Robotic assisted laparoscopic repair of vesico-vaginal fistula: the extravesical approach.

The optimal method of vesico-vaginal fistula (VVF) repair remains undetermined. Almost all previous descriptions of laparoscopic/robotic fistula repair involve utilizing a vertical cystotomy to identify the fisula. Avoidance of an intravesical approach to vesico-vaginal fistula repair may decrease patient morbidity. Patient selection, patient positioning, fistula recognition, port placement, intra-operative dissection techniques, flap formation, and repair are outlined in this video of robotic repair of vesico-vaginal fistula utilizing an extravesical approach. The extravesical robotic repair

Hand-assisted laparoscopic nephroureterectomy: analysis of distal ureterectomy technique, margin status, and surgical outcomes.

OBJECTIVES: To review our experience with various techniques used to manage the distal ureter during hand-assisted laparoscopic nephroureterectomy and to evaluate the surgical outcomes, including pathologic margin status and the incidence of disease recurrence. METHODS: We retrospectively analyzed 55 hand-assisted laparoscopic nephroureterectomies performed to treat transitional cell carcinoma (TCC), with the distal ureter managed as follows: cystoscopic disarticulation in 16 patients, stapled division in 7, open distal ureterectomy in 3, and hand-assisted laparoscopic extravesical en bloc di

Robotic extramucosal excision of bladder wall leiomyoma.

INTRODUCTION: Multiple case reports and reviews have been described in the literature for bladder wall leiomyoma resection via different approaches. The minimally invasive partial cystectomy remains the most widely accepted technique; however, case reports for enucleation of bladder wall leiomyoma have also been described. The purpose of this video is to demonstrate the robotic extramucosal excision of a bladder wall leiomyoma, without cystotomy, but with complete removal of the muscular layer. MATERIALS AND METHODS: A 35-year old male present with lower urinary tract symptoms and imaging sho

Transvesical NOTES: survival study in porcine model.

BACKGROUND AND OBJECTIVES: The optimal access route and method for natural orifice transluminal endoscopic surgery (NOTES) has not been established. A transvesical approach, with its low rate of peritoneal contamination, is an effective clean portal of entry, but a safe urinary bladder closure has been a challenge. We developed a new technique for a safe, pure transvesical NOTES approach. METHODS: Four female piglets were used in the study. With the pigs under anesthesia, a flexible cystoscope (15Fr) was used to make an endoscopic cystotomy; diagnostic peritoneoscopy of the abdominal quadrant

One-stage transthoracic operation for the treatment of right lung and liver hydatid cysts.

In this study we reviewed our experience of hydatid disease of the lung and the liver and discussed the safety and the follow-up results of the one-stage operation. Between 1990 and 2004, 142 patients with pulmonary hydatid disease underwent operation in our clinic. Of these, 27 (19%) patients had cysts located on the dome of the liver, treated with phrenotomy through a right thoracotomy. Hydatid cysts located in the lungs were managed by means of cystotomy. For liver cysts, cystotomy and the inversion of the cavity with sutures was the surgical method of choice, and a drain was left in place.

Laparoscopic pancreatic cystgastrostomy: the lesser sac technique.

Management of pancreatic pseudocysts has remained largely unchanged during the past century. Excision and drainage procedures remain the mainstay of therapy for large, persistent, or symptomatic pseudocysts. Laparoscopic approaches to the management of pancreatic pseudocysts have been previously described. The lesser sac approach is an effective means of creating a hemostatic cystgastrostomy through a single posterior gastrotomy. This procedure is performed by creating a cystotomy and posterior wall gastrotomy through which an endoscopic stapler is applied. The cystotomy-gastrotomy is closed u

Morphologic Factors Associated With Open Conversion During Holmium Laser Enucleation of the Prostate.

OBJECTIVE: To determine if there is an association between patient body habitus as measured by body mass index (BMI), body surface area (BSA), preoperative prostate volume, postoperative specimen weight, and open conversion with cystotomy or perineal urethrotomy (PU) during holmium laser enucleation of the prostate (HoLEP). We attempt to provide meaningful criteria to assist in preoperative patient counseling. MATERIALS AND METHODS: Three hundred consecutive patients underwent HoLEP between August 3, 2018 and February 20, 2020 by a single surgeon. Patient metrics were recorded in a database i

Laparoscopic removal of an intravesical foreign body after laparoscopically assisted vaginal hysterectomy: a case report and review of the literatures.

