Fitting of electronic elbow on an elbow disarticulated patient by means of a new surgical technique.

Rehabilitation of trans-humeral amputees represents a challenge. One of the difficulties to overcome is prosthetic suspension. In the case of elbow disarticulation the presence of humeral condyles permits better suspension, but there is no room for elbow joints, particularly electronic ones, and the only available mechanisms are provided by external hinges. The purpose of this report is to present a case of a primarily elbow disarticulated patient, with indication for surgical revision due to bad skin coverage, neuroma and the wish to improve prosthetic fitting. The surgical plan outlined was

[Extensive coxo-femoral resection with preservation of the thumb for prosthesis. An alternative to disarticulation at the hip].

To avoid the severe mutilation of a hip disarticulation and to improve limb-fitting, a method of partial limb preservation is proposed. Two cases of septic arthritis of the hip with neurological and atrophic lesions sufficient to justify disarticulation are described. Disarticulation was avoided by partial conservation of the thigh with an extensive resection of the upper end of the femur, a through-knee amputation and a large posterior musculo-cutaneous flap including the triceps surae whose muscle provided a good cover for an end-bearing myoplasty. The stump obtained was of good quality whic

Design and optimization of a hip disarticulation prosthesis using the remote center of motion mechanism.

BACKGROUND: Hip disarticulation prostheses (HDPs) are not routinely seen in clinical practice, and traditional hip prostheses rotate around an axis at the front side of the pelvic socket. OBJECTIVE: This study proposes a mechanism to restore the rotation center to the acetabulum of the amputated side and uses comparative experiments with traditional HDP to verify the validity of the novel design. METHODS: A double parallelogram design of HDP based on a remote center of motion (RCM) mechanism was presented in this paper. Optimization was achieved by a genetic algorithm with the maximal integr

Osseointegration as Treatment for a Knee Disarticulation Because of a Congenital Femoral Deficiency: A Case Report.

CASE: A 25-year-old woman underwent osseointegration surgery as treatment for an amputation that had been performed because of congenital femoral deficiency. The undersized custom-designed implant for the congenitally hypoplastic femur subsequently loosened, causing substantial pain and loss of function. The patient underwent revision surgery with a larger standard-sized osseointegration implant, and experienced no additional complications. CONCLUSION: This case demonstrates the possibility of stimulating a hypertrophic response in underdeveloped bone, which allows the subsequent insertion of

Wrist-driven prehension prosthesis for amputee patients with disarticulation of the thumb and index finger.

Partial hand amputations may leave significant functional limitations that are difficult to ameliorate by either orthoses or prostheses. Disarticulation of the thumb and index finger at the metacarpophalangeal joint level and the range of motion of the three remaining fingers would be insufficient to provide any type of grasp. In cases like this, a cosmetic hand usually seems to be the best solution. This report describes the design and use of a wrist-driven prehension prosthesis that was applied to a patient and provided a considerable improvement in function and cosmesis.

Peripheral Arterial Disease and Prosthetic Joint Infection Resulting in Hip Disarticulation: A Case Report.

INTRODUCTION: The aim of this study was to report a patient with refractory prosthetic joint infection (PJI) and severe peripheral arterial disease that necessitated hip disarticulation (HD), a rare and aggressive procedure. While this is not the first HD performed due to PJI, this is the first reported incidence that deals with profound infection burden along with immense vascular disease that has failed all other treatment options. CASE REPORT: We report a case of an elderly patient with a prior history of the left total hip arthroplasty, PJI, and severe peripheral arterial disease who unde

A pelvic kinematic approach for calculating hip angles for active hip disarticulation prosthesis control.

BACKGROUND: Control system design for a microprocessor-controlled hip-knee-ankle-foot (HKAF) prosthesis is a challenge since hip disarticulation amputees lack the entire leg and, therefore, only have pelvis movement as user-guided input. This research proposes a method for determining hip joint angles from pelvis movement in a control system for the next generation of powered prostheses. METHOD: Three-dimensional pelvic motion and stance time of 10 transfemoral (TF) prosthetic users were used to identify important features and to develop an algorithm to calculate hip angles from pelvis moveme

A Novel Technique of Hip Disarticulation with a Stump with Soft Tissue of the Proximal Thigh: A Case Report.

CASE: Two women (aged 35 and 29 years) with intractable chronic knee infection of tumor prostheses opted for amputation. As only the trochanteric region could be preserved, we resected the remaining proximal femur and created a 15-cm long stump using unaffected proximal thigh soft tissue to avoid the drawbacks of ultra-high-thigh amputation and conventional hip disarticulation. Both had no contracture, could move their stump voluntarily, and ambulated using a standard transfemoral prosthesis without a pelvic belt at 4-year and 3-year follow-ups, respectively. CONCLUSION: Our procedure might b

Is the prevention of condylar sag with maxillomandibular fixation the key to functional reconstruction of a mandibular disarticulation resection?

OBJECTIVE: The aim of this study was to evaluate the long-term outcomes of the use of the free fibula flap for reconstruction with mandibular disarticulation resections. STUDY DESIGN: A retrospective chart cohort review was performed in patients reconstructed with a fibula free flap for a mandibular disarticulation resection at an academic tertiary care institution over a 5-year period. RESULTS: Twenty-four patients were included in the study. Fourteen patients were treated for benign pathology, and 10 cases were malignant. Average follow-up was 20 months. Reviving the debate: Articular cartilage preservation during disarticulation at the lower limb? A systematic review.

