Continuous glucose monitoring and Rituximab treatment in insulin autoimmune syndrome.

Insulin autoimmune syndrome (IAS) or Hirata's disease is a rare cause of hypoglycemia. It is characterized by hyperinsulinemic hypoglycemia, elevated insulin autoantibody titers, no prior exposure to exogenous insulin and no pathological abnormalities of pancreatic islets. Hypoglycemia usually occurs in the post prandial and post absorptive state. Most cases of IAS are self-limiting, with resolution of symptoms within six months to one year. In intractable cases, treatment modalities include low-carbohydrate meals; acarbose; diazoxide; glucocorticoids; immune-suppressants like Azathioprine, cy

Accuracy of Continuous Glucose Monitoring Measurements in Normo-Glycemic Individuals.

BACKGROUND: The validity of continuous glucose monitoring (CGM) is well established in diabetic patients. CGM is also increasingly used for research purposes in normo-glycemic individuals, but the CGM validity in such individuals is unknown. We studied the accuracy of CGM measurements in normo-glycemic individuals by comparing CGM-derived versus venous blood-derived glucose levels and measures of glycemia and glycemic variability. METHODS: In 34 healthy participants (mean age 65.7 years), glucose was simultaneously measured every 10 minutes, via both an Enlite® CGM sensor, and in venous blood

Radiographic appearances of a continuous glucose monitor in a patient with lipodystrophy.

We report the case of a 50-year old woman with a known history of lipodystrophy. A pelvic radiograph was taken for the investigation of right hip pain. The image shown demonstrates an indeterminate artefact projected over the right iliac fossa. A previous CT renal study was reviewed, demonstrating the same device in the subcutaneous tissues of the contralateral left lower quadrant which on close inspection was consistent with a continuous glucose monitoring device. Features of lipodystrophy were also noted on review of the CT imaging. Although many devices such as vagal stimulators and prosthe

Roadmap to the Effective Use of Continuous Glucose Monitoring in Pregnancy.

The landscape for managing type 1 diabetes during pregnancy has been transformed by increasing use of continuous glucose monitoring (CGM). Women are aiming for pregnancy-specific glucose targets or 70% time in range for pregnancy (TIRp; 63-140 mg/dL) as soon as possible, knowing that every extra 5% TIRp has benefits for reducing the risks of complications in their babies. Ongoing monitoring of maternal A1C (at pregnancy confirmation and at 20, 28, and 36 weeks' gestation) remains useful. Intensification of glycemic management and instruction in using CGM (if not already used) is recommended fo

The Use of Continuous Glucose Monitoring in Comparison to Self-Monitoring of Blood Glucose in Gestational Diabetes: A Systematic Review.

BACKGROUND: Continuous glucose monitoring (CGM) has emerged as an important tool for managing gestational diabetes mellitus (GDM), offering real-time glucose data and the potential for improved glycemic control. Unlike traditional self-monitoring of blood glucose (SMBG), which provides intermittent readings, CGM captures continuous glucose fluctuations, including postprandial and nocturnal changes, which are critical in GDM management. OBJECTIVE: This systematic review aimed to assess the effectiveness of CGM compared with SMBG in managing glycemic control in women with GDM, focusing on key g

Continuous glucose monitoring in kidney transplant recipients: a narrative review.

Continuous glucose monitoring (CGM) has transformed diabetes management, offering real-time and dynamic insights into glucose variability and addressing the limitations of traditional glucose assessment methods. Kidney transplantation, the most common solid organ transplant, carries a considerable burden of post-transplant diabetes mellitus (PTDM), which is linked to increased cardiovascular events, graft dysfunction, and increased mortality. This review explores the role of CGM in kidney transplant recipients, particularly its impact on glycemic profiles and its predictive value for post-tran

Prediction of gestational diabetes mellitus using continuous glucose monitoring metrics.

AIMS: We evaluated continuous glucose monitoring (CGM)-derived metrics for predicting gestational diabetes mellitus (GDM). METHODS: We analyzed data from 167 pregnant participants who had ≥ 3 days of CGM data at 18-24 weeks' gestation and who underwent 75-gram oral-glucose-tolerance-tests at 24-28 weeks in a multi-ethnic prospective cohort. Predictive performance of CGM metrics was assessed using the area-under-the-receiver-operating-characteristic-curve (AUROC) with 20 repetitions of 5-fold cross-validation; optimal cut-points were determined using Youden's index. RESULTS: There were 30 (18

Continuous glucose monitoring in gestational diabetes mellitus: hope or hype?

