What is the effectiveness of systemic corticosteroids in children with croup?

Systemic corticosteroids constitute standard treatment in children with acute obstructive laryngitis (croup). However, there is some uncertainty in relation with the magnitude of the benefits and risks associated with their use. To answer this question, we used Epistemonikos, the largest database of systematic reviews in health, which is maintained by screening multiple information sources, including MEDLINE, EMBASE, Cochrane, among others. We identified six systematic reviews including 25 randomized trials relevant for the question of interest. We extracted data from the systematic reviews, r

Delivery of Epinephrine in the Vapor Phase for the Treatment of Croup.

OBJECTIVES: The Vapotherm system delivers high humidity to the airway of patients by using semipermeable tubules where heated liquid water is in contact with air. The humidified air is conducted to the patient via a heated tube. Preliminary clinical observations in infants with croup suggested that epinephrine added to the water supplying the humidity was delivered successfully in the vapor phase. The purpose of this study was to evaluate the efficiency of the delivery of epinephrine in the vapor phase and to develop the feasibility criteria for a clinical pilot study. DESIGN: Thirty milligra

Respiratory viruses in laryngeal croup of young children.

OBJECTIVES: To determine the viral cause of laryngeal croup by use of highly sensitive methods, and including recently recognized viruses in the analysis. STUDY DESIGN: One hundred forty-four consecutive children with hoarse voice and inspiratory stridor attending the emergency department were enrolled. Age- and season-matched children presenting with a wheezing illness served as control subjects (n = 76). Nasopharyngeal swabs were analyzed by polymerase chain reaction for rhinovirus and enterovirus, coronavirus, respiratory syncytial virus (RSV), parainfluenza virus (PIV), influenza A and B

The mechanics of breathing in children with acute severe croup.

RATIONALE: The assessment of the severity of croup and response to therapy has remained a clinical one. Despite recognition of the importance of a reproducible and easily applicable method for objectively measuring severity, currently, no such technique exists. OBJECTIVES: We postulated that measurements of air flow and intrathoracic pressure changes in patients with severe croup would provide detailed information about the mechanics of breathing and the potential for the development of continuous bedside methods for objective monitoring of upper airway obstruction. METHODS: Twenty out of 21

Pulmonary arterial pressure in children with croup syndrome.

PURPOSE: Croup syndrome or laryngotracheitis is one of the frequent causes of transient upper airway obstruction by laryngeal and tracheal blockage in infants and children. Upper airway obstruction may lead to increased pulmonary arterial pressure in children. In this study, the relationship between croup syndrome and pulmonary arterial pressure was investigated. MATERIALS AND METHODS: Forty children with croup syndrome and 14 healthy, age- and sex-matched control subjects were enrolled in this study. Of all patients, 12 with severe symptoms, such as dyspnea, cyanosis, hypoventilation, were a

Interobserver variability of croup scoring in clinical practice.

BACKGROUND: Viral laryngotracheobronchitis croup is the most common cause of acute upper airway obstruction in young children. Clinical assessment of children with croup is often performed using 'croup scores'; however, these scores have not been validated outside of the research setting. OBJECTIVE: To determine the reliability of clinical observation items in croup scores in a paediatric emergency department (ED) setting. DESIGN: Literature review identified 12 observation items (level of consciousness or mental status, inspiratory breath sounds, air entry, stridor, cough, cyanosis or colou

A risk-benefit assessment of corticosteroids in the management of croup.

Croup is an acute clinical syndrome of childhood characterised by a barking cough, hoarse voice, stridor and a variable degree of respiratory distress. A meta-analysis and subsequent controlled trials clearly demonstrate that corticosteroids are efficacious in the management of croup, with their benefits conclusively outweighing their risks. In mild to moderate cases of croup either systemic or nebulised corticosteroids decrease symptoms and need for hospitalisation. Most reports use IM dexamethasone 0.6 mg/kg, although it is likely that dexamethasone 0.15 mg/kg has a similar effect. In contro

Prolonged croup due to herpes simplex virus infection.

