Therapeutic thoracentesis is an invasive procedure performed to relieve symptoms in patients with large pleural effusions identified on diagnostic imaging. Untreated effusions can lead to respiratory compromise or hemodynamic instability. Factors such as poor patient compliance, worsening disease progression, or limited outpatient follow-up availability may increase the need for therapeutic thoracentesis in the emergency department (ED). This article provides a comprehensive overview of the procedure with point-of-care ultrasound (POCUS) guidance and discusses indications, contraindications, a

Subclavian vein catheterization is safe as long as proper indications and contraindications are observed. In the hands of highly qualified personnel, subclavian vein catheterization has a minimum rate of complications. Pneumothorax is by far the most common complication of the procedure. We describe a 64-year-old man who developed pneumothorax late after left subclavian vein catheterization.

PURPOSE OF REVIEW: Migraine is a common disorder which in many cases can be adequately treated with medications. However, there are some patients who may either not respond to medications or have contraindications to their use. In this review, we will evaluate the available literature on the interventional procedures available to treat patients with episodic migraine. We will review the technical details of performing the procedures, the potential mechanisms of action, and available data on their effectiveness.
RECENT FINDINGS: Recent studies conducted on the subject of interventional procedu

The present study describes a new reconstructive technique that can be performed after pancreaticoduodenectomy, modified Child's with intestinal pouch, and with pancreaticogastrostomy. This new reconstructive procedure was developed, and termed it "Oida's intestinal pouch reconstruction after modified Child's procedure and pancreaticogastrostomy" with the aim of obtaining a sufficient volume of the remnant stomach after antrectomy with pancreaticoduodenectomy in order to prevent delayed gastric emptying in cases wherein pylorus-preserving pancreaticoduodenectomy is contraindicated. Lingual and labial frenulectomy are commonly performed as an outpatient procedure, either in an office setting or under general anesthesia. Frenulectomy is generally regarded by both otolaryngologists and dentists as a straightforward and low-risk procedure with limited evidence-based indications and similarly few contraindications. We describe two cases of hypovolemic shock occurring after outpatient frenulectomy requiring emergent interventions of cardiopulmonary resuscitation and blood transfusion. These rare, but life-threatening outcomes warrant recognition as potential complications for

The Camitz procedure is a simple tendon transfer for patients who have loss of opposition in longstanding carpal tunnel syndrome. Historically, the Camitz procedure restored abduction only. However, through modification with the use of a pulley, the ideal axis of opposition can be achieved. The purpose of this article was to revisit an established method of the modified Camitz transfer in 6 patients with severe thenar wasting and weakness resulting from median nerve compression, identified between 2010 and 2012. We review the indications, contraindications, surgical technique, complications, a

The results of treatment of 249 patients aged 15 to 78 years with perforated gastroduodenal ulcers were analyzed. The study group consisted of 128 patients in whom video-laparoscopic procedure was used for suturing of ulcer, control group -- of 107 patients treated conventionally. Conversion of laparoscopic procedure to open surgery was in 14 cases. It is demonstrated that the intensity of pain syndrome, hospital stay and the number of postoperative complications in the study group were less that in the control group. Indications and contraindications for video-laparoscopic method in these pat

Free microvascular flap transfer is indicated in situations in which flap coverage is required and no local flap is available. The technique of transfer of a groin flap based on the superficial circumflex iliac vessels was described. The procedure has been applied to two patients with acute electrical burns with extensive soft-tissue loss, three patients with scar contractures, and one with osteoradionecrosis. Satisfactory results were achieved in all patients. The procedure is contraindicated if recipient or donor vessels are abnormal or inadequate, and in the face of fulminating infection, g

The results of 32 patients who underwent transcranial monobloc frontofacial advancement by the senior author (Wolfe) are examined. Fifteen of the patients were Crouzon's and 17 were Apert's, 14 of whom also underwent simultaneous facial bipartition. The ages ranged from 7 months to 14 years. Although a transcranial facial advancement carries with it substantial risks, with careful consideration of airway control, the anterior cranial base dura, and the retrofrontal dead space, the procedure is recommended for carefully selected patients. The indications and contraindications for the procedure

At the inception of a general practice well child clinic, a checklist card was introduced into the clinic notes to summarize specific and relative contraindications to immunizations. This card was used by the practice nurses as they ran the immunization procedures during the clinic. A failure on the checklist led to a consultation with the clinic doctor who decided whether to proceed with the immunization. Of 155 immunizations given during the six-month period, only 23 (15%) failed the checklist and required the child to be assessed by the clinic doctor. Of these, nine (39%) were for simple up

A highly successful transvaginal bladder neck suspension for female stress urinary incontinence is described and illustrated. Precise lateral placement of sutures, maximum mobilization of the anterior vaginal wall and bladder neck, and ease of approach are keys to the technique. The procedure can be utilized in any setting with minimal complications and with neither obesity nor multiple previous surgeries as contraindications.

BACKGROUND: Left atrial appendage (LAA) thrombus is a contraindication for LAA closure (LAAC). However, in selected cases, oral anticoagulants are strictly contraindicated because of a history of life-threatening bleeding, and LAAC remains the only possible therapy to avoid systemic and especially cerebral embolization.
CASE SUMMARY: We report a case of LAAC despite a massive proximal thrombus in a patient who had an absolute contraindication to anticoagulant therapy, with thorough pre-planning using CT scan, device modelling and thrombus trapping techniques to reduce the risk of systemic emb

Transjugular liver biopsy (TJLB) is widely used in adults when percutaneous liver biopsy (PLB) is contraindicated. The authors evaluated the safety, efficacy and utility of TJLB in pediatric patients with contraindications to PLB. Twenty-three children and adolescents (13 male) underwent 24 (one patient had two separate biopsies) consecutive biopsies (TJLB) under general anaesthesia (18) or intravenous sedation (6). PLB was contraindicated because of elevated prothrombin time (PT)/international normalized ratio (INR) (˃ 15.5/1.5) in 19 (79.2%) and thrombocytopenia (< 60,000/dL) in 15 (62.5%) p

OBJECTIVE: Cervical disc arthroplasty (CDA) has emerged as a motion-preserving treatment for symptomatic cervical disc disease refractory to nonoperative management in carefully selected patients. Given the interest in expanding on-label uses to include procedures such as hybrid arthroplasty-fusion constructs and 3- and 4-level arthroplasty, authors of this study sought to define the contemporary prevalence of absolute and relative contraindications to cervical arthroplasty.
METHODS: The authors conducted a retrospective observational cross-sectional study of all patients in whom > 6 weeks of

Percutaneous closure of the left atrial appendage may be indicated in patients with contraindications to anticoagulation therapy, for example, after recurrent gastrointestinal bleeding. It is an effective and safe procedure but is not without complications. We present a patient who presented with severe aortic insufficiency due to migration of the left atrial appendage closure device, which required urgent cardiac surgery for its removal.

OBJECTIVES: Transjugular intrahepatic portosystemic shunt (TIPS) insertion is an established treatment to lower portal pressure. There are no obligatory evidence-based recommendations addressing procedure and anticoagulation. Therefore, a survey was performed to establish current practice at different German hospitals.
METHODS: A three-page survey was sent out via postal mail to 76 different hospitals addressing the topics indication, contraindication, follow-up and anticoagulation.
RESULTS: Forty-three hospitals completed the survey: the median number of TIPS/year was 28.6 ± 23. Ascites and
