Repetitive transcranial magnetic stimulation and constraint-induced movement therapy combined in the treatment of post-stroke movement disorders: a narrative review.

Stroke is a significant cardiovascular and cerebrovascular condition and is among the primary causes of prolonged neurological impairment globally. Approximately 55%-75% of stroke survivors will experience some form of long-term sensorimotor impairment. Post-stroke, the upper limb typically exhibits restricted mobility, complicating daily chores for 70% of patients and impairing normal limb utilization. Repetitive Transcranial Magnetic Stimulation (rTMS), a prominent non-invasive neuromodulation technique designed to enhance functional recovery post-stroke, has garnered significant attention i

Effect of Modified Constraint-Induced Movement Therapy on Upper Limb Function in Children with Hemiplegic Cerebral Palsy.

To assess the effectiveness of modified constraint-induced movement therapy (mCIMT) in improving upper limb function and grip strength in children with hemiplegic cerebral palsy (CP). A comprehensive search was conducted from inception to August 2024. Eligibility criteria were studies evaluating the effectiveness of mCIMT on upper limb function in children with hemiplegic CP aged over 2 years. The following data was extracted from each study: participant characteristics, intervention, outcome measures, follow-up, and key findings. The risk of bias and the quality of the evidence were evaluated

Brain Stimulation and Constraint Induced Movement Therapy in Children With Unilateral Cerebral Palsy: A Randomized Controlled Trial.

BACKGROUND: There is a crucial need to devise optimum rehabilitation programs for children with cerebral palsy (CP). OBJECTIVE: This study aimed to assess the feasibility, safety, and efficacy of combining 6-Hz primed, low-frequency, repetitive transcranial magnetic stimulation (rTMS) with modified constraint-induced movement therapy (mCIMT) in improving upper limb function in children with unilateral CP. METHODS: Children aged 5 to 18 years with unilateral CP were randomized (23 in each arm) to receive 10 sessions of mCIMT with real rTMS (intervention arm) or mCIMT with sham rTMS (control a

Combination of early constraint-induced movement therapy and fasudil enhances motor recovery after ischemic stroke in rats.

PURPOSE: Constraint-induced movement therapy (CIMT) is a promising technique for the recovery of upper extremity movement in chronic stroke patients. However, the effectiveness of its use in acute ischemia has not been confirmed. Myelin-associated inhibitors, which have upregulated functions in tissues affected by acute focal infarction, limit axonal regeneration via activation of the Rho-Rho-associated protein kinase (ROCK) pathway. The present study examined whether early CIMT combined with the ROCK inhibitor fasudil promotes motor recovery after acute ischemic stroke. MATERIALS AND METHODS

INCITE: A randomised trial comparing constraint induced movement therapy and bimanual training in children with congenital hemiplegia.

BACKGROUND: Congenital hemiplegia is the most common form of cerebral palsy (CP) accounting for 1 in 1300 live births. These children have limitations in capacity to use the impaired upper limb and bimanual coordination deficits which impact on daily activities and participation in home, school and community life. There are currently two diverse intensive therapy approaches. Traditional therapy has adopted a bimanual approach (BIM training) and recently, constraint induced movement therapy (CIMT) has emerged as a promising unimanual approach. Uncertainty remains about the efficacy of these int

Multisite trial on efficacy of constraint-induced movement therapy in children with hemiplegia: study design and methodology.

OBJECTIVE: In the past decades, several treatment approaches have been used to improve upper limb function in hemiplegic cerebral palsy. Only recently has constraint-induced movement therapy emerged as a treatment approach for children with hemiplegic cerebral palsy with the aim of reversing the behavioral suppression of movement in the affected upper limb. To date, evidence on this treatment has been very poor and limited, because all currently available trials reveal methodological limitations and a need for additional research to support the application of this treatment technique. Effect of constraint-induced movement therapy with modified opposition restriction orthosis in chronic hemiparetic patients with stroke.

OBJECTIVES: Constraint-induced movement therapy (CIMT) has been demonstrated to be effective in improving hemiparetic upper extremity function in stroke patients, but few studies have been performed to assess orthosis modification. We investigated the effect of the newly designed small orthosis named modified opposition restriction orthosis (MORO) in chronic hemiparetic patients with stroke. DESIGN: Twenty-one stroke patients were randomly assigned to the CIMT group or control group. Thirteen patients in the CIMT group wore MORO confining the thumb and index finger for at least 5 hours of eac

Constraint-induced movement therapy in the treatment of the upper limb in children with hemiplegic cerebral palsy.

BACKGROUND: Children with hemiplegic cerebral palsy learn strategies to manage daily tasks (for example play) using one hand and often the affected limb is disregarded or not used. Constraint-induced movement therapy (CIMT) is emerging as a treatment approach for use with children with hemiplegic cerebral palsy. It aims to increase spontaneous use of the affected upper limb and thereby limit the effects of developmental disregard. CIMT is based on two fundamental principles: constraint of the non-affected limb and massed practice of therapeutic tasks with the affected limb. OBJECTIVES: The ob

[Constraint-induced movement therapy in neurological rehabilitation: which modality to choose? Our experience in rehabilitation in Geneva].

OBJECTIVE: The purpose of constraint-induced movement therapy (CIMT) is to stimulate the use of the affected upper arm following stroke and to improve the integration of the arm in activities of daily living. CIMT consists of restraining the unaffected arm to limit its use, combined with a training program based on the shaping principle. We aimed to explore how this method can be used in practice by studying 4 patients after stroke. METHOD: We investigated 4 patients, 2 with chronic disability (10 and 17 months after the stroke), and 2 with subacute disability (1 month after the stroke). Pati

Health-related quality of life following lower-extremity constraint-induced movement therapy after stroke: an intervention study.

INTRODUCTION: The aim of this study was to explore potential effects of lower extremity Constraint-Induced Movement Therapy (LE-CIMT) on Health-Related Quality of Life (HRQoL) after stroke, and the possible influence of descriptive characteristics. MATERIALS AND METHODS: Twenty men and 10 women (median age 53 years, IQR 45-59) participated in a single-arm, intervention study at an outpatient physiotherapy clinic. Participants underwent intensive, task-specific LE-CIMT for 6 h/day over 10 consecutive weekdays, followed by a home-exercise program until a 3-month follow-up. HRQoL was assessed us

Effects of a Distributed Form of Constraint-Induced Movement Therapy for Clinical Application: The Keys Treatment Protocol.

BACKGROUND/AIM: Currently, there are limited evidence-based protocols for improving upper extremity (UE) motor function after stroke. The Keys protocol, a distributed form of constraint-induced movement therapy (CIMT), delivers CIMT components in fewer hours per day over an extended period, fitting outpatient rehabilitation schedules and third-party payor models. This pilot study aimed to assess the effectiveness of the Keys protocol in enhancing UE capacity and performance poststroke. METHODS: Ten adults with chronic stroke (>6 months) participated in an 8-week intervention. The protocol inc

Constraint-induced movement therapy promotes motor recovery after neonatal stroke in the absence of neural precursor activation.

Neonatal stroke is a leading cause of long-term disability and currently available rehabilitation treatments are insufficient to promote recovery. Activating neural precursor cells (NPCs) in adult rodents, in combination with rehabilitation, can accelerate functional recovery following stroke. Here, we describe a novel method of constraint-induced movement therapy (CIMT) in a rodent model of neonatal stroke that leads to improved functional outcomes, and we asked whether the recovery was correlated with expansion of NPCs. A hypoxia/ischemia (H/I) injury was induced on postnatal day 8 (PND8) vi
