One of the challenges posed by diastema closure treatment in the presence of tooth size discrepancy is to achieve adequate distribution of the spaces between the teeth. The use of the Digital Smile Design can assist the clinician in visualizing and measuring dentogingival discrepancies with maximum predictability. The present clinical case describes an approach to space distribution with assistance of the digital tool, allowing adequate restorative procedures. The protocol used was shown to be efficient, achieving the esthetics desired by the patient both during and after multidisciplinary tre

Maxillary midline diastema is a common esthetic concern with a multifactorial etiology that includes aberrant maxillary labial frenum attachment. A papillary penetrating labial frenum may contribute to the persistence of a maxillary midline diastema and may warrant consideration during treatment planning. We report a 19-year-old woman with a maxillary midline diastema associated with a papillary penetrating labial frenum. Comprehensive fixed orthodontic treatment was completed over 16 months, achieving closure of the diastema. Because the aberrant frenum persisted after space closure, a 980 nm

INTRODUCTION: Dentofacial aesthetics plays an important role in social interaction and psychological well-being because it affects how people perceive themselves and how they are perceived by society. The maxillary labial frenum is a fold of tissue, usually triangular in shape, extending from the maxillary midline area of the gingiva into the vestibule and mid portion of the upper lip. Maxillary anterior spacing or diastema is a common aesthetic complaint of patients and is frequently seen in children especially in the mixed dentition stage.
AIMS AND OBJECTIVES: 1. To estimate the prevalence

INTRODUCTION: We tested whether the labial frenum attachment types may affect the occurrence or type of maxillary canine impaction.
METHODS: This case-control study was performed on 96 subjects (48 with maxillary canine impaction [20.50±4.99 years, 32 females], 48 controls [21.94±3.79, 31 females]). Types of canine impaction (buccal/palatal, bilateral/unilateral, left/right), types of upper lip middle frenums (mucosal, gingival, papillary), the anatomy of lateral incisors (impacted, peg shaped, small, normal), and the midline diastema were evaluated clinically and radiographically. The aim of this study was to compare the esthetic, functional, and psychosocial impact of mandibular crowding and maxillary midline diastema in black adolescents. A cross-sectional study was conducted with 420 black (brown and black, distinguished according to Brazilian Institute of Geography and Statistics - IBGE) adolescents aged 12 with normal occlusal relationships. Esthetic (OASIS) and functional/psychosocial (OHIP-14) impact related to the need for orthodontic treatment in groups with mandibular crowding and maxillary midline diastema, and those without these conditions: G1, without crow

BACKGROUND: Maxillary frenectomy in children is a common procedure, but concerns about scar tissue affecting diastema closure prevent many clinicians from treating prior to orthodontics.
OBJECTIVES: To determine if maxillary frenectomy is safe and if diastema size is affected by early treatment.
MATERIALS AND METHODS: Paediatric patients with hypertrophic maxillary frena were treated under local anaesthesia with diode laser and CO2 laser. Diastema width was compared by calibrating and digitally measuring initial and postoperative intraoral photographs.
RESULTS: In total, 109 patients were i

BACKGROUND: One of the causes of the maxillary midline diastema (MMD) may be discrepancy between teeth and maxilla dimension. That can relate to two situations: when teeth have correct size but maxilla is too large or maxilla bone is in the proper size but teeth have reduced dimensions (microdontia). The present study has been conducted to investigate the differences in the linear dimensions of upper central and lateral incisors and canines in diastematic dentition and to compare them with the control group without diastema.
MATERIALS AND METHODS: The study was conducted on Caucasian individu

Extensive gingival recession in the mandibular anterior region with diastema poses both functional and aesthetic challenges, where conventional free gingival grafts (FGG) often fail to provide adequate results in cases of combined vertical and horizontal tissue deficiencies. Hencce, an inverted T-shaped FGG design has been introduced, enabling simultaneous root coverage and interdental tissue augmentation. The technique involves a vertical graft segment to address recession depth and a horizontal arm to bridge the diastema, thereby enhancing stability, vascularization, and esthetic harmony. In

