Planning a medical relief mission.

Surveillance of drug consumption and disease incidence from medical relief missions in two locations--southeast Asia and eastern Africa--revealed close similarities. Analgesics, antibiotics, antimalarials, antihistamine or decongestant preparations (or both), bronchodilators, and scabicides were the 10 most commonly used drugs. Orthopedic, respiratory, gastrointestinal, dermatologic, and ophthalmic conditions, malaria, sexually transmitted diseases, and parasitic infestations were encountered most frequently. Recognition and early treatment of xerophthalmia, trachoma, and onchocerciasis, which

A case of chromomycosis treated by a combination of cryotherapy, shaving, oral 5-fluorocytosine, and oral amphotericin B.

A case of chromomycosis from Comoro Islands was first treated without success with high doses of oral amphotericin B (3 g per day). Treatment with itraconazole (400 mg per day) was also unsuccessful. Then, in vitro tests were done to study the susceptibility of this Fonsecaea pedrosoi strain to antifungal drugs. It was resistant to itraconazole, sensitive to 5-fluorocytosine, and the combination of 5-fluorocytosine with amphotericin B was synergistic. The patient was then treated with this last combination of drugs, which seemed to be effective. The patient stopped this treatment after six mon

Diagnosing psychopathy in an intercultural setting: Applications and implications in postcolonial contemporary Mayotte.

International scientific research has extensively studied psychopathy, but few studies focus on an intercultural and postcolonial context. Mayotte, a French overseas collectivity located in East Africa, offers a unique opportunity to study the application and effects of psychopathy diagnosis in the criminal justice field within a social context shaped by colonial legacy. This research uses a mixed-method approach, combining quantitative and qualitative data, to show that in Mayotte, the majority of individuals diagnosed with psychopathy are young, low-income individuals who act in groups. Rift Valley fever virus: A review of diagnosis and vaccination, and implications for emergence in Europe.

Rift Valley fever virus (RVFV) is a mosquito-borne virus, and is the causative agent of Rift Valley fever (RVF), a zoonotic disease characterised by an increased incidence of abortion or foetal malformation in ruminants. Infection in humans can also lead to clinical manifestations that in severe cases cause encephalitis or haemorrhagic fever. The virus is endemic throughout much of the African continent. However, the emergence of RVFV in the Middle East, northern Egypt and the Comoros Archipelago has highlighted that the geographical range of RVFV may be increasing, and has led to the concern

Poliovirus surveillance in Mayotte, Indian Ocean, reveals encephalomyocarditis virus type 1 and a wide diversity of non-polio enteroviruses.

BACKGROUND: In 2023, Mayotte, a French department in the Mozambique channel, experienced a long drought that led to potable water restrictions. Although the French vaccination schedule makes polio vaccination compulsory for children, the large proportion of migrants on the island coupled with the water crisis raised concerns about the establishment of poliovirus transmission chains. Therefore, a surveillance was implemented to detect polioviruses in sewage sampled in the two main wastewater treatment plants. METHODS: Samples collected from September 2023 through January 2024 were processed fo

Progress toward poliomyelitis eradication--Southern Africa, 2001-March 2003.

Since the 1988 World Health Assembly resolution to eradicate poliomyelitis globally, substantial progress has been made in all World Health Organization (WHO) regions, and three regions (Americas, European, and Western Pacific) are classified as polio-free. The African Region comprises four epidemiologic blocks (Central, Eastern, Southern, and Western). The Southern African block comprises 14 countries--10 on the mainland (Angola, Botswana, Lesotho, Malawi, Mozambique, Namibia, South Africa, Swaziland, Zambia, and Zimbabwe) and four in the Indian Ocean (Comoros, Madagascar, Mauritius, and Seyc

Discovery of hantavirus circulating among Rattus rattus in French Mayotte island, Indian Ocean.

Hantaviruses are emerging zoonotic viruses that cause human diseases. In this study, sera from 642 mammals from La Réunion and Mayotte islands (Indian Ocean) were screened for the presence of hantaviruses by molecular analysis. None of the mammals from La Réunion island was positive, but hantavirus genomic RNA was discovered in 29/160 (18 %) Rattus rattus from Mayotte island. The nucleoprotein coding region was sequenced from the liver and spleen of all positive individuals allowing epidemiological and intra-strain variability analyses. Phylogenetic analysis based on complete coding genomic se

Advancements in Rift Valley fever vaccines: a historical overview and prospects for next generation candidates.

Rift Valley fever (RVF) is a zoonotic viral disease transmitted by mosquitoes and causes abortion storms, fetal malformations, and newborn animal deaths in livestock ruminants. In humans, RVF can manifest as hemorrhagic fever, encephalitis, or retinitis. Outbreaks of RVF have been occurring in Africa since the early 20th century and continue to pose a threat to both humans and animals in various regions such as Africa, Madagascar, the Comoros, Saudi Arabia, and Yemen. The development of RVF vaccines is crucial in preventing mortality and morbidity and reducing the spread of the virus. While se

Evidence of Eretmapodites subsimplicipes and Aedes albopictus as competent vectors for Rift Valley fever virus transmission in Mayotte.

