Antegrade stenting of the descending thoracic aorta during DeBakey type 1 acute aortic dissection repair.

Several studies have shown that after DeBakey type 1 acute aortic dissection (DB1-AAD) surgery, 70% of the surviving patients still present with a dissected distal aorta that can eventually dilate, rupture, lead to distal malperfusion or require secondary extensive interventions. In order to minimize these complications, different surgeons have advocated total thoracic aorta remodelling procedures during primary aortic repair to promote false-lumen obliteration and distal thrombosis. Such management, which includes arch replacement and antegrade stenting of the dissected descending thoracic ao

Feasibility of measuring magnetic resonance elastography-derived stiffness in human thoracic aorta and aortic dissection phantoms.

Type B aortic dissection (TBAD) represents a serious medical emergency with up to a 50% associated 5-year mortality caused by thoracic aorta, dissection-associated aneurysmal (DAA) degeneration, and rupture. Unfortunately, conventional size-related diagnostic methods cannot distinguish high-risk DAAs that benefit from surgical intervention from stable DAAs. Our goal is to use DAA stiffness measured with magnetic resonance elastography (MRE) as a biomarker to distinguish high-risk DAAs from stable DAAs. This is a feasibility study using MRE to (1) fabricate human-like geometries TBAD phantoms w

Management of left subclavian artery in type B aortic dissection treated with thoracic endovascular aorta repair.

OBJECTIVE: Thoracic endovascular aortic repair (TEVAR) has been the favored approach for the treatment of type B aortic dissection (TBAD). To obtain an adequate proximal landing zone, coverage of the left subclavian artery (LSA) will often be necessary. The occurrence of possible neurologic complications has continued to be debated. We investigated the management of the LSA in patients with TBAD undergoing endovascular repair. METHODS: We searched the PubMed and MEDLINE databases to October 2020 for studies of TEVAR for TBAD. Data on the study design, demographics, endograft details, LSA cove

Esophageal fistula after endovascular treatment in a type B aortic dissection of the descending thoracic aorta.

We report a type B aortic dissection treated with stenting of the descending thoracic aorta that subsequently developed an ischemic necrosis of the esophagus with a posterior mediastinum abscess. The surgical treatment consisted of an extra-anatomic bypass to revascularize the supra-aortic trunks and the distal abdominal aorta through a middle sternal laparotomy, the resection of the thoracic aorta, and the drainage of the mediastinal abscess. Despite this aggressive surgical approach and an initial favorable postoperative course, the patient suddenly died 3 weeks later, likely from a rupture

The Utility of Paraspinal Acoustic Windows for the Evaluation of Acute Dissection of Descending Thoracic Aorta in the Emergency Setting.

Acute aortic dissection (AAD) is the prevalent acute aortic syndrome characterized by rapid onset and progression with time-dependent prognosis. When suspecting AAD of descending thoracic aorta in the context of the emergency department setting, computed tomography scanning and trans-esophageal echocardiography are the most useful imaging modalities. The sensitivity of transthoracic echocardiography in diagnosing for type B dissection is only 31%-55% when compared with other modalities. We describe the case of a 62-year-old female with a clinical history of Marfan syndrome where the low sensit

Targeted genetic analysis in a large cohort of familial and sporadic cases of aneurysm or dissection of the thoracic aorta.

PURPOSE: Thoracic aortic aneurysm/aortic dissection (TAAD) is a disorder with highly variable age of onset and phenotype. We sought to determine the prevalence of pathogenic variants in TAAD-associated genes in a mixed cohort of sporadic and familial TAAD patients and identify relevant genotype-phenotype relationships. METHODS: We used a targeted polymerase chain reaction and next-generation sequencing-based panel for genetic analysis of 15 TAAD-associated genes in 1,025 unrelated TAAD cases. RESULTS: We identified 49 pathogenic or likely pathogenic (P/LP) variants in 47 cases (4.9% of those

Aneurysmal dilation of the ascending thoracic aorta and the aortic arch following surgical repair of type A dissection.

