Juvenile dermatomyositis.

Thirteen cases of dermatomyositis seen in children are described. Records of hospitals serving 250,000 children were reviewed for the 10-year period during which these children were seen and no other cases were found. Eleven of the 13 are living, including seven believed to be in complete remission; the remainder have ongoing disease of varying severity. Literature concerning this unusual childhood disease is reviewed and there is general agreement as to the characteristic nature of the clinical picture. Special features, including rash, muscle findings, deformity, calcinosis and gastrointesti

Magnesium abnormalities of skeletal muscle in dermatomyositis and juvenile dermatomyositis.

OBJECTIVE: To characterize abnormalities in magnesium levels in the muscles of patients with dermatomyositis (DM) and juvenile dermatomyositis (JDM) and to evaluate the beneficial effects of prednisone and immunosuppressive therapy in elevating free magnesium (Mg(2+)) and ATP-bound magnesium (Mg-ATP). METHODS: The study groups consisted of 12 adult patients with DM and 10 juvenile patients with JDM. The 2 control groups were 11 normal adults and 6 healthy children. Levels of total ATP in the quadriceps muscles of the subjects were determined during rest, exercise, and recovery, using noninvas

Dermatomyositis in two siblings and a brief review of familial dermatomyositis.

Juvenile dermatomyositis is an uncommon autoimmune disease with classic heliotrope discoloration of the eyelids, erythematous skin rash of joints, and proximal muscle weakness. It is most frequently sporadic and only rarely familial. We present juvenile dermatomyositis in a 5-year-old brother and a 3 1/2-year-old sister, both are very responsive to corticosteroids. Familial dermatomyositis can occur in different family members, and even dermatomyositis and polymyositis can coexist in the same family.

Dermatomyositis pemphigoides: a case with coexistent dermatomyositis and bullous pemphigoid.

We describe a patient with coexistent dermatomyositis and bullous pemphigoid; both appeared within a few weeks. Protein blotting showed binding of the patient's serum to the classic 220 kd bullous pemphigoid antigen. Because of the close temporal association of the two disorders, we believe that they are almost certainly etiologically linked. One possibility is that exposure of basement membrane antigens by dermatomyositis led to exposure of bullous pemphigoid antigen and subsequent antibody formation.

Features distinguishing clinically amyopathic juvenile dermatomyositis from juvenile dermatomyositis.

OBJECTIVE: We examined features of clinically amyopathic JDM (CAJDM), in which patients have characteristic rashes with little to no evidence of muscle involvement, to determine whether this is a distinct phenotype from JDM. METHODS: Demographic, clinical, laboratory and treatment data from 12 (9 hypomyopathic, 3 amyopathic) patients meeting modified Sontheimer criteria for CAJDM and from 60 matched JDM patients meeting Bohan and Peter criteria were examined. Differences were evaluated by Fisher's exact and Mann-Whitney tests, random forests and logistic regression analysis. RESULTS: Nine (7

[Juvenile dermatomyositis--statistical observation of 105 patients with dermatomyositis].

Statistical observation about the clinical items of 105 patients with dermatomyositis (DMS) attending Department of Dermatology, Nagoya University Hospital and some hospitals in Aichi Prefecture, during the year of 1965 to 1989, was carried out. Clinical features of the patients with juvenile DMS obtained from the observation were as follows. 1) The male:female ratio was 1.3:1 in juvenile DMS. The evident difference such as the predominance of females in adult DMS was not found. 2) The tendency that cutaneous manifestations usually preceded muscular manifestations was observed, and the muscula

Juvenile dermatomyositis: a statistical study of 114 patients with dermatomyositis.

We conducted a statistical review of 114 cases of dermatomyositis (DMS) treated primarily at the Department of Dermatology at Nagoya University Hospital over 27 years from 1965 to 1991 in order to determine the primary characteristics of juvenile DMS with the following results. 1) Juvenile DMS was found slightly more often in males than females; the male-to-female ratio was 1.4:1. Therefore, unlike adult DMS with its preponderance of females, there was no clear gender predominance. 2) Muscular manifestations tended to follow the appearance of cutaneous manifestations, but the frequency of mino

Innovation in Dermatomyositis.

Dermatomyositis (DM) is a rare autoimmune disease defined by the presence of characteristic cutaneous findings, an increased cancer risk, and variable extracutaneous pathology involving the muscles, lungs, gastrointestinal tract, heart, and/or joints. Although the pathogenesis of DM remains incompletely understood, the discovery of myositis-specific autoantibodies has been an important step forward in understanding disease heterogeneity in DM and stratifying risk for extracutaneous disease and malignancy. Moreover, the recent elucidation of key immunologic drivers of DM has laid the groundwork

Dermatomyositis and MIG.

Several studies demonstrated the presence of an elevated expression of chemokine (C-X-C motif) ligand (CXCL)9/monokine induced by interferon (IFN)-γ (MIG) on macrophages and in T cells in perimysial infiltrates of patients with dermatomyositis (DM), and of chemokine (C-X-C motif) receptor (CXCR)3 expression on the majority of T cells in the same patients. This underlines the importance of T helper 1-mediated immunity, and in particular the MIG/CXCR3 interaction, in the immunopathogenesis of DM. The importance of MIG has been confirmed by a study on patients with DM about the presence of chemok

[Dermatomyositis and Autoantibodies].

