Spasm of the near reflex triggered by disruption of normal binocular vision.

A 26-year-old healthy female was referred by her optometrist to the binocular vision clinic of our institution for the investigation of an accommodative spasm occurring during monocular conditions. Corrected binocular visual acuity was 20/20 (6/6), with normal pupils and good ocular alignment. When the fellow eye was covered, visual acuity was <20/200 (6/60) in each eye, miosis was present in both eyes, and the occluded eye was in esodeviation, indicating a spasm of the near reflex. The spasm disappeared when a translucent occluder was used instead of an opaque black occluder. Further investig

OculoMotor Assessment Tool Test Procedure and Normative Data.

SIGNIFICANCE: This study establishes normative data and a testing procedure for the oculomotor assessment tool. The oculomotor assessment tool standardizes visual targets for the Vestibular/OculoMotor Screening assessment and provides additional metrics that may aid in the differentiation between those with normal and those with abnormal oculomotor function potentially caused by a concussion. PURPOSE: This study aimed to assess the oculomotor endurance of healthy participants with no self-reported history of concussions using the oculomotor assessment tool. METHODS: Healthy participants (n =

Objectively-based vergence and accommodative dynamics in mild traumatic brain injury (mTBI): A mini review.

Vergence and accommodation have been critical areas of investigation in those with mild traumatic brain injury (mTBI). In this mini-review, the major laboratory studies in the area using objective assessment of vergence and accommodative dynamics in this population are discussed. These include the basic findings, their diagnostic and therapeutic implications, potential study limitations, and suggested future research directions. All studies provided important new information, and insights, into the area. There were two key outcomes of the reviewed studies common to both the vergence and accomm

Effect of 3-Dimensional Central Stimuli on Near Point of Convergence.

AIM: Since an interaction between binocular disparity and the vergence system takes place in order to reduce retinal disparity, it is likely that vergence control, as measured with the near point of convergence (NPC), will be better if the target used for measurement has 3-dimensional (3-D) features in its most central parts. The aim of the present study was therefore to investigate if the NPC would give a better result using a fixation target with centrally placed 3-D features when compared with a two-dimensional (2-D) target. METHODS: Twenty-three asymptomatic subjects (2 men, 21 women; ave

Paralysis of the near-vision triad in a child.

The near-vision triad, or complex, consists of convergence, miosis, and accommodation. Neuronal pathways that control each of these components are distinct but interrelated. Abnormalities affecting 1 or more components of the complex may present as eye pain, headache, blurred vision, or diplopia at near fixation. Although isolated abnormalities in any one of the components are common, a severe and concurrent defect in all three is rare.(1,2) We describe an 11-year-old child who presented with complete paralysis of the near triad without identifiable neurological defect. The child benefited fro

Spasm of the near reflex in a patient with multiple sclerosis.

A 33-year-old woman with a history of relapsing-remitting multiple sclerosis presented with complaints of episodic horizontal diplopia and blurred vision over the past two months. The patient gave a past history of similar episodes of diplopia that had occurred with previous clinical relapses of her multiple sclerosis. On eye movement examination, the patient exhibited intermittent spasms of convergence coupled with miosis. On upward gaze, the convergence and miosis became more marked. An auto-refraction revealed variable measurements in both eyes, consistent with accommodative spasms. A cyclo

Accommodation Responds to Optical Vergence and Not Defocus Blur Alone.

PURPOSE: To determine whether changes in wavefront spherical curvature (optical vergence) are a directional cue for accommodation. METHODS: Nine subjects participated in this experiment. The accommodation response to a monochromatic target was measured continuously with a custom-made adaptive optics system while astigmatism and higher-order aberrations were corrected in real time. There were two experimental open-loop conditions: vergence-driven condition, where the deformable mirror provided sinusoidal changes in defocus at the retina between -1 and +1 diopters (D) at 0.2 Hz; and blur-driven

Postural control in dyslexic and non-dyslexic children.

Postural control relies to visual-motion processing (afferent or efferent) and this is thought to be deficient in dyslexics. There is a controversy between clinic and fundamental studies as to the presence of posture abnormalities in dyslexics. To explore further this issue, this study examines posture stability in quite stance in 13 dyslexics (mean age: 13.5 years) and in 13 non-dyslexics (mean age: 13 years). Experiment 1 shows that, similarly to adults and elderly, all children (dyslexics and non-dyslexics), present better stability at near distance (i.e. smaller surface area of the COP, sm

Reduced vergence adaptation is associated with a prolonged output of convergence accommodation in convergence insufficiency.

