BACKGROUND: Cardiac implantable electronic device (CIED)-related bacteremia necessitates device extraction, but if there are large lead-related vegetations, surgical extraction is recommended. The associated higher morbidity and mortality complicate the management of these patients.
OBJECTIVES: The aim of this study was to assess the outcomes of percutaneous mechanical debulking (PMD) of lead-associated vegetations using a manual vacuum aspiration device during CIED extraction.
METHODS: This was a retrospective analysis of patients from 3 centers undergoing CIED extraction with lead-associat

BACKGROUND: Venous occlusion is a common consequence of chronic indwelling cardiovascular implantable electronic device (CIED) leads within the venous system, posing a significant barrier to lead revision or system upgrade. Traditional options for regaining access, such as transvenous lead extraction (TLE), epicardial systems, or contralateral tunneling, are associated with increased morbidity or technical complexity. Retained wire techniques, which leverage preexisting leads to traverse occlusions, represent a practical alternative, but data remain limited.
OBJECTIVES: This study sought to e

INTRODUCTION: Right bundle branch block (RBBB) following cardiac device extraction has not been previously reported but may have catastrophic consequences.
METHODS AND RESULTS: We present two cases of young male patients who developed RBBB following the extraction of single chamber TV ICD systems where the coil was adherent close to the superior tricuspid valve annulus. Both patients had a subcutaneous ICD (SICD) implanted but suffered an inappropriate shock due to T-wave oversensing, requiring very early SICD removal for one patient.
CONCLUSION: The development of RBBB following the extract

Transvenous pacer and defibrillator lead extraction is the standard treatment for cardiac implantable electronic device (CIED) infection but carries significant procedural risk, particularly in patients with advanced cardiomyopathy, active infection, and long-standing lead dwell times. A 54-year-old male with nonischemic cardiomyopathy (ejection fraction <20%) and end-stage renal disease on hemodialysis presented with methicillin-resistant Staphylococcus aureus (MRSA) bacteremia in the setting of a biventricular implantable cardioverter-defibrillator (ICD) placed 13 years earlier. Cardiac implantable electronic device (CIED) implantation has steadily increased in the United States owing to increased life expectancy, better access to health care, and the adoption of updated guidelines. Transvenous lead extraction (TLE) is an invasive technique for the removal of CIED devices, and the most common indications include device infections, lead failures, and venous occlusion. Although in-hospital and procedure-related deaths for patients undergoing TLE are low, the long-term mortality remains high with 10-year survival reported close to 50% after TLE. This is likely demonstrat

The treatment of valvular endocarditis in patients with cardiac implantable electrophysiological device (CIED) includes valvular surgery and lead extraction. This can be challenging in patients with severely reduced left ventricular ejection fraction (LVEF). Reduced LVEF in combination with sepsis and cardioplegic cardiac arrest can make weaning from cardiopulmonary bypass difficult. Some of these patients require venoarterial extracorporeal membrane oxygenation (VA-ECMO) for postcardiotomy syndrome. Lead extraction by manual traction is often not possible in cases with a long lead dwell time.

Background: Left bundle branch area pacing (LBBAP) has emerged as a physiological alternative to conventional pacing, offering improved ventricular synchrony and clinical outcomes. However, extraction of deeply implanted LBBAP leads remains challenging, particularly in the context of device-related infections. Case Summary: We report two cases of successful extraction of chronically implanted LBBAP leads using a novel technique based on femoral countertraction with pigtail catheters. In the first case, a deep septal implanted 3830 lead was extracted in a patient with persistent bacteremia and

Infections involving internal cardiac devices can be very challenging to treat, and almost universally require complete device extraction for successful management. Of the pathogens that cause device-related endocarditis, fungi such as Candida albicans are notoriously difficult to manage because of their propensity to produce large vegetations and the need for long-term treatment with potentially toxic medications. Furthermore, individuals who develop fungal, device-associated endocarditis are typically among the most complex patients, with significant comorbidities that place them at high ris

Laser extraction of device leads offers an attractive alternative to countertraction and electrosurgical dissection sheath, potentially increasing efficacy and reducing complications. Wider adoption of this technology depends on relative ease of use. We report the experience of a new center to define the "learning curve." We performed 76 laser lead extractions in 75 patients (age 63 +/- 17 years, 59 male) from July 2001 to January 2004. Two experienced device implanters who were novice extractors underwent a 2-day site visit to a high volume extraction center for training. Lead extractions wer

