A multi-mode shock tube for investigation of blast-induced traumatic brain injury.

Blast-induced mild traumatic brain injury (bTBI) has become increasingly common in recent military conflicts. The mechanisms by which non-impact blast exposure results in bTBI are incompletely understood. Current small animal bTBI models predominantly utilize compressed air-driven membrane rupture as their blast wave source, while large animal models use chemical explosives. The pressure-time signature of each blast mode is unique, making it difficult to evaluate the contributions of the different components of the blast wave to bTBI when using a single blast source. We utilized a multi-mode s

Ocular tear film bubble formation after breath-hold diving.

Bubbles in the ocular tear film have been observed following both dry-chamber, simulated compressed air dives and in-water, recreational compressed air dives. The current paper reports on the formation of tear film bubbles in a breath-hold diver following repeated, extended breath-hold excursions to a maximum depth of -28.5 m. It is believed that this is the first time that ocular tear film bubbles have been reported in breath-hold divers.

Dysbaric disorders: aseptic bone necrosis in tunnel workers and divers.

Dysbaric osteonecrosis is a serious complication for those exposed to a hyperbaric environment, with prevalence of 17% amongst compressed air workers and 4.2% amongst divers. Bone lesions are characteristically multiple and bilateral, occurring frequently in the shafts of the femora or tibiae and the heads of the humeri or femora. A proportion of the lesions will lie next to the joint surface, the so called juxta-articular lesion, and these may progress to a structural failure and secondary osteoarthritis. These lesions can be severely disabling, especially in a young adult male. When related

Air Nozzle Injury: Barotrauma Resulted From an Industrial Accident.

Industrial accidents involving compressed air can lead to significant colonic injuries, ranging from minor tears to complete perforations. This study investigates a case of colonic barotrauma in a 40-year-old male oil refinery worker who suffered symptoms of lower abdominal discomfort, distension, and tenderness following the application of compressed air to his anus. Diagnostic tests, including blood count, abdominal X-ray, and ultrasonography, indicated fecal impaction, dilated bowel loops, and free gas under the diaphragm. An exploratory laparotomy revealed a 4 cm x 2 cm hole in the colon a

Massive subcutaneous emphysema and pneumomediastineum after finger subtotal amputation with barotrauma.

BACKGROUND: Advancement of pressurized air through subcutaneous tissue after barotraumas involving skin laceration has been documented in the literature. The type and anatomic location of injury, amount of pressure, and time elapsed all play a role in determining the destination of the air advancing through tissues. OBJECTIVES: To report a case demonstrating the vascular system as the anatomic pathway for subcutaneous pressurized air resulting from an industrial accident. CASE REPORT: We present the case of a 28-year-old laborer wounded by an air valve blast. An enormous accumulation of air

Postmortem intravascular bubbling: a decompression artifact?

The relationships between drowning occurring in divers using compressed air, with subsequent recovery of the body to the surface, and the finding of intravascular gas bubbles at autopsy were studied. Guinea pigs were exposed to compressed air in a hyperbaric chamber at various depths [98, 294, and 392 kPa (1,3, and 4 atm)] for various intervals and then drowned at depth. Various decompression schedules were used in returning the drowned animals to the surface. Autopsy examination indicated that large and small intravascular gas bubbles may be found, in the absence of traumatic air embolism or

Advanced high pressure hyperbaric techniques in tunnelling.

Work in compressed air and diving are both occupational activities that have been around since the mid-19th century, and those undertaking their work under elevated pressure. Meeting the demand to go to "higher pressure for longer" in tunneling has lagged in diving, but both activities have found it necessary to adopt mixed gas breathing and saturation exposure techniques. This paper explains how work in hyperbaric conditions at high pressure is undertaken in tunneling and is illustrated by the hyperbaric activity likely to be involved in constructing a large-diameter road tunnel below a body

Influence of environmental conditions in bovine bone ablation by ultrafast laser.

Ultrafast lasers are promising tools for surgical applications requiring precise tissue cutting. Shallow ablation depth and slow rate as well as collateral damage are common barriers limiting the use of laser in clinical applications. Localized cooling with water and/or air jet is known to reduce collateral thermal damage. We studied the influence of environmental conditions including air, compressed air flow, still water and water jet on ablation depth, ablation rate and surface morphology on bovine bone samples with an 800 nm femtosecond laser. At 15 J/cm2 , no thermal effect was observed by

Brain death of an infant caused by a penetrating air gun injury.

Air guns are shooting projectiles (pellets) from the expansion of compressed air without involving any chemical reactions. Air guns are often regarded as harmless by the public, but these weapons can produce severe, sometimes lethal injuries, especially in children. A case of a penetrating head injury of an eleven-month-old infant, caused by an ordinary air gun pellet, is presented. The air gun pellet created an exceptionally severe brain injury, and brain death. The factors influencing the severity of air gun injuries - muzzle velocity, muzzle energy, skull thickness - are discussed.

Surgical management of a patient with traumatic tension pneumorachis: A rare case report.

INTRODUCTION: Pneumorachis is a rare underdiagnosed phenomenon in which air enters the spinal canal mostly following injury to the respiratory system. PRESENTATION OF CASE: We presented a rare case of pneumorachis associated with neurological deficits following traumatic compressed air insufflation to the rectum. A 44-year-old, male, construction laborer presented to the accident and emergency department with a history of insufflation by compressed air through his rectum, as a prank by his workmates. Imaging studies revealed pneumoperitoneum and the exploratory laparotomy showed perforation o

The effect of jet parameters on jet injection.