Median maxillary labial frenum (MMLF) is one of the oral freni, found on the inner side of the centre of the upper lip. Maxillary midline diastema (MMD) is commonly associated with highly attached frenum, and frenectomy during early mixed dentition is controversial. A 6-year-old boy came with a chief complaint of unpleasant spacing between the two upper front teeth. A median maxillary high attached labial frenum with midline spacing of 5 mm was diagnosed. A consensus was made with the child's father to remove the frenum by laser. Diode laser frenectomy was done, and subsequent follow-up for 12

The authors used the technique of Z-palsty in the surgical treatment of hypertrophy of the superior labial fraenum. They consider that this operation is indicated when there is hypertrophy of the fraenum with a low insertion, associated with an inter-incisor diastema, and that the lateral incisors have appeared without causing the diastema to disappear. This technique eliminates the possibility of a hypertrophic scar, a possible source of failure.

INTRODUCTION: This study evaluated the esthetic perception of the mandibular anterior teeth during speech by comparing the assessments of laypeople (LP) and orthodontists (ODs).
METHODS: A Class I occlusion model was filmed pronouncing "Czechoslovakia" and smiling. Dynamic smile and speech images were extracted and digitally manipulated to create various dental conditions: ideal alignment, mandibular canine extrusion, mandibular incisor crowding, inclined mandibular occlusal plane, mandibular incisor diastema, and mandibular incisor extraction simulation. In mice, a toothless diastema separates the single incisor from the three molars in each dental quadrant. In the prospective diastema of the embryo, small rudimentary buds are found that are presumed to be rudiments of suppressed teeth. A supernumerary tooth occurs in the diastema of adult mice carrying mutations in either Spry2 or Spry4. In the case of Spry2 mutants, the origin of the supernumerary tooth involves the revitalization of a rudimentary tooth bud (called R2), whereas its origin in the Spry4 mutants is not known. In addition to R2, another rudimentary primordium (called MS) arises

Median diastema is a physiological occurrence that is frequently seen in the maxillary jaw. Therefore, a median diastema has been associated with a wide range of etiological variables, including thumb sucking, supernumerary teeth, tongue thrusting, some dentoalveolar discrepancy, and hypodontia. Patient esthetic and function demands are both negated by the labial frenum's abnormal location in relation to the maxillary anterior teeth, which results in diastema and gingival recession. Lasers are now being used in several fields of dentistry as an alternative to conventional scalpel operations. F

This case reports the early diagnosis and treatment of an iatrogenic acute localized periodontitis arising from the incorrect use of orthodontic elastic rubber bands to close a maxillary midline diastema in an eight-year-old girl. The case illustrates the misdiagnosis and mismanagement of a transitional diastema in the "ugly duckling" stage of the mixed dentition. The nine-year follow-up shows that thorough periodontal and orthodontic management has allowed successful retention of both maxillary permanent central incisors, which had initially pathological grade 3 vertical mobility, little rema

Periodontal disease is often associated with pathologic migration, which becomes an esthetic concern. A 17-year-old girl developed increasing gaps among her maxillary incisors. She had gingival enlargement in the palatal maxillary anterior region. The central incisors had pathologically migrated, resulting in a 2-mm diastema. Periodontal treatment was planned and completed. Following periodontal treatment, there was "spontaneous" repositioning of the central incisors. The 6-month follow-up revealed no change or deterioration of the periodontal condition. The patient was referred for orthodonti

We report the case of a 3-year-old girl referred to our hospital dentistry service, from a public health centre, due to a 4-week-old swelling in the area of the hard palate, causing displacement of the deciduous tooth and the appearance of an interincisal diastema. The clinical characteristics suggested the possibility of a reactive fibroma and we decided to intervene surgically by means of an excisional biopsy. Histology confirmed the presumptive diagnosis. Prompt referral and early surgical care spontaneously favoured both repositioning of the displaced primary tooth and closure of the diast

INTRODUCTION: The onset of one or several diastema in adults with no link of etiology to tooth loss or to a periodontal problem can be the first evocative sign of an acromegaly or a benign or malignant tumoral process.
OBSERVATION: A 40-year-old patient consulted for an implant to solve an esthetic problem caused by a diastema. The complementary exams revealed an osteolytic lesion marked by condensing areas. The biopsy proved the diagnosis of fibrous dysplasia.
DISCUSSION: Etiologies of fibrous dysplasia are varied. They can be related to a congenital defect of osteo mesenchyma development o
