New vascular ring connectors in surgery for intramural haematoma of the abdominal aorta progressing to rupture.

Aortic intramural haematoma is similar to classic aortic dissection, which causes a life-threatening medical condition, and immediate diagnosis and treatment are crucial. The optimal therapy for intramural haematoma of the abdominal aorta remains controversial. Conservative medical management is usually the first choice of treatment for uncomplicated cases. Surgical intervention is usually required for complicated intramural haematomas of the abdominal aorta, including conventional open repair and endovascular treatment with stent-grafts. A new vascular ring connector that achieves a quick, bl

Stented balloon fenestration before entry repair using the frozen elephant trunk technique for chronic aortic dissection.

Endovascular fenestration on the abdominal aorta is effective for preventing visceral malperfusion in aortic dissection. We report a case of stented balloon fenestration before residual entry repair using the frozen elephant trunk technique for chronic aneurysmal dissection after ascending aortic replacement for DeBakey I aortic dissection. We recognized poor communication between the true lumen and false lumen in the abdominal aorta, and visceral perfusion depended almost entirely on the proximal large entry. Therefore, we scheduled catheter angioplasty on the small re-entry before upstream e

[Nearly total aortic replacement for DeBakey type-I aortic dissection in a patient with Marfan's syndrome].

A successful nearly total aortic replacement for DeBakey type-I aortic dissection in a patient with Marfan's syndrome is reported. A 36-year-old woman developed a DeBakey type-I aortic dissection in October 1987. We replaced the ascending aorta and resuspended the aortic valve urgently, when she had not been diagnosed with Marfan's syndrome. In August 1989, we replaced the distal portion of the descending aorta and the infra-renal abdominal aorta because of dilatation in the diameter of these regions. In September 1990, she had sudden severe chest pain. Computed tomography and Doppler ultrason

Nutcracker syndrome due to chronic aortic dissection.

Nutcracker syndrome (NCS) is known as a status of compression of the left renal vein (LRV) between the abdominal aorta and superior mesenteric artery (SMA). We here report a case of NCS in a Marfan syndrome patient with type B aortic dissection who was presented with sudden gross hematuria. Computed tomography revealed the compression of the LRV sandwiched between the SMA and the dilated dissecting abdominal aorta. The compression was released by surgical intervention and the hematuria was promptly resolved. This report highlights that NCS should be considered as a differential diagnosis of un

[Risk Factors for Adverse Aortic Remodeling in Post-TEVAR Patients with Acute Stanford B Aortic Dissection].

OBJECTIVES: To assess morphological changes of distal aorta and the risk factors for adverse aortic remodeling inpost-TEVAR(thoracic endovascular aortic repair) patients with acute Stanford B aortic dissection. METHODS: We retrospectively investigated the patients who underwent TEVAR for a type B dissection between October 2005 and December 2015. CT angiogram (CTA) was obtained for each patients preoperatively, postoperatively and during the post-operational follow-up. Based on Criadol partition principle, we divided the aorta into descending thoracic aorta area, suprarenal abdominal aorta ar

A case of intraoperative acute aortic dissection caused by cannulation into an axillary artery.

Severe atherosclerotic disease of the ascending aorta is one of the risk factors of dissection of the ascending aorta and cerebral embolism during cardiac operations with cardiopulmonary bypass. Aortic dissection is rare, but once it happens, the mortality rate is high. For the patient with severely atherosclerotic or strongly calcified aorta, we should avoid cannulation into the aorta or clamping of it. In this case, we experienced aortic dissection although we chose the arterial cannulations into the axillary arteries because of the strong calcification of the ascending aorta and the abdomin

Comparison of β-aminopropionitrile-induced aortic dissection model in rats by different administration and dosage.

This study compared three β-aminopropionitrile (BAPN) treatment rats to find the optimal BAPN model for thoracic aortic dissection and aneurysm in one study. Sixty rats were divided into five groups: control, injected control, 0.25% and 0.4% BAPN treatment (orally), and 667 mg/kg/day BAPN injection subcutaneously. Incidence of aortic dissection and aneurysm, aortic weight and diameter were measured directly. Thickness of media and area of aorta were measured by hematoxylin and eosin and Victoria blue staining. The mortality, incidence of aortic dissection and the rupture rate of dissected aneu

Successful staged operation for acute type A aortic dissection with paraplegia.

A 75-year-old man presented with both paraplegia and acute occlusion of the abdominal aorta at onset. Extraanatomical bypass was performed following spinal drainage. After 3 days, the ascending aorta replaced under cardiopulmonary bypass using the extraanatomical bypass graft for arterial cannulation. The abdominal aorta was replaced after 6 months. A staged operation is one of the options for acute aortic dissection with paraplegia and acute occlusion of the abdominal aorta.

Surgical strategies in managing organ malperfusion as a complication of aortic dissection.

Between December 1978 and March 1994, 48 of 312 patients who underwent surgery for aortic dissection were diagnosed with major vascular complications. There were 18 patients with type A dissection and 30 patients with type B. In 23 patients with acute dissection, the site of vascular obstruction was the abdominal aorta in 12 patients, brachiocephalic artery in 7, iliac artery in 4, left common carotid artery in 3 and thoracic aorta in 2. In 26 patients with chronic dissection, the site of vascular obstruction was the abdominal aorta in 13 patients, brachiocephalic artery in 10, renal artery in

Decrease in aortic distensibility after an extended aortic reconstruction for Marfan's syndrome as a cause of postoperative acute aortic dissection DeBakey type I: a report of two cases.