Rift Valley fever (RVF) recently re-emerged in Mayotte. We described, for the first time, that the mosquito species Eretmapodites subsimplicipes, a highly abundant species in Mayotte, is a competent vector for the transmission of RVF virus using three distinct populations native to Mayotte. We also showed that Aedes albopictus specimens are able to transmit RVF virus (RVFV) as previously observed in mosquito populations of other countries emphasizing the need of the increase vigilance for this highly invasive species of global distribution. Altogether, these results underline the epidemiologic

Acute Hemolytic Anemia Caused by G6PD Deficiency in Children in Mayotte: A Frequent and Severe Complication.

Severe hemolytic anemia is a rare complication of glucose-6-phosphate dehydrogenase (G6PD) deficiency. It occurs with the Mediterranean (Med) variant corresponding to a class 2 deficiency according to the World Health Organization (WHO) classification, and it correlates with a severe deficiency in G6PD activity. In Mayotte, the majority of patients have the African (A-) variant as a WHO class 3 deficiency. Yet we have observed numerous cases of severe hemolytic anemia defined by a hemoglobin level of <6 g/dL. In this study, we aimed to describe the epidemiological, clinical, and biological fea

BIOMEDICAL EVALUATION OF A BROWN LEMUR (EULEMUR FULVUS SPP.) POPULATION FROM MBOUZI ISLET, MAYOTTE ISLAND.

The brown lemur population (Eulemur fulvus spp.) in Mbouzi islet is not native, and was introduced in 1997. Since then, the population has grown. In 2012 the National Council for Protection of Nature of Mayotte requested to remove this population of lemurs from Mbouzi, as they were suspected to be a threat to the protected endemic flora of the islet. The Association Francophone des Vétérinaires de Parcs Zoologiques (French-speaking Zoo Veterinarians Association, AFVPZ) was asked to conduct a biomedical evaluation of the population. Fifty-two animals were captured, anesthetized, and weighed. Th

Shoshin beriberi and thiamine-responsive right heart failure: A case report in Mayotte Recognition and management of infant Shoshin beriberi.

Infant Shoshin beriberi is an acute life-threatening condition for which the diagnosis is frequently delayed. Therefore, rapid recognition of right heart failure with lactic acidemia is a crucial element in the diagnosis and therapeutic management. We present the case of a 2-month-old girl with bronchiolitis, right heart failure, and lactic acidosis, who quickly and favorably responded to thiamine supplementation. Thiamine deficiency was established through laboratory tests. We present a brief review of the literature with the different thiamine dosages proposed in emergencies and provide an e

[AIDS and HIV infection in the Federal Islamic Republic of Comores (1988-1995)].

Since 1991 AIDS and HIV infection in the Federal Islamic Republic of the Comoros have been monitored through a network of sentinel sites including predetermined target populations. In all cases seropositivity for HIV was confirmed by western blotting after two ELISA screening tests. AIDS was diagnosed on Bangui clinical criteria and positive western blotting results. As of June 30, 1996, the cumulative number of AIDS cases reported to the World Health Organization was 7 for a prevalence of 1.4 per 100000 inhabitants. The prevalence of HIV antibodies has remained at less than 1% in target popul

Prevalence of crt and mdr-1 mutations in Plasmodium falciparum isolates from Grande Comore island after withdrawal of chloroquine.

BACKGROUND: In Comoros, the widespread of chloroquine (CQ)-resistant Plasmodium falciparum populations was a major obstacle to malaria control, which led to the official withdrawal of CQ in 2004. Continuous monitoring of CQ-resistant markers of the P. falciparum CQ resistant transporter (pfcrt) and the P. falciparum multiple drug resistance 1 (pfmdr-1) is necessary inder to obtain first-hand information on CQ susceptibility of parasite populations in the field. The objective of this study is to assess the prevalence and evolution of CQ-resistance in the P. falciparum populations on the Comoros

Effectiveness and equity of vaccination strategies against Rift Valley fever in a heterogeneous landscape.

Spatio-temporal variations in environmental and socio-agricultural factors create heterogeneity in livestock disease transmission risk, raising challenges in identifying populations most at risk and how this risk changes over time. Consequently, effective vaccination strategies require careful planning to achieve optimal or equitable outcomes across regions. We developed a metapopulation model for Rift Valley fever transmission in livestock across the Comoros archipelago which incorporates livestock vaccination in addition to heterogeneity in viral transmission rates and animal movements. We u

The role of livestock movements in the spread of Rift Valley fever virus in animals and humans in Mayotte, 2018-19.

Rift Valley fever (RVF) is a vector-borne viral disease of major animal and public health importance. In 2018-19, it caused an epidemic in both livestock and human populations of the island of Mayotte. Using Bayesian modelling approaches, we assessed the spatio-temporal pattern of RVF virus (RVFV) infection in livestock and human populations across the island, and factors shaping it. First, we assessed if (i) livestock movements, (ii) spatial proximity from communes with infected animals, and (iii) livestock density were associated with the temporal sequence of RVFV introduction into Mayotte c

Modelling the persistence and control of Rift Valley fever virus in a spatially heterogeneous landscape.

The persistence mechanisms of Rift Valley fever (RVF), a zoonotic arboviral haemorrhagic fever, at both local and broader geographical scales have yet to be fully understood and rigorously quantified. We developed a mathematical metapopulation model describing RVF virus transmission in livestock across the four islands of the Comoros archipelago, accounting for island-specific environments and inter-island animal movements. By fitting our model in a Bayesian framework to 2004-2015 surveillance data, we estimated the importance of environmental drivers and animal movements on disease persistenc
