[Shunt surgery in occlusive processes in the posterior cranial fossa of children].

Shunting of the cerebrospinal fluid by drainage systems of various design was conducted in 64 children aged from 4 months to 14 years with occlusive processes in the posterior cranial fossa. The occlusion was caused by a tumor in 57 cases and by an arachnoid cyst in 7 cases. All children were related to the high risk group, i. e. the prognosis of direct manipulation on the posterior cranial fossa was poor or doubtful. In 26 children surgery was limited to a shunting operation without subsequent intervention on the posterior cranial fossa. The successful implantation of the drainage system was

Venous hemodynamics of arteriovenous meningeal fistulas in the posterior cranial fossa.

The etiology of arteriovenous meningeal fistulas (AVMFs) of the posterior fossa often remains unclear. Analyses of three personal cases, however, suggest a change in venous hemodynamics due to a primary lesion, the cases being of sinus thrombosis, intracerebral abscess with oedema and arteriovenous malformation. It is suggested that increase of venous pressure in the posterior fossa leads to increased drainage and, consequently, to dilatation of infratentorial veins and of meningeal branches of the external carotid and vertebral artery. Clinical findings and anatomical facts underlying AVMF le

Vessel tortuosity as a cause of posterior fossa cranial nerve syndromes.

Neurologic syndromes may be caused by mechanical vascular compression of cranial nerves in the posterior fossa. The clinical expression is variable and can be divided into two main groups. Isolated cranial nerve involvement is most frequently associated with tortuous, elongated vertebral and basilar arteries as well as redundant loops of the anteroinferior cerebellar artery. Pseudotumoral manifestations of cranial nerve involvement at the level of the cerebellopontine angle are more frequently related to aneurysms of the vertebral and basilar arteries.

The growth of the posterior cranial fossa in FGFR2-induced faciocraniosynostosis: A review.

BACKGROUND: The growth of the posterior fossa in syndromic craniostenosis was studied in many papers. However, few studies described the pathophysiological growth mechanisms in non-operated infants with fibroblast growth factor receptor (FGFR) type 2 mutation (Crouzon, Apert or Pfeiffer syndrome), although these are essential to understanding cranial vault expansion and hydrocephalus treatment in these syndromes. OBJECTIVE: A review of the medical literature was performed, to understand the physiological and pathological growth mechanisms of the posterior fossa in normal infants and infants w

Morphometric Analysis of the Posterior Cranial Fossa in Syndromic and Nonsyndromic Craniosynostosis.

Posterior cranial fossa (PCF) anatomy can be abnormal in craniosynostosis, and hindbrain herniation may occur. This study analyzed PCF anatomy in single suture and complex craniosynostosis.Children with craniosynostosis and age-matched controls were identified. Cephalic index (CI) for cranial vault and PCF as well as tentorial (TA) and occipital angles (OA) were measured on preoperative imaging.Children with syndromic (N = 6), bicoronal (N = 4), sagittal (N = 12), and metopic synostosis (N = 4) as well as controls (N = 10) were enrolled. Mean CI for cranial vault was 0.89, 0.93, 0.65, 0.74, an

[Vascular pathologies and vascular anatomical variants of the posterior cranial fossa].

CLINICAL/METHODICAL ISSUE: The posterior fossa is the location of some specific vascular pathologies and vascular anatomical variants that are important for radiologists to know and recognize. Vascular malformations that are also found in the supratentorial region have some divergent characteristics in the posterior fossa. PRACTICAL RECOMMENDATIONS: Knowledge of the vascular anatomy of the posterior fossa and its variants is important for an understanding of vascular pathologies.

Growing skull fracture of the posterior cranial fossa and of the orbital roof.

BACKGROUND: Growing Skull Fractures (GSF) are rare complications of head trauma, primarily reported in infancy and early childhood. GSF are commonly located on calvaria, and rarely in other locations, including the skull base. METHOD: In this study, we report two cases of GSF occurring in unusual locations. The first, a 8-month old girl, with a GSF of the suboccipital posterior fossa region, and the second, a 4-year old boy with a GSF of the right orbital roof. Both cases underwent operative treatment of the GSF, with microsurgical dissection and excision of the protruding gliotic brain tissu

[Surgical treatment of cholesteatomas (epidermoid cysts) of the posterior cranial fossa].

