Selection against genetic defects in semen donors.

Artificial insemination donor selection requires predicting which men are likely to beget the healthiest offspring. Methods are developed for calculating the "offspring excess recurrence risk", delta R, for an anomaly in the offspring of an afflicted father. Mainly from published family survey and population data delta R is computed for 38 disorders. From a small survey a value for the with-treatment "affliction burden", Bt, is assigned to each anomaly. For each disorder the "offspring excess burden expectation" is delta RBt. Defects such as cataract, hereditary Parkinson disease, psoriasis, s

Harnessing HLA Divergence for Improved Donor Selection in Haploidentical Haematopoietic Stem Cell Transplantation.

Haploidentical haematopoietic stem cell transplantation (haplo-HSCT) with post-transplant cyclophosphamide (PTCy)-based graft-versus-host disease (GVHD) prophylaxis is commonly used in patients without a fully matched donor. HLA evolutionary divergence (HED), a surrogate for the diversity of the immunopeptidome, can predict HLA-matched HSCT outcomes. Because of the peculiarities of HED measurement in haplo-HSCT, we designed a new HEDD->R metric to proxy the fraction of the recipient-specific immunopeptidome not recognised as self by the haplo-identical donor, and evaluated its predictive value

Adult-to-Adult Living Donor Liver Transplant: Hemodynamic Evaluation, Prognosis, and Recipient Selection.

BACKGROUND: Living donor liver transplant (LDLT) is a well-established treatment for end-stage liver disease. A better recipient selection and hemodynamic evaluation may improve transplant outcomes. The aim of this study was to establish recipient parameters that could enhance the results of adult-to-adult LDLT. METHODS: We performed a retrospective study of all adult-to-adult LDLTs from a single center between January 2006 and December 2018. Variables analyzed included demographic and clinical parameters, laboratory tests, performance of intraoperative temporary portocaval shunt (TPCS), graf

Single-donor islet transplantation in type 1 diabetes: patient selection and special considerations.

Type 1 diabetes mellitus is an autoimmune disorder of the endocrine pancreas that currently affects millions of people in the United States. Although the disease can be managed with exogenous insulin administration, the ultimate cure for the condition lies in restoring a patient's ability to produce their own insulin. Islet cell allotransplantation provides a means of endogenous insulin production. Though far from perfected, islet transplants are now a proven treatment for type 1 diabetics. However, proper patient selection is critical for achieving optimal outcomes. Given the shortage of tran

Assessment of Graft Selection Criteria in Living-Donor Liver Transplantation: The Jikei Experience.

In living-donor liver transplantation, graft selection is especially important for the safety of the live donor and an acceptable outcome for the recipient. The essential medical requirements for living liver donation at Jikei University Hospital are as follows: an adult aged 65 years or younger, in good general condition, with partial liver volume of more than 35% of the standard liver volume (SLV) for the recipient, and without severe liver steatosis. Based on our criteria, we performed 13 living-donor liver transplantations between 2007 and 2013, including 1 retransplantation. Three cases w

Special issues in the management and selection of the donor for lung transplantation.

Lung transplantation is a viable treatment option for select patients with end-stage lung disease. Two issues hamper progress in transplantation: first, donor shortage is a major limitation to increasing the number of transplants performed. Secondly, recipient outcomes remain disappointing when compared with other solid organ transplant results. Outcomes are limited by primary graft dysfunction (PGD), the posttransplant acute lung injury that increases both short-and long-term mortality. Attempts to overcome donor shortage have included aggressively managing solid organ donors to increase the

Small-for-size liver syndrome after auxiliary and split liver transplantation: donor selection.

Small-for-size liver grafts can be defined by a recognizable clinical syndrome that results from the transplantation of too small a functional mass of liver for a designated recipient. A graft to recipient body weight ratio less than 0.8, impaired venous inflow, and enhanced metabolic demands in patients with poor clinical conditions must be considered as main factors leading to the small-for-size syndrome (SFSS) when using living and cadaveric partial grafts such as split and auxiliary liver grafts. Increased risk of graft dysfunction is currently observed in fatty infiltration of more than 3

Simplifying microvascular head and neck reconstruction: a rational approach to donor site selection.

The purpose of this study was to review the authors' 13-year experience with free tissue transfer for head and neck oncology patients. This study was a retrospective review of 728 free flaps performed in 698 patients. Recipient sites were subdivided by region into the mandible (N = 253), mid face/orbit (N = 190), hypopharynx (N = 134), oral cavity (N = 104), skull base (N = 36), and scalp (N = 11). The overall free flap success rate was 98.6%. Seventy-nine flaps (10.9%) were reexplored for vascular compromise. Ten flaps (1.4%) were lost in their entirety. The overall complication rate was 17.5

Current methodologies of human leukocyte antigen typing utilized for bone marrow donor selection.

