BACKGROUND: Renal interstitial fibrosis is a major complication of cisplatin (CP) treatment, and increased sodium intake may accelerate its progression by stimulating transforming growth factor (TGF)-β/Smad signaling. However, it is not clear whether a low-sodium diet has beneficial effects on the development of interstitial fibrosis because it activates the renin-angiotensin-aldosterone system. Here, we tested whether the TGF-β/Smad signaling pathway is stimulated in CP-treated rats, and whether the development of tubulointerstitial fibrosis in CP nephropathy can be checked by dietary sodium

Twelve moderately obese middle-aged male out-patients with untreated mild hypertension reduced their sodium intake by about 120 mmol/day during 4-6 weeks. The low sodium diet period was followed by a period of energy reduction as well as sodium restriction for 15 weeks. Mean body mass was then reduced by 7.5 +/- 1.0 kg. Intraerythrocyte sodium (IeNa), sodium influx (Na-influx) and sodium efflux rate constant (Na-efflux rate), were measured before intervention, during salt restriction and during salt and energy restriction. Plasma renin activity (PRA) and urinary excretion of aldosterone (U-Ald

An epidemiologic survey of the morbidity and mortality rates of hypertension and related disease was carried out in a population of 9,570 composed of salt workers and building workers. The survey rate was 96.59%. The age-standardized morbidity rate of hypertension in the salt workers was 27.88%, while that in the building workers was 8.5%. Na concentration and Na/K rate in 8 hrs' nighttime urine in salt workers were significantly higher than in building workers (P less than 0.001). Results of intervention of salt intake at the level of 10 g/person/day in 285 cases of hypertensives for 3 months

There are no indications to prescribed special diets for postoperative patients. Low-sodium and low-fat or low-cholesterol diets are examples of restricted diets, especially in patients with heart disease and atherosclerosis. These restricted diets are unpalatable. Postoperative nausea, paralytic ileus, and vomiting caused by residual anesthetic effects and opioids used for pain control further contribute to the problem. Long-term adherence to these diets is necessary to derive benefits. Prescribing regular and palatable diets in the immediate postoperative period to meet protein and energy go

BACKGROUND: Hypertension is the leading cause of premature death globally, and acute decompensated heart failure is among the most common causes of hospitalization. Both conditions are adversely affected by high dietary sodium intake. In contrast, potassium has been shown to lower blood pressure. Substituting dietary sodium with potassium may therefore reduce morbidity and mortality associated with hypertension and possibly heart failure.
OBJECTIVES: This review summarizes the current evidence on the cardiovascular effects of dietary sodium and potassium-containing salts.
CURRENT DATA: Epide

The management of decompensated cirrhosis is fraught with clinical dilemmas, often requiring a delicate balance between opposing physiological principles. One such enduring controversy is the role of dietary sodium restriction, long considered a cornerstone in the management of ascites and fluid overload. The study by Marrapu et al published in the World Journal of Methodology, directly challenges this convention in a specific and vulnerable population: Patients with cirrhosis and moderate to severe hyponatremia. Their open-label randomized controlled trial provides compelling evidence that a

In order to assess the congruity of two different methods for the characterization of blood pressure responsivity to alterations in sodium and extracellular fluid volume balance, we studied 40 normotensive and hypertensive humans. All subjects were initially studied with a protocol of rapid sodium and volume expansion induced by intravenous administration of 0.9% saline (2 L over 4 h) followed by a day of sodium and volume depletion achieved by a low (10 mmol) sodium diet and three 40 mg oral doses of furosemide. Subsequently the subjects underwent a dietary protocol consisting of 5 days of a

Sodium is the major cation of extracellular fluid (ECF) and, because of its osmotic action, is involved in the regulation of ECF volume and blood pressure. The ingested sodium is almost completely absorbed by the intestine. Circulating sodium is filtered by the glomeruli and its renal tubular handling is responsible for the maintenance of sodium and water balance. Sodium deficiency is rare and occurs only in some medical conditions. High dietary sodium intake is associated with ECF volume expansion and is a leading risk factor for hypertension and cardiovascular diseases; it also adds to risk