UNLABELLED: Herpes simplex virus (HSV) is an uncommon cause of acute laryngitis in immunocompetent patients since it mostly occurs in immunocompromised subjects. We present two previously healthy children with prolonged gingivostomatitis and stridor (lasting 3 and 4 weeks) in whom HSV-1 was isolated from subglottal ulcers. CONCLUSION: HSV should be considered a possible pathogen in cases of prolonged or atypical croup not only in immunocompromised or elderly patients but also in otherwise healthy children.

Clinical Presentation of Pediatric Recurrent Croup: Implications for Diagnosis.

Recurrent croup is a common clinical entity afflicting the pediatric population, but is not well-characterized in the literature. We describe the largest series of recurrent croup in North America from a single tertiary care academic center, and differentiate subpopulations based on demographic characteristics and clinical presentation. We identified 114 patients for inclusion. Common symptoms included barky cough (86.0%) and inspiratory stridor (60.5%). Many (26.3%) experienced full symptom resolution within a day, and 41.2% reported onset of symptoms at any time of the day. Male patients had

A subglottic foreign body mimicking croup: A case report.

INTRODUCTION: Foreign body (FB) aspiration is one of the causes of respiratory distress in infants is an extremely dangerous and potentially life-threatening event. The diagnosis of FB aspiration is difficult because the signs and symptoms vary according to the degree of airway blockage or location of the FB. PATIENT CONCERNS: An 11-month-old female infant visited a hospital because of a sudden onset cough. She was relatively healthy without fever, rhinorrhea cyanosis, or poor feeding. On physical examination, auscultation revealed inspiratory stridor without wheezing and crackles. DIAGNOSIS

Recurrent croup and persistent laryngomalacia: Clinical resolution after supraglottoplasty.

OBJECTIVES: To determine if children with recurrent croup (RC) and persistent laryngomalacia (LM) clinically improve after supraglottoplasty (SGP). MATERIAL AND METHODS: Retrospective chart review cohort at tertiary care children's hospital consisting of patients diagnosed with LM and RC that underwent SGP from July 2011 to August 2014. Clinical history, demographics, clinical outcomes, and operative complications were reviewed. Specifically, the episodes of croup requiring systemic steroids were compared pre- and post-SGP with statistical analysis. RESULTS: Out of 107 patients undergoing SG

Acute Respiratory Distress in Children: Croup and Acute Asthma.

Acute respiratory distress is one of the most common reason for emergency visits in children under 5 y of age. An accurate understanding of the epidemiology of these diseases, identification of risk factors and etiology is critical for successful treatment and prevention of related mortality. The cause of acute respiratory distress varies in etiology, and hence is amenable to different treatment modalities. Depending on the predominant symptoms and signs, a child presenting to the clinician can be divided into six groups, viz., stridor; cough, fever and difficulty in breathing or fast breathin

Unexpected combination of acute croup and myocarditis: case report.

BACKGROUND: Lower vaccination coverage among foreign-born children is of concern because they live in households and communities characterized by more intense exposure to infectious diseases. Because of their higher prevalence rates, there is an increasing occurrence of infectious diseases imported into developed countries. This case report emphasizes the emerging necessity for new clinicians and pathologists of having competence with old infectious disease pathology. CASE PRESENTATION: A three and a half year old girl, who presented with croup history of 5 days and has been in severe respira

Evaluation of Repeat Steroid Dosing in Children Admitted With Croup.

OBJECTIVE: Croup is a common cause of respiratory distress in young children due to airway obstruction from laryngeal edema secondary to viral upper respiratory infection. Our goal was to measure the effect of redosing steroids on the length of stay (LOS) and rates of recurrence of croup symptoms in children hospitalized with uncomplicated croup. PATIENTS AND METHODS: Non-critically ill children hospitalized with croup from 2013 to 2021 were identified using encounter diagnosis codes. Variables including demographic factors, total dexamethasone dose given during hospitalization or within 24 h

Standardization of Clinician Outcomes for Croup Using a Clinical Pathway.

OBJECTIVE: Croup management varies significantly between hospitals and clinicians. We assessed the level of clinician variation with a clinical croup pathway in the pediatric emergency department (PED) and the impact of a pathway change on admission rates. METHODS: This was a retrospective cohort study of children with croup in a PED at an academic referral hospital between 2018 and 2022. Primary outcome measures were the proportion of admissions to the hospital and mean PED length of stay. The main exposures were the primary clinician and their use of racemic epinephrine. We used generalized