BACKGROUND: The need for preservation(P) or removal(R) of articular cartilage during disarticulations remains unanswered. METHODS: Medline database was used to conduct a systematic review regarding all types of minor disarticulations and some types of major disarticulations in patients with diabetes mellitus, peripheral arterial disease or trauma related disarticulations. Fisher-exact statistical test was used to perform calculations for the entire group as for subgroups. RESULTS: A total of 444 disarticulations at the Chopart joint, ankle and knee were included (P = 255 vs. R = 189). There

Custom-made endoprostheses for the femoral amputation stump: an alternative to hip disarticulation in tumour surgery.

Disarticulation of the hip in patients with high-grade tumours in the upper thigh results in significant morbidity. In patients with no disease of the proximal soft tissue a femoral stump may be preserved, leaving a fulcrum for movement and weight-bearing. We reviewed nine patients in whom the oncological decision would normally be to disarticulate, but who were treated by implantation of an endoprosthesis in order to create a functioning femoral stump. The surgery was undertaken for chondrosarcoma in four patients, pleomorphic sarcoma in three, osteosarcoma in one and fibrous dysplasia in one

Healing of partial flap necrosis in ankle disarticulation amputation by débridement and continued weight-bearing.

Five patients with partial tissue loss of the weight-bearing surface of the heel pad following ankle disarticulation were treated with residual-limb debridement and continued end-weight-bearing using a total-contact cast. The patients, ranging in age from 53 to 76 years, had insulin-requiring diabetes and insensate heel pads and were low-demand, limited-activity, community walkers before amputation surgery. Each underwent amputation surgery as a consequence of peripheral vascular disease. All patients progressed to complete wound healing over 3 to 6 months and were able to return to their prev

Digital gangrene following intravenous quinine administration in an infant requiring surgical disarticulation: a case report.

INTRODUCTION AND IMPORTANCE: Distal digital gangrene following intravenous quinine for severe malaria is an infrequent but distressing complication. It is usually associated with severe Plasmodium falciparum malaria causing disseminated intravascular coagulation (DIC). Underlying mechanisms include microvascular thrombosis caused by parasite-infected erythrocytes. Additionally, the medication itself can act as a major contributing factor to local tissue ischemia. CASE PRESENTATION: An 8-month-old infant presented with progressive right-hand swelling and distal digital gangrene following intra

Ankle Disarticulation: An Underutilized Approach to Staged Below Knee Amputation-Case Series and Surgical Technique.

The staged approach to below knee amputation has proven to be an effective method of achieving functionality after amputation and reducing risk for postamputation complications. Transtibial or "guillotine" amputations are often used as the first-stage amputation. Disarticulation at the ankle joint is an alternative method that can be used with favorable results and benefits. These include rapid infection decompression while minimizing the blood loss of traditional tibial osteotomy. Here we describe our operative technique, perioperative management, and indications for this surgical procedure.

Functional Outcomes of Service Members With Bilateral Transfemoral and Knee Disarticulation Amputations Resulting From Trauma.

As longitudinal studies for those with bilateral transfemoral amputation (BTFA) or knee disarticulation (KD) are lacking, it is important to quantify performance measures during rehabilitation in an effort to determine reasonable expectations and trends that may influence the rehabilitation process. At initial evaluation (date of first independent ambulation) and follow up (median 135 [range = 47-300] days later), 10 participants with BTFA/KD completed 6 minute walk testing and Activity Specific Balance Confidence and Lower Extremity Functional Scale questionnaires. Of these, six participants

A Traumatic Partial Through-the-Elbow Amputation with Disarticulation of the Humerus: A Case Report and Review of the Literature.

CASE: A fifty-two-year-old fisherman sustained a severely mangled open elbow dislocation; the distal part of the humerus was protruding from the wound. Although all of the soft tissue attached to the distal part of the humerus was stripped off the bone, no neurovascular structures were damaged. A hinged external fixator was applied and a lateral collateral ligament repair was performed. At the final follow-up, the range of elbow motion was -10° in extension and 125° in flexion, with no instability. CONCLUSION: Despite the near amputation of the limb, a good functional outcome was achieved bec

Wrist disarticulation of a deformed hand: appropriate prosthesis and the habilitation of a severely retarded young man.

A 24-year-old man, who was late attaining developmental milestones and tested in the severely mentally retarded range, had incurred third- and fourth-degree burns at 2.5 years that resulted in a deformed, anesthetic, and useless distal left upper limb. Following wrist disarticulation, he was fitted with a cosmetic prosthesis that he accepted enthusiastically. It permitted him to use his left upper limb as an effective assister. Attempts to wear and use a functional prosthesis were not successful because the bulbous stump was covered with abnormal skin that ulcerated whenever he wore the prosth

Life-saving hip disarticulation following necrotizing myofasciitis from invasive mucormycosis after blast injury: A case report.

INTRODUCTION: Necrotizing myofasciitis is a rapidly progressing, life-threatening infection involving deep soft tissues, often caused by bacterial pathogens. However, fungal etiologies such as invasive mucormycosis, particularly in trauma patients, are rare and associated with high morbidity. Blast injuries create hypoxic environments conducive to fungal proliferation, complicating clinical management and often necessitating aggressive surgical interventions such as amputation. PRESENTATION OF CASE: We present the case of a 26-year-old male with no prior medical history, who sustained multipl