A foreign body in the bladder is a rare occurrence. Although most intravesical foreign bodies can be removed successfully using endoscopic transurethral extraction, suprapubic cystotomy may be required for the removal of large bodies. We report a case of an intravesical mass 8 cm in diameter that was successfully removed using laparoscopic cystotomy without further complications. The foreign body was a leiomyoma of the uterus that had not been removed during laparoscopically assisted vaginal hysterectomy. The myoma entered into the bladder through a defect that was not diagnosed nor repaired u

Simultaneous operation for coexisting lung and liver hydatid cysts: a treatment modality.

BACKGROUND/AIMS: Hydatid disease is very common in our region. Whether or not coexisting cysts of lung and liver should be operated on simultaneously is a challenging question. METHODOLOGY: Of 127 patients operated on for hydatid cysts of the lung between 1990 and 1995, 23 (18%) had coexisting hydatid cysts of the right lung and liver. There were 12 male and 11 female patients with an average age of 34 years. RESULTS: All patients had simultaneous operations for both organs via right thoracotomy with an incision in the diaphragm (phrenotomy). In the lung, cystotomy and capitonnage were perfo

A Case Report of Liver and Pancreatic Echinococcal Cysts.

A 54-year-old female patient with complaints of abdominal pain for 2 months was admitted to the General Surgery clinic of our tertiary care hospital. Abdominal ultrasound (US) and computed tomography revealed cholelithiasis, liver hydatid cyst, and hypodense multicystic mass lesion in the pancreatic body. In the endoscopic US performed, pancreatic hydatid cysts were considered as the pre-diagnosis. Cystotomy and external drainage were performed on the 8-cm cystic lesion in the pancreas head-class junction. Pancreatic hydatid cyst can be rarely seen isolated or can develop synchronously to live

Laser treatment of giant iris cyst with nanophthalmos.

We report a case of a giant iris cyst treated with neodymium-doped yttrium aluminum garnet laser cystotomy. A 33-year-old woman presented with nanophthalmos and ocular hypertension secondary to a giant iris cyst after congenital cataract surgery in the left eye. Examination with a rotating Scheimpflug camera and anterior-segment optical coherence tomography revealed a large iris cyst surrounding the pupillary region. She underwent laser iridocystotomy and irrigation of the anterior chamber. No recurrence of the iris cyst has been noted after 18 months and the IOP is normal. The cyst was treate

Supratrigonal VVF repair by modified O'Connor's technique: an experience of 26 cases.

OBJECTIVE: To report the technical modifications of O'Connor's procedure and their outcome in 26 supratrigonal vesico vaginal fistulae. MATERIALS AND METHODS: Twenty-six cases of supratrigonal VVF (17 primary, 9 recurrent) were operated using the described modifications. It consisted of approaching the bladder transperitoneally, without dissecting the retropubic space, making a short sagittal or parasagittal cystotomy in between stay sutures, liberal use of bladder rotation flaps instead of midline closure, using single layer, continuous, closely placed, interlocking stitches for bladder as w

Conservative surgery for treatment of hydatid cysts in children.

Human echinococcosis is endemic in Turkey and many other areas of the world. Fifteen years of surgical assessment related to involved structures, operative procedures, complications, and the recurrence rate is analyzed in pediatric patients with pulmonary and abdominal hydatid cysts. Between 1986 and 2001, the records of 42 pediatric patients (15 girls, 27 boys; ages 2-15 years) with pulmonary and abdominal hydatid cysts operated on in our clinic were analyzed retrospectively. The features of the disease, involved structures, operative procedures, complications, and recurrences were noted. A t

Calcified double J stent removed at 10 years: a case report.

In our current practice, the use of JJ probes has become extremely frequent. However, incrustation and fragmentation of JJ leads are still relatively common and sometimes complicate removal. A 61-year-old woman with a history of hysterectomy ten years ago, she had a double J endo-ureteral stent for preoperative identification of the right ureter. The patient has forgotten the double J stent. She currently has right lower back pain and urinary tract symptoms of urinary. Uroscan revealed a very important right hydronephrosis, a fully calcified right double J stent with a calcification of 6 cm at

Murine model for the evaluation of candiduria caused by Candida tropicalis from biofilm.

To evaluate the pathophysiology of catheter-associated candiduria, the bladders of female mice were infected with Candida tropicalis. One group was implanted with a catheter fragment with preformed biofilm by cystotomy technique, while another group received, in separate, a sterile catheter fragment and a correspondent yeast suspension. The bladder tissues were examined by histopathology and the quantity of colony forming units was evaluated. All the animals presented inflammation and the presence of C. tropicalis was observed in the tissue within 72 h of the introduction of biofilm, while 75%
