EUS guided gallbladder drainage.

Cholecystectomy is the gold standard treatment for acute cholecystitis, but it may not be appropriate for patients with significant comorbidities. Percutaneous gallbladder drainage (PT-GBD) and endoscopic transpapillary gallbladder drainage (ET-GBD) are alternatives with good technical and clinical success rates, but are limited by technical challenges and the need for definitive therapy. EUS-guided gallbladder drainage (EUS-GBD) is quickly becoming the preferred modality of treatment at expert centers in this cohort of patients due to increased efficacy and minimal adverse events. Lacrimal drainage system obstruction.

The lacrimal drainage system consists of the lacrimal puncta, inferior and superior canaliculi, common canaliculus, lacrimal sac, and nasolacrimal duct. It is classically divided into upper and lower portions. Obstruction of the drainage system can be congenital or acquired, and may result in persistent epiphora and mucopurulent discharge. This paper discusses the etiology, clinical picture, diagnosis, and surgical treatment of lacrimal drainage system obstruction.

Anomalous hepatic venous drainage.

We present a 3-year-old boy born with anomalous hepatic venous drainage into the left atrium and a small sinus venosus atrial septum defect, in whom pulmonary arteriovenous malformations developed with progressive cyanosis. Surgical redirection of the hepatic venous drainage to the right atrium and closure of the atrial septal defect led to regression of the pulmonary arteriovenous malformations. However, in contrast to other reports, progressive pulmonary hypertension developed postoperatively.

Laparoscopic gastric drainage procedures.

Gastric outlet obstruction continues to be an indication for drainage despite the common use of powerful proton pump inhibitors. Minimal invasive surgery techniques now play a significant role in the treatment of this pathology. Complicated peptic ulcer disease and cancer are the two most common causes. To accomplish drainage, advanced laparoscopic techniques are required. A variety of procedures are possible, and these are discussed in detail in this report. The advantages of the laparoscopic approach have been realized in this group of patients.

Drainage versus No Drainage after Laparoscopic Cholecystectomy for Acute Cholecystitis: A Meta-Analysis.

To conduct a randomized controlled trial (RCT), meta-analysis to assess the effectiveness of drains in reducing complications after laparoscopic cholecystectomy (LC) for acute cholecystitis needs to be carried out. An electronic search of PubMed, Embase, Science Citation Index, and the Cochrane Library from January 1990 to January 2018 was performed to identify randomized clinical trials that compare prophylactic drainage with no drainage in LC for acute cholecystitis. The outcomes were calculated as odds ratios (ORs) with 95 per cent confidence intervals (CIs) using RevMan 5.2. Four RCTs, whi

Outcome of patients with acute, necrotizing pancreatitis requiring drainage-does drainage size matter?

AIM: To assess the outcome of patients with acute necrotizing pancreatitis treated by percutaneous drainage with special focus on the influence of drainage size and number. METHODS: We performed a retrospective analysis of 80 patients with acute pancreatitis requiring percutaneous drainage therapy for infected necroses. Endpoints were mortality and length of hospital stay. The influence of drainage characteristics such as the median drainage size, the largest drainage size per patient and the total drainage plane per patient on patient outcome was evaluated. RESULTS: Total hospital survival

Computed Tomography-guided Drainage with Modified Trocar Technique Using a Drainaway Drainage Kit.

PURPOSE: Image-guided percutaneous drainage for abscesses is known as a safe and effective treatment. The computed tomography-guided percutaneous drainage kit Drainaway (SB Kawasumi Co., Ltd.), developed on the basis of a modified trocar method, has made it possible to complete the procedure only under computed tomography guidance without radiographic fluoroscopy. This study investigated the feasibility and safety of Drainaway for abscess drainage. MATERIAL AND METHODS: In this retrospective observational study, 28 procedures in 27 patients (18 men and 9 women; age 67.0 ± 12.3 years) who unde

Intelligent surgical drainage - digitizing the analysis of drainage fluid in patients with surgical drains.

