[The characteristics of the abdominal symptoms in craniocerebral trauma sustained in a state of alcoholic intoxication].

This paper focuses on 63 cases of syndrome of nonprimary acute abdomen (SNPAA) in craniocerebral injury (CCI) sustained in the state of alcoholic intoxication, and 59 observations connected with cranio-abdominal injuries (CAI). In the presence of cerebral injuries, one of the specific characteristics of alcohol was that it entailed changes in the abdominal symptomatology either concealing manifestations of severe damage to the abdominal organs or potentiating SNPAA. In all the observations the abdominal symptomatology was less pronounced that in those victims who do not take alcoholic drinks.

[Craniocerebral trauma in a newborn infant--precipitous labor or a fall after birth].

The autopsy of a newborn child that was found wrapped up in a plastic bag revealed a biparietal fracture of the skull. Only after knowing all circumstances it was possible to explain the origin of this fracture. A plausible reconstruction was possible after the inspection of the scene, the autopsy and by taking into consideration the statement of the mother. Thus it is could be decided that the death of the newborn child occurred by an accident while an infanticide, birth injury or labor precipitate causing the fracture was excluded.

[Prognostic factors in craniocerebral trauma without primary surgically correctable space occupying lesion].

By means of a retrospective study involving 154 patients with diffuse brain injury, we could show, that simple factors, like coexisting injuries, pupil abnormalities and the Glasgow coma scale, allow us to assess the prognosis of patients outcome a short time after trauma. This prognosis is further modified by cerebral perfusion pressure and secondary pupil abnormalities. The prognostic value of an EEG and a SEP is the same for both methods and was found to be correct in about 2/3 of all cases.

[Nosocomial pneumonia in patients with severe craniocerebral trauma in intensive care units].

Eighty-three patients with severe craniocerebral injuries (CCI) were treated at Institute of Neurosurgery in 1999. Pulmonary infectious complications occurred in 16 of 25 patients with severe CCI. Early nosocomial pneumonia (NP) was diagnosed in 18% and the so-called late NP (associated with artificial ventilation of the lungs) in 35%. Coma longer than 4 days increased the incidence of NP to 62%. The main pathogens of NP are gram-negative aerobic bacteria (61%), the predominant agent being Pseudomonas aeruginosa (18.9%). 76% isolated microorganisms were multiresistant. [The effect of multimodal pathogenetic therapy on microcirculatory function in combined craniocerebral trauma].

It is known that severe combined maxillocranial trauma causes impairment of microcirculatory vessels of bulbar conjunctiva as well as changes of local (cervical) and systemic blood flow, prevailing on the side of injury. New trigeminosympathetic regional block was used to produce chemical neurotomy of sympathetic and parasympathetic portions of autonomous nervous system in the neck. This blockade induced positive changes of minute CBF and microcirculation of CNS and facial area. Modified trigeminal sympathetic regional blockade along with intravenous infusion of dimephosphone improves both mic

[Treatment concepts of tertiary damage of the locomotor system after craniocerebral trauma].

Brain injuries are the most frequent cause of handicap in young adults. The success of rehabilitation depends mainly on the avoidance of tertiary lesions of the locomotor system. Between January 1989 and December 1991, 54 patients were treated at the neuro-rehabilitation unit of the Neurology Department of the University Hospital for severe brain injuries. On admission these patients were in different stages of recovery. All patients underwent physiotherapy adapted to their specific needs. The decision as to whether other kinds of treatment were indicated depended on the patients' problems in

[Diagnosis and surgical treatment of craniocerebral trauma within the scope of polytrauma management].

Head trauma of different degrees is present in about two-thirds of multiple trauma patients admitted to hospital. As the primary brain damage is irreversible, our objective should be the recognition and specific treatment of both early and late complications. Stabilization of cardiorespiratory parameters must first be achieved. Secondly, only the diagnosis and treatment of life-threatening hemorrhages is of greater importance than the diagnostic and therapeutic measures undertaken by the neurosurgeon. Coma is assessed according to the Glasgow Coma Scale. The indications for CT investigation in

[Schizophrenia complicated by craniocerebral trauma and other diseases of an exogenous and organic nature].

The authors examined 120 patients with schizophrenia (torpid and paroxysmal-progressive) whose disease at different stages of its course was complicated by exogenous impacts (head trauma in 66 cases, neuroinfection in 15, intoxication in 16 and vascular brain disease in 23). These complications affect the structure of individual psychopathological syndromes on the one hand and the rate of the development of the disease itself (with a faster formation of productive and negative syndromes) on the other. Correlation has been found between the degree of exogenous impact and the severity of astheni

[Significance of preclinical emergency treatment for the prognosis of patients with severe craniocerebral trauma].

In a prospective study in 147 unconscious patients (Glasgow-Coma-Scale less than or equal to 7) we examined the relevance of preclinical care for posttraumatic prognosis-basic life support (BLS) referring to ventilation/oxygenation and circulation, as well as advanced trauma-specific treatment (ATT) referring to medication and positioning. Procedures providing oxygenation and stable cardiovascular function are found to be most important. The overall survival rate amounts to 53.7 per cent (79 of 147 patients). Whereas 56.6 per cent (77 of 136 patients) of the patients being preclinically attend

[Bilateral hip ankylosis caused by heterotopic ossification after isolated severe craniocerebral trauma].

A 36-year old man suffered an isolated head injury with a fracture of the skull, epidural and subdural hematomas as well as brain contusion (Abbreviated Injury Scale (AIS): 5 points). The hematomas were evacuated by craniotomy. Because of high intracranial pressure (ICP) a barbiturate coma was necessary. Additionally, the patient demonstrated acute lung injury (ARDS) due to pneumonia 8 days after trauma. The patient recovered slowly and was transferred to neurorehabilitation on day 57 after injury. During the following 12 months the patient developed a significant deficit of hip motion (extens

[Peri-articular calcinosis after craniocerebral trauma--the natural course and treatment concepts].

Occurrence of periarticular ossification (PAO) is a serious complication in the rehabilitation of patients with neurological diseases, especially those with severe head injuries. The present concept for operative intervention in PAO postulates late surgical treatment after "maturation" of these heterotopic ossifications, because of a suspected higher rate of reoccurrence after early removal. In a retrospective analysis of nine arthrolysis in six patients with neurological diseases suffering from PAO (five patients with traumatic apallic syndrome and one patient with spontaneous subarachnoidal

[Craniocerebral trauma and aseptic osteonecrosis. Steroid-induced sequelae after therapy of brain edema].

In this publication we present three cases of avascular osteonecrosis (AON) of the femoral head and the talus in adolescent and young adult patients following short-term, high-dose steroid therapy for cerebral trauma. All patients were proven to be free of other risk factors for AON. The latency period between the steroid therapy and the occurrence of AON ranged from 12 to 60 months; steroid therapy lasted for 12 to 16 days with a total dosage equivalent to 2370-7180 mg prednisolone. Our case reports are added to a review of the literature with 14 similar cases reported receiving short-term, h

[Central laryngeal motor disorders following severe, closed craniocerebral trauma and in cerebrovascular diseases].

111 patients with voice disturbances after severe closed head trauma or cerebro-vascular accident were examined laryngologically. They presented no signs of peripheral nerve involvement. Their main symptoms of central motor dysfunction of the larynx were a reduced motility of the abductors and adductors, a shortening and thickening of the vocal folds, reduced fine control of vocal fold stiffness, and the presence of involuntary laryngeal movements. The frequency of these observations as well as their temporal relation to the onset of the disorder are reported. Possible pathophysiological subst
