A new dwarfing syndrome with extreme shortening of humeri and severe coxa vara.

A previously undescribed form of short-limbed dwarfism is reported in an 86-year-old man. All the long bones were shorter than normal, with extreme shortening of the humeri. Other features included dysplastic humeral heads, coxa vara with dysplasia of the femoral heads and acetabula, platybasia, and small vertebral bodies. In all other respects, including intelligence, the patient was normal for his age.

Percutaneous multiplanar subtrochanteric osteotomy with external fixation for developmental coxa vara (preliminary results).

UNLABELLED: Developmental coxa vara (DCV) develops during early childhood between the age of independent walking and 6 years. The deformity includes a decrease in the neck-shaft angle and femoral retroversion. Recently, good results have been reported using external fixator systems for the correction of proximal femoral deformities secondary to slipped capital femoral epiphysis, Perthes' disease in children, and percutaneous proximal femoral osteotomy for coxa vara. This preliminary study was performed to evaluate the results of percutaneous subtrochanteric osteotomy in restoring the normal al

Coxa vara infantum, hip growth disturbances, etiopathogenesis, and long-term results of treatment.

The literature concerning coxa vara infantum (CVI) contains only a few long-term follow-up reports of the treatment. This study was performed to define the clinical and roentgenographic features of coxa vara infantum (CVI) in children and skeletally mature patients. Special attention was given in follow-up evaluations to the growth and impairment of the hip joint (acetabulum, femoral head, and neck) before and after operative treatment in different age groups. Because in difficult cases the results of subtrochanteric osteotomy have not been satisfactory and have led to recurrences, the use of

Musculoskeletal ultrasound discriminating camptodactyly-arthropathy-coxa vara-pericarditis syndrome and juvenile idiopathic arthritis.

OBJECTIVE: Camptodactyly-arthropathy-coxa vara-pericarditis syndrome (CACP) and juvenile idiopathic arthritis (JIA) both feature joint effusions. This study aimed to identify preliminary MSUS-based criteria that may help discriminate children with CACP from those with polyarticular JIA. METHODS: A retrospective multicentre analysis of MSUS images from children with genetically confirmed CACP or a diagnosis of active polyarticular JIA was performed. Joint and tendon findings were evaluated using qualitative B-mode (BM) and Doppler-mode (DM) assessment and graded semi-quantitatively (0-3) accor

Post-traumatic coxa vara in children following screw fixation of the femoral neck.

BACKGROUND AND PURPOSE: The rare displaced fractures of the femoral neck in children need accurate reduction and rigid fixation. The implants commonly used for internal fixation in children are pins or screws. We evaluated the long-term outcome in children who sustained fractures of the proximal femur that were treated by screw fixation. PATIENTS AND METHODS: All 22 children (mean age 12 (5-16) years) with fractures of the femoral neck that were treated with screw fixation (mean 2.4 (1-3) screws) at our department between 1990 and 2006 were evaluated. For measurement of outcome, the Harris hi

Fixation of Intertrochanteric Valgus Osteotomy with T Plate in Treatment of Developmental Coxa Vara.

BACKGROUND: Although the valgus subtrochanteric osteotomy is considered as a standard surgical treatment for coxa vara, there is no consensus on the optimal method of fixation and osteotomy technique. Fixation of the osteotomy has been achieved by various methods including external fixation and internal fixation with pins and cerclage and a variety of plates. The aim of this study is the evaluation of the results of developmental coxa treated by Y intertrochanteric valgus osteotomy fixed with a T-buttress plate compared with other methods of fixation in the literature. METHODS: Eighteen corre

Results and complications of a surgical technique for correction of coxa vara in children with osteopenic bones.

BACKGROUND: Surgical correction of coxa vara in children with osteopenic bone diseases could be very challenging. In this study, we describe a modified surgical technique for the correction of coxa vara in children with bone fragility. We also report the results and complications of this technique in 16 children (21 coxa vara) with osteopenic bone diseases. METHODS: Charts, clinic notes, and radiology images of 16 patients (21 hips) who had a proximal femoral osteotomy for the treatment of coxa vara in osteogenesis imperfecta (18 hips) and fibrous dysplasia (3 hips) in our institution between

[Valgus osteotomy and lengthening of the femoral neck in secondary coxa vara in adolescents and adults].

