barriers to treatment access for patients seeking medications for opioid use disorder (OUD), and challenges of initiating buprenorphine in patients who use fentanyl, it is essential to explore novel approaches to expanding access to methadone treatment. An opioid treatment program (OTP) and a federally qualified health center (FQHC) partnered to develop and implement an innovative integrated methadone and primary care treatment model. The process for integrating an OTP and FQHC to provide methadone treatment

Addressing mpox at a Frontline Community Health Center: Lessons for the Next Outbreak.

The 2022 mpox (formerly monkeypox) outbreak affected predominantly men who have sex with men (MSM), likely through sexual transmission, which resulted in institutions specializing in sexual health being at the frontlines of the mpox outbreak. Fenway Health in Boston serves close to 10 000 MSM annually, which includes more than 2400 MSM who are living with HIV and 3320 MSM with active HIV preexposure prophylaxis (PrEP) prescriptions. We report on the programs implemented and changes to clinical practice at Fenway Health during the mpox outbreak. Fenway Health diagnosed its first case of mpox in

Colorectal cancer screening among a sample of community health center attendees.

To determine the rate of colorectal cancer screening in patients attending a sample of community health centers, medical records of 1,176 patients from eight community health centers were abstracted. Among the patients studied, 43.8% of patients had undergone at least one of the three colorectal screening tests (fecal occult blood test, colonoscopy, or flexible sigmoidoscopy) in the recommended interval. Colorectal screening in this community health center population was predicted by male gender, being African American, older age, having a greater number of chronic illnesses, a family history

Programmatic Effectiveness of Latent Tuberculosis Care Cascade in a Community Health Center.

Our aim was to quantify successful completion of steps of the latent tuberculosis (LTBI) care cascade in a community health center (CHC) with a large foreign-born population. We conducted a retrospective cohort study of individuals with a positive test for tuberculosis (TB) infection from 2015 to 2017 at Baltimore Medical Systems (BMS). During the study period, 3,984 individuals were tested for TB. Of the 418 individuals with positive tests, 152 (36%) were referred elsewhere for care, whereas 266 (64%) were retained in care at BMS. Treatment was prescribed for 157 of the 214 (73%) diagnosed wi

Strengthening Screening for HIV, Hepatitis C, and STIs: An Innovative Partnership Between the Health Department and Community Health Centers in New York City.

In New York City, federally qualified health centers (FQHCs) are ideal partners for health departments because of their location in neighborhoods with high rates of HIV, hepatitis C virus (HCV) infection, and gonorrhea. Providers have experienced many barriers to following screening and treatment recommendations. In 2013, the New York City Department of Health and Mental Hygiene partnered with six FQHCs, representing 14 clinics, to make screening for HIV and HCV routine and increase adherence to gonorrhea treatment guidelines through education, electronic health record modification, and progre

Self-reported habitual snoring and health-related quality of life among adults attending community health centers in Shanghai, China: a cross-sectional study.

BACKGROUND: Habitual snoring is a common symptom in primary care but is often overlooked as a minor nuisance. Although sleep-disordered breathing has been associated with adverse health outcomes, the association between self-reported habitual snoring and health-related quality of life (HRQoL) remains insufficiently characterized among adults attending community health centers in China. This study examined the association of habitual snoring with physical and mental HRQoL in this setting. METHODS: We conducted a cross-sectional study among 2,312 adults recruited from community health centers i

An antibiotic stewardship initiative focused on community-acquired bacterial pneumonia (CABP) in outpatient clinics and urgent care centers: a 2023-2024 community health system experience.

OBJECTIVE: This before-after study aimed to evaluate whether an order-set intervention would improve CABP-guideline concordance among outpatients. SETTING: This study included adult patients presenting to outpatient clinics (n = 92) and urgent care centers (n = 39) within a community-based health system without a formal outpatient antibiotic stewardship program (ASP). INTERVENTION: The intervention consisted of an antibiotic order-set and awareness campaign. Patient encounters were identified via CABP ICD-10 codes and IDSA-relevant patient comorbidities (chronic heart, lung, liver, or renal

Impact of teleretinal screening program on diabetic retinopathy screening compliance rates in community health centers: a quasi-experimental study.

BACKGROUND: Diabetic retinopathy (DR) remains a leading cause of preventable blindness, with inadequate screening rates even in urban areas with high concentrations of medical professionals. While medical guidelines recommend annual diabetic retinopathy screening for patients with diabetes mellitus, adherence to these recommendations remains low. This study evaluates the impact of a novel teleretinal DR screening program on screening compliance across urban community health centers in Boston, Massachusetts. METHODS: We conducted a quasi-experimental study comparing DR screening compliance bet

Preventive Care and Chronic Disease Management Comparison of Appalachian and Non-Appalachian Community Health Centers in the United States.

INTRODUCTION: The Appalachian region is often characterized by poor health outcomes and economic depression. Health centers (HCs) are community-based and patient-directed organizations that deliver comprehensive, culturally competent, high-quality primary healthcare services in high need areas, including Appalachia, where economic, geographic, or cultural factors can hinder access to healthcare services. PURPOSE: The study compares the clinical quality performance in preventive care and chronic disease management between Appalachian HCs and their non-Appalachian counterparts. METHODS: Using

Patient randomized trial of a targeted navigation program to improve rates of follow-up colonoscopy in community health centers.

BACKGROUND: Colorectal cancer (CRC) screening by annual fecal immunochemical test (FIT) is an accessible and cost-effective strategy to lower CRC incidence and mortality. However, this mode of screening depends on follow-up colonoscopy after a positive FIT result. Unfortunately, nearly one-half of FIT-positive patients fail to complete this essential screening component. Patient navigation may improve follow-up colonoscopy adherence. To deliver patient navigation cost-effectively, health centers could target navigation to patients who are unlikely to complete the procedure on their own. OBJEC

Community-based mental health centers in Ukraine - protocol for a mixed methods evaluation study using the RE-AIM framework.

BACKGROUND: Russia's full-scale invasion of Ukraine has deepened the strain on an already fragile mental health system, long shaped by Soviet institutionalized psychiatry. In response, Ukraine has started a national reform to modernize mental health services by shifting from an institutional to a community-based mental health care model. Implementation of his transition is supported by the international technical project Mental Health for Ukraine (MH4U). AIMS: To generate evidence-based evaluation of the community-based mental health centers as a basis for continued improvement and to guide n

Opportunities and barriers for providing HIV testing through community health centers in mainland China: a nationwide cross-sectional survey.

BACKGROUND: Primary care may be an avenue to increase coverage of HIV testing but it is unclear what challenges primary healthcare professionals in low- and middle-income countries face. We describe the HIV testing practices in community health centres (CHCs) and explore the staff's attitude towards further development of HIV testing services at the primary care level in China. METHODS: We conducted a national, cross-sectional survey using a stratified random sample of CHCs in 20 cities in 2015. Questionnaires were completed by primary care doctors and nurses in CHCs, and included questions r
