The efficacy and safety of sugammadex for reversing postoperative residual neuromuscular blockade in pediatric patients: A systematic review.

The aim of this study is to evaluate the efficacy and safety of sugammadex for reversing neuromuscular blockade in pediatric patients. MEDLINE and other three Databases were searched. Randomized clinical trials were included if they compared sugammadex with neostigmine or placebo in pediatric patients undergoing surgery involving the use of rocuronium or vecuronium. The primary outcome was the time interval from administration of reversal agents to train-of-four ratio (TOFr, T4/T1) > 0.9. Incidences of any drug-related adverse events were secondary outcomes. Trial inclusion, data extraction, a

Dexmedetomidine Dosing to Prevent Pediatric Emergence Delirium.

Because of the high incidence and untoward effects of emergence delirium in the pediatric population, investigating pharmacologic measures for preventing this phenomenon is important to the anesthesia provider. Dexmedetomidine, a highly selective α2 agonist, has been shown to prevent emergence delirium in the perioperative setting; however, recommendations for best practice regarding use of this medication are not widely available. Barriers to the use of dexmedetomidine may include side effects such as bradycardia and delayed emergence, as well as limited evidence for the best practice of timi

Effect of flumazenil on recovery from sevoflurane anesthesia in children premedicated with oral midazolam before undergoing herniorrhaphy with or without caudal analgesia.

PURPOSE: Oral midazolam is frequently used to treat children, but its effect on recovery from anesthesia is controversial. This study was designed to evaluate the effect of flumazenil on reversal of midazolam during recovery from sevoflurane-induced anesthesia in children who underwent caudal analgesia compared to those who did not. METHODS: A series of 60 children 1-8 years of age, with an American Society of Anesthesiologists (ASA) physical status of 1 or 2, who were scheduled to undergo herniorrhaphy were randomly assigned to one of four groups: group 1, control/placebo; group 2, control/f

Neuromuscular and Clinical Recovery in Thoracic Surgical Patients Reversed With Neostigmine or Sugammadex.

BACKGROUND: Patients undergoing thoracoscopic procedures may be at high-risk for incomplete neuromuscular recovery and associated complications. The aim of this clinical investigation was to assess the incidence of postoperative residual neuromuscular blockade in adult thoracic surgical patients administered neostigmine or sugammadex when optimal dosing and reversal strategies for these agents were used. The effect of choice of reversal agent on hypoxemic events and signs and symptoms of muscle weakness were also determined. Additionally, operative conditions in each group were graded by surge

Delayed Recovery From Anesthesia Revealing Unrecognized Metabolic Acidosis and Hypokalemia in a Patient With Urinary Diversion.

Delayed recovery from anesthesia may result from unrecognized metabolic and electrolyte abnormalities. We report a case of an 18-year-old female with prior ureterosigmoidostomy who underwent ureteric reimplantation under general anesthesia. She was found to have severe hypokalemia with metabolic acidosis in the perioperative period, which had not been identified preoperatively. Postoperatively, she exhibited neuromuscular weakness and inadequate respiratory effort, requiring elective ventilation. Correction of the underlying abnormalities resulted in rapid recovery and successful extubation. T

Dissociative amnesia after general anesthesia--a case report.

Psychogenic unconsciousness is a rare cause of failure of prompt recovery from general anesthesia. The diagnosis is only made by exclusion of other conditions. We describe a young, healthy female who failed to wake up promptly after total intravenous anesthesia with alfentanil and propofol. She regained consciousness 24 hours later without any specific treatment. However she sustained amnesia for a period of 48 hours. Dissociative disorder was diagnosed after an extensive workup. This case report emphasizes the importance of inclusion of psychiatric disorder in the differential diagnosis shoul

Perioperative hypothermia and its clinical consequences in geriatric patients undergoing abdominal surgery: a retrospective cohort study.

OBJECTIVE: Geriatric patients are highly susceptible to perioperative hypothermia due to diminished physiological reserve, comorbidities, and polypharmacy. Even mild temperature reductions may contribute to cardiac instability, delayed emergence from anesthesia, coagulopathy, increased blood loss, and elevated infection risk. This study aimed to evaluate the clinical impact of perioperative hypothermia in geriatric patients undergoing abdominal surgery. STUDY DESIGN: Retrospective observational study. METHODS: Medical records of 226 geriatric patients (ASA II–III) who underwent abdominal lap

Severe neurologic manifestations in acute intermittent porphyria developed after spine surgery under general anesthesia: a case report.

