A distributed fusion framework for breast cancer recurrence prediction using MapReduce.

Breast cancer recurrence remains a major clinical challenge, significantly influencing long-term survival and treatment planning. Accurate early prediction is hindered by heterogeneous clinical factors, imbalanced datasets, and the distributed nature of medical records stored across hospitals, registries, and laboratories. To address these challenges, this study proposes a MapReduce-aligned hybrid framework that combine with distributed Spark-based Gradient Boosted Trees, denoising autoencoder (AE)-derived latent representations, calibrated XGBoost, and deep tabular framework (FT-Transformer a

In the SARS-CoV-2 Pandora Pandemic: Can the Stance of Premorbid Intestinal Innate Immune System as Measured by Fecal Adnab-9 Binding of p87:Blood Ferritin, Yielding the FERAD Ratio, Predict COVID-19 Susceptibility and Survival in a Prospective Population Database?

SARS-CoV-2 severity predictions are feasible, though individual susceptibility is not. The latter prediction allows for planning vaccination strategies and the quarantine of vulnerable targets. Ironically, the innate immune response (InImS) is both an antiviral defense and the potential cause of adverse immune outcomes. The competition for iron has been recognized between both the immune system and invading pathogens and expressed in a ratio of ferritin divided by p87 (as defined by the Adnab-9 ELISA stool-binding optical density, minus the background), known as the FERAD ratio. Associations w

A Unified Framework for Statistical Inference and Study Design of Comparative F 1 and F β Scores under Paired Classifier Evaluations.

Background and objective The F 1 score and its generalized F β score are widely used to evaluate machine learning and artificial intelligence (AI) models in healthcare, particularly for imbalanced clinical datasets. In practice, competing prediction models are commonly evaluated on the same patient cohort, resulting in correlated classifier decisions. However, existing approaches for statistical inference of F 1 -related metrics typically assume independent classifier decisions or lack integrated procedures for comparative evaluation, power analysis, and sample size determination in paired val

A Stage IA and IB Endometrial Cancer Classification Method Using Genetic Programming on MRI Images.

BACKGROUND: Early diagnosis and accurate staging of endometrial cancer (EC) are crucial for effective treatment planning. Distinguishing stage IA from stage IB EC is challenging due to large variations in tumor and uterine morphology, as well as the limited availability of annotated magnetic resonance imaging (MRI) data for training robust models. METHODS: A genetic programming (GP)-based framework was developed for the classification of FIGO stage IA and IB EC using a small number of MRI images. The framework consists of three main components: (1) automatic detection of regions of interest (

Multi-Scale Residual Gated Attention U-Net for Liver Tumor Segmentation.

Accurate early diagnosis and treatment planning of liver cancer relies on precise liver and tumor segmentation in medical images. However, liver tumor segmentation faces complex anatomical structures, large lesion scale differences, and fuzzy lesion boundaries. Vanilla U-Net has three inherent structural drawbacks: inadequate cross-scale feature interaction in the encoder, semantic mismatch in skip connections, and insufficient multi-scale feature aggregation in the decoder. Therefore, this paper proposes a Multi-scale Residual Gated Attention U-Net (MRGA-UNet) for liver tumor segmentation. Th

An nnU-Net-based framework with adaptive feature representation for 3D brain tumor segmentation.

BACKGROUND: Accurate segmentation of glioma subregions from multimodal magnetic resonance imaging (MRI) is essential for diagnosis, treatment planning, and response assessment, but remains challenging because of boundary ambiguity, heterogeneous appearance, and small enhancing tumor (ET) components. This study aimed to develop and evaluate a controlled nnU-Net v2-based framework for three-dimensional (3D) brain tumor segmentation (BraTS) by improving adaptive feature representation and boundary-aware learning while preserving the reproducibility of the self-configuring nnU-Net pipeline. Challenges in implementing optimal perinatal regionalization: a scoping review.

BACKGROUND: Perinatal regionalization is a healthcare model designed to ensure that high-risk mothers and newborns receive care in facilities equipped with appropriate expertise and technology. While evidence supports its effectiveness in improving outcomes, the implementation of regionalized systems remains inconsistent and vulnerable to a range of clinical, structural, and financial barriers. This scoping review aimed to identify, categorize, and synthesize the reported challenges that hinder the effective functioning of perinatal regionalization systems across diverse settings. METHODS: A

Results of esketamine administration in a Greek population; A case series.

Esketamine is a non-selective, competitive antagonist of the N-methyl-D-aspartate (NMDA) receptor in the brain. Through NMDA receptor antagonism, esketamine causes a transient increase in glutamate release, leading to increases in neurotrophic signaling and restoration of synaptic function in brain regions involved in mood regulation and emotional behavior. Several randomized clinical trials have shown its effectiveness in reducing the symptoms of depression in some people, despite its short-term side effects that include mainly disorientation, dizziness, nausea, and increased blood pressure.

Aortic root disease in athletes: aortic root dilation, anomalous coronary artery, bicuspid aortic valve, and Marfan's syndrome.

Two professional athletes in the U.S. National Basketball Association required surgery for aortic root dilation in 2012. These cases have attracted attention in sports medicine to the importance of aortic root disease in athletes. In addition to aortic root dilation, other forms of aortic disease include anomalous coronary artery, bicuspid aortic valve, and Marfan's syndrome. In this review, electronic database literature searches were performed using the terms "aortic root" and "athletes." The literature search produced 122 manuscripts. Of these, 22 were on aortic root dilation, 21 on anomalo

Clinical monograph for drug formulary review: erectile dysfunction agents.

BackgroundSignificant advances in the pharmacologic treatment of erectile dysfunction (ERD) have occurred in recent years, most notably the introduction of sildenafil, the first oral selective phosphodiesterase type 5 (PDE5) inhibitor, in 1998. Sildenafil quickly gained acceptance by the medical community and the public because of its broad efficacy for different types of ERD and its ease of use. Two PDE5 inhibitors, vardenafil and tadalafil, have since joined sildenafil to compete in the ERD market. A review was conducted by the Drug Information Service of a pharmacy benefits manager (PBM) to
