Exosomes as Future Therapeutic Tools and Targets for Corneal Diseases.

The therapeutic potential of exosomes (Exos), a subpopulation of extracellular vesicles (EVs) secreted by various cell types, has been broadly emphasized. Exos are endosome-derived membrane-bound vesicles 50-150 nm in size. Exos can be general or cell type-specific. Their contents enable them to function as multi-signaling and vectorized vehicles. Exos are important for maintaining cellular homeostasis. They are released into extracellular spaces, leading to uptake by neighboring or distant cells and delivering their contents to modulate cell signaling. Exos influence tissue responses to injur

Overview of Cytomegalovirus Ocular Diseases: Retinitis, Corneal Endotheliitis, and Iridocyclitis.

Cytomegalovirus (CMV) infection is a significant clinical concern in newborns, immunocompromised patients with acquired immunodeficiency syndrome (AIDS), and patients undergoing immunosuppressive therapy or chemotherapy. CMV infection affects many organs, such as the lungs, digestive organs, the central nerve system, and eyes. In addition, CMV infection sometimes occurs in immunocompetent individuals. CMV ocular diseases includes retinitis, corneal endotheliitis, and iridocyclitis. CMV retinitis often develops in infected newborns and immunocompromised patients. CMV corneal endotheliitis and i

Metabolomics in Corneal Diseases: A Narrative Review from Clinical Aspects.

Corneal pathologies may have subtle manifestations in the initial stages, delaying diagnosis and timely treatment. This can lead to irreversible visual loss. Metabolomics is a rapidly developing field that allows the study of metabolites in a system, providing a complementary tool in the early diagnosis and management of corneal diseases. Early identification of biomarkers is key to prevent disease progression. The advancement of nuclear magnetic resonance and mass spectrometry allows the identification of new biomarkers in the analysis of tear, cornea, and aqueous humor. Novel perspectives on…

Non-infectious and non-hereditary diseases of the corneal epithelium.

The corneal epithelium serves as a physical barrier and a refractive element. Therefore, diseases of the corneal epithelium can increase the risk for infection and causes vision loss. The corneal epithelium can be affected by a multitude of conditions, such as infections, hereditary diseases, depositions, trauma, autoimmune conditions, factitious disorders, and iatrogenic causes. Non-infectious and non-hereditary corneal epithelial diseases represent a collection of conditions with diverse etiologies and clinical presentations but similar patient symptoms. The differing therapeutic interventio

The role of oxidative stress in corneal diseases and injuries.

In various corneal injuries (such as chemical burns or irradiation of corneas with UVB radiation) and ocular diseases (e.g. dry eye disease, keratokonus, bullous keratopathy, Fuchs' endothelial dystrophy), the expressions of malondialdehyde (a marker of lipid peroxidation) and nitrotyrosine (a marker of oxidative stress) appeared in cells of individual corneal layers and conjunctival cells (dry eye disease). This is in contrast to healthy corneas in which negligible levels of malondialdehyde and no expressions of nitrotyrosine are present. The injured or diseased corneas reveal decreased capac

Altered antigenicity of keratan sulfate proteoglycan in selected corneal diseases.

Monoclonal antibody against keratan sulfate (KS) was used for immunofluorescent staining of sections of human corneas from 8 normal eyes, 19 with keratoconus, 4 with pellucid marginal degeneration, 5 with primary macular corneal dystrophy, and 1 with recurrent macular corneal dystrophy. The anti-KS monoclonal antibody did not stain the corneas with primary macular corneal dystrophy, but stained all other corneas to varying degrees. Staining intensity was weaker than normal in most keratoconus and pellucid marginal degeneration corneas, and was very weak in a case of macular corneal dystrophy t

[Incidence and function of Langerhans cells in various corneal diseases].

Using immunohistochemical techniques, we investigated the distribution and frequency of Langerhans cells in corneal buttons obtained from patients who underwent corneal transplantation because of various corneal diseases. The frequency of these dendritic cells was similar to that in the normal epidermis in corneas with epidermalization after severe alkali burns. Numerous Langerhans cells, albeit in smaller numbers, were also present in the central corneal epithelium of patients with keratitis due to infection with herpes simplex virus, keratitis due to herpes zoster virus, bacterial corneal ul…

[The causes of blindness by corneal diseases in 3,499 cases].