Surgical drains are essential in post-operative care, where timely removal is critical to prevent complications. Early removal can result in seromas and hematomas, while delayed removal may lead to infections. Traditional manual analysis of drain output is time-consuming and often unreliable, necessitating a shift towards digital methods. This study used a compact mini-spectrometer to analyze surgical drain output quickly and non-invasively. The spectrometer operates in the 340-850 nm range with 288 discrete detection channels. A total of 528 samples were collected from 181 patients aged 0-85

Comparison of Simple Drainage Versus Drainage With Tract Destruction in the Management of Pilonidal Abscess.

INTRODUCTION: Pilonidal disease is an acquired condition primarily affecting the gluteal cleft and sacrococcygeal area, often complicated by the formation of sinuses and abscesses. Despite multiple surgical options, there is no universally accepted standard treatment. This study aimed to compare the outcomes of simple drainage alone versus drainage combined with sinus destruction using the same incision technique. METHODS: Between February 2019 and March 2024, a total of 480 patients with acute pilonidal abscesses received treatment. Of these, 235 underwent simple drainage (Group A), while 24

Endoscopic ultrasound-guided gallbladder drainage, transpapillary drainage, or percutaneous drainage in high risk acute cholecystitis patients: a systematic review and comparative meta-analysis.

BACKGROUND: Endoscopic transpapillary gallbladder drainage (ETGBD) and endoscopic ultrasound-guided gallbladder drainage (EUSGBD) are alternatives to percutaneous gallbladder drainage (PCGBD) for patients with acute cholecystitis who are unfit for surgery. Data comparing these modalities are limited and have reported conflicting results. METHODS: We searched multiple databases from inception to May 2019 to identify studies that reported on ETGBD, EUSGBD, and PCGBD in the management of acute cholecystitis in patients with a high surgical risk. Aims were to compare the pooled rates of technical

Outcomes of percutaneous transhepatic gallbladder drainage versus percutaneous transhepatic biliary drainage for obstructive jaundice.

Percutaneous transhepatic gallbladder drainage (PTGBD) is an alternative to percutaneous transhepatic biliary drainage (PTBD) for cases with obstructive jaundice in which the bile duct obstruction is below the confluence of the cystic ducts. This retrospective study aimed to evaluate the usefulness of PTGBD and PTBD in patients with obstructive jaundice. We recruited patients who had undergone percutaneous biliary drainage for acute cholangitis and obstructive jaundice at two institutions between January 2017 and March 2024. In principle, PTBD was the first choice. PTGBD was selected for cases

Percutaneous transhepatic biliary drainage may be the preferred preoperative drainage method in hilar cholangiocarcinoma.

Background and study aims  Preoperative biliary drainage of hilar cholangiocarcinoma (HC) is controversial. The goal of this study was to compare the clinical outcome and associated complications for types II, III, and IV HC managed by percutaneous transhepatic biliary drainage (PTBD) and endoscopic retrograde cholangiopancreatography (ERCP). Patients and methods  Between January 2011 and June 2017, a total of 180 patients with II, III, and IV HC were enrolled in this retrospective cohort study. According to the drainage method, patients were divided into two groups: PTBD (n = 81) and ERCP (n

Objective assessment of the drainage function of the lacrimal drainage system based on the dye disappearance test.

BACKGROUND: The assessment of lacrimal drainage function remains a diagnostic challenge in clinical practice. Current methods, such as the dye disappearance test, are limited by subjectivity and lack of quantitative analysis. This study aimed to develop an objective method for evaluating lacrimal drainage function using digital analysis of fluorescein retention patterns. METHODS: A prospective study was conducted on 70 patients (140 observations), including cases without anatomical lacrimal drainage impairment, dacryostenosis, and complete obstruction. The tear film was stained with fluoresce

The lymphatic drainage system of the CNS plays a role in lymphatic drainage, immunity, and neuroinflammation in stroke.

The lymphatic drainage system of the central nervous system (CNS) plays an important role in maintaining interstitial fluid balance and regulating immune responses and immune surveillance. The impaired lymphatic drainage system of the CNS might be involved in the onset and progression of various neurodegenerative diseases, neuroinflammation, and cerebrovascular diseases. A significant immune response and brain edema are observed after stroke, resulting from disrupted homeostasis in the brain. Thus, understanding the lymphatic drainage system of the CNS in stroke may lead to the development of
