Is it Trauma- or Fantasy-based? Comparing dissociative identity disorder, post-traumatic stress disorder, simulators, and controls.

OBJECTIVE: The Trauma Model of dissociative identity disorder (DID) posits that DID is etiologically related to chronic neglect and physical and/or sexual abuse in childhood. In contrast, the Fantasy Model posits that DID can be simulated and is mediated by high suggestibility, fantasy proneness, and sociocultural influences. To date, these two models have not been jointly tested in individuals with DID in an empirical manner. METHOD: This study included matched groups [patients (n = 33) and controls (n = 32)] that were compared on psychological Trauma and Fantasy measures: diagnosed genuine

A cross-sectional survey on French psychiatrists' knowledge and perceptions of dissociative identity disorder.

OBJECTIVE: The aim of this study was to determine French psychiatrists' level of general knowledge about dissociative identity disorder and to evaluate their perceptions of this condition. METHODS: In this study, French psychiatrists were invited by e-mail to answer an online survey. The questionnaire asked about their general knowledge and perceptions of dissociative identity disorder. RESULTS: We received 924 answers including 582 complete questionnaires. The survey revealed that almost two-thirds (60.8%) of psychiatrists working in France had never received any training on dissociative di

Seeing Horror through the Lens of Health: Embodying Dissociative Identity Disorder in The Babadook.

This essay analyzes the representation of dissociative identity disorder in Jennifer Kent's debut feature, The Babadook (2014). Although the film's exploration of psychological themes such as maternal ambivalence, grief, and repression have already been widely discussed in the critical literature, I argue that such readings tend to mitigate the embodied nature of the suffering that Kent's film depicts. Using Arthur Frank's concept of chaos stories as well as Edward Branigan's work on film narrative, I show how The Babadook provides a valuable, fictional example of how dissociative identity dis

Dissociative Identity Disorder in an Adolescent With Nine Alternate Personality Traits: A Case Study.

Since dissociative identity disorder (DID) has symptoms similar to schizophrenia, such as auditory hallucinations and delusional thoughts of being controlled, there are difficulties in its differential diagnosis. A 16-year-old adolescent male patient who was previously diagnosed with schizophrenia from a different hospital was admitted to our inpatient psychiatric unit for the evaluation of auditory hallucinations and suicide attempts. Through psychiatric evaluations, it was determined that the patient suffered from identity alternation, dissociation, and amnesia. As for the diagnostic evaluat

Dissociative identity (multiple personality) disorder in Poland: a clinical case description and diagnostic difficulties.

PURPOSE: Dissociative identity disorder (DID) is characterized by the presence of a minimum of two identities. Each identity has different fixed patterns of perception, establishing relationships, and thinking about itself and the environment in which it lives. The etiology of dissociative identity disorder is still not fully understood. The aim of the study is to review the literature on the psychopathology and diagnostics of DID and to describe the clinical case of a patient with co-existing DID and paranoid schizophrenia. CASE DESCRIPTION: The male patient discussed in the article is chara

Higher integration scores are associated with facial emotion perception differences in dissociative identity disorder.

BACKGROUND: Recovery from dissociative identity disorder (DID) is associated with the process of integration, which includes an increasing sense of self-cohesion and ownership over one's own emotions. Emotion perception is a construction based on interplay between stored knowledge (past experience), and incoming sensory inputs, suggesting changes in emotion perception might occur at different levels of integration - but this remains unexplored. Therefore, we examined the association between integration, psychiatric symptoms, and facial emotion perception. We hypothesized higher integration wou

Awareness of identity alteration and diagnostic preference between borderline personality disorder and dissociative disorders.

AIM: This study inquires into identity alteration among college students and its relationship to borderline personality disorder (BPD) and/or dissociative disorders (DDs). METHODS: Steinberg Identity Alteration Questionnaire (SIAQ), Childhood Trauma Questionnaire (CTQ), and self-report screening tool of the BPD section of the Structured Clinical Interview for DSM-IV (SCID-BPD) were administered to 1301 college students. Participants who fit the diagnostic criteria of BPD (n = 80) according to the clinician-administered SCID-BPD and 111 non-BPD controls were evaluated using the Structured Clin

Return to work: a case of PTSD, dissociative identity disorder, and satanic ritual abuse.

