Diabetic Ulnar Compression Neuropathy.

Ulnar compression neuropathy is the second most frequent entrapment neuropathy of the upper extremity and is observed with increased prevalence among diabetes mellitus patients. The metabolic burden of diabetes mellitus, including chronic hyperglycaemia, microvascular dysfunction, and systemic inflammation, promotes nerve susceptibility to mechanical compression. This review summarises current evidence on the association between diabetes mellitus and ulnar compression neuropathy, highlighting epidemiological data, metabolic contributors, clinical manifestations, diagnostic tools, and managemen

Painful forms of diabetic neuropathy.

Diabetic neuropathy is a frequent and severe degenerative complication of diabetes. The diagnosis is easily performed in painful symptomatic patients. Sensitivity disorders responsible for numbness, tingling, and loss of feeling are part and parcel of diabetic foot syndrome and require investigation in view of preventing trophic ulcers. To date, there exists no specific treatment for diabetic neuropathy possibly preventable by careful control of metabolic disorder. Effective management of diabetic patients would make it possible to limit the dramatic consequences of diabetic neuropathy while a

Gastrointestinal autonomic neuropathy in diabetes.

Gastrointestinal autonomic neuropathy represents an important and diverse, but poorly appreciated, manifestation of diabetic autonomic neuropathy that impacts negatively on quality of life. There is no test to assess gastrointestinal autonomic nerve damage directly in humans; cardiovascular autonomic reflex tests are often used as a surrogate, but are suboptimal. Gastrointestinal symptoms are common in diabetes, but usually correlate only weakly with disordered motility. Diabetic gastroparesis, or abnormally delayed gastric emptying, occurs frequently and is the best characterized manifestatio

Treatment-Induced Neuropathy of Diabetes.

PURPOSE OF REVIEW: Treatment-induced neuropathy of diabetes (TIND) is an under-recognized iatrogenic painful sensory and autonomic neuropathy. This review highlights the clinical symptoms and signs, raises awareness of the cause, and provides education about prevention of TIND. RECENT FINDINGS: TIND may be triggered by a rapid decline in the blood glucose levels following the use of insulin, oral hypoglycemic medications, or even diet only to control diabetes. This may be seen in up to 10% of patients with diabetic neuropathy and has the potential for significant long-term complications that

[Optic neuropathy in diabetic patients].

INTRODUCTION: Diabetes beyond the well described diabetic retinopathy symptoms leads also to other ophthalmology problems. Among them, there are less known neuropathies and other neuroophthalmological disorders. To determine whether optic neuropathy coexists with painful peripheral neuropathy. MATERIALS AND METHODS: 50 patients (99 eyes) with diabetes (type one 21 patients and type two 29 patients) and painful neuropathy. Their mean age was 56.14 +/- 11.31 standard deviation (SD) and their glycosylated hemoglobin (HbA1c) was 8.12 +/- 1.46 SD. Mean duration of diabetes was 17.05 +/- 9.47 SD. 5

Diabetic neuropathy and oxidative stress.

This review will focus on the impact of hyperglycemia-induced oxidative stress in the development of diabetes-related neural dysfunction. Oxidative stress occurs when the balance between the production of reactive oxygen species (ROS) and the ability of cells or tissues to detoxify the free radicals produced during metabolic activity is tilted in the favor of the former. Although hyperglycemia plays a key role in inducing oxidative stress in the diabetic nerve, the contribution of other factors, such as endoneurial hypoxia, transition metal imbalances, and hyperlipidemia have been also suggest

Evaluation and prevention of diabetic neuropathy.

Diabetic neuropathy is a debilitating disorder that occurs in nearly 50 percent of patients with diabetes. It is a late finding in type 1 diabetes but can be an early finding in type 2 diabetes. The primary types of diabetic neuropathy are sensorimotor and autonomic. Patients may present with only one type of diabetic neuropathy or may develop combinations of neuropathies (e.g., distal symmetric polyneuropathy and autonomic neuropathy). Distal symmetric polyneuropathy is the most common form of diabetic neuropathy. Diabetic neuropathy also can cause motor deficits, silent cardiac ischemia, ort

[Autonomic neuropathy in diabetes mellitus].

AIMS: We analyse the different studies conducted on the prevalence, aetiopathology, clinical symptoms and treatment of diabetic autonomic neuropathy (AN), together with its involvement in the mortality of patients. DEVELOPMENT: AN can affect any part of the nervous system to varying degrees and at different moments in time, and its prevalence is significant in any of the stages of diabetes. The aetiopathogenesis of AN results from a multifactorial process involving metabolic disorders triggered by the activation of the polyolic pathway, microangiopathy and alterations affecting the synthesis

Diabetic neuropathy masquerading as glossodynia.

