Teaching peritoneal dialysis: A position paper for the International Society for Peritoneal Dialysis.

Given the central importance of the peritoneal dialysis (PD) nurse in successfully training and supporting a patient with PD at home, as well as preventing complications as a result of the therapy, the International Society for Peritoneal Dialysis (ISPD) has provided guidance on the principles for training in two previous publications. Despite the lack of high certainty evidence in teaching PD, this ISPD 2025 Position Paper builds upon the two prior works to provide contemporary approaches to training a patient/care partner for PD to be performed at home, based upon an evolution in healthcare

Case Report: Recurring Peritonitis and Dialysis Failure in a Toddler on Peritoneal Dialysis.

We report a case of a 2-year-old-boy with end stage renal disease (ESRD) secondary to posterior urethral valves (PUV) on peritoneal dialysis (PD). Our patient developed multiple episodes of peritonitis, refractory anemia and feeding intolerance over a 12-month-period. He was treated with multiple courses of intraperitoneal antibiotics. Despite being on high-calorie formula, he was slowly thriving. The feeding intolerance was attributed to past history of prematurity, gastro-esophageal reflux disease and ESRD co-morbidities. He had anemia resistant to erythrocyte stimulating agents and iron sup

Cost analysis of hemodialysis and peritoneal dialysis access in incident dialysis patients.

BACKGROUND: Although several studies have demonstrated the economic advantages of peritoneal dialysis (PD) over hemodialysis (HD), few reports in the literature have compared the costs of HD and PD access. The aim of the present study was to compare the resources required to establish and maintain the dialysis access in patients who initiated HD with a tunneled cuffed catheter (TCC) or an arteriovenous fistula (AVF) and in patients who initiated PD. METHODS: We retrospectively analyzed the 152 chronic kidney disease patients who consecutively initiated dialysis treatment at our institution in

Peritoneal dialysis in infants: the experience of the Italian Registry of Paediatric Chronic Dialysis.

BACKGROUND: Although chronic peritoneal dialysis (CPD) is considered the replacement therapy of choice for infants with end-stage renal failure, many questions persist about treatment risks and outcomes. METHODS: We present data on 84 infants who started CPD at <1 year of age; these patients represent 12% of the total population of the Italian Registry of Paediatric Chronic Dialysis. We analysed patient records from all children consecutively treated with CPD between 1995 and 2007 in Italy. Growth data analysis was performed only in infants with complete auxological parameters at 0, 6 and 12

Growth of children following the initiation of dialysis: a comparison of three dialysis modalities.

Maintenance dialysis usually serves as an interim treatment for children with end-stage renal disease (ESRD) until transplantation can take place. Some children, however, may require dialytic support for an extended period of time. Although dialysis improves some of the problems associated with growth failure in ESRD (acidosis, uremia, calcium, and phosphorus imbalance), many children continue to grow poorly. Therefore, three different dialysis modalities, continuous ambulatory peritoneal dialysis (CAPD), cycler/intermittent peritoneal dialysis (CPD), and hemodialysis (HD), were evaluated with

Comparing Dialysis Modality and Cardiovascular Mortality in Patients on Hemodialysis and Peritoneal Dialysis.

Patients undergoing dialysis are at high risk for cardiovascular disease (CVD). Mortality differences between peritoneal dialysis (PD) and hemodialysis (HD) are widely debated. The question of whether dialysis modality affects the risk for CVD remains to be addressed. In the present study, we evaluated the influence of hemodialysis (HD) and peritoneal dialysis (PD) on survival and the risk of developing de novo CVD. Our observational prospective study enrolled 157 end-stage renal disease patients on HD or PD for 12 months. Patients with a history of malignancy, chronic rheumatic heart disease,

Early dialysis initiation and rates and timing of withdrawal from dialysis in Canada.

Background and objectivesThe number of elderly patients and those with higher estimated GFR (eGFR) initiating dialysis have recently increased. This study sought to determine rates of withdrawal from dialysis and variables associated with withdrawal.Design, setting, participants, & measurementsCanadian Organ Replacement Registry data were used to examine withdrawal rate and identify variables associated with withdrawal among the total cohort, patients age 10.5 ml/min per 1.73 m(2)) and those with late (eGFR ≤ 10.5 ml/min per 1.73 m(2)) initiation of dialysis, using a Cox proportional hazard mo

Sodium removal in peritoneal dialysis: the role of icodextrin and peritoneal dialysis modalities.

One of the main goals of dialysis is the control of extracellular volume, because inadequate sodium and fluid removal result in fluid overload and increased mortality. In the present study, we evaluated the roles of continuous ambulatory peritoneal dialysis (CAPD), continuous cycling peritoneal dialysis (CCPD), and the use of icodextrin on sodium removal in 29 patients (n = 18 on CAPD, n = 11 on CCPD). Daily removal of sodium by each modality and dialysis adequacy by Kt/V and creatinine clearance were evaluated. A significantly higher amount of sodium was removed in CAPD patients than in CCPD

Quality of life in chronic dialysis patients. Factors unrelated to dialysis modality.

The quality of life (medical, psychological, and social adaptation) of 66 chronic dialysis patients was studied cross-sectionally. The participants had similar levels of medical, psychological, and social adaptation to those found in previous studies of chronic dialysis patients. Medical, psychological, and social adaptation are independent dimensions that are only weakly intercorrelated. When categorized by specific individual or treatment characteristics, men and those over age 51 had poorer medical and psychological adaptation than did women and younger dialysis patients. Duration of dialys

Ascitic Fluid Drainage before Dialysis Catheter Removal for Peritoneal Dialysis-Related Peritonitis.

