Prescribed alternative stimulants among people who use unregulated stimulants: Examining the association between maximum dose reached and medication discontinuation in the first 3 years of implementation in British Columbia, Canada.

BACKGROUND AND AIMS: In 2020, British Columbia (BC), Canada, introduced 'Prescribed Alternatives' (PA) as an approach to offering regulated substances to people accessing the unregulated supply. This study aimed to estimate and compare prescribing characteristics of dextroamphetamine and methylphenidate stimulant PAs in BC, to estimate medication discontinuation probabilities for each PA type by episode order, and to estimate the hazard ratio for medication discontinuation comparing episodes that reached a maximum daily stimulant PA dose of ≥60 mg/day with episodes that did not. DESIGN: Popul

Clinical behavioral pharmacology: methods for evaluating medications and contingency management.

We evaluated methods for comparing the effects of dextroamphetamine (Dexedrine), thioridazine (Mellaril), and contingency management in the control of severe behavior problems. A reversal design was used in which medications were systematically titrated and assessed in unstructured as well as structured settings with three clients. Subsequently, behavioral procedures including timeout, differential reinforcement of other behavior, and visual screening, were used in a multiple-baseline design across settings. The assessment and design methods were useful in comparing the interventions. Dextroam

The triad of luteal phase ocular migraines, interstitial cystitis, and dyspareunia as a result of sympathetic nervous system hypofunction.

PURPOSE: To evaluate whether ocular migraines can be related to sympathetic nervous system hypofunction, especially when associated with interstitial cystitis and dyspareunia. MATERIALS AND METHODS: Dextroamphetamine sulfate was administered to a 34-year-old woman with a history of long-term interstitial cystitis, dyspareunia, and ocular migraines that were resistant to all other therapies. RESULTS: In a short length of time the sympathomimetic amine therapy almost completely abrogated all of her symptoms and they have remained controlled while she continues on the drug. CONCLUSIONS: This i

Psychostimulants in psychiatry.

The use of the psychostimulants in psychiatry is reviewed. A brief historical perspective on dextroamphetamine is presented, and a brief review of the psychopharmacology of dextroamphetamine, methylphenidate and magnesium pemoline is given. The literature on the use of stimulants in the treatment of resistant depression, apathetic geriatric patients and patients medically ill with a secondary depression is summarized and two case histories given to illustrate the clinical usefulness of the stimulants. The literature on the use of stimulants as an adjunct to antidepressant therapy and as a diag

Gambling when sleep deprived: don't bet on stimulants.

Recent evidence suggests that sleep deprivation leads to suboptimal decision-making on the Iowa Gambling Task (IGT), a pattern that appears to be unaffected by moderate doses of caffeine. It is not known whether impaired decision-making could be reversed by higher doses of caffeine or by other stimulant countermeasures, such as dextroamphetamine or modafinil. Fifty-four diurnally active healthy subjects completed alternate versions of the IGT at rested baseline, at 23 and 46 h awake, and following a night of recovery sleep. After 44 h awake, participants received a double-blind dose of caffein

Qualitative measurement of drugs.

It has been noted for several years that certain drugs produce abnormal test results in patients who are administered the electronystagmograph (ENG) test. Drug specific waveforms, termed drug evoked potentials (DEP), have been identified by using the ENG for alcohol, marijuana, cocaine, tranquilizers, amphetamines, barbiturates, opiates, hallucinogens, caffeine, aminophylline, dopamine, several gases and approximately 75 combinations of drugs (N = 821). Tests for double blind diagnosis of drug intake as well as tests for repeatability within the same subject were conducted. Subjects included s

Meta-analysis: treatment of attention-deficit/hyperactivity disorder in children with comorbid tic disorders.

OBJECTIVE: The Food and Drug Administration currently requires the package inserts of most psychostimulant medications to list the presence of a tic disorder as a contraindication to their use. Approximately half of children with Tourette's syndrome experience comorbid attention-deficit/hyperactivity disorder (ADHD). We sought to determine the relative efficacy of different medications in treating ADHD and tic symptoms in children with both Tourette's syndrome and ADHD. METHOD: We conducted a PubMed search to identify all double-blind, randomized, placebo-controlled trials examining the effic

Narcolepsy.