BACKGROUND: Transvenous lead extraction (TLE) plays a significant role in maintaining device therapy. Conventional extraction involves advancing an extraction sheath via the implanting vein over the targeted lead, however advanced techniques involve the use of the jugular and femoral accesses which provide geometrical advantages. Femoral extraction can be used as a "bail-out" and a primary extraction strategy but with significant challenges.
METHOD: We present a novel technique, transferring a 20-year-old passive fixation atrial lead with a deployed locking stylet from the subclavian vein to

BACKGROUND: Need for pacemaker or internal cardioverter defibrillator lead removal is increasing. Removal can be dangerous, difficult, or unsuccessful.
METHODS: We retrospectively reviewed our results and the techniques we used in 365 patients from 1992 through 2009 for successful complete removal of leads and complications. Various techniques of extraction were analyzed for effectiveness and complications. The eras before (1992 to 1999) and after the availability of laser sheath extraction (2000 to 2009) are compared.
RESULTS: Of 365 patients who underwent transvenous lead extraction, of wh

AIMS: Owing to the increasing use of cardiac implantable electronic devices, there is a growing need for safe and effective techniques to manage device-related complications and lead dysfunction. Lead extraction remains a challenging procedure with inherent risks. We present the 30-day and long-term outcomes of lead extractions in the University Hospitals Leuven.
METHODS AND RESULTS: We report a retrospective cohort study of 176 patients admitted to the University Hospitals Leuven between January 2005 and December 2011, for the transvenous extraction of 295 leads. Indications for extraction w

BACKGROUND: Lead extraction procedures typically use "over-the-lead" cutting tools (sheaths) to separate leads from the surrounding fibrotic, vascular, and cardiac tissue. These methods carry significant risks of collateral damage and major bleeding complications.
OBJECTIVES: The goal of this study was to evaluate the safety and efficacy of a novel vibration-based lead extraction technique that eliminates the need for cutting tools. The study included comprehensive preclinical testing and initial human experience.
METHODS: Development included 3 phases: 1) bench experiments; 2) animal studie

We present a complex case of cardiac implantable electronic device infection and extraction in the setting of bacteremia, large lead vegetation, and patent foramen ovale. Following a comprehensive preprocedural workup including transesophageal echocardiogram and computed tomography lead extraction protocol, in addition to the involvement of multiple subspecialties, an open chest approach to extraction was deemed a safer option for eradication of the patient's infection. Despite percutaneous techniques having evolved as the preferred extraction method during the last few decades, this case demo

Haemodialysis (HD) patients are at increased risk of vascular access complications with device therapy. We report a case of transvenous defibrillator lead extraction from the ipsilateral side as an HD fistula. The procedure was successfully performed using a hybrid approach, with extraction of the lead with a powered sheath, and surgical cut-down and closure of the subclavian vein defect.

A 53-year-old man with a subcutaneous implantable cardioverter-defibrillator (S-ICD) presented with inappropriate shocks. He underwent device extraction, and the lead was freed using a rotating mechanical dilator sheath. As patients with S-ICDs get older, extractions will become more complicated and more common. We have described a novel method of S-ICD lead extraction. (Level of Difficulty: Advanced.).

OBJECTIVES: Tricuspid regurgitation (TR) is associated with high morbidity and mortality. The TricValve bicaval valve system has emerged as a transcatheter option for patients with severe symptomatic TR who are not candidates for surgery or orthotopic interventions. However, its safety in patients with pre-existing cardiac implantable electronic devices (CIED) remains uncertain due to potential lead-related complications. This study aims to evaluate the feasibility and safety of TricValve implantation in patients with pre-existing CIED, with particular focus on lead-related complications and t

INTRODUCTION: Current guidelines recommend complete removal of a cardiac implantable electronic device (CIED) in infective endocarditis (IE), with surgical extraction indicated for large vegetations (> 20-25 mm).
CASE: We report a 68-year-old patient with CIED-related IE and a 3 × 3.5 cm lead vegetation successfully treated by percutaneous mechanical aspiration (PMA) using the FlowTriever system, followed by transvenous lead extraction (TLE). The combined approach achieved complete removal without complications or recurrence.
CONCLUSION: This case highlights PMA as a feasible, less invasive