The screening, diagnosis and management of gestational diabetes mellitus (GDM) have evolved over time. Similarly, continuous glucose monitoring (CGM) has advanced in parallel: from intermittent to real-time monitoring, shorter to longer wear-time and poor to increased precision. Thus, CGM has replaced multiple glucose fingerstick testing in type 1 diabetes mellitus (T1D), including pregnant women with T1D. Despite its advantages, the uptake of CGM in women with GDM remains limited. Doubts persist over its cost, accuracy and optimal utilisation, besides there is paucity of evidence supporting i

Intra-pancreatic fat is associated with continuous glucose monitoring metrics.

AIM: To investigate the relationship of fat in the pancreas with time spent in different glycaemic ranges. METHODS: Abdominal magnetic resonance imaging at 3.0 Tesla was used to quantify fat in the pancreas as both continuous [i.e. intra-pancreatic fat deposition (IPFD)] and binary (i.e. fatty change of the pancreas vs. normal pancreas) variables. Dexcom G6 devices were used to collect continuous glucose monitoring data every 5 min over a continuous 7-day period. Time above range (TAR), time in range (TIR) and time below range were computed. Statistical models were built to adjust for age, se

Near-Infrared Spectroscopy and Continuous Glucose Monitoring During Therapeutic Hypothermia.

The relation between glucose homeostasis and cerebral blood flow (CBF) and their correlation to outcome in neonatal hypoxic-ischemic encephalopathy are unclear. In this short communication, we tried to determine whether changes in regional oxygen saturation (rSO2), as measured by near-infrared spectroscopy (NIRS), in asphyxiated neonates during therapeutic hypothermia correlate with the glycemic profile and whether NIRS and continuous glucose monitoring are useful in identifying cooled asphyxiated neonates at high risk of brain injury. Although there was no correlation between blood glucose an

Continuous glucose monitoring for children with hypoglycaemia: Evidence in 2023.

In 2023, childhood hypoglycaemia remains a major public health problem and significant risk factor for consequent adverse neurodevelopment. Irrespective of the underlying cause, key elements of clinical management include the detection, prediction and prevention of episodes of hypoglycaemia. These tasks are increasingly served by Continuous Glucose Monitoring (CGM) devices that measure subcutaneous glucose at near-continuous frequency. While the use of CGM in type 1 diabetes is well established, the evidence for widespread use in rare hypoglycaemia disorders is less than convincing. However, i

Toward an Optimal Definition of Hypoglycemia with Continuous Glucose Monitoring.

BACKGROUND AND OBJECTIVE: As continuous glucose monitoring (CGM) becomes common in research and clinical practice, there is a need to understand how CGM-based hypoglycemia relates to hypoglycemia episodes defined conventionally as patient reported hypoglycemia (PRH). Data show that CGM identify many episodes of low interstitial glucose (LIG) that are not experienced by patients, and so the aim of this study is to use different PRH simulations to optimize CGM parameters of threshold (h) and duration (d) to provide the best PRH detection performance. METHODS: The algorithm uses particle Markov

Integrating Continuous Glucose Monitoring Into Clinical Practices and Patients' Lives.

Numerous studies have demonstrated the clinical benefits of continuous glucose monitoring (CGM) in individuals with diabetes. Within ongoing innovations in CGM technology, individuals now have an expanding array of options that allow them to select the device that meets their individual needs and preferences. Although demand for CGM in primary care continues to grow, many clinicians are reluctant to prescribe this technology due to their unfamiliarity with the various devices, uncertainty about which devices are best suited to each patient and the feasibility of using CGM. Continuous Glucose Monitoring in Women with Normal OGTT in Pregnancy.

Continuous glucose monitoring (CGM) might be an effective tool to improve glycemic control in gestational diabetes mellitus (GDM). Few data are available about its utilization as a diagnostic tool to find potential alterations of glycemia in subjects with normal oral glucose tolerance test (OGTT). In this preliminary prospective real-life observational study, we aimed to analyze the glycemic pattern in normal and gestational diabetes mellitus (GDM) women by continuous glucose monitoring (CGM) in order to detect potential differences between the two groups and glycemic alterations despite a nor