This case study investigated an intervention that enabled an individual with Posttraumatic Stress Disorder (PTSD), Dissociative Identity Disorder (DID), and satanic ritual abuse to return to work after discharge from psychiatric inpatient treatment. The Occupational Questionnaire [88] revealed past difficulties in organization, awareness of time, communication, cooperation, frustration tolerance, competition, stress management, goal setting, and amnesia resulting in incomplete tasks and sporadic attendance at work. The Role Checklist [72] identified alters valuing work and employed in the past

Familial and social support as protective factors against the development of dissociative identity disorder.

The incidence of dissociative identity disorder (DID) is strongly correlated with exposure to serious physical and sexual abuse. Although studies of more than 1,000 DID sufferers indicate that severe child abuse is a predisposing factor in 95% to 98% of cases (B. Braun, 1988), abuse alone is not, in fact, predictive of DID (B. Rind & P. Tromovitch, 1997). Disorganized/disoriented attachment style and the absence of social and familial support, in combination with abuse history, best predict DID (D. Howe, 2006; R. Kluft, 1984; K. Lyons-Ruth, L. Dutra, M. Schuder, & I. Bianchi, 2006). Individual

Electroconvulsive therapy in an adolescent with dissociative identity disorder and depression: a case report.

BACKGROUND: Dissociative identity disorder (DID) is a psychiatric condition characterized by the existence of at least two distinct identities. This disorder often serves as a defense mechanism, a response to severe childhood trauma, aimed at protecting the individual from overwhelming emotions or experiences. There is no specific targeted treatment for DID at present. OBJECTIVE: The purpose of this report is to explore the safety and efficacy of electroconvulsive therapy (ECT) in combination with pharmacological treatment for the treatment of concomitant depression in an adolescent female wi

A critical review of methodological quality in functional neuroimaging studies on dissociative identity disorder.

Background: Dissociative identity disorder remains contested. The debate hinges on whether memories carry over between identity states and whether those states are truly distinct, but most evidence rests on self report rather than direct memory tests. Neuroimaging has been advanced as an indirect, non self-report approach by scanning individuals with DID in different identity states and comparing them with simulators or other groups. Objective: To evaluate how studies that scan people with DID in more than one identity state inform the core memory claims of DID, by assessing their methodologic

Advancing Research on and Treatment of Dissociative Identity Disorder With People With Lived Experience.

Dissociative identity disorder is a posttraumatic, psychobiological syndrome that develops over time during childhood. Despite empirical evidence supporting the validity of this diagnosis and its relation to trauma, the disorder remains a misunderstood and stigmatized condition. This article highlights expert consensus guidelines and current empirical research on the treatment of dissociative identity disorder. In addition, the authors describe the Lived Experience Advisory Panel (LEAP), which was designed to leverage the expertise of individuals with dissociative identity disorder to combat s

Multimodal Treatment With ECT for Identity Integration in a Patient With Dissociative Identity Disorder, Complex Post-traumatic Stress Disorder, and Major Depressive Disorder: A Rare Case Report.

The legitimacy and etiology of Dissociative Identity Disorder (DID) remains a controversial topic within Psychiatry. The two schools of thought are the Post-Traumatic Model (PTM) and the Socio-Cognitive Model (SCM). This case highlights the validity of PTM in an individual who suffered severe and prolonged physical, psychological, and sexual abuse from 2 years old through adulthood. The reported abuse was corroborated and proven on two separate occasions via medical professionals/rape kit and the police. This resulted in the incarceration of one of her abusers. The only way for the patient to

[Treatment of dissociation in borderline personality disorder with dissociative identity disorder and complex post-traumatic stress disorder: A clinical case].

