Relationship between consciousness level and perfusion computed tomography in patients with prolonged disorders of consciousness.

PURPOSE: We assessed the relationship between consciousness level and values of cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit time (MTT), and time to peak (TTP) obtained by whole-brain perfusion computed tomography (pCT) in patients with prolonged disorders of consciousness (pDOC). METHODS: This study included 29 patients in vegetative state (VS), 34 with minimally consciousness state minus (MCS-), and 13 with minimally consciousness state plus (MCS+). All patients were evaluated using the Coma Recovery Scale-Revised (CRS-R), the Glasgow Coma Scale (GCS), and the Full O

Consciousness Disorders and Swallowing Difficulties.

With the advancement of medicine, the survival rate of patients with brain injuries has significantly increased. Among these patients, some remain in a chronic state of altered consciousness and experience difficulties in swallowing. Swallowing difficulties can lead to clinical issues such as aspiration and malnutrition. Since swallowing function is closely related to the level of consciousness, effective oral swallowing is an early sign of consciousness recovery. The relationship between swallowing difficulties and the prognosis and quality of life of patients with consciousness disorders is

Acute Onset of Impaired Consciousness.

BACKGROUND: Mortality in patients with acute onset of impaired consciousness is high: as many as 10% do not survive. The spectrum of differential diagnoses is wide, and more than one underlying condition is found in one-third of all cases. In this article, we describe a structured approach to patients with acute onset of impaired consciousness in the emergency department. METHODS: This review is based on pertinent articles retrieved by a selective search of PubMed and on the AWMF guidelines on the most common causes of impairment of consciousness. RESULTS: Impairments of consciousness are cl

[Consciousness disorders from cardiological view].

Consciousness disorders may have many causes, mainly cardiac arrhythmias. The incidence of bradyarrhythmias (BA) in patients with acute coronary syndrome (ACS) is 0.3-18 % and caused by sinus node dysfunction (SND), high degree atrioventricular (AV) block or bundle branch blocks. SND are sinus bradycardia or sinus arrest. 1st degree AV-block occurs in 4-13 % of patients with ACS caused by rhythm disturbances in atrium, AV node, bundle of His or the Tawara system. 1st or 2nd degree AV block is seen very frequently within 24 hours after beginning of ACS and these arrhythmias are frequently trans

Infectious diseases and impaired consciousness.

Any of a number of neuroinfectious diseases can cause a disorder of consciousness. The priority in the care of the patient is to identify an infectious disease that is treatable. This article examines disorders of consciousness that may be caused by a septic encephalopathy, bacterial meningoencephalitis, viral encephalitis, tick-borne bacterial disease, fungal meningitis, tuberculous meningitis, a focal infectious mass lesion, such as a brain abscess, or an autoimmune-mediated disorder as a complication of infection.

Differential engagement of thalamic nuclei orchestrates consciousness states across anesthesia, sleep, and disorders of consciousness.

Consciousness alterations occur across various states, encompassing changes in both arousal and awareness. The contributions of thalamic nuclei of consciousness remain incompletely understood. We analyzed fMRI data across general anesthesia (propofol N = 12, sevoflurane N = 12), disorders of consciousness (DoC) (minimally conscious state N = 9, unresponsive wakefulness syndrome N = 9), and sleep stages (wakefulness N = 31, NREM1 N = 24, NREM2 N = 19), investigating thalamocortical connectivity, local fluctuation (complexity/variability), and their coupling. In our results, propofol affected pu

Queering critical consciousness: Measurement and implications of critical consciousness among sexual and gender minority youth.

Developmental research on critical consciousness (CC) offers insight into processes that empower youth to challenge oppressive realities. This concept has been examined predominantly among low income Black and Latinx youth. We consider how CC is relevant to sexual and gender minority (SGM) youth from different racial/ethnic backgrounds with the goal of expanding CC research. Leveraging data from 823 youth in Massachusetts, we conducted factor analytic and measurement invariance analyses across three groups of interest: straight Black and Latinx youth (N=254); SGM Black and Latinx youth (N=111)

A review of the sufficient conditions for consciousness.