Two cases of Marfan's syndrome underwent a reconstruction of the abdominal aorta and descending thoracic aorta. A replacement of the abdominal aorta with visceral arteries as well as a wrapping of the descending thoracic aorta was performed in case 1 while a reconstruction of the descending thoracic aorta and infra-renal abdominal aorta was done in case 2. After the reconstruction, both cases developed acute aortic dissection DeBakey type I. Another reconstruction of the ascending aorta was then urgently performed. Cardiac catheterization after the second operation in case 1 revealed that the

Treatment of a chronic aneurysmal aortic dissection in a patient with Marfan syndrome using a staged hybrid procedure and a fenestrated endograft.

Patients with aneurysmal dissections involving both the thoracic and the abdominal aorta are particularly challenging to treat with endovascular techniques because of the natural communications at the level of the visceral arteries. We present the case of a patient with Marfan syndrome with an aneurysmal aortic dissection involving the thoracic and abdominal aorta who was treated by a combination of endografts, surgical bypass, and a fenestrated tube graft.

[Imaging of thoracic and abdominal aortic aneurysms using MR angiography].

Five patients with suspected dissecting aneurysm of the thoracic aorta and nine patients with fusiform aneurysm of the abdominal aorta underwent time-of-flight (TOF) MR angiography. MR angiography revealed dissecting aneurysms in four patients and a dilated ascending aorta without evidence of dissection in one patient. These findings correlated well with angiography, computed tomography and surgical findings. Proximal and distal extension as well as the relationship to vessels originating from the aortic arch was assessed correctly in all cases. There was a good correlation between MR angiogra

An Approach to Point-Of-Care Ultrasound Evaluation of the Abdominal Aorta.

Disorders of the abdominal aorta, including aneurysms and dissection, have potentially high rates of morbidity and mortality. While computed tomography (CT) is the current gold standard to image the abdominal aorta, the process of obtaining a CT may be time-consuming, requires the use of intravenous contrast dye, and involves exposure to ionizing radiation. Point-of-care Ultrasound (POCUS) can be performed at the bedside and has excellent sensitivity and specificity for the diagnosis of abdominal aortic aneurysm and excellent specificity for the diagnosis of abdominal aortic dissection. Fate of Residual Aorta After Surgery for Type A Aortic Dissection.

INTRODUCTION: Surgical treatment of type A aortic dissection is essentially palliative. Many patients who undergo the procedure still have a dissection flap in the residual aorta, with a persistent patent or partially thrombosed false lumen leaving them susceptible to the dilatation of distal aorta and aneurysm formation. METHODS: Patients who had undergone surgery for type A aortic dissection from January 2015 till December 2022 were recruited into the study. Two follow-up computed tomography scans were performed at least six months apart, the first one at least one month after the surgery.

Acute lower limb malperfusion triggered by a large vegetation located on the proximal entry tear of chronic type B aortic dissection.

A 74-year-old man who had been receiving antibiotic treatment for meningitis was transferred to our hospital because of a sudden decrease in lower limb blood pressure. Computed tomography revealed a type B aortic dissection with obstruction of the abdominal aorta. Furthermore, transesophageal echocardiography revealed a large vegetation on the proximal entry tear of the descending aorta. We performed successful emergency descending and abdominal aorta replacement, which prevented complications from intraoperative organ malperfusion. In the present report, we have described an effective treatme

Endovascular management of traumatic infrarenal abdominal aortic dissection.

Dissection is a recognized finding after blunt trauma to the abdominal aorta. Immediate and long-term prognosis is poor without surgical treatment especially since most patients present severe associated injuries. On the basis of encouraging results using endovascular techniques to treat experimental dissection of the descending thoracic aorta, we treated three patients with traumatic infrarenal abdominal aortic dissection by percutaneous stent placement. There were two men 34 and 41 years of age and one 89-year-old woman. In all patients, dissection began in the infrarenal portion of the aort

Dilation of peripheral vessels in Marfan syndrome: importance of thoracoabdominal MR angiography.

OBJECTIVES: Aim of this study was to assess vascular morphology and dimension of the entire aorta and branch vessels in MFS using thoracoabdominal MR angiography (MRA). BACKGROUND: Evaluation of vascular involvement in Marfan Syndrome (MFS) is focused to the ascending aorta, which has the major risk of dissection/rupture. METHODS: From March 2006 to January 2011, 64 patients (35 ± 13 years old) with MFS underwent echocardiography and MRA of thoracic and abdominal aorta. Measurements of ascending aorta, aortic arch, descending thoracic and abdominal aorta, and aortic branch vessels were asses

[Abdominal aortic surgery in Stanford type B chronic dissecting aortic aneurysm].

Three cases of a chronic type B dissecting aneurysm which required abdominal aortic reconstruction are presented. Constriction of the iliac artery due to dissection was found in case 1, in case 2 the left subclavian artery originated from a giant false lumen, and arteriosclerosis obliterans was observed in case 3. The left renal artery originated from a false lumen in all cases. Each patient underwent preliminary graft replacement of the abdominal aorta because using the femoral artery for extracorporeal circulation had a higher risk factor. Pathogenesis of thoracic and abdominal aortic aneurysms.

The major disease processes affecting the aorta are aortic aneurysms and dissections. Aneurysms are usually described in terms of their anatomic location, with thoracic aortic aneurysms (TAAs) involving the ascending and descending aorta in the thoracic cavity and abdominal aortic aneurysms (AAAs) involving the infrarenal abdominal aorta. Both thoracic and abdominal aortas are elastic arteries, and share similarities in their physical structures and cellular components. However, thoracic and abdominal aortas differ in their biochemical properties and the origin of their vascular smooth muscle
