Human factors and airway management: focus on the Swiss Airway Guidelines.

Although decision-making in airway management is crucial for performance and outcomes, it often deteriorates in crisis situations. Studies suggest that human decision-making can be unreliable in such situations and must be supported to reduce reliance on exceptional human performance to ensure safety. Airway guidelines provide essential evidence-based information intended to maximize the likelihood of successful airway management. However, the linear, textual and inflexible design of most algorithms does not align with the naturalistic decision-making processes involved in clinical activity. T

Advanced Diabetes Management Using Artificial Intelligence and Continuous Glucose Monitoring Sensors.

Wearable continuous glucose monitoring (CGM) sensors are revolutionizing the treatment of type 1 diabetes (T1D). These sensors provide in real-time, every 1-5 min, the current blood glucose concentration and its rate-of-change, two key pieces of information for improving the determination of exogenous insulin administration and the prediction of forthcoming adverse events, such as hypo-/hyper-glycemia. The current research in diabetes technology is putting considerable effort into developing decision support systems for patient use, which automatically analyze the patient's data collected by C

AI-Driven Heart Failure Decision Support in Skilled Nursing Facilities.

Heart failure management in skilled nursing facilities (SNFs) is complicated by limited access to specialists, incomplete clinical documentation, and patients with complex comorbidities. Artificial intelligence clinical decision support systems have been developed mostly for acute hospital settings but not for SNF settings. We present ForeSight-HF, an artificial intelligence clinical decision support tool designed specifically for SNF heart failure management. Built on a large language model with HIPAA-compliant infrastructure, ForeSight-HF synthesizes unstructured clinical documentation to ge

Design of a JAIN SLEE/ESB-based platform for routing medical data in the ICU.

The importance of computer aided decision making is continuously increasing. In the ICU, medical decision support services gather and process medical data of patients and present results and suggestions to the medical staff. The medical decision support services can monitor for example blood pressure, creatinine levels or the usage of antibiotics. If certain levels are crossed, they raise alerts so that the medical staff can take appropriate actions if required. This significantly reduces the amount of data needing to be processed by the medical staff. To handle the large amount of data that i

Clinical decision support systems for polypharmacy optimization in older patients: a narrative review.

PURPOSE: Multimorbidity and polypharmacy are increasingly prevalent in the older population and are associated with a higher risk of potentially inappropriate medications (PIM), drug-drug interactions (DDI), and adverse drug reactions (ADR). Although medication review (MR) and deprescribing are effective strategies, their manual implementation can be complex, time-consuming, and prone to clinical variability. Clinical Decision Support Systems (CDSS) offer advanced digital solutions to optimize polypharmacy by analyzing multidimensional clinical data and generating personalized recommendations.

Having and eating the cake? The centralisation paradox in decentralised healthcare governance: a scoping review.

BACKGROUND: Decentralisation in healthcare governance is widely promoted to enhance managerial autonomy, responsiveness, and performance. However, empirical evidence suggests that managers operating in decentralised systems continue to rely on centrally coordinated organisational structures. This interdependence remains insufficiently conceptualised in existing research. METHODS: A scoping review was conducted to synthesise empirical studies examining how central coordination in organisational systems shapes managers' effective decision space in decentralised healthcare organisations. Medical informatics academia and industry: a symbiotic relationship that may assure survival of both through health care reform.

There are often clear lines drawn identifying the demilitarized zone between medical informatics academics and industry. Academics were "pure" intellectuals sequestered in ivory towers that effectively shielded them from the realities of the world. Industry has historically focused on creating effective products that produce financial return to the corporation. Both the paradigms of academia and industry are quickly becoming dinosaurs in the era of health care reform where both medical informatics academia and industry are under increasing pressure to develop and prove that medical informatics

AGA Technical Review on the Medical Management of Moderate to Severe Luminal and Perianal Fistulizing Crohn's Disease.

The incidence and prevalence of Crohn's disease (CD) is rising globally. Patients with moderate to severe CD are at high risk for needing surgery and hospitalization and for developing disease-related complications, corticosteroid dependence, and serious infections. Optimal management of outpatients with moderate to severe luminal and/or fistulizing (including perianal) CD often requires the use of immunomodulator (thiopurines, methotrexate) and/or biologic therapies, including tumor necrosis factor-α antagonists, vedolizumab, or ustekinumab, either as monotherapy or in combination (with immun

Application of an enhanced spill management information system to inland waterways.

Spill response managers on inland waterways have indicated the need for an improved decision-support system, one that provides advanced modeling technology within a visual framework. Efforts to address these considerations led the authors to develop an enhanced version of the Spill Management Information System (SMIS 2.0). SMIS 2.0 represents a state-of-the-art 3D hydrodynamic and chemical spill modeling system tool that provides for improved predictive spill fate and transport capability, combined with a geographic information systems (GIS) spatial environment in which to communicate propagat

Algorithm-based decision support for symptom self-management among adults with Cancer: results of usability testing.

BACKGROUND: It is essential that cancer patients understand anticipated symptoms, how to self-manage these symptoms, and when to call their clinicians. However, patients are often ill-prepared to manage symptoms at home. Clinical decision support (CDS) is a potentially innovative way to provide information to patients where and when they need it. The purpose of this project was to design and evaluate a simulated model of an algorithm-based CDS program for self-management of cancer symptoms. METHODS: This study consisted of three phases; development of computable algorithms for self-management

Knowledge engineering using retrospective review of data: a useful technique or merely data dredging?

The process of extracting the knowledge or rules for medical decision making is not an easy task. One approach to knowledge engineering is to carefully review how decisions were made in the past with the goal of extracting the rules. The purpose of this project was to use previously collected data from ICU patients to derive the rules for the definition of hemodynamic stability. 97 ICU patients between 9/9/86 and 7/29/90 were included in the analysis. All of these patients had adult respiratory distress syndrome. Their mechanical ventilation was managed by a set of computerized protocols. We r

[Technology-driven management of early-onset scoliosis:new technologies and concepts].

Early-onset scoliosis (EOS) refers to spinal deformity occurring in children younger than 10 years of age. The main goals of EOS treatment are not only to correct spinal curvature, but also to preserve spinal and thoracic growth potential. In recent years, the rapid development and clinical application of robotic technology and artificial intelligence (AI) have driven a paradigm shift in EOS management from experience-based practice toward intelligent and data-driven care. Emerging technologies, including robotic navigation, virtual and augmented reality (AR), AI-based imaging analysis, wearab

Enhancing antibiotic stewardship by tackling "spurious" penicillin allergy.

Approximately 90-99% of patients with a label of penicillin allergy (PenA) are not allergic when comprehensively investigated. An inaccurate label of PenA has major public health implications-longer hospital stay, more frequent hospital admissions, greater use of fluoroquinolones, glycopeptides, cephalosporins and other expensive antibiotics resulting in significantly higher costs to the health service and predisposing to Clostridium difficile, methicillin-resistant Staphylococcus aureus infections and vancomycin-resistant enterococcus. We describe lessons learnt from recent studies regarding