INTRODUCTION: Persistent ascites requiring drainage following catheter removal because of peritoneal dialysis (PD)-related peritonitis is a common complication associated with prolonged hospitalization and increased mortality. To mitigate this risk, a free drainage protocol was introduced before catheter removal. This study evaluated its impact on post-removal paracentesis incidence. METHODS: This study retrospectively reviewed patients who underwent catheter removal because of refractory PD-related peritonitis between January 2009 and December 2021. Before 2015, 47 patients underwent cathete

Sustained dialysis with misplaced peritoneal dialysis catheter outside peritoneum: A case report.

BACKGROUND: In patients undergoing peritoneal dialysis (PD), catheter dysfunction is a common complication. A misplaced catheter is one of the reasons contributing to its dysfunction. The present study aimed to describe a case of misplaced PD catheter with an unusual location of the catheter tip. CASE SUMMARY: A 61-year-old man undergoing PD for 4 years was investigated for progressive nausea and fatigue of 3 mo. Dialysis adequacy studies indicated inefficient dialysis. Imaging discovered that the PD catheter tip was mispositioned in the pelvic cavity with its tip outside the peritoneal cavit

Tailoring the dialysate bicarbonate eliminates pre-dialysis acidosis and post-dialysis alkalosis.

BACKGROUND: Both metabolic acidosis and alkalosis increase hospitalizations, haemodynamic instability and mortality in haemodialysis patients. Unfortunately, current practices opt for a one-size-fits-all approach, leaving many patients either acidotic before or alkalotic after dialysis sessions. Therefore an individualized adjustment of these patients' dialysate bicarbonate prescriptions could reduce these acid-base imbalances. METHODS: This is a prospective single-cohort study of patients on a chronic haemodiafiltration programme. The dialysate bicarbonate prescription was modified according

Dialysis modality-related disparities in sudden cardiac death: hemodialysis versus peritoneal dialysis.

BACKGROUND: Patients require risk stratification and preventive strategies for sudden cardiac death (SCD) based on the dialysis modality because the process of dialysis is a risk factor for SCD. This study aimed to compare the risk of SCD in patients undergoing hemodialysis (HD) versus peritoneal dialysis (PD). METHODS: Patients on HD and PD were included in the end-stage renal disease registry of the Korean Society of Nephrology between 1985 and 2017. The incidence and associated factors of SCD were analyzed based on the dialysis modality. RESULTS: Of 132,083 patients, 34,632 (26.2%) died d

Peritoneal Dialysis Patient Outcomes under the Medicare Expanded Dialysis Prospective Payment System.

BACKGROUND AND OBJECTIVES: Peritoneal dialysis (PD) use increased in the United States with the introduction of a new Medicare prospective payment system in January 2011 that likely reduced financial disincentives for facility use of this home therapy. The expansion of PD to a broader population and facilities having less PD experience may have implications for patient outcomes. We assessed the impact of PD expansion on PD discontinuation and patient mortality. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: A prospective cohort study was conducted of patients treated with PD at 90 days of ESK

Functional status, pre-dialysis health and clinical outcomes among elderly dialysis patients.

BACKGROUND: Elderly patients comprise the fastest growing population initiating dialysis in United States. The impact of poor functional status and pre-dialysis health status on clinical outcomes in elderly dialysis patients is not well studied. METHODS: We studied a retrospective cohort of 49,645 incident end stage renal disease patients that initiated dialysis between January 1, 2008 and December 31, 2008 from the United States Renal Data System with linked Medicare data covering at least 2 years prior to dialysis initiation. Using logistic regression models adjusted for pre-dialysis health

Risk factors for peritoneal dialysis withdrawal due to peritoneal dialysis-related peritonitis.

BACKGROUND: Peritoneal dialysis has become commonly used for renal replacement therapy; however, some patients withdraw from peritoneal dialysis due to complications, including peritoneal dialysis-related peritonitis, resulting in the low number of patients on peritoneal dialysis. Risk factors for peritoneal dialysis withdrawal due to peritoneal dialysis-related peritonitis are less certain. This retrospective study aimed to investigate these risk factors. METHODS: We retrospectively analyzed clinical characteristics, laboratory data, and causative microorganisms of 204 episodes of peritoneal

Dialysis Membranes and Hemodialyzers.

Hemodialysis and related extracorporeal therapies utilize filters (dialyzers) comprising hollow fiber membranes to achieve solute and water removal. While membrane classification schemes based on composition and water permeability are the historical standard, recent membrane advances have created new perspectives. In particular, the introduction of medium cut-off membranes with mean pore sizes substantially larger than those typical of standard highly permeable membranes for a new therapy (expanded hemodialysis [HDx]) necessitates the development of refined schemes that clearly define the new

Dialysis and cognitive impairment.

People with chronic kidney disease who require maintenance dialysis characteristically experience accelerated and aggravated cognitive decline compared with those with advanced kidney disease who are not receiving this form of kidney replacement therapy. This effect is inadequately appreciated, but of crucial importance to patients, their carers and the health-care systems that support them. Although many of the comorbid conditions prevalent in this patient population have the potential to affect brain structure and function, an evolving body of evidence indicates that the dialysis therapy its

[The highlights of dialysis in 2024].

In 2024, dialysis is experiencing advancements, particularly in environmental sustainability. “Green dialysis” is emerging as a priority, aiming to reduce the carbon footprint of dialysis centers (water and resource consumption, patient transportation). Innovative strategies, such as adjusting dialysate flow rate and temperature, have demonstrated similar effectiveness to standard dialysis in terms of Kt/V while reducing water consumption. A recent Cochrane review confirms the equivalence between peritoneal dialysis (PD) and hemodialysis (HD) in terms of mortality. Additionally, hemodiafiltrat