In 1985 to 2000, the Academician N. N. Burdenko Research Institute of Neurosurgery, Russian Academy of Medical Sciences, examined and operated on 78 patients with epidermoidal cysts of the posterior cranial fossa. A complex of clinical symptoms of epidermoidal cysts of the posterior cranial fossa and their computed tomographic (CT) and magnetic resonance imaging (MRI) scans were studied. Based on clinical and X-ray evidence, their diagnosis was correctly made in 87.2% of cases. A procedure was developed for radical removal of epidermoidal cysts of the posterior cranial fossa. The stages and su

[The early postoperative period in patients with tumors of the posterior cranial fossa].

The studies carried out during an early postoperative period in patients with posterior fossa tumors were made to analyse the role of the degree and the level of brain stem damages in formation of cerebral reactions. The three groups of cases were selected in accordance with the course of the postoperative period: 1.1-with exitus letalis (irreversible damages); 1.2-rehabilitation in persistent focal damages; 2-restoration with functional safety of brain stem. Differences of the integral cerebral reactions (the state of consciousness, emotional-affective disorders, awakening state) as well as t

[Cesarean section with subsequent craniotomy in the area of the posterior cranial fossa].

We report on a 31-year old pregnant patient with von-Hippel-Lindau syndrome who presented to the emergency room with symptoms of increased intracranial pressure. She was found in premature labour with a normal foetus of 29 weeks' gestational age in breech presentation. We discuss an anaesthetic and neurosurgical management during emergent craniotomy and Caesarean section. Caesarean section and posterior fossa craniotomy with resection of an angioblastoma are performed in one setting. Following rapid sequence induction with thiopentone and succinylcholine, anaesthesia is maintained with fentany

[The diagnosis and surgical treatment of traumatic hematomas of the posterior cranial fossa].

The symptomatology and results of treatment of 26 patients with traumatic hematomas of the posterior cranial fossa (PCF) are analysed. The hematomas were verified on operation in 19 patients and on postmortem examination in the other patients. It is concluded that the diagnosis of PCF hematomas is based on a combination of signs: cervico-occipital mechanism of the trauma, fracture of the occipital bone, cerebello-stem symptomatology, and acute hydrocephalus. The formation of trephination openings on the line of the fracture of the occipital bone is considered expedient in the presence of the a

[Clinical picture and diagnosis of arteriovenous malformations of the posterior cranial fossa].

An anatomo-surgical classification of arteriovenous malformations of the posterior cranial fossa (PCF) is suggested, based on angiography and computer-aided tomography findings with due consideration for adequate surgical approaches to the defect. Conditions of blood supply and drainage are described with regard to localization of the malformation in the PCF structures. Analysis of the neurologic symptoms in PCF arteriovenous malformations permitted a conclusion on a comparatively rare correlation between the symptoms and the localization of the malformation in a certain PCF structure.

[Scintigraphic and EEG diagnosis of posterior cranial fossa tumors in children].

The purpose of this work was to assess the diagnostic value of scintigraphy and EEG in posterior fossa tumours. The analysis covered 22 children aged from 10 months to 13 years treated surgically in the period from 1975 through 1978 for expanding intracranial lesions of this location. Before the operation in all cases positive ventriculography, brain scintigraphy and EEG were done. In 21 cases ventriculograms were abnormal and scintigrams demonstrated in all these cases presence of pathological foci in the posterior fossa. This method helped in accurate localization of 11 out of 12 tumours of

Cranial venous system and the posterior fossa in spina bifida aperta.

A study of the cerebral venous system in children with S.B.A. was undertaken, and this report concerns the gross structure of this structure of this system in the posterior fossa. Noteworthy is the closeness of the torcular to the foramen magnum. The lateral sinuses ran near to the foramen magnum to the sigmoid sinuses. The dura of the posterior fossa consisted of a plexiform structure in which no clear distinction could be made between various sinus systems normally present in the dural layers of the posterior fossa, thus constituting an occipital plexus. The walls of the sinuses were weak an
