Transient corneal edema after phacoemulsification: comparison of 3 viscoelastic regimens.

PURPOSE: To evaluate the effect of different viscoelastic substances on the grade and time course of postoperative corneal edema. SETTING: Department of Clinical Sciences/Ophthalmology, Umeå University Hospital, Umeå, Sweden. METHODS: This study comprised 62 patients with otherwise healthy eyes who had routine phacoemulsification and intraocular lens (IOL) implantation. Patients were divided into 3 groups. Group 1 was given Healon GV (sodium hyaluronate 1.4%) at phacoemulsification and IOL implantation. Group 2 was given Viscoat (sodium hyaluronate 3.0%-chondroitin sulfate 4.0%) at phacoemul

Cold-induced corneal edema in patients with trigeminal nerve dysfunction.

Two previous cases of cold-induced corneal edema have been reported in patients with corneal anesthesia secondary to a trigeminal nerve disorder. We studied six patients with complete unilateral corneal anesthesia after trigeminal ablation. Subjects' eyes were exposed to 4 C air from a fan for one hour. We measured corneal thickness, corneal surface temperature, and endothelial permeability to fluorescein. During cold exposure, two of the six study eyes exhibited reversible corneal swelling (11% and 26% over baseline value). All anesthetic corneas were consistently colder (13.8 +/- 0.7 C) than

Corneal backscatter as a biomarker for edema severity in Fuchs endothelial corneal dystrophy: a cross-sectional study.

BACKGROUND: Fuchs endothelial corneal dystrophy (FECD) is characterized by progressive endothelial dysfunction and corneal edema. Reliable objective biomarkers for grading disease severity are currently evolving. Corneal backscatter may reflect edema-related changes in corneal transparency and could serve as an additional objective measure of FECD severity across different edema stages. The purpose of this study was to evaluate the potential of corneal backscatter as a biomarker for severity of FECD. METHODS: In this retrospective, observational, single-center cross-sectional study, 99 eyes o

Coenzyme Q10 in the Treatment of Corneal Edema in Kearns-Sayre: Is There an Application in Fuchs Endothelial Corneal Dystrophy?

PURPOSE: Corneal involvement in mitochondrial disease is seldom described. Kearns-Sayre syndrome (KSS) is a mitochondrial disorder characterized by retinitis pigmentosa, external ophthalmoplegia, and heart block. We report 2 patients with KSS with corneal lesions involving the endothelium, which improved with Coenzyme Q10 (CoQ10). Based on recent research regarding the role of dysfunctional oxidative metabolism in Fuchs Endothelial Corneal Dystrophy (FECD), we propose that mitochondrial diseases and FECD share a final pathway. METHODS: A chart review was performed and a review of the literatu

Permanent corneal edema resulting from the treatment of PTK corneal haze with mitomycin: a case report.

A 39-year-old man underwent phototherapeutic keratectomy via excimer laser for recurrent corneal erosions secondary to basement membrane dystrophy with the subsequent development of irregular astigmatism and central stromal opacity. The cornea was scraped and treated with 0.02% mitomycin C using a total of 14 drops over a period of 6 days. Corneal edema developed as a consequence of low endothelial cell count with dysfunctional cells. A corneal transplant restored acuity of 20/20 with binocular vision. It is believed that the underlying endothelium was exposed to toxic doses of mitomycin C suf

Corneal edema with hydrogel lenses and eye closure: time course.

Central corneal thickness changes during the wearing of hydrogel lenses under closed-eye conditions were measured for two subjects. Each wore an investigational ultrathin Bausch & Lomb Soflens contact lens (series X-176) on one eye only under closed-eye conditions for periods ranging from 30 min to 6 hr. The other eye served as a control. Corneal thickness of the control eyes increased gradually and reached a maximum increase of about 4% at the end of the 4-hr wearing trial. Corneal thickness in the experimental eyes also increased with wearing time during the first 4 hr and then remained fair

Corneal edema as a result of ischemic endothelial damage: a case report.

In a 79-year-old woman with temporal arteritis, an attack of corneal edema coincided with an ischemic attack of the anterior eye segment. Specular microscopy 17 months after the acute attack demonstrated a 72% endothelial cell loss on the side with previous edema as compared to the other side without corneal involvement. The development of endothelial damage is assumed to be caused by ischemia of the endothelial cells during the acute phase, which, in turn, caused the corneal edema.

Topical Ripasudil for the Treatment of Segmental Corneal Edema: A Case Series.

PURPOSE: Ripasudil, a rho-kinase inhibitor, has gained popularity as an adjunctive therapy after Descemet stripping only to hasten visual recovery. Ripasudil has been shown to increase corneal endothelial cell proliferation and intercellular adhesion, and decrease endothelial cell apoptosis. We present 4 cases in which persistent corneal edema after various anterior segment surgeries was successfully treated with topical ripasudil, and 1 case in which corneal edema did not improve with use of ripasudil. METHODS: Retrospective chart review revealed 5 patients treated with topical ripasudil for

Problems associated with penetrating keratoplasty for corneal edema in congenital glaucoma.

Corneal edema from congenital glaucoma is a rare indication for penetrating keratoplasty. We report the complications and problems associated with eight consecutive penetrating keratoplasties performed in adult eyes with a history of congenital glaucoma. Only 25% of the eyes achieved 20/40 or better vision after surgery. These generally poor results can be attributed to previous glaucomatous optic nerve damage, to complications related to previous surgery, and possibly to amblyopia. The most common surgical complication was postoperative elevation of intraocular pressure, which occurred in all

Results of anterior segment reconstruction for aphakic and pseudophakic corneal edema.

The authors studied prospectively 59 eyes with corneal edema after cataract surgery that required reconstructive surgery; penetrating keratoplasty in 59 (100%), anterior vitrectomy in 42 (71%), intraocular lens removal or exchange in 33 (56%), gonioplasty in 32 (54%), and iridoplasty in 30 (51%). With an average follow-up of 9.9 months, the corneal transplant was clear in 56 eyes (95%). The corrected visual acuity was improved in 49 eyes (83%) and was the same in seven eyes (12%). Intraocular pressure was less than or equal to 21 mmHg in 47 eyes (80%) and 22 to 29 mmHg in nine eyes (15%). The

Management of pseudophakic corneal edema with reconstruction of the anterior ocular segment.

The following surgical techniques were used to treat pseudophakic corneal edema in 13 consecutive eyes: penetrating keratoplasty (13 eyes), mechanical anterior vitrectomy (ten eyes), removal of intraocular lens (13 eyes), gonioplasty with opening of peripheral anterior synechiae using a dental mirror (seven eyes), iridoplasty using a round needle (six eyes), and insertion of a replacement intraocular lens (12 eyes). The most common intraoperative complication was hemorrhage in the angle. Although the angle was open 360 degrees to the ciliary body in 12 of the 13 eyes at the end of surgery, a f

Multi modal imaging in corneal edema after corneal collagen cross-linking (CXL); a case-based literature review.

BACKGROUND: Keratoconus (KCN) is a common ectatic disorder of the cornea. Corneal collagen cross-linking (CXL) is used as an effective option to slowdown the disease progression. Although CXL is considered a safe procedure, corneal endothelial damage, especially in corneal thickness of less than 400 μm, has been reported. CASE PRESENTATION: A 25-year-old man known case of KCN was referred with complaints about blurred vision and discomfort of the right eye 3 days after performing CXL. The preoperative thinnest point was 461 μm. His presenting BCVA was CF at 1 m. Examination showed central cor