Matching for human leukocyte antigen (HLA) loci is the most crucial factor in the selection of bone marrow donors. The techniques utilized to assign HLA type have evolved significantly since the application of polymerase chain reaction-based methodologies. A significant challenge for DNA typing methodologies is to be able to maintain high resolution typing with the ever-increasing numbers of defined HLA alleles. To date, over 800 HLA class I and class II alleles have been identified by sequencing analysis, and this number shows no sign of reaching a plateau. It is only with high allelic level

Molecular analysis of HLA genes for the selection of unrelated bone marrow donor.

The selection of fully matched unrelated volunteer donors (UVD) in BMT requires a molecular characterization of MHC polymorphism, since most phenotypically HLA-identical donors can be non-identical when analyzed at a genomic level. The present report describes a molecular typing protocol for HLA genes developed for the selection of UVD, and its application to some donor-recipient pairs. The protocol involves three successive steps. Firstly, PCR with sequence-specific primers for HLA-DRB1 and -DQB1 genes is performed to identify the major alleles of the recipient. PCR-fingerprint matching is th

Reassessment of blood donor selection criteria for United States travelers to malarious areas.

In the United States (US), travelers who have had malaria or who have taken antimalarial chemoprophylaxis are deferred as blood donors for 3 years to prevent transfusion-transmitted malaria. To assess the impact of shortening this 3-year exclusion period, national malaria surveillance data from 1972 to 1988 were reviewed. The average annual rate of transfusion-transmitted malaria is 0.25 cases per million units of blood collected. Of 45 reported cases, 38 percent were caused by Plasmodium malariae, 29 percent by P. falciparum, 24 percent by P. vivax, and 9 percent by P. ovale. Thirty-two donor

Comparing donor selection barriers for living donor liver transplantation among Hispanic and non-Hispanic candidates in a large urban center.

Hispanics represent the largest-growing ethnic group awaiting liver transplantation. Though racial/ethnic barriers to living donor liver transplantation have been described among recipients, little is known about challenges for minority living liver donor (LLD) candidates during donor evaluation. We aimed to characterize LLD candidates and compare donor-related barriers by ethnicity through a single-center retrospective cohort study of all LLD referrals from December 2017 to August 2021. The primary outcome was LLD approval with OR estimated using multivariable logistic regression. Among 1508

Donor Selection and Human Leukocyte Antigen Loss Leukemia Relapse After Hematopoietic Cell Transplantation.

PURPOSE: For patients with hematologic malignancies, relapse is the leading cause of death after allogeneic hematopoietic cell transplantation (allo-HCT). Frequently, relapses are explained by immune evasion through alterations of human leukocyte antigens (HLAs), but determinants and clinical consequences remain poorly defined. METHODS: We analyzed 533 relapses of hematologic cancers after allo-HCT from different donor types, conducted at 27 centers worldwide. Genomic loss of mismatched HLA (HLA loss) was assessed using a newly developed next-generation sequencing pipeline. Clinical and immun

"Pinch Test" Facilitates Donor Sensory Nerve Selection Before Corneal Neurotization for Neurotrophic Keratopathy.

PURPOSE: Neurotrophic keratopathy (NK), caused by deficient corneal innervation, leads to progressive corneal ulceration. Corneal neurotization (CN) uses nerve transfers to direct new nerve fibers to the insensate cornea to treat NK. However, selecting a donor nerve with functional nociception remains challenging, especially in pediatric patients with congenital NK. This study evaluates the "pinch test" as a reliable method to assess nociceptive function in potential donor nerves for CN. METHODS: A retrospective chart review identified patients who underwent CN at a single center between 2021

Donor selection criteria for liver transplantation in Argentina: are current standards too rigorous?

Organ shortage is the major limitation for the growth of deceased donor liver transplant worldwide. One strategy to ameliorate this problem is to maximize the liver utilization rate. To assess predictors of liver utilization in Argentina. The national database was used to analyze transplant activity in 2010. Donor, recipient, and transplant variables were evaluated as predictors of graft utilization of number of rejected donor offers before grafting and with the occurrence of primary nonfunction (PNF) or early post-transplant mortality (EM). Of the 582 deceased donors, 293 (50.3%) were recover

Surgical issues in lung transplantation: options, donor selection, graft preservation, and airway healing.

To present an overview of the surgical issues in lung transplantation, including the historical context and the rationale for choosing a particular procedure for a specific patient, we reviewed and summarized the current medical literature and our personal experience. Several surgical options are available, including single lung transplantation; double lung transplantation; heart-lung transplantation; bilateral, sequential single lung transplantation; and (recently) single lobe transplantation. Although single lung transplantation is preferred for maximal use of the available organs, bilateral
