Simultaneous Integrated Dose Reduction Radiation Therapy Versus Conventional Fractionated Radiation Therapy With Chemotherapy for Limited-Stage Small Cell Lung Cancer in the Modern Treatment Era: A Phase 3, Randomized Clinical Trial (SDR-RT trial).

PURPOSE: Simultaneous integrated dose reduction (SDR) radiation therapy (SDR-RT) with a definitive dose to the gross tumor volume (GTV) and a relatively lower dose to the clinical target volume (CTV) might be beneficial in improving normal tissue sparing with low toxicity. We aimed to compare the efficacies of SDR-RT and conventional fractionated radiation therapy (CF-RT) for limited-stage small cell lung cancer (LS-SCLC). METHODS AND MATERIALS: Eligible patients with LS-SCLC were 1:1 randomized to receive CF-RT (60 Gy in 30 once-daily fractions over 6 weeks for both CTV and GTV) or SDR-RT (5

Ultra-high dose radiation for Liver metastasis using MR-guided TReatment with stereotactic Ablative Single-fraction (ULTRAS): study protocol for a phase III randomized controlled trial.

INTRODUCTION: Liver metastases are common in several malignancies, with only 15-20% of patients eligible for surgical resection. For non-surgical candidates, stereotactic body radiation therapy (SBRT) is an effective alternative with acceptable local control (LC) rates. Higher biologically effective doses (BED10 > 100 Gy10) are associated with improved LC, and single-fraction ultra-high dose SBRT (35-40 Gy) has demonstrated excellent LC with minimal toxicity. MR-guided adaptive SBRT enhances precision and may optimizing single-fraction SBRT for liver metastases. METHODS AND ANALYSIS: ULTRAS i

A final report of a phase I study of veliparib (ABT-888) in combination with low-dose fractionated whole abdominal radiation therapy (LDFWAR) in patients with advanced solid malignancies and peritoneal carcinomatosis with a dose escalation in ovarian and fallopian tube cancers.

BACKGROUND: The combination of low-dose radiation therapy with PARP inhibition enhances anti-tumor efficacy through potentiating DNA damage. We combined low-dose fractionated whole abdominal radiation (LDFWAR) with ABT-888 in patients with peritoneal carcinomatosis with a dose escalation in ovarian and fallopian cancer patients (OV). METHODS: Patients were treated with veliparib, 40-400mg orally BID on days 1-21 of 3 28-day cycles on 6 dose levels. Dose levels 5 and 6 included only OV patients. LDFWAR consisted of 21.6Gy in 36 fractions, 0.6Gy twice daily on days 1 and 5 for weeks 1-3 of each

Inter-Fractional Variations in Volume and Radiation Dose to the Organs at Risk, High-Risk Clinical Target Volume and Implication of Image-Guided Adaptive Planning During Intracavitary Brachytherapy of Carcinoma Cervix.

Background Geometrical and anatomical variations occur during the brachytherapy of carcinoma cervix and dose optimization is necessary for every fraction of high‑dose rate intracavitary brachytherapy (HDR-ICBT) for carcinoma of the cervix. A single planned treatment is usually delivered for multiple fractions without consideration of inter-fractional applicator positioning variations and organ motion, which may lead to substantial differences between the planned and delivered doses. Aim and objectives This study was aimed at evaluating the inter-fractional variation in volume and radiation dos

A comparative study of the treatment outcome of moderately accelerated radiation fractionation (with concurrent chemotherapy in daily dosing) to conventional chemo-radiotherapy in locally advanced head and neck cancers: Supportive in a resource constrained environment.

BACKGROUND: In our country, head and neck cancers account for about a third of all cancers. Moreover, the patients typically present in advanced stages, which entails intensive multimodality therapy; but there is not much scope for improved survival outcomes. In view of this, a study was conducted to know the effects of treatment intensification, wherein moderately accelerated fractionation radiotherapy was given to patients presenting with advanced cancer of head and neck. This treatment was further intensified by accompanying radiation with concurrent cisplatin chemotherapy in daily doses. T

Radiation doses and fractionation schedules in non-low-risk ductal carcinoma in situ in the breast (BIG 3-07/TROG 07.01): a randomised, factorial, multicentre, open-label, phase 3 study.

BACKGROUND: Whole breast irradiation (WBI) after conservative surgery for ductal carcinoma in situ (DCIS) reduces local recurrence. We investigated whether a tumour bed boost after WBI improved outcomes, and examined radiation dose fractionation sensitivity for non-low-risk DCIS. METHODS: The study was an international, randomised, unmasked, phase 3 trial involving 136 participating centres of six clinical trials organisations in 11 countries (Australia, New Zealand, Singapore, Canada, the Netherlands, Belgium, France, Switzerland, Italy, Ireland, and the UK). Eligible patients were women age

Effect of triple therapy with low-dose total body irradiation and hypo-fractionated radiation plus anti-programmed cell death protein 1 blockade on abscopal antitumor immune responses in breast cancer.