Narcolepsy is a neurological condition with a prevalence of up to 1 per 1,000 that is characterized by irresistible bouts of sleep. Associated features include the pathological manifestations of rapid-eye-movement (REM) sleep: cataplexy, sleep paralysis, hypnagogic hallucinations, and abnormal sleep-onset REM periods and disturbed nocturnal sleep. The condition is strongly associated with the HLA-DR2 and DQw1 phenotype. The phenomenology of narcolepsy is discussed, and diagnostic procedures are reviewed. Exploring the Theoretical Potential of Benzphetamine in the Treatment of Attention-Deficit/Hyperactivity Disorder (ADHD).

Benzphetamine HCl (Didrex) is a Schedule III amphetamine indicated for the treatment of obesity. Amphetamine and methylphenidate are CNS stimulants regarded as the most effective treatment for attention-deficit/hyperactivity disorder (ADHD). Currently, both classes comprise the nationwide shortage of stimulants used to treat ADHD. Benzphetamine is a prodrug yielding dextroamphetamine (Zenzedi) and dextromethamphetamine (formerly Desoxyn). Both metabolites hold FDA approval to treat ADHD, suggesting their potential to aid in the shortage. Nonetheless, the lack of data on the compound limits its

Effects of fluoxetine on mood and immune status in depressed patients with HIV illness.

BACKGROUND: When present, depression in the context of human immunodeficiency virus (HIV) illness not only detracts from quality of remaining life but may interfere with motivation to obtain good medical care as well, thus directly influencing life's duration. This study was undertaken to assess the efficacy of fluoxetine in treating depression occurring in the context of HIV illness and to assess effects, if any, of fluoxetine on immune status (T-cell subsets). METHOD: Patients had originally participated in a double-blind, placebo-controlled, 6-week study of imipramine. Imipramine nonrespon

The acute effects of psychoactive drugs on emotional episodic memory encoding, consolidation, and retrieval: A comprehensive review.

Psychoactive drugs modulate learning and emotional processes in ways that could impact their recreational and medical use. Recent work has revealed how drugs impact different stages of processing emotional episodic memories, specifically encoding (forming memories), consolidation (stabilizing memories), and retrieval (accessing memories). Drugs administered before encoding may preferentially impair (e.g., GABAA sedatives including alcohol and benzodiazepines, Δ9-tetrahydrocannabinol or THC, ketamine), enhance (e.g., dextroamphetamine and dextromethamphetamine), or both impair and enhance (i.e.

Age-stratified safety signal mining of ADHD medications in pediatric populations using the FAERS database (2014-2024): a pharmacovigilance study using multi-method disproportionality analysis.

BACKGROUND: Pediatric attention-deficit/hyperactivity disorder (ADHD) medications have been analyzed in the FDA Adverse Event Reporting System (FAERS), yet systematic age-stratified signal profiling under ICH E11(R1) developmental categories, covering the recently approved agents viloxazine and serdexmethylphenidate, remains unreported. METHODS: We performed a retrospective disproportionality analysis of pediatric (age <18 years) FAERS reports, 2014Q1-2024Q4, after FDA-standard three-step deduplication. Ten active ingredients (eight in the main analysis, two in a new-drug branch) were identif

The effect of dextroamphetamine sulfate (Dexedrine) on plasma opioid peptides, hormones, and other metabolites was studied in eight female subjects with idiopathic (orthostatic) edema and five healthy females. All subjects were given 20 mg of dextroamphetamine sulfate, a drug widely used in the treatment of this disorder, and blood samples were collected before and 30, 60, and 90 minutes after treatment. Patients with idiopathic (orthostatic) edema had significantly lower plasma sodium levels but higher blood urea nitrogen, aldosterone, and renin levels. D-amphetamine decreased aldosterone and

Pharmacological treatment for attention deficit hyperactivity disorder (ADHD) in children with comorbid tic disorders.

BACKGROUND: This is an update of the original Cochrane Review published in Issue 4, 2011.Attention deficit hyperactivity disorder (ADHD) is the most prevalent of the comorbid psychiatric disorders that complicate tic disorders. Medications commonly used to treat ADHD symptoms include stimulants such as methylphenidate and amphetamine; non-stimulants, such as atomoxetine; tricyclic antidepressants; and alpha agonists. Alpha agonists are also used as a treatment for tics. Due to the impact of ADHD symptoms on the child with tic disorder, treatment of ADHD is often of greater priority than the me