Porphyrias are inherited metabolic disorders resulting from a specific enzyme defect in the heme biosynthetic pathway. Porphyrias are induced by various precipitants. Clinical features include abdominal pain, neurologic manifestations, autonomic neuropathy, and mental disturbance. Diagnosis may be delayed because of variable symptoms that mimic other diseases and because of the rarity of of porphyrias. Although most patients with known porphyria can complete anesthesia and surgery safely, undiagnosed porphyric patients are in danger of porphyric crisis due to inadvertent exposure to precipitat

Randomised controlled trial of sugammadex or neostigmine for reversal of neuromuscular block on the incidence of pulmonary complications in older adults undergoing prolonged surgery.

BACKGROUND: Residual neuromuscular block has been associated with postoperative pulmonary complications. We hypothesised that sugammadex reduces postoperative pulmonary complications in patients aged ≥70 yr having surgery ≥3 h, compared with neostigmine. METHODS: Patients were enrolled in an open-label, assessor-blinded, randomised, controlled trial. At surgical closure, subjects were equally randomised to receive sugammadex 2 mg kg-1 or neostigmine 0.07 mg kg-1 (maximum 5 mg) for rocuronium reversal. The primary endpoint was incidence of postoperative pulmonary complications. Secondary endpo

Small-dose droperidol effectively reduces nausea in a general surgical adult patient population.

UNLABELLED: In this prospective, randomized, placebo-controlled study, we (1) determined whether 0.625 mg of IV droperidol given 30 min before emergence from general anesthesia reduces the incidence of immediate and delayed postoperative nausea and vomiting (PONV) in a general surgical adult patient population, and (2) compared the efficacy of droperidol, ondansetron, and promethazine for the rescue treatment of PONV. One hundred fifty adult patients receiving general anesthesia for >2 h received either droperidol (0.625 mg IV) or a placebo before emergence. Patients requiring treatment for PO

Delayed emergence after general anesthesia: working through the differential diagnosis.

INTRODUCTION AND IMPORTANCE: Delayed emergence (DE) following general anesthesia is a critical postoperative complication characterized by the patient's inability to regain an appropriate level of consciousness 30-60 minutes after surgery. Rapid identification and management of potential causes, including metabolic disturbances and pharmacologic effects, are essential to prevent adverse outcomes. CASE PRESENTATION: We present the case of a 67-year-old female with multiple comorbidities, including chronic obstructive pulmonary disease, hemochromatosis, and hypothyroidism, who underwent emergen

Sunset Sign Due to Intraventricular Tension Pneumocephalus: A Key Clue to Evaluating Delayed Emergence After General Anesthesia.

Prompt emergence from general anesthesia is crucial after neurosurgical procedures, such as craniotomies, to facilitate timely neurological evaluation for identification of intraoperative complications. Delayed emergence can be caused by residual anesthetics, metabolic imbalances, and intracranial pathology, for which an eye examination can provide early diagnostic clues. The sunset sign (or setting sun sign), characterized by a downward deviation of the eyes, can be an early indicator of raised intracranial pressure (ICP) or midbrain compression, as is commonly observed in states of hydroceph

Non-convulsive status epilepticus as a cause of delayed emergence after a thoracic surgery: a case report.

Non-convulsive status epilepticus (NCSE) is an electrical discharge which occurs without prominent motor symptoms. NCSE is one of the causes of delayed emergence from anesthesia; however, as far as we know, previous reports of postoperative NCSE were related to patients after neurological surgery. Herein, we report a case of an elderly male who developed initial NCSE after thoracic surgery. The patient remained unresponsive and developed hemiplegia after lung resection, and then the symptoms fluctuated between better and worse. Metabolic disorders and stroke were ruled out, and NCSE was diagno

Warning and Nursing Experience of Anesthesia Depth Monitoring for Patients with General Anesthesia Delayed to Leave Anesthesia Recovery Room and Delirium.

Affected by the residues of narcotic drugs, patients under general anesthesia are vulnerable to emergence of agitation, delirium, hemodynamic changes, and other adverse events in the recovery period of anesthesia. Therefore, it is necessary to strengthen the observation and care of these patients. Depth of anesthesia monitoring (DAM) has always been a concern for anesthesiologists, but there are few reports related to it. This study compared the early warning value of DAM for patients under general anesthesia with delayed exit from the anesthesia recovery unit (PACU) and delirium and summarize

Risk factors for unanticipated hypertension during emergence from general anesthesia in elderly surgical patients: A retrospective cohort analysis.

Unanticipated hypertension in the postanesthesia care unit (PACU) refers to acute blood pressure elevation in patients whose preoperative baseline blood pressure was normal, possibly resulting in adverse postoperative complications. This study aimed to identify potential factors and the impact of unanticipated hypertension during emergence from general anesthesia in the elderly surgical population. A retrospective study was done at the Affiliated BenQ Hospital of Nanjing Medical University. Elderly surgical patients who had been admitted to the PACU for recovery from general anesthesia were en
