Peripheral effects of sex steroids: implications for patient management.

Sex steroids--estrogens, progesterone, and testosterone and other androgens--have effects on the anatomy and physiology of many nonreproductive organ systems. These changes can influence the sex ratio of certain diseases. They also affect the "normal range" for certain parameters, and data from men often do not apply to normal female physiology. Sex steroids also affect the response to certain toxins, pharmacologic agents, and other therapies. Replacement sex steroid therapy and oral contraceptives may affect these processes differently than natural hormones do. The normal monthly changes of f

An appraisal of strategies to reduce the incidence of breast cancer.

The current focus of breast cancer research is to develop a novel strategy to prevent the disease. In this review a potential model of breast cancer development is proposed based upon the results of laboratory models of the induction of mammary carcinogenesis. It is clear that susceptibility to initiation occurs in young female animals, and a preventive strategy is more effective the sooner it is started after initiation occurs. In humans we do not know the timing or the nature of the carcinogenic insult, but epidemiologic studies suggest that the process is long and initiation is most likely

Prophylactic contraceptives for HIV/AIDS.

The current pandemic of sexually transmitted human immunodeficiency virus (HIV) infection--the causative agent of acquired immunodeficiency syndrome (AIDS), has created an urgent need for a new type of contraceptive: one that is both a spermicide and a microbicide. Because most women at risk for HIV infection are of reproductive age (15-44 years), effective use of dual-function contraceptives is important to prevent HIV transmission and unintended pregnancies. In the absence of an effective prophylactic anti-HIV therapy or vaccine, new emphasis has been placed on the development of intravagina

Current contraceptive research.

No contraceptive panacea is near or likely. The most exciting prospect, man-made analogs of LHRH (a brain hormone) for both male and female use, could not be available before the late 1980s. Other potential methods in human trials face serious obstacles.

Noncontraceptive effects of hormonal contraceptives: bone mass, sexually transmitted disease and pelvic inflammatory disease, cardiovascular disease, menstrual function, and future fertility.

Noncontraceptive actions of oral contraceptives (OCs) include hormonal effects on bone mass, sexually transmitted disease, pelvic inflammatory disease, the cardiovascular system, menstrual function, and future fertility. Information about the effects of OCs on bone mass is limited and contradictory. Two recent studies failed to show a positive effect of OC use on bone mass or density compared with the three cross-sectional studies and one longitudinal study that showed favorable effects. A recently completed study of 156 healthy females indicated a significant correlation between OC use and th

Acute Pancreatitis in a Young Patient With Type 2 Diabetes Mellitus Taking Sitagliptin and the Combined Oral Contraceptive Pill.

Acute pancreatitis is a common cause of hospital admission; however, cases secondary to medication use are rare. We report a case of drug-induced mild acute pancreatitis in a 33-year-old female with type 2 diabetes mellitus. The patient presented with a one-day history of vomiting and lower chest and epigastric pain after five months of starting sitagliptin therapy. She was also taking the combined oral contraceptive pill. Clinical examination revealed epigastric tenderness and signs of dehydration. Laboratory findings showed elevated white cell count, C-reactive protein, and triglyceride leve

Recent advances in contraception.

The world population is expected to increase by 2.6 billion to 9.1 billion in 2050. This will occur, if fertility decreases from today's 2.6 children to about 2 children per woman. If fertility were to remain at the present level, 34 million persons would be added annually by mid-century and thus the world population would reach 10.6 billion by 2050. The most notable increase in the world population will occur in third world countries. Therefore immense investments are being made to develop safe, reliable and easily used contraceptive methods. It has not proven easy. Further, acceptance of the

[Contraception during perimenopause: The spermicides option].

AIM: In perimenopause, the increased cardiovascular and cancer risk and the increased incidence of gynaecologic disorders have a major impact on the choice of an appropriate contraceptive method. The aim of this review is to assess the potential benefit of spermicides at this stage of woman's life. MATERIALS AND METHODS: This review presents the eligibility criteria of the different contraceptive methods according to national and international guidelines, together with a literature review of the efficacy and side-effects of the two major spermicidal preparations, benzalkonium chloride and non

'Most women really actually do just appreciate being asked': clinicians' views on integrating sexual wellbeing into contraceptive care.

A growing body of literature suggests that contraceptives' impact on sexual experiences influences willingness to initiate and continue with a method. Little is known, however, about how clinicians engage with clients on this topic during contraceptive consultations. Fifteen clinicians in South Eastern Australia participated in semi-structured interviews between April and June 2019. Interviews were analysed using inductive, semantic thematic analysis. Participants varied in their evaluation and management of contraceptives' sexual side effects and the legitimacy given to women's bodily experie