Intralymphocytic sodium and potassium concentrations, 22Na efflux rate constants (EFRC) and blood pressures (BPs) were studied in nine normotensive young males without a history of essential hypertension before, during and after 5 weeks of severe sodium depletion. Diastolic and median BPs decreased significantly as estimated by both ambulatory and home readings. A slight decrease in intralymphocytic potassium content and a significant increase in sodium content was found. Measurements of 22Na-EFRC were reduced by 15% in the ouabain-sensitive component of the sodium pump, while the absolute sod

BACKGROUND: Salt restriction is a cornerstone in managing ascites in cirrhotic patients; however, its impact on nutritional status, sarcopenia, and mortality remains unclear.
AIM: To evaluate the effects of a salt-restricted diet (SRD) on ascites control, body composition, sarcopenia development, and patient survival in cirrhotic patients with decompensated liver disease.
METHODS: This prospective study included 102 patients with grade III ascites, categorized into two groups based on dietary adherence: Salt-Restricted Diet (SRD), (n = 46) and Salt-Unrestricted Diet (SUD) (n = 56). Sodium in

Excessive salt consumption is an important risk factor for cardiovascular diseases (CVD), especially through increased blood pressure. In Brazil, the average salt intake is almost twice the international recommendations, and current population-level salt-reduction policies include only voluntary maximum sodium targets for packaged foods and awareness strategies. This study aims to quantify the potential health and economic impacts of additional salt-reduction strategies in Brazil from 2019 to 2038. We used a previously validated microsimulation of a close-to-reality synthetic population (IMPAC

As noted in part I of this article, emerging evidence indicates that overt and (more commonly) subclinical volume expansion is frequent in patients on peritoneal dialysis (PD). That expansion in turn leads to hypertension. With loss of residual renal function, the hypertension becomes difficult to control even with increasing doses and varieties of antihypertensive drugs. Poor volume and blood pressure control aggravates already existing left ventricular hypertrophy and leads to increased cardiovascular morbidity and mortality. Indeed, cardiovascular events are the leading cause of death in PD

1. Rats on normal sodium diet (group 1) and on chronically maintained low sodium diet (group 2) were studied during a control period, after clipping the renal artery (two-kidney, one-clip hypertension) and after nephrectomy (one-kidney, one-clip hypertension). 2. The low sodium diet neither prevented the development nor changed the severity of two-kidney, one-clip hypertension, and the latter was not accompanied by an increase in plasma renin activity. 3. After nephrectomy arterial pressure further increased and plasma renin activity decreased in group 1, and both remained unchanged in group 2

In a group of four young patients with stable chronic renal failure and hyperkalemia sodium restriction induced a remarkable increase in plasma renin activity (PRA) and plasma aldosterone (PA), a decrease in the elevated serum potassium (SK) and a rise in potassium excretion. During high sodium intake the levels of PRA and PA were lower than those found in the healthy control group suggesting that enhanced suppressibility of the renin-angiotensin-aldosterone system (RAAS) was the main cause of hyperkalemia. During sodium restriction despite a marked increase in PRA and PA levels poor correlati

Six patients with angiographically proven renal artery stenosis were given a low salt diet, consisting of 10 mEq sodium per day for 4-6 days. Five patients showed a remarkable decrease in blood pressure (BP), from 169 +/- 12/107 +/- 4 to 131 +/- 12/84 +/- 5 mmHg, despite a rise in renin level. The decrease in BP was paralleled by a decrease in urinary sodium excretion. Restoration of the regular diet caused immediate elevation of blood pressure. We conclude that severe sodium restriction can reduce blood pressure in patients with renovascular hypertension.

BACKGROUND: Ménière's disease or syndrome is a chronic inner ear disorder that results in sporadic attacks of vertigo, sensorineural hearing loss, aural fullness and tinnitus. There is no definitive treatment for Ménière's disease and treatment options range from dietary modification through medication to surgery.Modification of diet, including restriction of salt, caffeine and alcohol intake, is a management option that is widely recommended to patients with Ménière's as a first-line treatment. There has not previously been a systematic review of this intervention.

A low salt diet is beneficial for the whole population but has particular advantages for hemodialyis patients because of the role of salt restriction in the management of hypertension and interdialytic weight gain (IDWG). Education on dietary salt intake based on general healthy eating guidelines, such as the "DASH-sodium" diet, should be provided for staff, families, and carers as well as patients. Anuric hemodialysis patients will need to take in approximately 1 l of water for every 8 g salt consumed. Patients who restrict salt intake to <6 g/day, and drink only when thirsty, should gain no