This report describes the case of a 71-year-old lady who was diagnosed with a Stanford type A dissecting aortic aneurysm which resulted in paraplegia secondary to spinal artery injury at T12 level. She had surgical repair with a tube graft. At a routine review CT scan 2 years postdissection, she presents with asymptomatic but significant dilation, of maximum diameter 78 mm, of the superior part of the ascending thoracic aorta, extending into the arch, suggestive of false aneurysm formation at the surgical anastomoses. There was also thrombosis of the false lumen in the distal arch and descendi

Thoracic endovascular aortic repair for retrograde type A aortic dissection with an entry tear in the descending aorta.

PURPOSE: To report the immediate and follow-up outcome of thoracic endovascular aortic repair (TEVAR) in highly selected patients with retrograde type A aortic dissection (RAAD) and an entry tear in the descending aorta. MATERIALS AND METHODS: TEVAR was performed in 17 patients with RAAD and an entry tear in the descending aorta. None of the patients had severe aortic regurgitation, cardiac tamponade, coronary artery involvement, or brain ischemia. The false lumen in the ascending aorta was patent in nine patients. Two patients had acute malperfusion of the branched artery. Computed tomograph

Risk of development of abdominal aortic aneurysm and dissection of thoracic aorta in a postpartum woman with Marfan's syndrome.

A 24-year-old pregnant woman with Marfan's syndrome delivered by cesarean section during the 38th week of gestation. Although aortic root dilatation did not increase during pregnancy, three months after delivery, the patient noticed a pulsatile abdominal mass. Aortic aneurysm was diagnosed and surgical replacement of the infrarenal abdominal aorta to the common iliac arteries and reconstruction of the inferior mesenteric artery were performed. Moreover, the patient subsequently developed a Stanford type B thoracic aortic dissection, even after more than four months of beta-blockade.

[A coronary aneurysm of left main trunk associated with localized dissecting aneurysms of thoracic and abdominal aorta].

A 47-year-old man who had undergone surgery of a dissecting thoracic aortic aneurysm was admitted for a localized, dissecting abdominal aortic aneurysm. Preoperative coronary arteriogram incidentally showed a coronary artery aneurysm (CAA) of the left main trunk. Prior to the operation for abdominal aortic aneurysm, that for the CAA was performed under standard cardiopulmonary bypass conditions with antegrade and retrograde coronary sinus cold blood cardioplegia. A longitudinal incision of the aneurysmal wall was made for the removal of a small thrombus and, the ostia of the left anterior desc

Treatment of chronic expanding dissecting aneurysms of the descending thoracic and upper abdominal aorta by extended aortotomy, removal of the dissected intima, and closure.

PURPOSE: The incidence of complications, particularly paraplegia, remains high after surgical repair of DeBakey type IIIB chronic expanding dissecting aneurysms. In this report we cautiously advocate a new approach termed tailored aortoplasty, which allows the surgeon to achieve two primary goals: reduced renal/visceral and spinal cord ischemic time, and reperfusion of all patent inferior intercostal and superior lumbar arteries. METHODS: Tailored aortoplasty consists of incising the controlled aorta on the left posterolateral surface from T-8 to the infrarenal portion. The membrane separatin

[Periaortic gas and signs of dissection of a mycotic aneurysm in the descending thoracic aorta due to Clostridium septicum].

The case of 92-year-old male with a history of sepsis due to Clostridium septicum is described. Since the bacteria were also found at the site of joint aspiration, the patient underwent knee prosthesis removal. Due to the association between colorectal cancer and Clostridium septicum, a colonoscopy was performed. An adenocarcinoma was extracted in situ. At 5 months, the patient presented again with shortness of breath, acute retrosternal pain and fever. CT revealed a mycotic aneurysm of the descending aorta with Stanford B dissection and periaortic gas. Due to the fragility of the patient, con

Controlled balloon false lumen obliteration for the endovascular management of chronic dissection in the descending thoracic aorta.

OBJECTIVE: Retrograde false lumen perfusion has limited the utility of aortic stent grafting for chronic aortic dissection. It is unknown whether balloon septal rupture can improve the outcomes for endovascular management of chronic aortic dissection. METHODS: Included patients underwent false lumen obliteration and creation of a single-lumen aortic landing zone using balloon aortoplasty during thoracic endovascular aortic repair. The distal thoracic stent graft was sized to the total aortic lumen diameter, and septal rupture was performed within the stent graft with a compliant balloon in th