Shortening of the femoral neck and proximal displacement of the greater trochanter are main problems of coxa vara after avascular necrosis of the capital epiphysis. In 24 patients an osteotomy to lengthen the femoral neck was performed to normalize the anatomic and functional situation. A clinical and radiological follow-up showed good or satisfactory results of the operation in 23 cases. Elimination or relief of pain and a negative Trendelenburg sign are important short-termed aims. Prevention or deceleration of degenerative changes in the hip joint are primary long-termed aims.

Congenital coxa vara: computed tomographic analysis of femoral retroversion and the triangular metaphyseal fragment.

Three patients with congenital coxa vara studied with two- and three-dimensional computed tomographic (2DCT and 3DCT) methods are reported. In all cases, the femoral retroversion was documented and subsequently corrected by proximal femoral osteotomy. In two patients with isolated coxa vara, the physeal-femoral neck angle was decreased as seen in slipped capital femoral epiphysis in adolescents. Our studies suggest that the triangular metaphyseal fragment reflects a Salter-Harris type II separation pattern through the defective femoral neck. The epiphysis and attached triangular fragment slip

Midterm results after subtrochanteric end-to-side valgization osteotomy in severe infantile coxa vara.

BACKGROUND: For the treatment of the severe infantile coxa vara it is mandatory for the orthopaedic surgeon to observe the mechanobiology of the growing hip before and after the surgical intervention. We hereby would like to present our experiences with the subtrochanteric end-to-side valgization osteotomy and to compare the procedure with the alternatively used Y-shaped osteotomy as described by Pauwels. METHODS: Thirteen patients (20 hips) who had undergone subtrochanteric end-to-side valgization were followed for a mean 6.2 years (range, 0.8 to 12.8 y). At the time of surgery the mean age

Double Angle Varus Corrective Osteotomy in a Case of Severe Coxa Vara - A Case Report.

INTRODUCTION: The most common site of tumors is the femur in the adolescent group. As the tumour progresses in the proximal and distal metaphysis, the biomechanics of the hip and knee joints are changed, respectively. This case report is a coxa vara in an adolescent boy with fibrosis dysplasia - shepherd crook deformity, and a corrective osteotomy was done with some complications and success. CASE REPORT: A 12-year-old adolescent came with deformity of the right thigh with shortening, and radiograph shows fibrous dysplasia with coxa vara, which was treated with corrective osteotomy and medica

Proximal femoral valgus osteotomy for the treatment of developmental coxa vara: a systematic review of the literature.

BACKGROUND: Developmental Coxa Vara (DCV) consists on a pathological reduction in head-shaft angle (HSA) and increased femoral retroversion. Several case series reported outcomes on proximal femoral valgus osteotomy (PFVO), but no evidence synthesis had been conducted. This systematic review aimed to (1) analyze success rate and complications, (2) report the degree of correction according to the HSA and the Hilgenreiner Epiphyseal Angle (HEA), compare success rate and degree of correction of subtrochanteric (SVO) vs intertrochanteric (IVO) osteotomy, and (4) difference in success rate and corr

[Subcapital osteotomy of the femoral neck in the treatment of coxa vara in adolescence--long-term results].

The authors present a group of 16 adolescents suffering from coxa vara treated in the years 1967-1987 for a severe slipped capital femoral epiphysis by subcapital osteotomy. The authors deal with the etiology, symptomatology and treatment with special regard to the severe slip. Spica cast was applied in most operations and no internal fixation was used. In the so far latest female patient the femoral head was fixed after osteotomy by Kirschner wires according to Dyas Aries. In the discussion the authors report considerably lower percentage of complications in the longterm examination, mainly a

Single-stage surgical correction of a bilateral acquired coxa vara deformity in a young woman.

A 23-year-old female-treated patient of osteomalacia and secondary hyperparathyroidism with hypophosphatemia presented with a 5-year history of bilateral groin pain and stiffness of both hips and difficulty in walking. Plain radiographs of the pelvis showed bilateral coxa vara deformity. She was managed surgically by a single-stage bilateral subtrochanteric corrective osteotomy with the internal fixation. After the osteotomy healing at 3 months, the patient was pain free and walked comfortably with an increased range of hip motion.