Current jet injection devices often utilize compressed air or springs to create a high-pressure fluid jet capable of piercing the skin. However, these devices are limited to a single invariable injection profile based on the impulse created by the compressed air or spring and therefore the parameters that affect injection are not well understood. To determine the effect of injection parameters on jet injection into tissue, including the effect of the fluid ejection profile on the injection, a controllable jet injection device was used to perform experiments into sheep and pig tissue. This pape

Pneumomediastinum or lung damage in breath-hold divers from different mechanisms: a report of three cases.

Normally pulmonary over-inflation is not an issue during breath-hold diving, in contrast to lung squeeze. Compared with compressed air diving, pulmonary barotrauma is rare in breath-hold diving. Several mechanisms can lead to an increase in intrathoracic pressure in breath-hold diving that may cause alveolar rupture. Here we report three cases of pulmonary barotrauma in breath-hold diving. Using high-resolution chest tomography, bullous damage in Case 1, and pneumomediastinum in Cases 2 and 3 were detected. Transient neurological symptoms in Cases 1 and 2 suggested cerebral arterial gas emboli

Nonpneumonic, short-incubation-period Legionellosis (Pontiac fever) in men who cleaned a steam turbine condenser.

Pontiac fever affected ten men who had cleaned a steam turbine condenser with compressed air. Previous epidemics of Pontiac fever and Legionnaires' disease--both caused by Legionella Pneumophila (proposed sp. nov.)--involved "airborne spread" from air-conditioning cooling towers or evaporative condensers. Aerosols of contaminated water in heat-rejection systems appear to be important sources of epidemic legionellosis.

Estimation of gas-phase diffusivities in hyperbaric environments.

Diffusion of a particular gas in a mixture of three or more gases depends on diffusion characteristics and concentrations of the other gases and also on environmental pressure. 1) Estimates of gas-phase diffusivities in hyperbaric environments can be calculated from binary coefficients by the Wilke equation. Sample calculations show that addition of carbon dioxide and water to inspired gas has very little effect on diffusivity of oxygen but that neglect of lesser components of a mixture, such as the nitrogen in "trimix" or the helium in crude neon, would lead to errors of 10% to 20%. 2) It is

Medical Management and Risk Reduction of the Cardiovascular Effects of Underwater Diving.

Undersea diving is a sport and commercial industry. Knowledge of potential problems began with Caisson disease or "the bends", first identified with compressed air in the construction of tunnels under rivers in the 19th century. Subsequently, there was the commercially used old-fashioned diving helmet attached to a suit, with compressed air pumped down from the surface. Breathhold diving, with no supplementary source of air or other breathing mixture, is also a sport as well as a commercial fishing tool in some parts of the world. There has been an evolution to self-contained underwater breath

Effect of hyperbaric oxygenation on peripheral nerve regeneration in adult male rabbits.

Oxygen environments were used to study the regenerative effects of hyperbaric oxygen on crushed sciatic nerves in 30 adult male rabbits. Six different oxygen environments were used, and treatments were initiated 4 days post injury. Transmission electron microscopy and light microscopy were used to evaluate the regenerative morphology of crushed nerves. The morphology of crushed nerves after 7 wk of treatment with compressed oxygen at 202, 242, and 303 kPa resembled normal uncrushed nerves, with nerve fibers uniformly distributed throughout the section. The treatment groups receiving 202 kPa co

Pneumatic colonic rupture accompanied by tension pneumoperitoneum.

Rupture of the colon caused by high pressure compressed air is a rare, unique and traumatic intra-abdominal injury. As the use of compressed air in industrial work has increased, so has the risk of associated pneumatic injuries from its improper use. Recently we experienced a case of pneumatic rupture of the sigmoid colon accompanied by tension pneumoperitoneum, which caused respiratory distress. The patient's respiration was very rapid with the rate of 44 breaths per minute. On arterial blood gas analysis, pH was 7.40, pO2 68 mmHg, pCO2 44 mmHg, and SaO2 90%. Chest X-ray film showed marked pn

Efficacy of flexible endoscope drying using novel endoscope test articles that allow direct visualization of the internal channel systems.

BACKGROUND: Thorough drying of flexible endoscope channels has been identified as an essential reprocessing step. Yet, instructions are not specific on how to dry endoscopes. There is lack of data supporting efficacy of current drying practices, due to limitations in determining channel dryness. METHODS: Novel endoscope test articles were used to evaluate the effectiveness of alcohol flush and hanging in an ambient endoscope storage cabinet. Prepared test articles were hung in a storage cabinet for 5 days and visually inspected for residual liquid. The procedure for preoperative inspection of

Effect of alpha-adrenergic blockade on exercise-induced asthma and conditioned cold air.

Cooling and drying of the intrapulmonary airways have been shown to be important stimuli for the development of bronchospasm induced by exercise and isocapnic cold air hyperventilation. It has also been suggested that alpha-adrenergic receptor activity is increased at lower temperatures. To evaluate the role of alpha-adrenergic activity in the development of bronchoconstriction during airway cooling, we examined the effects of alpha-adrenergic blockade with phentolamine on bronchospasm induced by exercise and isocapnic cold air hyperventilation in 8 asthmatics. Exercise consisted of 6 min of s

Exaggerated perception of normal physiological responses to stress and hypercapnia in young women with numerous functional somatic symptoms.

OBJECTIVE: This study tested whether functional somatic symptoms are associated with exaggerated increases in self-reported anxiety and somatic complaints in response to stress and CO(2)-enriched air breathing, and whether this association exists in parallel to or in the absence of exaggerated physiological responses. METHODS: Out of 499 young somatically healthy undergraduate women, 18 participants high in functional somatic symptoms (HSS group) and 18 participants low in symptoms (LSS) were selected. They were submitted to mental stress, mild physical exercise and relaxation during conditio