Convergence insufficiency (CI) is a developmental visual anomaly defined clinically by a reduced near point of convergence, a reduced capacity to view through base-out prisms (fusional convergence); coupled with asthenopic symptoms typically blur and diplopia. Experimental studies show reduced vergence parameters and tonic adaptation. Based upon current models of accommodation and vergence, we hypothesize that the reduced vergence adaptation in CI leads to excessive amounts of convergence accommodation (CA). Eleven CI participants (mean age=17.4±2.3 years) were recruited with reduced capacity

Heterophoria and fusional convergence and divergence in preschool children.

AIM: To determine the prevalence of heterophoria, fusional convergence and divergence values and the degree of relationship between heterophoria and fusional vergence in preschool children. METHODS: For assessment of far and near heterophoria in 152 children aged 6-7, cover and prism cover tests were used. Far and near fusional vergences were measured by step vergence testing using prism bars. RESULTS: Orthophoria (-tropia) with the average values of heterophoria around 0 PD (prism diopter) were most common at far, and exophoria around 2 PD at near. The average break values of far fusional c

The influence of the vergence system on strabismus diagnosis and management.

The vergence system is an important element in human eye movement control. It comprises horizontal, vertical, and torsional components, the largest of which is convergence. Vergence performance is largely involuntary, although voluntary effort can influence convergence. Vergence function can be damaged by trauma or organic disease and, because convergence can be exerted voluntarily, it is susceptible to purposeful subjective disturbances. Vergence function is important in the maintenance of binocular control, being synonymous with motor fusion. The most common vergence dysfunction affects conv

[A case of post-infectious wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) syndrome].

The patient is a 72-year-old man who had tonsillitis and underwent incision on March 6, 1998. He complained headache and nausea from March 10 and double vision from March 12. He was admitted to our hospital on March 14. Neurological examination on March 18 revealed bilateral internuclear ophthalmoplegia with exotropia (WEBINO), bilateral ptosis and disturbance of convergence. Lumbar puncture revealed moderately elevated protein (46 mg/dl) with normal pressure and cells. The T2-weighted images of head MRI showed multiple high intensity lesions in anterior commissure and right pons. After he was

Using evidence-based psychological approaches to accommodation anomalies.

INTRODUCTION: Accommodation anomalies are frequently caused or exacerbated by psychological problems such as anxiety. Patients share many features with those with other anxiety based somatic symptoms such as stomach-ache, palpitations and headaches. They can be difficult to treat, and the ophthalmic literature rarely goes beyond diagnosis and ocular treatment. This study reports characteristics and outcomes of a short case series of patients with accommodation spasms and weaknesses assessed objectively, and outlines a psychological approach to treatment. METHODS: 23 patients (13 severe accomm

Objective estimation of fusional reserves using infrared eye tracking: the digital fusion-range test.

CLINICAL RELEVANCE: Horizontal fusional reserves are used in the diagnosis and monitoring of common vergence disorders, such as convergence insufficiency, which can cause asthenopia and impact near work. Infrared eyetracking technology shows promise for obtaining automated and objective measurements of fusional reserves, expanding options for screening, clinical testing, and at-home monitoring/vision training. BACKGROUND: Current clinical tests for fusional reserves rely on subjective judgements made by patients (for diplopia) and clinicians (for eye movements). This paper describes an object

Vision-related symptoms, accommodative and binocular vision performance in young diabetics vs. normal controls.

PURPOSE: To compare accommodative and binocular vision performance between young diabetic subjects and normal controls, and to investigate the correlation of accommodative/binocular indices with the severity of diabetes. METHODS: Thirty young subjects with diabetes mellitus (DM) and 30 age-matched normal controls were recruited in this hospital-based cross-sectional study. DM was diagnosed by a haemoglobin A1c (HbA1c) higher than 6.5%. The status of vision-related symptoms was examined by the convergence insufficiency symptoms survey (CISS). All participants underwent a complete optometric ex

Convergence substitution in pontine tegmental hemorrhage: an observation with Frenzel goggles.

We report a case in which convergence movement was substituted for bilateral horizontal gaze palsy due to a pontine tegmental hemorrhage. When visual fixation was suppressed with Frenzel goggles (FG), substituted convergence movements were prominent in our patient. The observation suggests that both vergence eye movements and horizontal gaze movements act synergistically and lesioning of the version system results in overactivity of the convergence system after a pontine hemorrhage. Furthermore, it may be worth using FG in the observation of substituted eye movements in patients with horizonta

Nonaccommodative factors of refractive accommodative esotropia.

The clinical characteristics of 45 cases of refractive accommodative esotropia were compared with 31 cases of the control group who remained orthotropic in the presence of uncorrected same degree of hypermetropia. In addition to the general ophthalmologic and orthoptic examinations, the accommodative convergence/accommodation (AC/A) ratio, fusional vergences and random dot stereopsis were investigated. The hypermetropic patients without esotropia were found to have a lower AC/A ratio and normal fusional function, while in cases with refractive accommodative esotropia, their AC/A ratios were no
