Eye drops used for diagnostic mydriasis may produce systemic side effects in preterm infants. The dynamics of pupillary dilation induced by Phenylephrine 10% and Cyclopentolate 1% have been examined by flash photography. A correlation between anterior chamber depth and the pupil shape on dilation with Phenylephrine Hydrochloride 10% is described. It is postulated that these pupillary dilation dynamics support a sympathetic abnormality as a trigger for acute primary angle closure glaucoma.

In a prospective study of 26 premature infants, 5 microliters microdrops were compared with standard 26 microliters eye drops of cyclopentolate 0.5% and phenylephrine 2.5%. There was no statistical difference in pupil dilatation. The 5 microliters microdrops have potentially fewer adverse effects in premature infants.

Five children, admitted with acute psychotic reactions after use of Scopolamine eye drops 0.2%, are presented. The predominant symptoms were visual hallucinations, strange behaviour, and restlessness. In one case peripheral symptoms were observed. Recovery was complete in 24 h. Prescription of Scopolamine eye drops should be followed by information of possible adverse effects. Cyclopentolate might be used instead.

OBJECTIVE: This study was aimed to determine mydriatic regimen(s) used in neonatal units in Aotearoa, New Zealand (NZ), and Australia and to estimate the frequency of adverse drug events following mydriatic administration in preterm neonates.

We compared the depth of the anterior chamber and the optimal distance refraction in a group of patients with soft and rigid intraocular implants under pilocarpine (maximal ciliary contraction) and cyclopentolate (maximal ciliary relaxation) in order to determine if lens movement might account for the apparent accommodation phenomenon. Lens shifts ranging from 1.5 to 0.02 mm and refractive variations up to 1 D were found. However, the discrepancies between amount of shift and refractive variations suggest that lens movement does not play a relevant role in this phenomenon.

A 2-month-old girl who had been born at 27-weeks' gestation was admitted for her screening examination for retinopathy of prematurity and given two drops each of cyclopentolate 0.5% and phenylephrine 2.5%. Approximately 2 h after completion of the examination, the infant had episodes of apnoea and vomiting. She was noted to be distended, and an abdominal radiograph demonstrated acute gastric dilatation. Apnoea, vomiting and distension resolved after 18 h and a repeat abdominal radiograph demonstrated resolution of the gastric dilatation.

Acute onset of transient myopia may be induced by several drugs, different mechanisms having been described. We report the occurrence of such an event after intravenous administration of equine antilymphocyte globulins in a patient treated for aplastic anemia. Clinical ocular examination revealed no other abnormality, and ultrasonography disclosed no swelling of the ciliary body.