BACKGROUND: Diabetic neuropathy, or DN, occurs in approximately 50 percent of patients who have type 2 diabetes mellitus, or DM. Oral burning and symptoms consistent with glossodynia (burning mouth syndrome) may occur secondary to DN. CASE DESCRIPTION: A 54-year-old woman reported to a university dental clinic with a chief complaint of oral burning. No clinical signs were evident. Her medical history was positive for type 2 DM. The initial diagnosis was glossodynia, and she was evaluated with relevant blood studies, which indicated that her diabetes was not well-controlled. The patient was re

Postural characteristics of diabetic neuropathy.

OBJECTIVE: To explore the posturographic correlates of diabetic neuropathy by comparing the performances of three groups of diabetic patients (severe, moderate, and absent neuropathy) with those of normal subjects and four clinical control groups. RESEARCH DESIGN AND METHODS: Using the Interactive Balance System (Tetrax, Ramat Gan, Israel), based on the assessment of the interaction of vertical pressure fluctuations on four independent platforms, one for each heel and toe part, respectively, posturographic examinations were given to 28 diabetic patients (8 with severe, 12 with moderate, and 8

Diabetic autonomic neuropathy and the heart.

This brief review summarises the relationship between autonomic nerve damage, diabetes, and the heart. Clinical aspects of cardiac autonomic involvement are first considered, then the evidence, both direct and indirect, for cardiac autonomic nerve involvement is reviewed. This evidence includes studies of heart rate, cardiovascular reflexes, heart rate variability, QT interval length, cardiac function, pathological studies, and cardiac scintiscanning. Brief consideration is given to the relation with other aspects of diabetes and treatment to prevent or reverse autonomic damage. Potential futu

[Severe neuropathy in a young diabetic].

A 19-year-old woman with insulin-dependent diabetes mellitus (IDDM) of 3.5 years duration had been suffering from recurrent episodes of diabetic ketoacidosis (DKA), dizziness, and weight loss (16 kg, 29%) for 6 months. History and physical examination gave evidence of severe peripheral and autonomic neuropathy. Radionuclide retention on gastric emptying test at 60 min was greater than 90% (normal < 60%). On autonomic cardiovascular testing there was evidence of both parasympathetic and sympathetic damage. There was no evidence of nephropathy or retinopathy. Optimal diabetic control using 4 ins

Diabetic neuropathy. Human and experimental.

Human diabetic neuropathy consists of various clinical syndromes and it is unlikely that any single aetiological process is responsible for all of them. There is probably a complex interaction of different metabolic factors, microvascular disease, and perhaps even a genetic predisposition and other as yet unidentified environmental factors. Abnormalities of sorbitol and myo-inositol metabolism have been described in animal models and human studies of neuropathy and must be regarded as of major importance, especially since they can be profoundly influenced by blocking the enzyme aldose reductas

[Diabetic neuropathy and lesion of the vessels].

Data from the literature concerning the frequent lack of correlation between the clinical manifestations of diabetic neuropathy on the one hand and the morphofunctional status of the appropriate nerves, disorders in carbohydrate and lipid metabolism, and the influence of antidiabetic therapy on metabolism and the clinical picture of diabetic neuropathy on the other, are presented. In this connection, abundant evidence of frequent affection of various blood vessels, particularly small and smallest, in diabetic neuropathy is considered. But even these data attest to a non-infrequent lack in diab

Cardiac denervation in diabetic neuropathy.

Measurement of heart rate variation during deep breathing has been used as a diagnostic test of autonomic function. Normal subjects have considerable heart rate variation that is accentuated during deep breathing; this variation is diminished or sometimes absent in diabetics with autonomic neuropathy. Recording heart rate variation during deep breathing is a good method for establishing the presence of autonomic neuropathy: All normal subjects have a score greater than 9, and autonomic neuropathy is probably absent if the score is greater than 12. The optimal breathing rate for this test is si

Mortality in diabetic autonomic neuropathy.

Thirty-seven diabetics with symptoms and clinical features suggestive of autonomic neuropathy were followed up for 33 months. Of the twenty with abnormal Valsalva or handgrip tests initially, ten (50%) died. There were no other features at presentation that differentiated those who subsequently died from those who survived. The causes of death were renal failure (six patients), cerebrovascular accident (two patients), hypoglycaemic coma (one patient), and "sudden death" (one patient). Of the survivors whose autonomic-function tests were repeated 18 months to 2 years later, five had new or wors