The chief causes of corneal blindness was found to be corneal inflammatory diseases and injury in 83.4% of the 3,499 cases (3,993 eyes) of corneal blindness that presented themselves during 1983-1985 at the authors' hospital. The important diseases in order of frequency were herpes simplex keratitis (42.8%), bacterial keratitis (17.4%), corneal injuries (15.7%), keratomalacia (10.0%), and trachoma (3.8%). Patients of the latter two diseases were mostly old cases. The authors discussed the incidence of corneal diseases and their prevention and treatment.

Corneal stromal demarcation line after collagen cross-linking in corneal ectatic diseases: a review of the literature.

Collagen cross-linking (CXL) is a relatively new conservative approach for progressive corneal ectasia, which is able to strengthen corneal tissue reforming new covalent bonds. Subjective and objective results following this method seem to be promising. In recent years, newer CXL protocols have been developed to perform more effective and less invasive procedures. The increasing diffusion of CXL in the corneal ectatic disease has increased the need to have actual indices regarding the efficacy of the treatment. Evaluation of demarcation line (DL), a transition zone between the cross-linked ant

Clinical correlates of common corneal neovascular diseases: a literature review.

A large subset of corneal pathologies involves the formation of new blood and lymph vessels (neovascularization), leading to compromised visual acuity. This article aims to review the clinical causes and presentations of corneal neovascularization (CNV) by examining the mechanisms behind common CNV-related corneal pathologies, with a particular focus on herpes simplex stromal keratitis, contact lenses-induced keratitis and CNV secondary to keratoplasty. Moreover, we reviewed CNV in the context of different types of corneal transplantation and keratoprosthesis, and summarized the most relevant

Clinical evaluation of corneal diseases associated with floppy eyelid syndrome.

We examined 18 cases of floppy eyelid syndrome, first reported in the Orient, in patients ranging in age from 11 to 55 years (mean 24 years); 16 were men and two were women. In contrast to previous reports in which almost all patients were obese men, only three of our patients were mildly obese. The most common abnormal corneal finding was punctate epithelial keratopathy (five patients-28%). Keratoconus was detectable in three patients (17%) overall; it was bilateral in one case and unilateral in two. Other miscellaneous corneal findings were corneal astigmatism and corneal opacity. In two cas

[EGF in therapy of corneal diseases. Principles and possible uses].

Numerous recent research results stress the importance of the epidermal growth factor (EGF) for the growth and differentiation of corneal tissues. EGF, a physiological ingredient in the tears, plays a predominant role in the regeneration of corneal epithelium. Furthermore, EGF has been shown to increase the proliferation and differentiation of corneal keratocytes and endothelial cells. Topically applied EGF significantly enhances both the re-epithelialization of denuded areas and the tear strength of stromal cicatrices. These results may lead to interesting pharmacological management procedure

[Place of the transplantation of the amniotic membrane in the treatment of corneal diseases concomitant with corneal neovascularization].

Published data and authors' independent observations related with the influence produced by the transplantation of the amniotic sac on corneal neovascularization are described. The epithelial-and-stromal corneal defect as well as the limbic epithelium deficit with a subsequent conjunctivization of the corneal epithelium and corneal neovascularization are one of the key mechanisms of corneal neovascularization. The amniotic sac transplantation, made for the purpose of reconstructing the eye surface, ensures the conditions for a fast recovery of the corneal epithelium and, thus, prevent the corn

Sjögren Disease and the Corneal Surface.

Dry eye in Sjögren disease is characterized by lacrimal gland dysfunction, often resulting in decreased lacrimal fluid secretion, tear film thinning, epithelial cell damage, and ocular surface inflammation. Beyond lacrimal gland impairment, corneal nerves play a crucial role in the disease, with patients exhibiting a spectrum of symptoms depending on structural and functional nerve abnormalities. Two phenotypes are recognized in patients with Sjögren disease: one with diminished symptoms despite worsening clinical signs (neurotrophic aspect), and another with heightened symptoms such as hypera
