COVID-19 vaccination hesitancy and uptake: Perspectives from people released from the Federal Bureau of Prisons.

People in United States (US) prisons and jails have been disproportionately impacted by the COVID-19 pandemic. This is due to challenges containing outbreaks in facilities and the high rates of health conditions that increase the risk of adverse outcomes. Vaccination is one strategy to disrupt COVID-19 transmission, but there are many factors impeding vaccination while in custody. We aimed to examine the perspectives of former residents in the Federal Bureau of Prisons (BOP) regarding COVID-19 vaccine hesitancy and acceptance. Between September-October 2021, we conducted semi-structured interv

[UHSI in the somatic care system for prisoners].

Inter-regional secure hospital units have noted an increase in the complex pathologies and care load of detained patients they receive for somatic care in a short-stay scheduled hospitalisation. Communication between health care providers and prison staff is essential for the proper functioning of these closed units. Transmissions with level 1 health units are essential to ensure continuity of care upon the patient's return to custody.

Challenges Within North Carolina Correctional Medicine.

North Carolina has approximately 36,500 prisoners. As the population becomes more complex, the Department of Public Safety still has the responsibility to provide medical care to all those in its custody. Despite staffing shortages, limited financial resources, and logistical challenges the state must continue to provide access to care.

Sexual harassment in the hospital industry: an empirical inquiry.

A 1994 survey of hospital HRMs has revealed that charges of sexual harassment within the health care industry are increasing at an alarming rate. Most charges are filed by women who claim they were victims of "hostile environments." Nurses levy the largest number of complaints, followed by clerical/secretarial personnel, technicians, custodial workers, food service personnel, and therapists. Most charges are filed against coworkers. Approximately 10 percent of all charges of sexual harassment in hospitals are brought against physicians.

National consensus statement on opioid agonist treatment in custodial settings.

INTRODUCTION: Opioid use and dependence are prevalent among incarcerated people, contributing to elevated rates of overdose and other harms in this population. Opioid agonist treatment (OAT) has been shown to be an effective intervention to mitigate these risks. However, challenges to health care implementation in the custodial sector result in suboptimal and variable access to OAT in prisons nationally. MAIN RECOMMENDATIONS: Among a national multidisciplinary expert panel, we conducted a modified Delphi study that yielded 19 recommendations to government, relevant health authorities and cust

'I'm the One With the Degree:' Challenges to Medical Autonomy in Correctional Healthcare in the United States.

Correctional healthcare workers face numerous challenges delivering healthcare to incarcerated patients. This study aims to understand the ways these healthcare workers struggle to assert their medical and clinical independence from custody staff. This study employed a qualitative design, using in-depth interviews with 46 correctional healthcare workers across the United States in 2022. Results indicate that the delivery of correctional healthcare is a product of negotiated order in detention facilities. Participants and custody staff coproduce an informal hierarchy that promotes deference to

Training Among Noncustodial Health Care Workers Caring for Patients Experiencing Incarceration: A Preliminary Investigation.

People experiencing incarceration in the United States receive much of their health care outside of custodial settings. Optimizing care in this setting requires further understanding of the training and experiences of noncustodial health care workers. We conducted a cross-sectional, exploratory survey of health care workers at a single academic institution to assess their training and experiences related to caring for this patient population. Of 333 respondents, 94.1% had cared for patients experiencing incarceration but only 22.5% had received any formal training, with 94.6% somewhat or very

Emergency department presentation of 'delusional parasitosis by proxy'. Delusional parent, injured child.

We report a case of 'delusional parasitosis by proxy'. A sixyear old child was brought to the emergency department by a mother with concerns that her son had a skin and scalp infestation. Despite the absence of any clinical findings being found on exam, the mother remained disproportionately concerned. Follow up care was recommended with the child's primary care. The mother returned to the ED with her child three weeks later with concerns that her son had an inflamed scalp and eyes. The mother remained insistent that the child was infested with bugs and she had sought care at two other locatio

Providing Mental Health and Substance Use Services to People Released From Custody During the COVID-19 Pandemic: A Qualitative Study of Service Providers.