We describe the trajectory of a 24-year-old woman with a history of borderline personality disorder (BPD) and complex trauma who presented with significant dissociative episodes. The initial assessment-including a psychiatric interview, the use of standardized measures (PCL-5, CAPS-5), and collateral information gathering-identified complex post-traumatic-stress disorder (PTSD) comorbid with dissociative identity disorder (DIID) and incomplete diagnostic criteria for BPD. Treatment consisted of an integrated psychiatric and psychotherapeutic approach based on the DBT-PTSD model. After just ove

Predicting stabilizing treatment outcomes for complex posttraumatic stress disorder and dissociative identity disorder: an expertise-based prognostic model.

The purpose of this study was to develop an expertise-based prognostic model for the treatment of complex posttraumatic stress disorder (PTSD) and dissociative identity disorder (DID). We developed a survey in 2 rounds: In the first round we surveyed 42 experienced therapists (22 DID and 20 complex PTSD therapists), and in the second round we surveyed a subset of 22 of the 42 therapists (13 DID and 9 complex PTSD therapists). First, we drew on therapists' knowledge of prognostic factors for stabilization-oriented treatment of complex PTSD and DID. Second, therapists prioritized a list of progn

Overt and covert processing of self-relevance information in dissociative identity disorder: controlled fMRI study.

BACKGROUND: Dissociative identity disorder (DID) manifests with distinct trauma-avoidant and trauma-related identity states. Overtly conscious trauma-related knowledge processing is identity state-dependent. Previous research on covertly subconscious knowledge processing in DID lacks subject-specific trauma-related stimuli. AIMS: Our controlled functional magnetic resonance imaging (fMRI) study explored neural and behavioural differences of overt and covert knowledge processing of individualised self-relevant words in DID. METHOD: Behavioural data were gathered while 56 participants underwen

Comparison of dissociative identity disorder with other diagnostic groups using a structured interview in Turkey.

Twenty patients with dissociative identity disorder (DID), 20 with schizophrenic disorder, 20 with panic disorder, and 20 with complex partial epilepsy were evaluated with the Dissociative Disorders Interview Schedule (DDIS) and the Dissociative Experiences Scale (DES). Subjects with dissociative identity disorder were more frequently diagnosed as having somatization disorder, past or concurrent major depressive episode, borderline personality disorder, depersonalization disorder, and dissociative amnesia than other groups. They reported Schneiderian symptoms and extrasensory perceptions more

A brief cognitive-behavioural treatment approach for PTSD and Dissociative Identity Disorder, a case report.

BACKGROUND AND OBJECTIVES: We described a new treatment model for Posttraumatic Stress Disorder (PTSD) and Dissociative Identity Disorder (DID), based on cognitive-behavioural principles. In this model, dissociation is seen as a maladaptive avoidant coping strategy. In addition, we stress that patients have dysfunctional beliefs about dissociation. Both elements, avoidance behaviour and dysfunctional beliefs, are challenged during the brief, intensive trauma-focused treatment. When the PTSD-symptoms decrease, the patient is offered a fare-well ritual to say goodbye to their identities in one o

Inter-identity amnesia for neutral episodic self-referential and autobiographical memory in Dissociative Identity Disorder: An assessment of recall and recognition.

Amnesia is a core diagnostic criterion for Dissociative Identity Disorder (DID), however previous research has indicated memory transfer. As DID has been conceptualised as being a disorder of distinct identities, in this experiment, behavioral tasks were used to assess the nature of amnesia for episodic 1) self-referential and 2) autobiographical memories across identities. Nineteen DID participants, 16 DID simulators, 21 partial information, and 20 full information comparison participants from the general population were recruited. In the first study, participants were presented with two vign

Cognitive abuse within the incestuous family as a factor in the development of dissociative identity disorder.

The polarized nature of the ongoing controversies surrounding the genesis and validity of dissociative identity disorder pit advocates who see and work with dissociative identity disorder sufferers against skeptics who claim it to be an artificial iatrogenically produced phenomenon. This paper suggests that such a dichotomy is unwarranted and that, in fact, both sides of the argument have merit. A short description of the controversy that rages among professionals is followed by a personal-experiential review of the development of dissociative identities in the author.