The neuroscience of consciousness has set out to find the sufficient conditions for subjective experience. Theoretical and empirical endeavours have placed a particular focus on the cortex and subcortex, whilst discounting the cerebellum. However, when looking at neuroimaging research, it becomes clear there is substantial evidence that cerebellar, cortical, and subcortical functions are correlated with consciousness. Neurostimulation evidence suggests that alterations in both the cortex and the cerebellum may provoke alterations in experience, however experimental stimulation of the subcortex

Frontal Disorders and Consciousness: A Review.

PURPOSE OF REVIEW: The frontal cortex plays a key role in many functions considered to be relevant for consciousness. Furthermore, altered consciousness appears to exist in those with disorders that disrupt or damage frontal areas and/or networks. This article reviews recent research discussing the impact of frontal disorders on consciousness. RECENT FINDINGS: Many theories of consciousness have been developed over the last century to help explain the neural correlates of consciousness. Some theories argue that the frontal cortex is necessary for consciousness, while others argue that posteri

Functions of consciousness in emotional processing.

Contrary to the leading theories of consciousness on offer, it is a fruitful working hypothesis that conscious experiences facilitate a variety of functional capacities that are distinct to particular psychological tasks, individuals, and species (i.e., functional pluralism). In this paper, I illustrate this novel methodological point by identifying some of the functional contributions that consciousness makes to (human) emotional processing. I first consolidate empirical evidence of the capacities and limitations of unconscious emotional processing, drawing on a) experimental paradigms that e

Arousal and Consciousness in Focal Seizures.

Impaired consciousness during seizures severely affects quality of life for people with epilepsy but the mechanisms are just beginning to be understood. Consciousness is thought to involve large-scale brain networks, so it is puzzling that focal seizures often impair consciousness. Recent work investigating focal temporal lobe or limbic seizures in human patients and experimental animal models suggests that impaired consciousness is caused by active inhibition of subcortical arousal mechanisms. Focal limbic seizures exhibit decreased neuronal firing in brainstem, basal forebrain, and thalamic

Loss of consciousness in a little traveler.

BACKGROUND: Loss of consciousness in children can be caused by a wide spectrum of factors, including infection, metabolic disorders, trauma, and poisoning which requires timely and accurate evaluation. CASE PRESENTATION: In this paper, we introduce a three-year-old boy who was first referred to the Emergency ward of Mashhad Imam Reza Hospital due to unconsciousness. Having spent a few days in a hotel, this boy, who was a visitor to Mashhad, lost consciousness. During evaluations, hypotension and severe high anion gap metabolic acidosis was observed. Finally, the patient was diagnosed with eth

Origin and evolution of human consciousness.

The study of the origin and evolution of consciousness presents several problems. The first problem concerns terminology. The word consciousness comes from the Latin term conscĭentĭa that means "knowledge shared with others." However, the term consciousness also refers to several other aspects involving both its levels (sleep, coma, dreams and waking state) and contents (subjective, phenomenal and objective). A second issue is the problem of other minds, namely, the possibility to establish whether others have minds very like our own. Moreover, human consciousness has been linked to three diff

Impaired Consciousness in the Emergency Department.

BACKGROUND: The approach to unconscious patients in the emergency department (ED) is difficult, often depends on local resources and interests, and workup strategies often lack standardization. One reason for this is that data on causes, management, and outcome of patients who present to the ED with sudden onset unconsciousness of unknown cause is limited. OBJECTIVES: This study was performed to analyze the causes of acute impaired consciousness in patients in an interdisciplinary ED. METHODS: Here, we analyzed all patients who were admitted to the ED of a tertiary care hospital with the dom

An algorithmic information theory of consciousness.

Providing objective metrics of conscious state is of great interest across multiple research and clinical fields-from neurology to artificial intelligence. Here we approach this challenge by proposing plausible mechanisms for the phenomenon of structured experience. In earlier work, we argued that the experience we call reality is a mental construct derived from information compression. Here we show that algorithmic information theory provides a natural framework to study and quantify consciousness from neurophysiological or neuroimaging data, given the premise that the primary role of the bra

Life and consciousness - The Vedāntic view.

In the past, philosophers, scientists, and even the general opinion, had no problem in accepting the existence of consciousness in the same way as the existence of the physical world. After the advent of Newtonian mechanics, science embraced a complete materialistic conception about reality. Scientists started proposing hypotheses like abiogenesis (origin of first life from accumulation of atoms and molecules) and the Big Bang theory (the explosion theory for explaining the origin of universe). How the universe came to be what it is now is a key philosophical question. The hypothesis that it c