This paper explores the perspectives of mental health and substance use providers who supported people released from custody during the pandemic in Ontario, Canada, with a focus on service disruptions, adaptations, and experiences. We conducted 20 in-depth qualitative interviews and analysed these using Applied Thematic Analysis. Pandemic public health responses limited the ability of providers to support people released from custody. Policies designed to mitigate viral spread were not tailored to the mental health and substance use context, leading to disruptions in service delivery. This had

A Thirty-Year Forensic Case Report of Chronic Psychosis: Systemic Implications for Italian Forensic Psychiatry.

Background: Italy's forensic psychiatric system has undergone major reforms over recent decades, shifting from custodial institutions to a right-based, community-oriented model. While internationally praised for its commitment to dignity, recovery, and social reintegration, this framework presupposes a degree of clinical recoverability that is not always achievable. Individuals with chronic, treatment-resistant disorders-particularly those who remain socially dangerous-pose complex challenges that strain the system. Methods: We report a thirty-year longitudinal case of G.D.M.: a man initially

PTSD Among Aboriginal and Torres Strait Islander People in Custody in Australia: Prevalence and Correlates.

Mental disorder and trauma experiences are highly prevalent among individuals in custody; however, the impact of posttraumatic stress disorder (PTSD) on functioning is rarely considered. Indigenous Australians are incarcerated at 13 times the rate of nonindigenous Australians and report high levels of trauma exposure and psychological distress. In analysis of the largest systematic study of mental disorder among indigenous Australians in custody (N = 396), we found that the 12-month prevalence of PTSD was high in both men (12.1%) and women (32.3%). Having PTSD was also associated with high rat

Development of an evidence-based hepatitis C education program to enhance public health literacy in the Australian prison sector: The Hepatitis in Prisons Education program (HepPEd).

BACKGROUND: Australia's prisons have a high chronic hepatitis C (HCV) prevalence (8 %). Antiviral therapies and prison-based hepatitis services are available, but only a minority of those eligible are being treated. Improving the HCV public health literacy of the prison sector via targeted education may overcome key barriers to scale-up treatment. This paper describes the: i) HCV public health literacy of the prison setting; ii) barriers and solutions for HCV education and service engagement; iii) HCV education program co-design and development processes; and iv) HepPEd resources. METHODS: A

Understanding barriers and facilitators of implementing a new assessment and bridging tool to support those at risk of repeat self-harm in prison: the qualitative RAPPS study.

BACKGROUND: Rates of self-harm and suicide in prisoners in England and Wales are high, exceeding rates observed in the general population, of similar age and gender. Assessment, Care in Custody and Teamwork (ACCT) is the prison service's self-harm monitoring process. Closure of this process is a high-risk time where people may be at risk of self-harm. Mechanisms to manage risk, particularly following closure of the ACCT management process, mean that many people subsequently self-harm while not being monitored. This creates an opportunity to evaluate the examination of a new tool that could be

Psychosocial and behavioral factors related to the post-partum placements of infants born to cocaine-using women.

OBJECTIVE: One objective was to determine if cocaine-using women who did not maintain infant custody (NMC) would report more psychological distress, domestic violence, negative coping skills, lower social support and more childhood trauma than cocaine-using women who maintained custody (MC) of their infant. A second objective was to evaluate the relative contribution of psychosocial factors to infant placement. METHODS: Psychosocial profiles of MC women (n=144) were compared with NMC (n=66) cocaine-using women. Subjects were low income, urban, African-American women who delivered an infant at

Aboriginal and non-aboriginal Australian former prisoners' patterns of morbidity and risk of hospitalisation.

BACKGROUND: People who have been in custody are more likely to experience multiple, long standing health issues. They are at high risk of illness and injury post release and experience poor access to health services both of which contribute to high rates of recidivism. The study was conducted to examine Aboriginal and non-Aboriginal former prisoners' risk of hospitalisation and rehospitalisation in the five years post release from custody and identified the common reasons for hospitalisations. METHODS: Common reasons for hospital admission were identified by conducting descriptive analysis of

Treatment for substance use disorders in pregnant women: Motivators and barriers.

BACKGROUND: Pregnancy is a unique opportunity to provide broad and necessary medical care for women- including treatment for Substance Use Disorders (SUD). The standard of care for SUD in pregnant women is treatment at a comprehensive care facility. There is little existing qualitative research exploring what brings pregnant women with SUD to treatment and what barriers to treatment exist for this population. This study explored women's self-reported reasons for pursuing treatment or hesitating to do so. METHODS: This qualitative study used interviews to explore common factors that motivate p