Immunostimulatory effects of radiotherapy can be synergistically augmented with immune checkpoint blockade to act both on irradiated tumor lesions and distant, non-irradiated tumor sites. Our hypothesis was that low-dose total body irradiation (L-TBI) combined with hypo-fractionated radiotherapy (H-RT) and anti-programmed cell death protein 1 (aPD-1) checkpoint blockade would enhance the systemic immune response. We tested the efficacy of this triple therapy (L-TBI + H-RT + aPD-1) in BALB/c mice with bilateral breast cancer xenografts. The L-TBI dose was 0.1 Gy. The primary tumor was treated w

Normal tissue radiosensitivity: prediction on deterministic or stochastic basis?

For the same standardized physical radiation dose there is considerable variation, among different patients, of the magnitude of early and late normal tissue reactions. Technical and clinical factors account for about one third only of these variations. Genetic or epigenetic differences between patients account for the greater proportion of interpatient radiosensitivity differences. Attempts have been made to correlate differences in the tissue radiosensitivity with the in vitro radiosensitivity of fibroblasts or lymphocytes as well as with other biological cellular processes related to cell d

Morphological correlates of fractionated radiation of the mouse lung: early and late effects.

PURPOSE: The definition and quantitation of radiation-induced morphologic alterations in murine lungs is presented. METHODS AND MATERIALS: The extent of injury to the lung, which is the dose-limiting organ in the thorax, may be reduced by fractionating the total radiation exposure to permit partial repair of radiation-induced damage between fraction administration and also to permit a larger total exposure to be administered. We previously reported that, following fractionated radiation exposures, as the dose/fraction decreases, the total dose to reach an isoeffect increases, with an alpha/be

Mediators of Racial Disparities in Heart Dose Among Whole Breast Radiotherapy Patients.

BACKGROUND: Racial disparities in survival of patients with cancer motivate research to quantify treatment disparities and evaluate multilevel determinants. Previous research has not evaluated cardiac radiation dose in large cohorts of breast cancer patients by race nor examined potential causes or implications of dose disparities. METHODS: We used a statewide consortium database to consecutively sample 8750 women who received whole breast radiotherapy between 2012 and 2018. We generated laterality- and fractionation-specific models of mean heart dose. We generated patient- and facility-level

Enhanced regeneration response of laryngeal and hypopharyngeal mucosa with accelerated hyperfractionated radiation therapy for glottic cancers.

The course and severity of acute mucosal reactions in 22 patients with previously untreated T1-2N0 glottic cancers were compared between two treatment schedules with different dose intensities: accelerated hyperfractionated radiation therapy (AHF) and standard conventional fractionation radiation therapy (CF). AHF consisted of a twice-daily fractionation of 1.5 Gy 10 times weekly to a total dose of 66 Gy given in 30-40 (median, 33) days. For CF, the fractionation was 2 Gy five times weekly for a total dose of 66 Gy in 45-51 (median, 49) days. Both treatment schedules were well tolerated and no

Radiotherapy of high-grade gliomas: dealing with a stalemate.

This article discusses the studies on radiotherapy of high-grade gliomas published between January 1, 2022, and June 30, 2022, with special reference to their molecular biology basis. The focus was on advances in radioresistance, radiosensitization and the toxicity of radiotherapy treatments. In the first half of 2022, several important advances have been made in understanding resistance mechanisms in high-grade gliomas. Furthermore, the development of several radiosensitization procedures for these deadly tumors, including studies with small molecule radiosensitizers, new fractionation protoc

Radiation-Induced Oral Mucositis.

Radiation-induced oral mucositis (RIOM) is a major dose-limiting toxicity in head and neck cancer patients. It is a normal tissue injury caused by radiation/radiotherapy (RT), which has marked adverse effects on patient quality of life and cancer therapy continuity. It is a challenge for radiation oncologists since it leads to cancer therapy interruption, poor local tumor control, and changes in dose fractionation. RIOM occurs in 100% of altered fractionation radiotherapy head and neck cancer patients. In the United Sates, its economic cost was estimated to reach 17,000.00 USD per patient with

Role of stereotactic body radiotherapy in spinal metastasis and subsequent fracture risk: identifying and treating the at-risk patient.

The treatment of spinal metastasis has considerably improved with the advent of stereotactic body radiotherapy. Technological advances have enabled the precise delivery of high-dose radiation that may supplant surgery and standard fractionation postoperative radiation as a treatment for spinal metastasis without cord compression. Unfortunately, the higher biologically equivalent doses conferred by stereotactic body radiotherapy can also result in radiation toxicity, notably myelitis and vertebral body fracture. These are toxicities that the radiation oncologist must be able to anticipate, miti
