Percutaneous cervical cordotomy and subarachnoid phenol block using fluoroscopy in pain control of costopleural syndrome.

We examined the efficacy of percutaneous cervical cordotomy (PCC) and subarachnoid phenol block using fluoroscopy (SAPB-F) for control of chest and/or back pain from costopleural syndrome. The efficacy of each block was evaluated by changes in pain score (PS), analgesic dose and performance status 1 week after the block, as well as by the complications. Between 1980 and 1986, PCC was performed in 10 patients. SAPB-F was performed in 13 patients between 1987 and 1991. Pain was not well controlled by analgesics in any of these patients. For PCC the follow-up period was 94.7 +/- 71.1 days. PS (VA

[The effects of CO2 laser posterior cordotomy on quality of life in patients with bilateral vocal cord paralysis].

OBJECTIVES: In this study, we evaluated the effects of CO2 laser posterior transverse cordotomy on the quality of life in patients with bilateral vocal cord paralysis (BVCP). PATIENTS AND METHODS: Between April 2009 and October 2011, 14 patients with BVCP who underwent unilateral CO2 laser posterior cordotomy were included. The functional outcome of the procedure was assessed by the changes in the subscales of the Quality of Life Survey (SF-36). Postoperative glottic space and voice quality were evaluated using an arbitrary classification system. RESULTS: There was a statistically significan

Transoral Flexible Laser Posterior Transverse Cordotomy with Blue Laser: A Case Describing a New Surgical Approach.

The posterior transverse cordotomy (PTC) can be performed for posterior glottic stenosis (PGS) in the operating room through suspension laryngoscopy. This procedure requires adequate exposure of the vocal cords. An alternative PTC approach was carried out on a 56-year-old man without a view of the vocal cords during suspension laryngoscopy. After anteriorizing the hypertrophic tongue with a glide scope, the surgeon reached the vocal folds through a flexible nasofibroscopy with an operative channel. The adequate exposure of the posterior part of the vocal cords allowed the blue laser PTC throug

Bilateral high-level percutaneous cervical cordotomy in cancer pain due to lung cancer: a case report.

BACKGROUND: Computed tomography-guided high-level percutaneous cordotomy has been used unilaterally or bilaterally for the treatment of localized intractable pain in malignancies. CASE DESCRIPTION: A 57-year-old man was admitted to the hospital with the complaint of intractable pain involving the left side of the chest, axillary region, and shoulder. He was operated for small cell lung cancer on the left side in December 2003 and received radiotherapy and chemotherapy. His neurological examination was normal. Magnetic resonance imaging of the thorax revealed contrast-enhancing lesions on the

Aerodynamic, acoustic and functional results of posterior transverse laser cordotomy for bilateral abductor vocal fold paralysis.

Bilateral abductor vocal fold paralysis (BAVFP) is a rare but life-threatening condition which may require an emergency tracheotomy procedure. The ideal surgical technique for this condition should improve quality of life by relieving the airway obstruction while preserving laryngeal functions such as phonation and deglutition. Posterior transverse laser cordotomy (PTLC) was first described by Dennis and Kashima as a technique for providing an airway at the posterior glottis without pre-operative tracheotomy; they reported it as a successful method with satisfactory functional results. The aim

Contralateral approach for ipsilateral target: Stimulation-guided percutaneous cordotomy for refractory cancer pain - A case report.

Percutaneous cervical cordotomy is an advanced neuroablative technique used in the management of medically refractory cancer pain, particularly in patients with unilateral body or limb pain. The procedure is based on targeting the spinothalamic tract (STT) on the side contralateral to the patient's pain, guided by clinical findings and topographic anatomy. However, standardized and reliable guidance on how to interpret ipsilateral sensory responses that may occur during intraoperative stimulation remains limited in the current literature. In this case report, we discuss an ipsilateral (pain-si

Cordotomy for pain control and opioid reduction in cancer patients: A cancer center 11-year experience.

BACKGROUND: Percutaneous lateral cervical cordotomy (PLCC) is a treatment option for predominantly nociceptive pain of oncological origin that is refractory to conservative methods, with unilateral distribution, particularly in the lower trunk or lower limbs of patients with a life expectancy of less than one year. OBJECTIVE: The aim of this study was to assess the analgesic efficacy and opioid utilization alteration in patients undergoing PLCC. METHODS: We retrospectively collected data from patients undergoing PLCC between 2011 and 2021 at the AC Camargo Cancer Center in São Paulo, Brazil.

Use of local mitomycin C in enhancing laryngeal healing after laser cordotomy: a prospective controlled study.

BACKGROUND: The purpose of this study was to investigate the role of intraoperative application of mitomycin C in prevention of glottic restenosis after posterior transverse CO2 laser cordotomy (PTLC) for patients with post-thyroidectomy bilateral vocal fold paralysis. METHODS: Twenty-five patients with an impaired airway because of bilateral vocal fold paralysis were treated with PTLC. Patients were divided into groups: the mitomycin C group (13 patients) had PTLC and topical mitomycin C; and the control group (12 patients) had PTLC only. RESULTS: No patients in the mitomycin C group develo

Laser arytenoidectomy and posterior cordotomy in a patient with bilateral vocal cord paralysis due to multiple system atrophy.

Bilateral vocal cord paralysis leading to stridor is a known but rare complication of Parkinson's disease (PD) and a recognised complication of multiple system atrophy (MSA). Tracheostomy is a commonly offered treatment, leading to substantial adaptations and lifestyle changes for the patient. Patients can struggle to manage a tracheostomy due to the tremor and bradykinesia associated with their parkinsonism. We report a case of bilateral vocal cord paralysis leading to significant stridor in a patient with atypical parkinsonism (probable MSA). To avoid tracheostomy, our patient underwent succ

[Spino-thalamic cordotomy in cancerous pain. Results of a series of 124 patients operated on by the direct posterior approach].

The authors -- about a series of 124 cancerous patients treated during the 12 last years with open spino-thalamic cordotomy for intractable pain -- have tried to evaluate effectiveness of the operation with regard to its levels in relation to the site of pain. Patients suffering median or bilateral perineo-pelvic pain, isolated or associated with algias in one or both legs (group I: 50%) underwent a bilateral C8-C6 cordotomy in one stage. Patients with the same perineo-pelvic cancers but suffering only unilateral pain (group II : 31,8%) and patients with painful cancers in the leg (group III :

Percutaneous computed tomography-guided transdiscal low cervical cordotomy for cancer pain as a method to avoid sleep apnea.

Rheumatoid arthritis (RA) is a systemic inflammatory form of arthritis characterized by joint inflammation, pain, swelling, and stiffness. While contemporary treatment strategies based on early diagnosis, aggressive treatment, and regular monitoring have helped a significant number of patients achieve evidence-based treatment goals, RA still presents substantial management challenges to both clinicians and patients, and has the potential to lead to severe disability over time. In addition to its significant clinical consequences, RA has important economic implications. Both direct and indirect