Recent studies have identified novel dermatomyositis-specific autoantibodies and revealed that disease-specific autoantibodies become positive at a high rate in this disease. Moreover, these autoantibodies have been demonstrated to show a strong correlation with distinct clinical manifestations and complications such as interstitial lung disease and malignancy. Thus, these autoantibodies are now recognized as useful tools to classify this varied disease into more homogeneous subsets. In this review, the clinical significance of five dermatomyositis-specific autoantibodies, anti-Mi-2, anti-MDA5

The pathogenesis of dermatomyositis.

This review looks at the many different factors thought to play a role in idiopathic inflammatory myopathies (IIM), concentrating mainly on the dermatomyositis (DM) subtype. Subject areas addressed include looking at the different clinical features of IIM, paying particular attention to the skin manifestations. There is a discussion around investigations needed with their perceived value, followed by a description of the immunohistochemical findings of DM. This review goes on to address other attributing factors such as genetic associations with the different subtypes of IIM, and environmental

Dermatomyositis and chemokines.

Abundant expression of IFN-γ-inducible protein 10 (IP-10) was observed on macrophages and in T cells in perimysial infiltrates of dermatomyositis (DM), and strong chemokine (C-X-C motif) receptor (CXCR)3 expression was observed on the majority of T cells in DM. This suggests that the T helper (Th)1-mediated immunity in general, and the CXCR3/IP-10 interaction in particular, is important in the immunopathogenesis of DM. The study of infiltrating cells, related cytokines and chemokine receptors in lesional skin of patients with DM, confirmed the importance of Th1 chemokines. Type I Interferon ch

[Dermatomyositis and pregnancy].

INTRODUCTION: Dermatomyositis is a disorder known as a pathology whose etiology remains unknown. It can occur at any age with clinical symptoms of weakness and wasting, especially of the proximal musculature, due to inflammatory infiltration of muscles and destruction of muscle fibers. It is exceptionally seen during pregnancy, conditioning adverse effects in pregnant women. OBJECTIVE: The principal objective here is the presentation of a clinical case of dermatomyositis during pregnancy. CASE REPORT: Women of 33 years of age with term pregnancy, suffering from dermatomyositis from infancy,

Dermatomyositis and Polymyositis.

Dermatomyositis (DM) and polymyositis (PM) are idiopathic inflammatory myopathies characterized by proximal greater than distal muscle weakness, elevated serum creatine kinase levels, electrophysiologic abnormalities, and inflammation on muscle biopsy. Clinically and electrophysiologically, DM and PM appear very similar, and muscle biopsy is the gold standard for diagnosis. Much of the PM literature based the diagnosis on Bohan and Peter's criteria, which is now obsolete given the advances of immunopathology. As diagnostic criteria for the inflammatory myopathies have been refined, it has beco

[Dermatomyositis in childhood].

AIM: The aim of this study is to review the presenting signs and symptoms, laboratory findings and therapeutic regimens of juvenile dermatomyositis in a tertiary hospital. METHODS: We reviewed retrospectively the available medical records of patients who met the clinicopathologic criteria of Bohan and Peter for definite juvenile dermatomyositis. They were followed between 1986 and july 1999 at the pediatric rheumatology section at our institution. RESULTS: The patient population included 3 male and 6 female patients. The mean age at diagnosis was 7 years. Clinical features demanding medical

[Polymyositis and dermatomyositis].

This review summarizes the current progress in the clinical research on pathogenesis, diagnosis and treatment of polymyositis (PM) and dermatomyositis (DM). Recent studies on immunohistochemical and molecular findings of biopsied muscle tissues have shed light on pathogenetic mechanisms in myositis. Muscle imaging techniques such as ultrasonography and magnetic resonance imaging facilitate the assessment of the type (edema, inflammation, fat, and fibrosis), degree, and localization of these alterations. They are useful for the diagnostic evaluation and the assessment of treatment. Measurement

Dermatomyositis and drugs.

Dermatomyositis (DM) is an idiopathic inflammatory disorder consisting of skin and skeletal muscle involvement. Patients with skeletal muscle involvement have polymyositis (PM), and those unresponsive to therapy and with characteristic findings on muscle biopsy have inclusion body myositis. Patients without muscle damage and typical skin lesions have amyopathic dermatomyositis. Disease in children (juvenile dermatomyositis) is not associated with malignancy as it may be in adults (paraneoplastic dermatomyositis). Overlap syndrome (OS) is mixed connective tissue disease combining some features

Dermatomyositis and malignancy.

During the past 12 years, many studies applying strict diagnostic criteria have been published that have attempted to settle the controversy about the reality of the association between dermatomyositis and malignancy. Although retrospective, recent studies have shown an increased incidence of malignancy among patients with dermatomyositis when compared with controls without myositis. In contrast, an increased frequency of malignancy in dermatomyositis as compared to polymyositis still has to be demonstrated. In most cases, malignant disease precedes or occurs concurrently with dermatomyositis
